Many transgender and gender diverse (TGD) individuals will be considering gender-affirming treatments during their reproductive lifespan. These medically necessary treatments have a negative impact on reproductive potential. All TGD individuals should be counseled regarding fertility. Options for fertility preservation for individuals who have undergone puberty include mature oocyte, embryo, and sperm cryopreservation. In prepubertal individuals, ovarian tissue cryopreservation may be considered, but testicular tissue cryopreservation remains experimental only. While there have been advances in the technology and standardization of reproductive health care for this population, many gaps remain in our knowledge which require further research.

Children’s right to have a say in matters relevant to their lives is a universal legal entitlement but it is realized in settings that differ across cultures and places. Yet, studies that account for these diverse contexts are lacking. This study aimed to fill gaps in the literature by applying latent variable mixture modeling to cross-national data from the International Survey of Children’s Well-being (N= 23,733) to identify profiles of participation among children in 18 countries across four continents and to assess how these profiles compare in different contexts. Results show that in their views on participatory opportunities in key socio-developmental contexts (family, school, local community, country), children belong to one of four groups: Respected and Engaged, Family Supported, Respected but Disengaged and Disengaged. Comparative analyses reveal nuanced variations in the distribution of participation profiles among children in different societies. The article discusses these findings in the context of research and practical efforts aimed at understanding how children in modern societies view participatory opportunities that reflect their diverse roles across multiple social-relational contexts.

AbstractWe conducted a post hoc analysis of an antibiotic stewardship intervention implemented across our health system’s urgent-care network to determine whether there was a differential impact among patient groups. Respiratory urgent-care antibiotic prescribing decreased for all racial, ethnic, and preferred language groups, but disparities in antibiotic prescribing persisted.

A Hermitian metric on a complex manifold is said to be pluriclosed or SKT if the torsion of the associated Bismut connection is closed, and it is called balanced if its fundamental form is co-closed. In the paper we give an overview of recent results on pluriclosed and balanced metrics, provide new constructions of compact non-Kähler manifolds and also present a few open problems.

Low-cost, noble metal-free and environmentally friendly potassium glasses constitute a new class of promising catalysts for soot combustion. The effect of doping of potassium silicate glasses with aliovalent metals of prominent redox properties (Mn, Fe, Co, Ni, Cu, Ce) on their catalytic performance in soot combustion was investigated. A series of glasses were synthesized by the melt-quenching method and characterized in terms of composition, structure, and morphology by a plethora of techniques. The structural verification by XRD, FTIR, and RS confirmed the amorphous structure of all investigated samples. High hydroxylation and carboxylation of the surface of glassy materials were observed by DRIFT. The chemical state of metal additives in the doped samples was determined by XPS and UV/vis-DR as Mn2+/3+, Fe3+, Co2+/3+, Ni2+, Cu+/2+, and Ce3+/4+. The typical morphology of glasses obtained by the high-temperature method was observed in SEM images. Thermal stability tests by SR-TAD permitted us to order the dopants as follows: Cu > undoped > Co Mn > Ni > Fe Ce, corresponding to a decreasing stability of potassium in the investigated glasses. Catalytic activity was investigated under both tightand loose contactconditions, in an atmosphere of NO or without this additive. The investigated materials exhibited high activity in soot combustion (T50= 359–386 °C for tight contactand 442–500 °C for loose contact). The most promising dopants were Cu and Co, based on their activity and potassium stability. The low surface concentration of redox metals suggested that they influenced activity towards the modification of the glass structure rather than playing a role of active centers in the soot oxidation. Those dopants also influenced the formation and stability of potassium carbonates on the surface of the investigated glasses. A correlation between the observed thermal stability of surface potassium species and catalytic activity was established.

Purpose: To report a successful revascularization case using the Rotarex™S atherothrombectomy system in a recent iliac limb thrombosis, and chronic hypogastric stent obstruction after previous aortoiliac aneurysm endovascular repair (EVAR).Case Report: A 72-year-old patient was treated for recent right iliac limb thrombosis and left iliac branch chronic hypogastric stent occlusion, 5 years after EVAR. A total endovascular approach, using both upper extremity and femoral vascular access, was settled with 2 Rotarex™S (6Fr and 10Fr) devices. The Rotarex™S catheters removed most of the intraluminal material, allowing additional endografts and bare metal stents to be deployed to support a new healthy lumen surface.Conclusion: The total endovascular approach provided by the Rotarex™S device appears to be safe and effective in treating aortoiliac endografts occlusions, both in subacute and chronic phases. Larger studies could highlight differences and eventual advantages compared with more traditional solutions.

This article critically reviews the UN Strategy and Guidance on Hate Speech as examples of practice and uses them as a springboard to engage with the current legal framework. In doing so, it considers not only international human rights law but the related fields of international criminal and humanitarian law. Building upon the UN Strategy and Guidance, whilst also informed by their shortcomings, the article offers a definition and typology of ‘hate speech’ firmly rooted in the rigours of the rules of interpretation, as crystallised in the Vienna Convention on the Law of Treaties. The law-based approach ensures more transparency, certainty, and predictability of the law of free speech and its limitations. As such, it should also garner the support of more States, the primary duty-holders in international human rights law and lead to a more holistic approach to countering ‘hate speech’, thus achieving the aims of the UN Strategy.

Cancer is the second leading cause of death worldwide, with a dramatic impact due to the acquired resistance of cancers to used chemotherapeutic drugs and treatments. The enzyme lactate dehydrogenase (LDH-A) is responsible for cancer cell proliferation. Recently the development of selective LDH-A inhibitors as drugs for cancer treatment has been reported to be an efficient strategy aiming to decrease cancer cell proliferation and increase the sensitivity to traditional chemotherapeutics. This study aims to obtain a stable and active biocatalyst that can be utilized for such drug screening purposes. It is conceived by adopting human LDH-A enzyme (hLDH-A) and investigating different immobilization techniques on porous supports to achieve a stable and reproducible biosensor for anticancer drugs. The hLDH-A enzyme is covalently immobilized on mesoporous silica (MCM-41) functionalized with amino and aldehyde groups following two different methods. The mesoporous support is characterized by complementary techniques to evaluate the surface chemistry and the porous structure. Fluorescence microscopy analysis confirms the presence of the enzyme on the support surface. The tested immobilizations achieve yields of ≥80%, and the best retained activity of the enzyme is as high as 24.2%. The optimal pH and temperature of the best immobilized hLDH-A are pH 5 and 45 °C for the reduction of pyruvate into lactate, while those for the free enzyme are pH 8 and 45 °C. The stability test carried out at 45 °C on the immobilized enzyme shows a residual activity close to 40% for an extended time. The inhibition caused by NHI-2 is similar for free and immobilized hLDH-A, 48% and 47%, respectively. These findings are significant for those interested in immobilizing enzymes through covalent attachment on inorganic porous supports and pave the way to develop stable and active biocatalyst-based sensors for drug screenings that are useful to propose drug-based cancer treatments.

The Light Ion Detector for ALTEA (LIDAL) is a new instrument designed to measure flux, energy spectra and Time of Flight of ions in a space habitat. It was installed in the International Space Station (Columbus) on January 19, 2020 and it is still operating. This paper presents the results of LIDAL measurements in the first 17 months of operation (01/2020–05/2022). Particle flux, dose rate, Time of Flight and spectra are presented and studied in the three ISS orthogonal directions and in the different geomagnetic regions (high latitude, low latitude, and South Atlantic Anomaly, SAA). The results are consistent with previous measurements. Dose rates range between 1.8 nGy/s and 2.4 nGy/s, flux between 0.21 particles/(sr cm2s) and 0.32 particles/(sr cm2s) as measured across time and directions during the full orbit. These data offer insights concerning the radiation measurements in the ISS and demonstrate the capabilities of LIDAL as a unique tool for the measurement of space radiation in space habitats, also providing novel information relevant to assess radiation risks for astronauts.

Vibration is a widely used mode of haptic communication, as vibrotactile cues provide salient haptic notifications to users and are easily integrated into wearable or handheld devices. Fluidic textile-based devices offer an appealing platform for the incorporation of vibrotactile haptic feedback, as they can be integrated into clothing and other conforming and compliant wearables. Fluidically driven vibrotactile feedback has primarily relied on valves to regulate actuating frequencies in wearable devices. The mechanical bandwidth of such valves limits the range of frequencies that can be achieved, particularly in attempting to reach the higher frequencies realized with electromechanical vibration actuators ($>$100 Hz). In this paper, we introduce a soft vibrotactile wearable device constructed entirely of textiles and capable of rendering vibration frequencies between 183 and 233 Hz with amplitudes ranging from 23 to 114 g. We describe our methods of design and fabrication and the mechanism of vibration, which is realized by controlling inlet pressure and harnessing a mechanofluidic instability. Our design allows for controllable vibrotactile feedback that is comparable in frequency and greater in amplitude relative to state-of-the-art electromechanical actuators while offering the compliance and conformity of fully soft wearable devices.

Machine learning potentials have emerged as a means to enhance the accuracy of biomolecular simulations. However, their application is constrained by the significant computational cost arising from the vast number of parameters compared with traditional molecular mechanics. To tackle this issue, we introduce an optimized implementation of the hybrid method (NNP/MM), which combines a neural network potential (NNP) and molecular mechanics (MM). This approach models a portion of the system, such as a small molecule, using NNP while employing MM for the remaining system to boost efficiency. By conducting molecular dynamics (MD) simulations on various protein–ligand complexes and metadynamics (MTD) simulations on a ligand, we showcase the capabilities of our implementation of NNP/MM. It has enabled us to increase the simulation speed by ∼5 times and achieve a combined sampling of 1 μs for each complex, marking the longest simulations ever reported for this class of simulations.

Background: PICC-ports may be defined as totally implantable central venous devices inserted in the upper limb using the current state-of-the-art techniques of PICC insertion (ultrasound-guided venipuncture of deep veins of the arm, micro-puncture kits, proper location of the tip preferably by intracavitary ECG), with placement of the reservoir at the middle third of the arm. A previous report on breast cancer patients demonstrated the safety and efficacy of these devices, with a very low failure rate.Methods: This retrospective multicenter cohort study—developed by GAVeCeLT (the Italian Group of Long-Term Venous Access Devices)—investigated the outcomes of PICC-ports in a large cohort of unselected patients. The study included 4480 adult patients who underwent PICC-port insertion in five Italian centers, during a period of 60 months. The primary outcome was device failure, defined as any serious adverse event (SAE) requiring removal. The secondary outcome was the incidence of temporary adverse events (TAE) not requiring removal.Results: The median follow-up was 15.5 months. Device failure occurred in 52 cases (1.2%), the main causes being local infection (n= 7; 0.16%) and CRBSI (n= 19; 0.42%). Symptomatic catheter-related thrombosis occurred in 93 cases (2.1%), but removal was required only in one case (0.02%). Early/immediate and late TAE occurred in 904 cases (20.2%) and in 176 cases (3.9%), respectively.Conclusions: PICC-ports are safe venous access devices that should be considered as an alternative option to traditional arm-ports and chest-ports when planning chemotherapy or other long-term intermittent intravenous treatments.

The Anomalous Long Term Effects in Astronauts (ALTEA) project originally aimed at disentangling the mechanisms behind astronauts’ perception of light flashes. To this end, an experimental apparatus was set up in order to concurrently measure the tracks of cosmic radiation particles in the astronauts’ head and the electroencephalographic (EEG) signals generated by their brain. So far, the ALTEA data set has never been analyzed with the broader intent to study possible interference between cosmic radiation and the brain, regardless of light flashes. The aim of this work is to define a pipeline to systematically pre-process the ALTEA EEG data. Compared to the analysis of standard EEG recording, this task is made more difficult by the presence of unconventional artifacts due to the extreme recording conditions that, in particular, require the EEG cap to be positioned next to another noisy electronic device, namely the particle detectors. Here we show how standard tools for the analysis of EEG data can be tuned to deal with these unconventional artifacts. After pre-processing the available data we were able to elucidate a shift of the center frequency of the αrhythm induced by visual stimulation, thus proving the effectiveness of the implemented pipeline. This work represents the first study presenting results of signal processing of ALTEA EEG time series. Further, it is the starting point of a future work aimed at analyzing the interaction between EEG and cosmic radiation.

ABSTRACTThis article shares collaborative scientific work done by researchers from the Centre for Social Studies (University of Coimbra, Portugal) and from the Technology, Restoration and Arts Enhancement Center (Polytechnical Institute of Tomar, Portugal) within the REDE ARTÉRIA in cooperation with MOVTOUR. REDE ARTÉRIA is an action research project that aims to create cultural presentations and performances to tour the Centro Region of Portugal. It aspires to encourage a greater professionalization of agents, while aiming to enrich and enhance the cultural offering. Simultaneously, it intends to stimulate a community practice of participatory cultural mapping influencing creation processes and cultural policies in this region. The analysis is made in the specific context of the artistic creation VAGAR, conceived by the Portuguese choreographer Marina Nabais and co-produced by the Municipality of Ourém (Portugal). The analysis results from participatory workshops with local agents, field notes from rehearsal observation, interviews with artists, local participants, local government, and from the final report produced by the dance association responsible for the performance. This work critically reflects on how processes of community participation and of tangible and intangible resource mapping – by providing dialogue opportunities between artists, local agents, municipalities and the local cultural fabric – can develop innovative projects of artistic intervention that link heritage to local, social and cultural civic dynamics. It thus epitomizes the importance of the social dimension of cultural activity.

Nowadays, phosphorus natural reserves are being depleted, while P fertilizers demand is increasing. Phosphorus is well contained in waste materials such as sewage sludge. Only a small amount (1–3%) of the soil total phosphorus is bioavailable for plant nutrition. More in detail, the present study focuses on the determination of the kinetics of bioavailable phosphorus concentrations in a sandy calcareous soil after the application of sewage sludge. A centrifuged (C) and dried (D) anaerobic digestate from sewage sludges obtained from the same wastewater treatment plan were separately tested to fertilize a calcareous sandy soil. Falcon tubes (50 mL) containing negative control (T) and soil treated with C and D were incubated from 1 to 90 days. Soil phosphorus fractionation was performed with the SMT method and bioavailable-P was extracted through the Olsen method. Phosphorus was spectrophotometrically quantified by the molybdovanadate method. Lastly, kinetics of bioavailable-P on soils were evaluated using four kinetic models. Phosphorus fractions were constant throughout the experiment. Conversely, the bioavailable-P significantly decreased from day 1 to day 90 in C treatment (from 34.9 ± 2.9 to 23.8 ± 1.5 ppm) and T treatment (from 4.2 ± 1.2 to 0.3 ± 0.6 ppm). This decrease might be due to the precipitation of P with calcium; in fact, high concentration of Ca2+ions and the alkaline soil pH can induce the sequential formation of calcium phosphates, even less soluble over time. Whereas D treatment showed a peak of bioavailable-P concentration on day 14 (26.6 ± 3.0). This trend could be due to organic carbon compounds, competitive sorption and metal bridging. The fitting of experimental data revealed that the Elovich model best described the adsorptive-precipitate process of bioavailable-P in T (r2= 0.90) and C (r2= 0.93). Conversely, none of the models satisfactorily described the behavior of bioavailable-P in D samples. This study increases the knowledge on P-related phenomena for designing and optimizing fertilizers and reducing their drawbacks such as eutrophication.

In the present work, an attempt has been made to do a combined hot isostatic pressing (HIP) & solution treatment in a single-step and the effect of different cooling rates (quenching and annealing) on the microstructure was examined. For a comparison, solution treatment (without HIP) was also analysed. It was observed that HIP treatment was successful in reducing the macro-porosities of the L-PBF part. Although, it was not an efficient treatment for the dissolution of the θ-Al2Cu phase. Furthermore, in both HIP-Quenched and HIP-Annealed treatments a sign of incipient melting was observed along the grain boundaries. Solution treatment without HIP did not show incipient melting.

Natural polyamines (PAs) are key players in cellular homeostasis by regulating cell growth and proliferation. Several observations highlight that PAs are also implicated in pathways regulating cell death. Indeed, the PA accumulation cytotoxic effect, maximized with the use of bovine serum amine oxidase (BSAO) enzyme, represents a valuable strategy against tumor progression. In the present study, along with the design, synthesis, and biological evaluation of a series of new spermine (Spm) analogues (1–23), a mixed structure-based (SB) and ligand-based (LB) protocol was applied. Binding modes of BSAO-PA modeled complexes led to clarify electrostatic and steric features likely affecting the BSAO-PA biochemical kinetics. LB and SB three-dimensional quantitative structure–activity relationship (Py-CoMFA and Py-ComBinE) models were developed by means of the 3d-qsar.comportal, and their analysis represents a strong basis for future design and synthesis of PA BSAO substrates for potential application in oxidative stress-induced chemotherapy.

In this paper, we consider the Cauchy problem in RN, N≥1, for semi-linear Schrödinger equations with space–time fractional derivatives. We discuss the nonexistence of global L1or L2weak solutions in the subcritical and critical cases under some conditions on the initial data and the nonlinear term. Furthermore, the nonexistence of local L1or L2weak solutions in the supercritical case are studied.

Traditionally, anaerobic digestion (AD) is applied for the simultaneous generation of energy and treatment of various organic wastes. For example, despite orange peels (OP) and seaweed (SW) are highly available for AD, their physicochemical composition can inhibit the microbiome and collapse the process. In the present work, we examined the combination of OP and SW with livestock manure (MN) at a co-digestion process. At first, batch toxicity tests revealed that D-limonene and gallic acid at 0.2% v/v and 2.28 g L−1respectively solely inhibit methane generation. At batch assays and based on added volatile solids (VS), the mono-digestion of OP and SW had methane yields of 398.4 ± 9.4 and 348.6 ± 10.2 m3tVS−1, respectively. Furthermore, continuous mode experiments were conducted in order to examined process performance and bioenergy outcome at the co-digestion of OP, SW, and MN at various organic loading rates and substrate contributions. This work revealed two successful operation strategies when OP and SW were co-digested with manure resulting in methane yields of 264.5 ± 5.2 and 220.5 ± 5.2 m3tVS−1, respectively. The results of this work potentially could be applied when dealing with other types of algal biomass.

Neurosarcoidosis is an uncommon and multiform clinical entity. Its presentation as an isolated longitudinal extensive transverse myelitis (LETM) is rare and challenging to identify. We report a case of LETM in a 60-year-old patient with no significant systemic symptoms nor relevant medical history. The peculiar spinal magnetic resonance imaging finding characterized by a posterior and central canal subpial contrast enhancement, the so-called “trident sign,” together with chest computed tomography scan and lymph node biopsy led to the diagnosis of sarcoidosis. We also discuss the main differential diagnoses of LETM and therapeutic options for sarcoidosis-related myelitis.

In the current study, sugarcane bagasse-derived biochar catalysts were synthesized by doping with metal-free heteroatoms including nitrogen, phosphorus, oxygen, and boron and their efficacy was tested for catalytic co-pyrolysis of walnut shells (WS) and palm oil fatty acid distillate (PFAD) using Py-GC/MS. The biochar catalysts’ properties and their effectiveness on co-pyrolytic bio-oil (overall product distribution, selectivity towards aliphatic and aromatic hydrocarbons, and carbon number ranges) were extensively examined. Among the various catalysts considered, boron-doped biochar (BCB) demonstrated effective performance due to an improved surface area, porosity, and acidity and resulted in a significantly increased hydrocarbon yield, particularly in the gasoline and diesel range, and preference for aromatic over aliphatic hydrocarbons. The optimum bio-oil quality was achieved at a feedstock-to-catalyst ratio of 2:1 and a pyrolysis temperature of 750 °C, with a hydrocarbon yield of 90.8 % and aromatic hydrocarbon selectivity of 63.5 %. Overall, the study underscores the role of BCB in fostering deoxygenation, decarboxylation, and aromatic selectivity via thermal and catalytic pathways, highlighting the potential of non-metallic doped biochars for bio-oil upgrading.

Aim of this paper is to describe a simple and helpful technique for challenging visceral vessel catheterization during complex aortic endovascular procedures.

Introduction: Several studies report that medical students are at high risk of depression. Despite the variability in students’ vulnerability to depression, the role of individual differences in depression risk among medical students has hardly been investigated. Studies outside of medical student populations have shown that individual differences in attachment style and emotion regulation participate in vulnerability to depression. Objectives: This study investigates to what extent medical students’ depressive symptoms are related to differences in students’ insecure attachment styles and their perception of reduced access to emotion regulation strategies. Methods: In a cross-sectional quantitative study, undergraduate medical students at the beginning of their second academic year completed online questionnaires measuring their attachment style, difficulties in emotion regulation, and depressive symptoms. Results: Out of the 390 medical students invited, 267 participated in the survey. Higher secure attachment was associated with fewer depressive symptoms. Medical students’ insecure attachment style and emotion dysregulation were significantly related to depressive symptoms. Difficulties in employing strategies to disengage from one’s own negative affect partly mediated the effects of two dimensions of insecure anxious attachment—need for approval and preoccupation with relationships—on depressive symptoms. Discussion: Anxious attachment style and limited access to emotion regulation strategies participate in medical students’ depressive symptoms. These findings highlight the need for acknowledging medical students’ attachment style and students’ perceived access to emotion regulation strategies for the early identification of and intervention programs for the risk of depression.

Purpose: We performed a survey among European semen banks enquiring safety protocols during the COVID-19 pandemic. We report the experience from a center searching SARS-CoV-2 mRNA in semen of patients undergoing cryopreservation from May 2020 to January 2021. Methods: A questionnaire was submitted to accredited semen banks of the European Academy of Andrology (EAA) and the Italian Society of Andrology and Sexual Medicine (SIAMS). A total of 22 centers answered to the survey. SARS-CoV-2 mRNA in semen was evaluated by RT-PCR in 111 subjects banking in the Semen Bank of Careggi University Hospital (Florence, Italy). Results: No particularly drastic safety measures were adopted by the majority of the centers to prevent the risk of contamination or transmission of the virus. The most common strategy (77.3%) was the administration of an anamnestic questionnaire. About half of the centers request a negative nasopharyngeal swab (NPS) before cryopreservation. Few centers use a quarantine tank, in case of late response of NPS, and only 4 store in a dedicated tank in case of infection. SARS-CoV-2 mRNA was not found in 111 semen samples cryopreserved in the Florentine bank. Conclusions: European semen banks use different measures to handle semen samples for cryopreservation during COVID-19 pandemic. The request of NPS is advised to better manage couples undergoing ART and to protect the personnel operating in the bank/ART center. Finally, due to the areas of uncertainties of an almost unknown virus, it is absolutely recommended the use of safe devices for sample handling and storage.

Conformal cooling coils have been developed during the last decades through the use of additive manufacturing (AM) technologies. The main goal of this study was to analyze how the presence of an internal channel that could act as a conformal cooling coil could affect compressive strength and quasi-elastic gradient of AlSi10Mg lattice structures produced by laser powder bed fusion (LPBF). Three different configurations of samples were tested in compression at 25 °C and 200 °C. The reference structures were body centered cubic (BBC) in the core of the samples with vertical struts along Z(BCCZ) lattices in the outer perimeter, labelled as NC samples. The main novelty consisted in inserting a straight elliptical channel and a 45° elliptical channel inside the BCCZ lattice structures, labelled as SC and 45C samples respectively. All the samples were then tested in as-built (AB) condition, and after two post process heat treatments, commonly used for AlSi10Mg LPBF industrial components, a stress relieving (SR) and a T6 treatment. NC lattice structures AB exhibited an overall fragile fracture and therefore the SC and 45C configuration samples were tested only after thermal treatments. The test at 25 °C showed that all types of samples were characterized by negligible variations in their quasi-elastic gradients and yield strength. On the contrary, the general trend of stress-strain curves was influenced by the presence of the channel and its position. The test at 200 °C showed that NC, SC and 45C samples after SR and T6 treatments exhibited a metal-foam like deformation.

ABSTRACTCooper Hewitt, Smithsonian Design Museum is a leader in the field of collecting and displaying contemporary design across a wide range of media, including born-digital and interactive works, 3D computer-aided designed and printed objects, as well as more traditional materials. As digital acquisitions have increased, an interdisciplinary group was formed in order to best guide this expanding area of collecting. This group worked collaboratively to steer the recent Digital Collections Materials Project, a critical review to develop enhanced protocols for preserving extant digital collections. This paper reports on the results of its collections survey and case studies, demonstrating how theories of contemporary art and time-based media conservation can be applied to the unique needs of a design institution. Concerns circling obsolescence and scarcity – referring both to expert knowledge and replacement materials – are examined with regard to mass-produced consumer electronics and newer born-digital works. In order to unpack these questions in a novel way, the term “riskscape,” borrowed from anthropologist Arjun Appadurai and co-opted by environmental and social disaster studies scholars, is introduced as a tool to better reflect the interconnected nature of commercially produced objects and the processes by which they become institutionalized.

Current guidelines indicate iliac-branched endografts (IBDs) as the ideal means to preserve pelvic perfusion during endovascular aortic repair. Because patient vascular anatomy represents the main limitation to extensive use of these devices, off-label application may be considered to expand the number of patients being treated. The aim of this study is to evaluate long-term outcomes obtained using the Cook ZBIS endograft in the treatment of aortoiliac aneurysms as per or outside manufacturer's instruction for use (IFU).

Rhodospirillum rubrumis a gram-negative bacterium that naturally takes advantage of CO and which, in the presence of acetate, accumulates carbon and energy units as polyhydroxybutyrate (PHB). Since the conversion of CO depends on a large protein membrane complex that is expressed after the exposure to carbon monoxide, this study presents the effects of a CO-based acclimation in R. rubrumon the growth trend and on the production of PHB. The strain was cultured in two consecutive fermentation cycles on 15% of CO, and the behaviour of this species, in the presence of acetate or a reducing sugar, such as fructose, was compared. The exposure of R. rubrumto CO during the first adaptation phase led to the development of a metabolically active population characterised by a greater biomass growth. The supply of fructose ensured a shorter lag-phase and a higher biomass titre, but it also determined a decrease in the biopolymer accumulation. However, R. rubrumshowed the best carbon utilisation in the absence of fructose, with a growth molar yield of 48 mg mol−1, compared to the 12 mg mol−1obtained for fructose feeding.

Rice is one of the most important crops throughout the world, as it contributes toward satisfying the food demand of much of the global population. It is well known that rice production generates a considerable number of by-products, among which rice bran deserves particular attention. This by-product is exceptionally rich in nutrients, since it contains a wide spectrum of macronutrients (proteins, fats, carbohydrates) as well as dietary fibers and bioactive compounds. However, rice bran is usually wasted or just used for the production of low-cost products. The lipidic fraction of rice bran contains an unsaponifiable fraction that is rich in such functional components as tocopherols, γ-oryzanol, tocotrienols, and phytosterols. This lipidic fraction can be extracted to obtain rice bran oil (RBO), a high value-added product with unique health properties as a result of its high concentration in γ-oryzanol, a powerful antioxidant mixture of bioactive molecules. Conventional extraction methods employ hexane as the solvent, but these methods suffer from some drawbacks linked to the toxicity of hexane for humans and the environment. The aim of the review presented herein is to point out the new green technologies currently applied for the extraction of RBO, by highlighting reliable alternatives to conventional solvent extraction methods that are in line with the twelve principles of green chemistry and a circular economy.

While walking humans generally plan foot placement two steps in advance. However, it is often necessary to rapidly alter foot placement position just before stepping due to the appearance of a new obstacle. While humans are quite capable of rapidly altering foot placement position, such changes can have major effects on centre of mass dynamics. We investigated how rapid changes to planned foot placement impact centre of mass dynamics, and how such changes influence the control of balance and forward progress, during both straight- and turning-gait. Thirteen young adults walked along a virtually projected walkway with precision footholds oriented either in a straight line or with a single 60°, 90° or 120° turn. On a subset of trials, participants were required to rapidly avoid stepping on select footholds. We found that if the centre of mass was disrupted such that it interfered with task success (i.e. staying upright and continuing along the planned path), walkers were more likely to sacrifice forward progress than the upright stability. Further, walkers appear to control centre of mass dynamics differently following inhibited steps during step turns than during spin turns, which may reflect a larger threat to task success when spin turns are interrupted.

Poly(3-hydroxybutyrate) (PHB) is one of the most well-known biodegradable and biocompatible biopolymers produced by prokaryotic microorganisms. It belongs to the family of polyhydroxyalkanoates (PHAs), and it has gained significant attention in recent years due to its potential as a sustainable alternative to traditional petroleum-based plastics. Cupriavidus necatorhas been identified as a potential producer of PHB for industrial applications due to its ability to produce high amounts of the polymer under controlled conditions, using a wide range of waste substrates. In this study, the ability of Cupriavidus necatorDSM 545 strain to produce PHB was tested in a fed-batch strategy providing two different organic substrates. The first is a sugar-based syrup (SBS), derived from cereal waste. The second is an acetate-rich medium obtained through CO2-H2fermentation by the acetogenic bacterium Acetobacterium woodii. The carbon sources were tested to improve the accumulation of PHB in the strain. C. necatorDSM 545 proved to be able to grow and to perform high accumulation of biopolymer on waste substrates containing glucose, fructose, and acetate, reaching about 10 g/L of PHB, 83% of biopolymer accumulation in cell dry mass, in 48 h of fed-batch fermentation in 0.6 L working volume in a bioreactor. Moreover, a Life Cycle Assessment analysis was performed to evaluate the environmental impact of the process converting the sugar syrup alone and the integrated one. It demonstrated that the integrated process is more sustainable and that the most impactful step is the PHB production, followed by the polymer extraction.

Objectives: Falls in hospitals pose a significant safety risk, leading to injuries, prolonged hospitalization, and lasting complications. This study explores the potential of augmented reality (AR) technology in healthcare facility design to mitigate fall risk.Background: Few studies have investigated the impact of hospital room layouts on falls due to the high cost of building physical prototypes. This study introduces an innovative approach using AR technology to advance methods for healthcare facility design efficiently.Methods: Ten healthy participants enrolled in this study to examine different hospital room designs in AR. Factors of interest included room configuration, door type, exit side of the bed, toilet placement, and the presence of IV equipment. AR trackers captured trajectories of the body as participants navigated through these AR hospital layouts, providing insights into user behavior and preferences.Results: Door type influenced the degree of backward and sideways movement, with the presence of an IV pole intensifying the interaction between door and room type, leading to increased sideways and backward motion. Participants displayed varying patterns of backward and sideways travel depending on the specific room configurations they encountered.Conclusions: AR can be an efficient and cost-effective method to modify room configurations to identify important design factors before conducting physical testing. The results of this study provide valuable insights into the effect of environmental factors on movement patterns in simulated hospital rooms. These results highlight the importance of considering environmental factors, such as the type of door and bathroom location, when designing healthcare facilities.

Introduction: Women are generally underrepresented in trials focusing on aortic aneurysm. Nevertheless, sex-related differences have recently emerged from several studies and registries. The aim of this research was to assess whether sex-related anatomical disparities existed in fenestrated and branched aortic repair candidates and whether these discrepancies could influence endovascular repair outcomes.Methods: Data from all consecutive patients treated during the 2008–2019 period within the Italian Multicenter fenestrated or branched endovascular aortic repair (F/BEVAR) Registry were included in the present study. Propensity matching was performed using a logistic regression model adjusted for demographic data and comorbidities to obtain comparable male and female samples. The selection model led to a final study population of 176 patients (88 women and 88 men) among the total initial cohort of 596. Study endpoints were technical and clinical success, overall survival, aneurysm-related death, and reintervention rates evaluated at 30 days and during follow-up.Results: Twenty-eight patients (15.9%) received urgent/emergent repair. In most of the cases (71.6%), women received treatment for extensive thoracoabdominal pathology (Crawford type I, II, or III aneurysm rather than type IV or juxta-pararenal) versus 46.6% of men (p=0.001). Female patients presented with more challenging iliac accesses with at least one side considered hostile in 27.3% of the cases (vs 13.6% in male patients, p=0.039). Finally, women had significantly smaller visceral vessels. Women had significantly worse operative outcomes, with an 86.2% technical success rate versus 96.6% in the male population (p=0.016). No differences were recorded in terms of 30-day reinterventions between men and women. The 5-year estimate of freedom from late reintervention, according to Kaplan-Meier analysis, was 85.6% in men versus 81.6% in women (p=ns). No aneurysm-related death was recorded during follow-up (median observational time, 23 months [interquartile range, 7–45 months]).Conclusion: Women presented a significantly higher incidence of thoracoabdominal aneurysms, smaller visceral vessels, and more complex iliofemoral accesses, resulting in a significantly lower technical success after F/BEVAR. Further studies assessing sex-related differences are needed to properly determine the impact on outcomes and stratify procedural risks.Clinical Impact Women are generally underrepresented in trials focusing on aortic aneurysms. Aiming to assess whether sex may affect outcomes after a complex endovascular aortic repair, a propensity score selection was applied to a total population of 596 patients receiving F/BEVAR aortic repair with the Cook platform, matching each treated female patient with a corresponding male patient. Women presented more frequently a thoracoabdominal aneurysm extent, smaller visceral vessels, and complex iliofemoral accesses, resulting in significantly worse operative outcomes, with an 86.2% technical success versus 96.6% (p=0.016). No differences were recorded in terms of short-term and mid-term reinterventions. According to these results, careful and critical assessment should be posed in case of female patients receiving complex aortic repair, especially regarding preoperative anatomical evaluation and clinical selection with appropriate surgical risk stratification.

Creatinine and cystatin-C are recommended for estimating glomerular filtration rate (eGFR) but accuracy is suboptimal. Here, using untargeted metabolomics data, we sought to identify candidate filtration markers for a new targeted assay using a novel approach based on their maximal joint association with measured GFR (mGFR) and with flexibility to consider their biological properties. We analyzed metabolites measured in seven diverse studies encompasing 2,851 participants on the Metabolon H4 platform that had Pearson correlations with log mGFR and used a stepwise approach to develop models to < —0.5 estimate mGFR with and without inclusion of creatinine that enabled selection of candidate markers. In total, 456 identified metabolites were present in all studies, and 36 had correlations with mGFR < —0.5. A total of 2,225 models were developed that included these metabolites; all with lower root mean square errors and smaller coefficients for demographic variables compared to estimates using untargeted creatinine. Seventeen metabolites were chosen, including 12 new candidate filtration markers. The selected metabolites had strong associations with mGFR and little dependence on demographic factors. Candidate metabolites were identified with maximal joint association with mGFR and minimal dependence on demographic variables across many varied clinical settings. These metabolites are excreted in urine and represent diverse metabolic pathways and tubular handling. Thus, our data can be used to select metabolites for a multi-analyte eGFR determination assay using mass spectrometry that potentially offers better accuracy and is less prone to non-GFR determinants than the current eGFR biomarkers.

AbstractBackground and objectives:Antibiotic allergy labels are common and are frequently inaccurate. Previous studies among adults demonstrate that β-lactam allergy labels may lead to adverse outcomes, including prescription of broader-spectrum antibiotics, increased costs, and increased lengths of stay, among others. However, data among pediatric patients are lacking, especially in the United States. In this study, we sought to determine the impact of β-lactam allergy labels in hospitalized children with regards to clinical and economic outcomes.Method:This retrospective cohort study included pediatric patients 30 days to 17 years old, hospitalized at Intermountain Healthcare facilities from 2007 to 2017, who received ≥1 dose of an antibiotic during their admission. Patients with β-lactam allergies were matched to nonallergic patients based on age, sex, clinical service line, admission date, academic children’s hospital or other hospital admission, and the presence of chronic, comorbid conditions. Outcomes included receipt of broader-spectrum antibiotics, clinical outcomes including length of stay and readmission, and antibiotic and hospitalization costs.Results:In total, 38,906 patients were identified. The prevalence of antibiotic allergy increased from 0.9% among those < 1 year peaked at 10.6% by age 17. Patients with β-lactam allergy received broader-spectrum antibiotics and experienced higher antibiotic costs than nonallergic controls. However, there were no differences in the length of stay, readmission rates, or total number of days of antibiotics between allergic and nonallergic patients.Conclusions:Hospitalized pediatric patients with β-lactam allergy labels receive broader-spectrum antibiotics and experience increased antibiotic costs. This represents an important opportunity for allergy delabeling and antibiotic stewardship.

Ti-48Al-2Cr-2Nb (Ti-48-2-2) is an intermetallic alloy belonging to a family of gamma-TiAl intermetallic alloys that are attracting significant attention. Electron Beam Melting (EBM) process is today the only manufacturing process that allows effective production of parts made by these kinds of alloys. Proper process control avoids high temperatures in the surrounding areas that may generate significant residual stresses that could cause micro-cracks. In this paper, an investigation on the residual stress state on Ti-48-2-2 parts is carried out using the hole drilling method. In particular, the influence of EBM process parameters is evaluated in order to understand the effects of the residual stresses on part integrity.

To describe a novel technique developed for the treatment of patients with thoracoabdominal aortic aneurysms having narrow aortic lumens using branched endografts.

Introduction: Long-term use of bisphosphonates (BPs) has been associated with a specific type of tensile side femoral stress fracture known as Atypical Femoral Fracture (AFF). Theoretically periprosthetic femoral fractures (PFF) should be excluded from the diagnosis of AFF. However, emerging evidence correlates prolonged BPs use with the occurrence of a type of PFF with an atypical pattern (atypical PFF, APFF). The aim of the present study is to report 3 cases of APFF treated at a single centre.Methods: Clinical and radiographic records of PFF that occurred between January 2016 and August 2018 were retrospectively reviewed. All patients meeting the American Society for Bone and Mineral Research (ASBMR) criteria for definition of PFF were included. Management strategies for APFF and patient outcomes, including fracture healing and hip function (assessed by the Oxford Hip Score [OHS]) were collected.Results: 3 patients in the study period were identified as APFF (1 incomplete, 2 complete). All patients were females with a mean age of 83.3?years. All patients were treated with lateral plating. The application of a contralateral strut allograft resulted in fracture healing in cases of complete fractures. Mean OHS at final follow-up was 34.3.Conclusions: Despite occurring around a hip stem like PFF, APFF had peculiar clinical and radiographic features, making them more similar to AFF. Therefore, the orthopaedic surgeon should also consider the natural history and healing problems associated with AFF prior in order to choose the most appropriate management for APFF.

Recently, we have introduced a new bioeconomic indicator in order to avoid the difficulties in evaluating the process and technologies for sustainability. In this paper, we wish to improve this new indicator for the analysis of sustainability. Indeed, the indicator has been based on the exergy analysis of dissipation and irreversibility, and it was proven in some social and technical application. In this work, a more general definition has been introduced in order to use it in any evaluation of sustainability. In particular, it has been applied to improve the biofuel production obtained by microorganisms, starting from the biophysical behaviour of the microorganisms themselves. Indeed, in industrialised countries, the management of CO2emissions represents one of the present compelling issues. In this context, the improvement of the energy efficiency, and its rational use, can be considered a fundamental economic strategy for the sustainable development of the industrialised countries. Our indicator takes into account all these requests for the development and sustainability, resulting a very interesting thermoeconomic quantity to be used by decision makers. Moreover, it is used to prove that mutualism can represent a new approach for the optimisation of biofuels production.

By means of the method of test functions, the nonexistence of nontrivial global weak solutions is demonstrated. Simultaneously, the nonexistence of nontrivial weak solutions for the corresponding elliptic case is established.
