Artificial intelligence (AI) systems for automated chest x-ray interpretation hold promise for standardising reporting and reducing delays in health systems with shortages of trained radiologists. Yet, there are few freely accessible AI systems trained on large datasets for practitioners to use with their own data with a view to accelerating clinical deployment of AI systems in radiology. We aimed to contribute an AI system for comprehensive chest x-ray abnormality detection.

This review highlights the current applications and future potential of molecular imaging techniques as companion diagnostics during anti-cancer immunotherapy to stratify patients, monitor molecular dynamics, and determine treatment response.

Efforts to address the poor prognosis associated with esophageal adenocarcinoma (EAC) have been hampered by a lack of biomarkers to identify early disease and therapeutic targets. Despite extensive efforts to understand the somatic mutations associated with EAC over the past decade, a gap remains in understanding how the atlas of genomic aberrations in this cancer impacts the proteome and which somatic variants are of importance for the disease phenotype. We performed a quantitative proteomic analysis of 23 EACs and matched adjacent normal esophageal and gastric tissues. We explored the correlation of transcript and protein abundance using tissue-matched RNA-seq and proteomic data from seven patients and further integrated these data with a cohort of EAC RNA-seq data (n = 264 patients), EAC whole-genome sequencing (n = 454 patients), and external published datasets. We quantified protein expression from 5879 genes in EAC and patient-matched normal tissues. Several biomarker candidates with EAC-selective expression were identified, including the transmembrane protein GPA33. We further verified the EAC-enriched expression of GPA33 in an external cohort of 115 patients and confirm this as an attractive diagnostic and therapeutic target. To further extend the insights gained from our proteomic data, an integrated analysis of protein and RNA expression in EAC and normal tissues revealed several genes with poorly correlated protein and RNA abundance, suggesting posttranscriptional regulation of protein expression. These outlier genes, including SLC25A30, TAOK2, and AGMAT, only rarely demonstrated somatic mutation, suggesting post-transcriptional drivers for this EAC-specific phenotype. AGMAT was demonstrated to be overexpressed at the protein level in EAC compared to adjacent normal tissues with an EAC-selective, post-transcriptional mechanism of regulation of protein abundance proposed. Integrated analysis of proteome, transcriptome, and genome in EAC has revealed several genes with tumor-selective, posttranscriptional regulation of protein expression, which may be an exploitable vulnerability.

BackgroundThere is increasing evidence for the use of exercise in cancer patients and data supporting enhanced tumour volume reduction following chemotherapy in animal models. To date, there is no reported histopathological evidence of a similar oncological benefit in oesophageal cancer.MethodsA prospective non-randomised trial compared a structured prehabilitation exercise intervention during neoadjuvant chemotherapy and surgery versus conventional best-practice for oesophageal cancer patients. Biochemical and body composition analyses were performed at multiple time points. Outcome measures included radiological and pathological markers of disease regression. Logistic regression calculated ORs with 95% CI for the likelihood of pathological response adjusting for chemotherapy regimen and chemotherapy delivery.ResultsComparison of the Intervention (n=21) and Control (n=19) groups indicated the Intervention group had higher rates of tumour regression (Mandard TRG 1–3 Intervention n=15/20 (75%) vs Control n=7/19 (36.8%) p=0.025) including adjusted analyses (OR 6.57; 95% CI 1.52 to 28.30). Combined tumour and node downstaging (Intervention n=9 (42.9%) vs Control n=3 (15.8%) p=0.089) and Fat Free Mass index were also improved (Intervention 17.8 vs 18.7 kg/m2; Control 16.3 vs 14.7 kg/m2, p=0.026). Differences in markers of immunity (CD-3 and CD-8) and inflammation (IL-6, VEGF, INF-y, TNFa, MCP-1 and EGF) were observed.ConclusionThe results suggest improved tumour regression and downstaging in the exercise intervention group and should prompt larger studies on this topic.Trial registration numberNCT03626610.

Standardizing convolutional filters that enhance specific structures and patterns in medical imaging enables reproducible radiomics analyses, improving consistency and reliability for enhanced clinical insights.

Fluoropyrimidine-based chemoradiation (CRT) is used routinely for locally advanced rectal cancer to shrink the tumor preoperatively, improve lateral surgical clearance at total mesorectal excision, prevent local recurrence, and preserve organ function. In Northern Europe, short-course preoperative radiotherapy (SCPRT) is preferred to achieve locoregional control. However, with recent improvements in the quality of surgery, in magnetic resonance imaging (MRI), and in pathologic reporting, we question whether 'routine' CRT or SCPRT should be offered indiscriminately for all patients. MRI is considered the optimal modality for locoregional staging and evaluation of the potential for an involved circumferential resection margin. MRI also provides detailed anatomic information for surgical planning, and may identify poor prognostic features, which influence the way in which the pathologist processes specimens. MRI can predict the likelihood of good/poor tumor response to neoadjuvant CRT and can categorize responders/nonresponders following treatment. Using MRI to define the risk of both local recurrence and metastatic spread allows clinicians to determine which patients might benefit from or safely avoid neoadjuvant treatment. The survival of oesophageal cancer is poor as most patients present with advanced disease. Radiological staging of oesophageal cancer is complex but is fundamental to clinical management. Accurate staging investigations are vitally important to guide treatment decisions and optimise patient outcomes. A combination of baseline computed tomography (CT), endoscopic ultrasound (EUS) and positron emission tomography (PET) are currently used for initial treatment decisions. The potential value of these imaging modalities to re-stage disease, monitor response and alter treatment is currently being investigated. This review presents an essential update on the accuracy of oesophageal cancer staging investigations, their use in re-staging after neo-adjuvant therapy and introduces evolving imaging techniques, including novel biomarkers that have clinical potential in oesophageal cancer.

First-order histogram parameters (mean, median, entropy) and model-based fractal parameters (fractal dimension mean, fractal dimension standard deviation) consistently demonstrated good repeatability, with narrow limits of agreement.

We evaluated magnetic resonance imaging (MRI) voxel heterogeneity following trastuzumab and/or cisplatin in a HER2+ esophageal xenograft (OE19) as a potential response biomarker. OE19 xenografts treated with saline (controls), monotherapy, or combined cisplatin and trastuzumab underwent 9.4-T MRI. Tumor MRI parametric maps of T1 relaxation time (pre/post contrast), T2 relaxation time, T2* relaxation rate (R2*), and apparent diffusion coefficient obtained before (TIME0), after 24hours (TIME1), and after 2weeks of treatment (TIME2) were analyzed. Voxel histogram and fractal parameters (from the whole tumor, rim and center, and as a ratio of rim‐to‐center) were derived. Tumors were stained for immunohistochemical markers of hypoxia (CA-IX), angiogenesis (CD34), and proliferation (Ki-67). Combination therapy reduced xenograft growth rate (relative change, ∆ +0.58±0.43 versus controls, ∆ +4.1±1.0; P=0.008). More spatially homogeneous voxel distribution between the rim to center was noted after treatment for combination therapy versus controls, respectively, for contrast-enhanced T1 relaxation time (90th percentile: ratio 1.00 versus 0.88, P=0.009), T2 relaxation time (mean: 1.00 versus 0.92, P=0.006; median: 0.98 versus 0.91, P=0.006; 75th percentile: 1.02 versus 0.94, P=0.007), and R2* (10th percentile: 0.99 versus 1.26, P=0.003). We found that combination and trastuzumab monotherapy reduced MRI spatial heterogeneity and growth rate compared to the control or cisplatin groups, the former providing adjunctive tumor response information.

Imaging biomarkers (IBs) are used extensively in drug development and cancer research, but important differences exist between IBs and biospecimen-derived biomarkers. A tailored 'roadmap' is required for the development of new IBs to be used either in clinical research or for decision-making in healthcare. In this Consensus statement, a group of experts assembled by CRUK and the EORTC present 14 key recommendations for accelerating the clinical translation of IBs.

Our ability to accurately assess the skeleton for metastases in breast and prostate cancers has improved significantly in recent years with hybrid imaging methods. Nevertheless, no consensus has been reached on the best imaging modality for diagnosis and treatment response assessment of skeletal disease. Hybrid SPECT/CT has low false-positive and false-negative rates compared with planar bone scintigraphy (BS) or BS augmented with SPECT in breast and prostate cancers. In breast cancer, 18F-FDG PET is more sensitive and accurate at detecting bone metastases than BS. Currently, little evidence has accrued to support the superiority of 18F-fluoride (18F-NaF) PET in diagnosing osseous metastases or monitoring treatment response in breast cancer when compared with conventional imaging. In prostate cancer, the sensitivities of 18F-NaF PET/CT, 18F-fluorocholine (18F-choline), or 11C-choline PET/CT are equivalent, although 11C-/18F-choline PET/CT scans are more specific. Whole-body MRI, using anatomical sequences complemented by diffusion-weighted MRI, shows early evidence of utility for diagnosis and monitoring therapy response. We review the literature for staging and response assessment in metastatic breast and prostate cancer. While staging accuracy has significantly improved with hybrid imaging, optimal methods for assessing early treatment response have not been determined, and this is an area of active research.

Pegargiminase (ADI-PEG 20I) degrades arginine in patients with argininosuccinate synthetase 1-deficient malignant pleural mesothelioma (MPM) and NSCLC. Imaging with proliferation biomarker 3'-deoxy-3'-[18F] fluorothymidine (18F-FLT) positron emission tomography (PET)-computed tomography (CT) was performed in a phase 1 study of pegargiminase with pemetrexed and cisplatin (ADIPemCis). The aim was to determine whether FLT PET-CT predicts treatment response earlier than CT.

Hidradenitis suppurativa is a rare chronic inflammatory disorder of apocrine gland-bearing skin, which commonly affects the anogenital region. There has been very little literature to date on the MRI appearances of anogenital hidradenitis suppurativa.

Radiomics is an evolving field in which the extraction of large amounts of features from diagnostic medical images may be used to predict underlying molecular and genetic characteristics, thereby improving treatment response prediction and prognostication and potentially allowing personalisation of cancer treatment. There is increasing interest in extracting additional data from PET images, particularly novel features that describe the heterogeneity of voxel intensities, but a number of potential limitations need to be recognised and overcome. Nevertheless, some early data suggest that extraction of additional quantitative data may offer further predictive and prognostic information in individual patients.

Perfusion computed tomography (CT) is an imaging technique that enables radiologists to evaluate the tumour vasculature, quantifying tumour perfusion, blood volume, and microcirculatory changes, which can provide both prognostic and predictive information to the clinician. Its increasing acceptance within oncology in recent years has been related to a combination of factors: an increasing use of anti-vascular therapy (anti-angiogenic agents and vascular disrupting agents) both as stand-alone first line agents or in combination with standard chemotherapy and/or radiotherapy, and technological improvements in CT, including faster tube rotation speeds, higher temporal sampling rates, the development of dynamic 3D acquisitions and development of commercial software programmes embedded within the clinical workflow. This review summarises current Perfusion CT techniques and their clinical application in oncological practice.

Manipulation of tumor vasculature has emerged as a promising therapeutic strategy in advanced non-small cell lung cancer (NSCLC). Bevacizumab has been licensed for first-line therapy in combination with standard chemotherapy in advanced NSCLC, whereas inhibitors of platelet-derived growth factor receptors and other tyrosine kinases are currently in clinical trials for the treatment of advanced NSCLC. Perfusion computed tomography is a promising biomarker in NSCLC. As a measure of tumor perfusion and angiogenesis, perfusion computed tomography may provide invaluable pharmacodynamic information in early-phase clinical trials, monitor the vascular effects of antiangiogenic or vascular-targeting drug treatment, and provide prognostic and predictive information. Current volumetric techniques, affording a coverage of up to 28 cm, permit clinically relevant, high-quality, whole tumor evaluation.

In the past 10 years, overall survival and disease-free survival of patients with colorectal cancer (CRC) has improved substantially because of a combination of factors: (1) more accurate staging as a result of advances in imaging technology; (2) refinements in surgical technique; (3) ‘curative’ metastasectomy for patients with limited metastatic disease; (4) improvements in radiation therapy planning and greater precision of radiation therapy delivery; and (5) increasing chemotherapeutic options, including antiangiogenic and vascular targeting drugs. In this era of ‘personalized medicine,’ the increasingly individualized treatment of patients with CRC has highlighted the need for functional imaging techniques in addition to conventional anatomic-based imaging. This review discusses the contribution of positron emission tomography to the clinical management of CRC. In addition, evolving techniques such as dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI), DCE computed tomography (perfusion CT), diffusion-weighted MRI, and blood oxygenation level–dependent MRI that might have a future role will be covered.

Neoadjuvant systemic therapy is used to enable breast-conserving surgery in patients with large primary operable breast cancers. It is important to be able to accurately assess response to systemic therapy, both to assist the surgeon and for prognostic purposes. Moreover, a proportion of women will fail to respond to treatment and would potentially benefit from either a change in therapy or earlier surgery rather than continuing completion of the planned course of treatment. Conventional techniques of assessing response (clinical examination, x-ray mammography and breast ultrasound) rely on changes in tumor size, which are often delayed and do not always correlate with pathologic response. This review examines the evidence for functional imaging techniques including scintimammography, functional computed tomography, dynamic magnetic resonance imaging, spectroscopy and positron emission tomography. These techniques measure changes in tumor vasculature, metabolism or proliferation and may prove to be earlier and more sensitive measures of response to systemic therapy, thus enabling tailoring of an individuals treatment.

With the emergence of novel targeted therapies, imaging techniques that assess tumor vascular support have gained credence for response assessment alongside standard response criteria. Computed tomography (CT) perfusion techniques that quantify regional tumor blood flow, blood volume, flow-extraction product, and permeability-surface area product through standard kinetic models are attractive, but the level of evidence for CT perfusion to be a truly mature biomarker remains insufficient. Studies to date have not been powered to assess this. Future studies that include good quality prospective validation correlating perfusion CT to outcome end points in the trial setting are needed to take CT perfusion forward as a biomarker in oncology.
