Purpose:To investigate nystagmus and other visual system abnormalities among children exposed to opiates and benzodiazepines in utero.Methods:Retrospective case series comprising clinical examination and case note review of 25 children with nystagmus and reduced vision who were exposed to controlled drugs during pregnancy.Results:Twenty-four children were exposed to opiates, of whom 13 were also exposed to diazepam. One child was exposed to diazepam alone. All children had horizontal nystagmus, which was either fine pendular or jerk type. The nystagmus had a latent element in 4 children and 8 adopted a compensatory head posture. Where the time of onset of nystagmus was known, it was always prior to 6 months of age. At least 9 children (36%) had delayed visual maturation. The mean initial logarithm of the minimum angle of resolution binocular best-corrected visual acuity (BCVA) was 0.54 at an average of 22 months of age. Thirteen children were followed up for 6 months or longer and their BCVA improved to 0.4 at an average age of 48 months. The nystagmus was clinically improved in only 5 patients. Electroretinogram testing was normal in the 4 children tested. The only ocular structural abnormality was binocular optic nerve hypoplasia in 2 children.Conclusion:Exposure to opiates and benzodiazepines in utero may be associated with permanent nystagmus and reduced visual acuity. This is most likely the result of insult(s) to the central nervous system rather than the eyes.

To study the time course of retinopathy of prematurity (ROP) regression and reactivation after treatment with intravitreal ranibizumab or laser in the ranibizumab compared with laser therapy for the treatment of infants born prematurely with ROP trial.

The aim of this study was to investigate whether the mean around which arterial oxygen fluctuations take place was important in a unique animal model of oxygen-induced retinopathy. Retinopathy of prematurity (ROP) is associated with fluctuating arterial oxygen. A recent retrospective study suggested that management of high-risk preterm infants at lower oxygen saturations was associated with less severe ROP. Rat pups were raised in a variable oxygen environment around a high (24), normal (21) or low (17) mean inspired oxygen for 14 d. Rat pups raised in the high (24) mean variable oxygen environment had more retarded retinal vascular development than did rats raised in an environment that fluctuated around 21 mean oxygen. In contrast, rats raised in a lower mean (17) but still variable oxygen environment had no discernible retinal differences from controls raised in constant room air. Rats raised in a relatively hypoxic but variable oxygen environment develop less severe retinal vascular abnormalities than those raised in variable oxygen around higher oxygen means.

The incidence of ROP is birth weight dependent and restricting therapeutic oxygen levels has dramatically reduced the incidence of ROP in infants of birth weight >1000g. However, the incidence of ROP has remained high in very low birth weight (VLBW) infants and this appears to be related to these babies being more ill. Several risk factors have been identified in this group, however oxygen variability, rather than high levels, has been correlated with severity of disease in recent clinical and animal studies. Difficulties in defining ‘normal’ oxygen in this group has meant the optimal range of oxygen therapy has not yet been defined. Clinical studies are now underway using even lower oxygen therapy ranges. The impact this may have on ROP, neurological and respiratory outcomes will require further study.
