- Abstract
The relationship of stroke and atrial fibrillation seems to become more complex as we gain more knowledge of the issue. Recent studies have questioned the temporal relationship between the two, which also questions causation. Left atrial appendage closure is an attractive, but unproven technology when compared with the 50-year experience with warfarin. In a patient who is on warfarin and having no issues with bleeding, medication intolerance, or lack of efficacy, it is difficult to justify stopping the drug and placing a closure device as sole therapy to prevent a stroke.

4. REALIZABILITY PROBLEM FOR COMMUTING GRAPHS

- Abstract
We investigate properties which ensure that a given finite graph is the commuting graph of a group or semigroup. We show that all graphs on at least two vertices such that no vertex is adjacent to all other vertices is the commuting graph of some semigroup. Moreover, we obtain complete classifications of the graphs with an isolated vertex or edge that are the commuting graph of a group and the cycles that are the commuting graph of a centrefree semigroup.

5. Locally s‐distance transitive graphs

- Abstract
We give a unified approach to analyzing, for each positive integer s, a class of finite connected graphs that contains all the distance transitive graphs as well as the locally s‐arc transitive graphs of diameter at least s. A graph is in the class if it is connected and if, for each vertex v, the subgroup of automorphisms fixing vacts transitively on the set of vertices at distance ifrom v, for each ifrom 1 to s. We prove that this class is closed under forming normal quotients. Several graphs in the class are designated as degenerate, and a nondegenerate graph in the class is called basic if all its nontrivial normal quotients are degenerate. We prove that, for s≥2, a nondegenerate, nonbasic graph in the class is either a complete multipartite graph or a normal cover of a basic graph. We prove further that, apart from the complete bipartite graphs, each basic graph admits a faithful quasiprimitive action on each of its (1 or 2) vertex‐orbits or a biquasiprimitive action. These results invite detailed additional analysis of the basic graphs using the theory of quasiprimitive permutation groups. © 2011 Wiley Periodicals, Inc. J Graph Theory 69:176‐197, 2012

We establish natural bijections between three different classes of combinatorial objects; namely certain families of locally 2‐arc transitive graphs, partial linear spaces, and homogeneous factorizations of arc‐transitive graphs. Moreover, the bijections intertwine the actions of the relevant automorphism groups. Thus constructions in any of these areas provide examples for the others. © 2005 Wiley Periodicals, Inc. 9. Completely Transitive Codes in Hamming Graphs

- Abstract
A codein a graph Γis a non-empty subset Cof the vertex set Vof Γ. Given C, the partition of Vaccording to the distance of the vertices away from Cis called the distance partition of C. A completely regular codeis a code whose distance partition has a certain regularity property. A special class of completely regular codes are the completely transitive codes. These are completely regular codes such that the cells of the distance partition are orbits of some group of automorphisms of the graph. This paper looks at these codes in the Hamming Graphs and provides a structure theorem which shows that completely transitive codes are made up of either transitiveor nearly complete, completely transitive codes. The results of this paper suggest that particular attention should be paid to those completely transitive codes of transitive type.

10. The Ablate and Pace Trial: A Prospective Study of Catheter Ablation of the AV Conduction System and Permanent Pacemaker Implantation for Treatment of Atrial Fibrillation

- Abstract
Background: The Ablate and Pace Trial (APT) prospectively assessed the effects of catheter ablation of the AV conduction system and permanent pacemaker implantation on health-related quality of life, survival, exercise capacity, and ventricular function in 156 patients with symptomatic atrial fibrillation. Methods: All patients referred for catheter ablation and permanent pacemaker implantation because of medically-refractory atrial fibrillation at 16 centers were screened for enrollment in a prospective registry. Baseline assessment prior to ablation included measurement of quality of life, including the Health Status Questionnaire, the Quality of Life Index and the Symptom Checklist: Frequency and Severity. Exercise capacity was assessed with metabolic treadmill exercise testing and ventricular function was quantitated with echocardiography. The quality of life instruments, exercise capacity, and echocardiography were repeated at 3 and 12 months after catheter ablation. Results: The APT population included 90 men and 66 women (66.1 ± 11.5 years of age) with either chronic (n = 70), recurrent (n = 31), or paroxysmal atrial fibrillation (n = 55). Structural heart disease was present in 78.2% of patients. Successful ablation of AV conduction was achieved in 155 of 156 patients (99.4%). Survival at 1 year was 85.3%, with 5 of 23 deaths being sudden cardiac deaths. Survival over the first year of follow-up was significantly lower for patients with a baseline left ventricular ejection fraction (LVEF) <0.45 (0.73) than for patients with a LVEF ≤0.45 (0.88, p = 0.03). The NYHA functional class improved from 2.1 at baseline to 1.8 at 3 months and 1.9 at 12 months of followup (p = 0.0001). Significant improvement in quality of life scores were noted for all 8 subscales of the Health Status Questionnaire, for the overall rating of the Quality of Life Index, the Health and Function subscales; Arrhythmia-related symptoms were markedly reduced as measured by the Symptom Checklist: Frequency and Severity scale. The mean LVEF improved from 0.50 ± 0.20 at baseline to 0.54 ± 0.20 at 3 months (p = 0.03). The LVEF 12 months after ablation was 0.52 ± 0.20, not statistically different from baseline. Individuals with reduced systolic function at baseline had the greatest improvement, from LVEF 0.31 ± 0.20 at baseline to 0.41 ± 0.20 at 3 months and 0.41 ± 0.30 at 12 months (p = 0.0001). There were no significant changes in treadmill exercise duration (10.0 ± 4.3 min at baseline and 11.6 ± 3.6 min at 12 months) or VO2max(1467 ± 681 ml O2min baseline and 1629 ± 739 ml O2min at 12 months). Conclusions: Catheter ablation of the AV conduction system and permanent pacemaker implantation were associated with improved quality of life and left ventricular function in this population of highly symptomatic patients with atrial fibrillation refractory to medical therapy.

- Abstract
The SAMe-TT2R2score helps identify patients with atrial fibrillation (AF) likely to have poor anticoagulation control during anticoagulation with vitamin K antagonists (VKA) and those with scores >2 might be better managed with a target-specific oral anticoagulant (NOAC). We hypothesized that in clinical practice, VKAs may be prescribed less frequently to patients with AF and SAMe-TT2R2scores >2 than to patients with lower scores.

Introduction:Catheter ablation for atrial fibrillation (PVI) is being performed with increasing frequency. This time and labor-intensive procedure is under increasing scrutiny as we look for means to decrease costs of delivering care. Performing these procedures on therapeutic warfarin could shorten hospital stays, eliminate costly low-molecular weight heparin (LMW) use, and decrease procedural heparin administration which could reduce hemorrhage from access sites.

- Abstract
Body: BackgroundIbutilide fumarate is a class III antiarrhythmic agent whose sole indication is the acute conversion of atrial fibrillation or atrial flutter to sinus rhythm. Ibutilide blocks potassium channels and activates slow inward sodium channels, which delays repolarization. These actions prolong the action potential in atrial and ventricular refractory periods, leading to arrhythmia termination. Many institutions restrict the use of ibutilide because of the potential risk of polymorphic ventricular tachycardia (PMVT). Limitations include administration only in the presence of a physician, use only in certain monitored units, or an Intensive Care Unit.
