Les neuroleptiques sont souvent prescrits chez les patients âgés présentant des troubles du comportement. Ces molécules ont cependant de nombreux effets indésirables. L’objectif de cette étude était d’évaluer l’impact à huit jours de la diminution des neuroleptiques sur l’intensité des troubles du comportement de patients âgés hospitalisés. Une étude prospective observationnelle multicentrique (5 hôpitaux du groupe hospitalo-universitaire Paris Nord Val-de-Seine) incluant des patients de plus de 70 ans, hospitalisés, souffrant de troubles du comportement et traités par neuroleptiques a été conduite. Les participants ont été classés en 2 groupes à j0 selon l’avis des médecins référents des patients et l’importance des troubles évaluée par le score de l’inventaire neuropsychiatrique–équipes soignantes (NPI-ES) : « groupe 1 : patients pour lesquels le traitement neuroleptique a été maintenu à l’identique » et « groupe 2 : patients pour lesquels les neuroleptiques ont été diminués ». L’évolution des troubles du comportement a été mesurée à j8 par le score NPI-ES. L’impression clinique globale selon l’équipe médicale (ICG médecin) et paramédicale (ICG soignant) a également été recueillie à j8. Soixante-dix-huit patients (81,5+5,7 ans) présentant une maladie soit psychiatrique (15,4 %), soit démentielle (84,6 %) avec des troubles du comportement (NPI-ES (moyenne±ET) : 26,06+23,31/144) ont été inclus : 40 dans le « groupe 1 » et 38 dans la « groupe 2 ». La baisse des neuroleptiques était associée à une diminution significative de la sévérité des troubles du comportement dans le groupe ayant bénéficié d’une diminution de doses de neuroleptiques (groupe 2) à j8 par rapport aux troubles du comportement des patients ayant conservé la même dose de neuroleptiques (groupe 1) après ajustement sur la sévérité des troubles à j0 (ΔNPI-ES : groupe 1 vs groupe 2=−0,13 vs −0,34, p=0,003). L’ICG était concordant entre l’équipe médicale et paramédicale et variait dans le même sens que la ΔNPI-ES. Ainsi, cette étude met en évidence une association entre la diminution des doses de neuroleptiques et la diminution des troubles du comportement dans cette population selon la NPI-ES. Ces résultats renforcent les recommandations concernant la révision systématique de la prescription de neuroleptiques.

The few studies that have focused on Time between Onset of Signs and Symptoms and Referral (TOSR) for dementia to a memory center suggest a substantial delay of 1–3 years. This delay has a negative impact on both patients’ and their caregivers’ quality of life.

Anastomotic leakages are severe and often lethal adverse events of surgery for esophageal cancer. The endoscopic treatment is growing up in such indications. The aim was to evaluate the efficacy and describe the strategy of the endoscopic management of anastomotic leakages/fistulas after esophageal oncologic surgery. Single-center retrospective study on 126 patients operated for esophageal carcinomas between 2010 and 2014. Thirty-five patients with postoperative fistulas/leakages (27 %) were endoscopically managed and included. The primary endpoint was the efficacy of the endoscopic treatment. The secondary endpoints were: delays between surgery, diagnosis, endoscopy and recovery; number of procedures; material used; and adverse events rate. Uni- and multivariate analyses were carried out to determine predictive factors of success. There were mostly men, with a median age of 61.7 years ± 8.9 [43–85]. 48.6 % underwent Lewis-Santy surgery and 45.7 % Akiyama’s. 71.4 % patients received neo-adjuvant chemo-radiation therapy. The primary and secondary efficacy was 48.6 and 68.6 %, respectively. The delay between surgery and endoscopy was 8.5 days [6.00–18.25]. Eighty-eight percentages of the patients were treated using double-type metallic stents, with removability and migration rates of 100 and 18 %, respectively. In the other cases, we used over-the-scope clips, naso-cystic drain or combined approach. The mean number of endoscopy was 2.6 ± 1.57 [1–10]. The mortality rate was 17 %, none being related to procedures. No predictive factor of efficacy could be identified. The endoscopic management of leakages or fistulas after esophageal surgery reached an efficacy rate of 68.8 %, mostly using stents, without significant adverse events. The mortality rate could be decreased from 40–100 to 17 %.

The management of post-operative anastomotic leakage and fistulas of the upper GI tract remains challenging. Fully covered stents are used despite a high risk of migration because of a better removability. The goal of our study was to evaluate the effectiveness of this new type of endoscopic stent in this indication. The secondary objective was to determine the ability of withdrawing this stent. Thirty-six patients treated for upper GI fistula using a double-type metallic stent (DTMS) (Taewoong, Korea) for a benign indication were included in this retrospective study. This stent associates an outer uncovered metallic stent, decreasing the risk of migration, to an inner fully covered stent that ensured its tightness. The DTMS was removed after 4 weeks of treatment. Twenty-four patients had a post-operative fistula (15 sleeve gastrectomies), eight had an anastomotic leakage, and four had an esophageal perforation. Seventeen patients underwent a previous failed stenting, and fourteen had an associated treatment with OTSC clips. A final complete healing was achieved in twenty-six patients (72 %). For patients with fistulas, the overall success rate was 66.6 % (16/24) mostly in case of post sleeve fistula (80 %), and it was 75 % (6/8) for patients with anastomotic leakages (3/4). We reached a primary success (one session) in twenty-one cases (58.3 %), and a second session was required in five cases. All the stents were removed without complications after a median stenting time of 32 [20–71] days. The spontaneous migration rate was 16.6 %. This new double-type stent is a new and efficient way to treat post-operative fistulas and leakages in the upper GI tract. The stents were always removable despite the external uncovered part with a low migration rate.

Afin d'evaluer le potentiel en eau thermale du secteur de Haddage, Tunisie, une etude hydrogeothermique multidisciplinaire—comportant la geologie, la geophysique, l'hydrochimie, la geothermometrie et l'hydrogeologie—a ete menee dans le bassin de Maknassy. Cette approche est capitale pour determiner les differents facies chimiques des eaux thermales, et les relier a leurs reservoirs d'origine. Elle constitue un outil adequat pour la delimitation des bassins hydrogeothermiques. En effet, le traitement integre de toutes les donnees existantes ou nouvellement acquises confirme bien que l'eau thermale de la region de Haddage est d'origine meteorique. Elle s'est infiltree profondement dans les sables et gres du Cretace inferieur (formation Boudinar) et a atteint une temperature elevee (60°C) avant de resurgir au niveau de la source par un drainage vertical en suivant les dysharmonies tectoniques. En outre, il a ete possible d'esquisser un modele conceptuel de fonctionnement hydrogeothermique du systeme aquifere dans la region de Maknassy.

L’apparition de la ponction dirigée sous échoendoscopie, a complètement transformé la prise en charge des maladies néoplasiques et en particulier la prise en charge des tumeurs du pancréas. En effet, le pancréas est un organe difficile à prélever sur un plan histologique. Le risque d’essaimage sur le trajet de l’aiguille, au cours des ponctions guidées sous échographie ou sous scanner, a également contribué au développement des ponctions sous échoendoscopie. Si l’on veut répondre à la question «faut-il toujours ponctionner ?», il faut d’abord évaluer les raisons qui doivent conduire à la ponction et les éléments décisionnels de la ponction sous échoendoscopie. The development of ultrasound guided fine needle aspiration (EUS-FNA) thoroughly modified the management of neoplastic pathology and particularly the management of pancreatic tumours. As a matter of fact, samples for histology are difficult to obtain on pancreas. The risk of tumour seeding on the pathway of the needle, during US or scanner guided biopsies largely contributed to the development of EUS FNA. Before answering the question “is EUS-FNA always required?” it is first of all necessary to evaluate the reasons which should lead to perform EUS-FNA and the decisionmaking elements related to this technique. Le champ d’application de l’échographie endoanale aux affections de la sphère anorectale est de plus en plus étendu et de mieux en mieux évalué. L’échographie endo-anorectale est devenue la technique de référence dans l’évaluation des ruptures sphinctériennes responsables d’incontinence ou bien dans l’exploration des affections tumorales du canal anal [1–3]. Elle s’impose actuellement dans l’évaluation des suppurations anopérinéales (abcès ou fistules) en alternative avec l’IRM [4–6]. Toutefois, la plupart des explorations sont effectuées avec une sonde à balayage radial où le canal anal apparaît en coupe transverse [4]. L’exploration à l’aide d’une sonde linéaire permet pourtant une approche plus anatomique du canal anal, une évaluation dynamique de la sphère anale et du périnée et de compléter les images obtenues par coupe radiale, définissant au mieux l’angle de la rupture. Le balayage sagittal permet en effet, une évaluation dynamique de la région anopérinéale, les repères anatomiques se déplaçant dans l’axe de la sonde échographique [7]. Jusqu’à présent, l’échographie endoanale constituait l’imagerie de référence pour apprécier l’intégrité sphinctérienne mais l’IRM est une technique émergente qui semble de plus en plus performante [8, 9]. The scope of anal endosonography applied to disorders of the anorectal area is more and more enlarged with a constant improved assessment. Endorectal echography is considered as a reference for technical investigation in cases of disrupted anal sphincter responsible of fecal incontinence and is a basic method of anal tumors diagnosis [1–3]. It is also currently used to evaluate perianal sepsis i.e. abscess and fistula as an alternative to the MRI [4–6]. Nevertheless, in most cases, the anal endosonography is performed with probes using a radial scanning which gives a transversal view of the anal canal [4], The approach with linear probe is closer to the anatomic structure, offers a possible dynamic evaluation of the anal and perineal area and provides a complement to the radial view more accurate for an appraisal of the angle of the rupture. In fact, the sagittal scanning provides a dynamic investigation of the anoperineal area because the anatomic landmarks are moving together with the probe axis [7]. Until now, endoanal sonography was the best off in imaging for assessment of the sphincter integrity but MRI emerges more and more with an increasing efficiency [8, 9]. [ABSTRACT FROM AUTHOR]

The authors aim to study geological formations that may satisfy needs in drinking water for rural agglomeration in the Mech plain and south Sned plain in the South of Central Tunisia. The geophysics coupled to structural and stratigraphical analysis shows that the superficial water tables are located in the heteregenous covering Mio-Plio-Quaternary. The deep water tables are located in carbonates terms of upper Cretacious (Zebbag). The structural frame of Mech plain consists of a north flanc of E-W Bouhedma anticline borded in the East by NW-SE Mech Fault. The structural frame of south Sned plain consists of a major collapse borded by E-W faults passage way exhibiting transversal faults thus isolating minor breaking down tilted bloks. The faults trace are followed by rivers overbooks among which that of Oued Drag. Favorable areas for superficial and deep water tables exploitation by drilling hole in the two plains are delimited.

La dysfonction oddienne est une affection bénigne non tumorale du sphincter d’Oddi. Elle survient principalement dans les suites de cholécystectomie mais peut précéder dans un nombre non négligeable de cas, l’ablation de la vésicule. Son expression clinique est principalement biliaire mais peut aussi se traduire par des douleurs pancréatiques ou des pancréatites aiguës récurrentes. Elle correspond dans la moitié des cas à une sténose fibreuse de la papille, probablement due à un traumatisme local lors du passage de calculs biliaires, et dans l’autre moitié des cas à une dysfonction oddienne. Le diagnostic de dysfonction oddienne est d’abord un diagnostic d’élimination d’une autre pathologie biliaire comme la lithiase cholédocienne ou une tumeur ampullaire par la réalisation d’une écho-endoscopie et d’une IRM bilio-pancréatique. Son diagnostic positif est basé sur la pratique d’une manométrie oddienne, de moins en moins effectuée en raison d’une morbidité pancréatique difficilement acceptable pour un acte diagnostique. La scintigraphie biliaire peut la remplacer avantageusement. Le traitement médical fait appel aux dérivés nitrés et à la trimébutine. L’efficacité modérée en pratique de ces traitements médicaux conduit parfois mais de façon la plus limitée, à la réalisation d’une intervention sur la papille principale, à type de sphinctérotomie endoscopique mais avec une importante morbidité postopératoire. L’indication de ces traitements doit donc être portée avec une grande rigueur, en utilisant les critères de la classification de Milwaukee. Seuls doivent être éventuellement traités, par voie endoscopique, les patients qui présentent un tableau de douleur biliaire associée à une perturbation du bilan biliaire et/ou à une dilatation de la voie biliaire. Les patients qui ne présentent que des douleurs (type III Milwaukee) souffrent en effet de troubles de la sensibilité ou de la motricité duodénale.

The area of study, located in the northern part of Gabes (South of Tunisia), remains other slightly unknown as far as its deep water table is concerned. The sole existing witness is the 'El Hicha' drilling, dug more than 20 years ago, reaching the water table having a salinity of 5.6 g/l at a depth exceeding 100 m. On the other hand, 15 km North West of this point, another drilling 'Sbih' has reached a water table, with a salinity of 17 g/l at a depth exceeding 150 m. A geophysics study undertaken by VES is deemed necessary in order to know the limits of this deep water table at 'El Hicha' area. To reach this objective, a measuring study was done in collaboration with CRDA-Gabes. Results have allowed to locate the deep water table in three suitable zones to plant new drilling for exploration.

At Sidi Abdel Hamid (Sousse-Tunisia), after having discovered an artesian superficial water-table, we undertook 31 V.E.S. of geophysical measurements. The underground is made of at least five beds. The artesian acquifer is wedged between two impermeable levels. Two faults (F1 and F2) cross the prospected zone. F1 has basculed down the South-East compartment. It represents a barrier between two parallel ground-water flows from West to East. The flow at the north of F1 may cross the sediments of Sebket-Sousse, and that could explain the high sulfure concentration at the level of P1 well (more than 40 mg/l). Geoelectric sections and maps of the acquifer have been made in order to better understand its general structure. Finally, we have made some recommandations and proposed the emplacement of a mechanical boring prospection.
