•Haplo-bone marrow (BM) transplantation is moderately cost effective versus cord blood for persons under age 65 and cost-saving for those over 65.•Haplo-BM is a fair value choice for commercially insured patients with high-risk leukemia and lymphoma who require HCT.•For Medicare enrollees, haplo-BM is a preferred choice when considering costs and outcomes.
