OBJECTIVE: We investigated the potential concordance of 2 different magnetic resonance (MR) sequences — short-tau inversion recovery (STIR) and fat-saturated T1-weighted spin-echo after application of gadolinium (Gd) contrast medium to detect active bone marrow abnormalities at the sacroiliac joints (SIJ) in patients with spondyloarthritis (SpA). METHODS: Blinded and using the Danish scoring method, we evaluated transaxial MR images of the 2 sequences in 40 patients with SpA with disease duration of 3–14 years. Both the cartilaginous and ligamentous portions of the SIJ were analyzed. RESULTS: There was a significant positive correlation between the activity scores obtained by STIR and Gd-enhanced sequences (p < 0.0001). Agreement in the detection of bone marrow abnormalities occurred in 60 of the 80 joints, 35 with and 25 without signs of active disease. Discordance with STIR-positive marrow activity scores occurred in only 11 joints; Gd-enhanced positive scores in 9 joints. The STIR sequence detected remnants of marrow activity in the periphery of chronic fatty replacement not seen or partly obscured on the Gd sequence. Small subchondral enhancing lesions may not be scored on the STIR sequence, mostly because of reduced image resolution. CONCLUSION: Active bone marrow abnormalities were detected nearly equally well with STIR and Gd-enhanced fat-suppressed T1 sequences in patients with SpA, with STIR being most sensitive to visualize active abnormalities in the periphery of chronic changes.

OBJECTIVE: To evaluate the association between occupational kneeling and degenerative meniscal tears. METHODS: Magnetic resonance imaging (MRI) of both knees was conducted in 92 male floor layers and 49 male graphic designers (referents), with a mean age of 55.6 years (range 42–70 yrs). The presence of grade 3 MRI signal intensities indicating degenerative tears of the anterior, middle, and posterior one-third of the lateral and medial menisci was assessed on 1.5-Tesla MRI scans. The odds ratio (OR) of meniscal tears was determined among floor layers compared to graphic designers. Using logistic regression, models were adjusted for age, body mass index, and knee-straining sports. RESULTS: Degenerative tears were significantly more prevalent in the medial meniscus among floor layers than among graphic designers [OR 2.28, 95% confidence interval (CI) 1.10–4.98] and significantly more floor layers had medial tears in both knees (OR 3.46, 95% CI 1.41–8.48). Tears extending to the tibial aspect and localized in the middle and posterior one-third of the medial meniscus were most prevalent. Lateral meniscal tears were predominantly unilateral and the prevalence of lateral tears did not differ between the 2 study groups. Knee complaints occurred in about 50% of all floor layers, irrespective of the presence of meniscal tears. CONCLUSION: Occupational kneeling increases the risk of degenerative tears in the medial but not the lateral menisci in both knees.

Eleven shoulders in 10 patients with rheumatoid arthritis were examined by conventional radiography and CT prior to cup hemiarthroplasty of the humeral head and the results were compared with the surgical findings. There was good agreement between preoperative CT and surgical findings. Humeral head cavities and erosions, with cortical boundaries, could be seen more accurately at CT than at conventional radiography. The HU of their contents corresponded to those of soft tissue, being granulomatous in nature at surgery. In 8 humeral heads CT disclosed large areas of fatty degeneration of bone marrow with HU between - 10 HU and -76 HU that were not visible on the conventional radiographs. These “fatty cysts” had no cortical boundaries, unlike inflammatory granulomas, but both lesions may influence the surgical approach to hemiarthroplasty.

In 34 femoral neck fractures, CT was performed within 1-32 days after internal fixation. All the cases except one showed an increased distance between the femoral neck and the anterior aspect of the joint capsule as compared with the intact side, indicating varying degrees of hip joint effusion and/or synovitis. Hip joint aspiration in 11 patients revealed increased intracapsular pressure varying between 10 and 112 mmHg and volumes of aspirated joint effusion up to 23 ml. Pain relief and increased joint motion after drainage of the intracapsular effusion was observed in 3 patients whose postoperative mobilization was facilitated.

After 2 to 4 years, 25 patients (27 feet) who had a proximal closing wedge osteotomy of the first metatarsal and an adductor tenotomy were reviewed. A total of 20 patients (22 of 27 feet) were completely satisfied; 5 patients not completely satisfied had metatarsalgia because of dorsal displacement of the first metatarsal head. Radiographic measurements showed a narrowing of the forefoot rather than a large change in the intermetatarsal angle. The recovery period was long, an average of 11 weeks. Pin inflammation occurred in 5 patients and incisional neuromas in 2 patients. The risk of these complications must be taken into consideration when using this operation.

Twenty-one children with transient synovitis of the hip were treated in two consecutive groups. In 12 patients aspiration of the synovial effusion was performed within 24 h of admission. In a second group of nine patients no aspiration was performed. There was no other difference in treatment. Joint effusion was studied sonographically in comparison with the nonsymptomatic contralateral hip. Sonography was performed immediately after admission, within 12 h after aspiration, and repeatedly during a follow-up period of up to 15 days. Following aspiration the capsular distention decreased to 49% but recurred within 24 h to 72% of the preoperative value. However, during the first 4 days of follow-up the capsular distention in the aspirated cases was significantly and permanently lower than in the nonaspirated cases. We conclude that arthrocentesis in the acute stage of transient synovitis of the hip in the child permanently reduces the intracapsular effusion.

To assess the ability of conventional radiography to identify subsidence of tibial components

Manual tests and 2 external devices were used together with roentgen stereophotogrammetry (RSA) and an active weight-bearing radiographic method to measure the sagittal laxity in 11 knees with anterior-cruciate-ligament rupture. In 5 knees no ligament surgery had been performed (unstable knees) and in 6 knees a reconstruction had been performed one year before the examination (stable knees). There were positive correlations between all methods, including the manual tests when all knees, both stable and unstable, were analyzed together. However, the mean values of the total displacement differed between the methods, especially when comparing the weight-bearing radiographs with the three other methods. Some knees with substantial displacement during passive loading did not show any displacement when weight bearing; the measurements thus depended on both the ligamentous laxity and the patient's neuromuscular control of the joint. When the stable knees were analyzed separately, higher mean values were recorded with the external devices than with RSA using 180 N load. This could be explained by an error from soft tissue deformation which added to the skeletal displacement when the external devices were used.

Hemiarthroplasty of the humeral head, using a stainless steel cup, was performed in 26 shoulders of patients with rheumatoid arthritis who had severe pain and loss of function. All the shoulders were Larsen's radiographic Grade 4 or 5. After 2 (1-5) years, all the shoulders were painless and had satisfactory function. Partial radiolucent zones exceeding 1 mm were seen in three shoulders.

Nineteen consecutive children with transient synovitis of the hip were examined for intracapsular joint effusion using computed tomography (CT) and ultrasonography (US). The two techniques were highly correlated in measurements of the anterior displacement of the joint capsule, indicating both to be reliable diagnostic tools. Such an effusion is clinically important because of the increase in intracapsular pressure with ensuing disturbance in the vascular supply to the proximal femoral epiphysis, as demonstrated experimentally and clinically in transient synovitis.

Two elderly patients had pain after hip trauma with no radiographic evidence of fracture. Computed tomography demonstrated capsular haema-toma. Scintimetry revealed femoral head ischaemia. Intracapsular hip joint pressure in extension was 240 and 176 mm Hg, respectively, in neutral position and 280 and 360 mm Hg in internal rotation. The hip joints were aspirated for 8 ml and 5 ml of blood, respectively, leading to pain relief and regained radiotracer uptake in the femoral head. It is concluded that traumatic hip joint tamponade may cause femoral head ischaemia which may be reversed by aspiration.

Abstract:  Objective. To review our experience of chronic recurrent multifocal osteomyelitis (CRMO) and to assess the value of MRI in this rare disease, which mainly affects children and adolescents. Design and patients. Seventeen patients from our departments were reviewed. All underwent conventional radiography and MRI, and most had bone scintigraphy. All had undergone bone biopsy, with microbiological and histopathological examinations, to exclude infectious disease, tumours and tumour-like lesions. Results and conclusion. CRMO affects predominantly the tubular bones of the limbs, followed by the clavicle and the spine. Other locations are rare. Diagnosis is important in avoiding unnecessary diagnostic procedures and to initiate appropriate therapy, and is usually based on a characteristic course and the appearances on radiography. However, CRMO lesions of tubular bones and the spine exhibit quite characteristic MRI features which support the diagnosis, while the appearance of the early clavicular lesion is non-specific. At all sites of CRMO in the skeleton, MRI is valuable in assessing the extent and activity of the lesion. It may exclude pyogenic involvement of the bone and soft tissues and guide effective biopsy.

We studied 21 children with Legg-Calve-Perthes Disease with a prognostically poor development, including lat-eralization, poor containment, anterolateral flattering, and deformation of the femoral head, as evaluated on serial magnetic resonance (MR) imaging. These children were treated with proximal femoral varus derotation osteotomy. The sphericity of the cartilaginous and bony femoral epiphysis was evaluated postoperatively on serial radiography and MR imaging. There was an early postoperative continuous spherical remodeling over a follow-up period of 3.0 years (1.0–5.1; SD, 1.3).

The normal and abnormal anatomy of the hip joint in childhood was evaluated by ultrasonography (US) with reference to examination technique and errors. When compared with computed tomography (CT), it was confirmed that the two sonographic schoes from the hip joint represent the femoral neck and the external surface of the capsule and that the thickness of the echo received from the capsule depends on the frequency of the transducer. Partial volume effects may simulate or increase the sonographic appearance of hip joint effusion. The normal relationship between joint capsule and femoral neck was established.

39 children with Legg-Calve-Perthes Disease (LCPD) and with a radiographically unsatisfactory development were examined with sonography regarding synovitis and with intracapsular pressure recording and aspiration performed 15 (2-36) months after the onset of symptoms. The mean anterior sonographic capsular distension was 3.0 (1.0-7.0) mm greater than that of the contralateral, asymptomatic hip. The mean intracapsular pressure was 4.5 (0-11.5) kPa with the hips in extension and neutral rotation, 9.7 (1.3-27.3) kPa with the hips in extension and inward rotation and 0.9 (-0.8-4.7) kPa in 45° of flexion. We conclude that these children have synovitis as diagnosed sonographically. This synovitis is probably symptomatically and prognostically important in LCPD due to increased intracapsular pressure, with pain, a decreased range of motion and, potential joint contracture.

Two series of patients operated on with total hip replacements for arthrosis are reported. The same prosthesis and operative and cementing techniques were used in both series, except that in Series 2 a centralizing device consisting of four triangular fins passed over the tip was also used to improve the positioning of the femoral stem. It was found that the position of the tip was more central in Series 2. No drawbacks were associated with the use of the device. We concluded that the centralizing device facilitated the positioning of the femoral stem tip.

In eight undisplaced intracapsular fractures of the femoral neck, an intracapsular hematoma was diagnosed by computed tomography. 99mTc-MDP scintimetry revealed markedly reduced or absent blood supply to the head of femur. The intracapsular pressure was 23 (2.7-43) kPa with the hip in neutral position. Following aspiration of 12 (0.5-36) ml of blood, pressure was reduced to zero, and postaspiration scintimetry revealed restitution of blood supply to the femoral head. Hip joint tamponade in these patients has caused femoral head ischemia, reversible by aspiration.

Sonography was used in the preoperative evaluation of the hip joint in 14 patients with juvenile chronic arthritis (JCA). The joint capsule distension found at sonography and the intracapsular pressure were increased in patients with severe synovitis revealed at arthroscopy. Sonography and intracapsular pressure-recording can be recommended for assessments of synovitis of the hip joint in JCA.

Fourteen consecutive children with symptoms of transient synovitis of the hip were examined with sonography regarding intracapsular effusion, with scintimetry regarding blood-flow in the proximal femoral epiphysis (PFE) and with intracapsular pressure recording and aspiration. All patients had an intracapsular effusion. Intracapsular pressure was found to depend on the position of the hip. The mean pressure with hips in extension and inward rotation was 22.6 kPa (170 mm Hg) whereas in 45 of flexion it was 2.3 kPa. In two cases scintimetry demonstrated reduced blood flow to the PFE; after aspiration, isotope uptake returned to normal, indicating that increased intracapsular pressure has a harmful effect on circulation to the PFE.Children with transient synovitis should be treated with the hips in 45 of flexion to reduce intracapsular pressure. Forcing the hip in extension causes a risk of ischaemia of the PFE.

A roentgen stereophotogrammetric analysis of hip prostheses was made in four patients with rheumatoid arthritis during a postoperative period of 2 years.

Conventional radiography was performed at diagnosis and at follow-up 5 to 9 months later in 70 children with transient synovitis of the hip. Twenty-four of the patients also had CT examination at diagnosis. The cartilaginous and osseous reaction in the conventional antero-posterior radiographs was studied as was the relation between the radiographic and CT findings concerning fat planes, joint effusion, and position of the hip joint. At diagnosis there was significant increase in the medial joint space and at follow-up examination there was significant increase in medial and cranial joint space, metaphyseal width and acetabular roof width. There was a correlation of a bulging lateral ‘capsular fat plane’ to the position of abduction in the affected hip which explains the asymmetry in the lateral fat plane observed in these patients.

To improve the positioning of the tibial component in compartmental knee arthroplasty a guide instrument was introduced. With the guide instrument the position in the frontal plane significantly improved from a mean medial slope of 10 degrees to 2 degrees. In the sagittal plane the change from a mean posterior slope of 5 degrees to 3 degrees was insignificant. The mean value for the Hip-Knee-Ankle angle (HKA) (ideal 180 degrees) was 186 degrees after "free-hand" operation and 183 degrees with the modified operative technique. Thus, it is possible with the aid of a guide instrument to improve the precision in compartmental knee arthroplasty.

High tibial osteotomy was performed for medial gonarthrosis in 28 patients. Preoperatively, and at 3, 12, and 24 months after surgery, clinical and radiographic examinations were made, and joint-fluid samples were aspirated. Arthroscopy was performed preoperatively and at 24 months. Immunoassay of proteoglycan epitope in joint fluid showed an increase in concentration at all times as compared with a reference population with normal knee joints. An increase in both the concentration and the total amount of proteoglycan epitope in joint fluid was noted at 3 months postoperatively with a return to preoperative values at later times. Regrowth of fibrocartilage did not correlate with proteoglycan epitope data

In a prospective series of 52 knees selected for high tibial osteotomy because of early medial arthrosis, special care was taken to obtain a final over-correction in valgus. The operative procedure and the pre- and post-operative radiographic examinations were standardized and the follow up was 1–3 years. 49 of the 52 knees had obtained a normal or over-corrected mechanical axis. 45 of these had improved painfree walking distance, and 31 were painfree. In 45 knees the arthrotic condition had not progressed, and in 5 of these there were signs of cartilage restitution.

In three cases operated with high tibial osteotomy for medial gonarthrosis the exact method of roentgen stereophotogrammetry with tantalum balls as bone markers (Selvik 1974) was used to study angular and translational movements in three dimensions at the operation and during the healing period. Tibial osteotomy caused angular and translational movements even in planes where correction was not intended, and the stereo technique revealed that stability was not present when knee mobilisation started. Correlation between the stereo values and conventional radiographic measurements were best in the frontal plane (root mean square value of discrepancies 1.3°). Roentgen stereophotogrammetry gives superior information compared with the conventional radiographic technique, but it is concluded that the latter has sufficient accuracy for the clinical assessment of corrections in the frontal plane.

Longitudinal growth determined by roentgen stereophotogrammerry was registered in three patients with physial injuries in distal femur and in two patients with physial injuries in proximal tibia during 18 months. The injuries in distal femur were classified as Type I, Type I + III and Type IV and in proximal tibia as Type I + III and Type IV in the different cases according to Salter and Harris. Markers of tantalum balls were implanted into the metaphysis and bony epiphysis of distal femur and proximal tibia permitting regular determination of longitudinal growth.

Sonography was used successfully in the investigation of septic conditions in the hip region of four children. In two patients, septic arthritis of the hip was confirmed; in two patients, intracapsular involvement was excluded in osteomyelitis of the hip region.

Computed tomography (CT) was employed to evaluate the femoral component of the prosthesis in surface-replacement total hip arthroplasty in seven patients suffering from rheumatoid arthritis. Image reconstruction using the high frequency filter resulted in an image of the cementbone interface inside the cup which allowed the detection of bone resorption. This new application of CT could prove to be a valuable complement to conventional methods in the radiological assessment of metal cup arthroplasties.

Double-contrast arthrography and arthroscopy were performed in a prospective study on 46 consecutive patients clinically suspected of having a medial meniscal tear only. At arthrography, 38 torn medial menisci were found; four of them were, however, false-positive and another false-negative as compared with arthroscopic examination. Moreover, seven unsuspected lateral meniscal tears were diagnosed at both arthrography and arthroscopy. The diagnostic accuracy of the double-contrast arthrography was well above the clinical accuracy, and we conclude that arthrography is are-liable method for diagnosing both medial and lateral tears.

In a prospective study, 32 knees in 32 patients were randomized to either a cylinder plaster cast (17 knees) or hinged cast-brace (15 knees) after high tibial osteotomy for medial gonarthrosis. At 6 weeks, 3 months, and still 1 year after surgery, the range of motion was better in the cast-brace group. There was no difference in the other clinical results at 3 months and at 1 year after surgery, nor in changes of osseous correction or in the final knee alignment. All the patients in the cast-brace group were satisfied with early motion.

Twenty-seven patients (28 knees), with a median age of 42 (27-50) years treated with a high tibial osteotomy for early medial gonarthrosis (Ahlbäck 's Stage I) were examined after 11 (7-18) years. Twenty-four were men; 18 knees had had previous meniscus and ligament lesions. At follow-up, 22 knees were satisfactory and 9 patients managed high-activity sports or heavy work. In 25/28 knees, the arthrotic process had not progressed radiographically.

In a preliminary report of a clinical trial of gadolinium-DTPA (Gd-DTPA) for enhancement of soft tissue tumors in magnetic resonance imaging (MRI), experience of the first five cases is presented. Enhancement was found to give better delineation of the tumor in richly vascularized parts of the tumors, compressed tissue immediately surrounding the tumor, and in atrophic, but richly vascularized, muscle.

In 16 patients with chronic, symptomatic anterior cruciate ligament-deficient knees, sagittal displacement was studied in the standing position using fluoroscopic control and a simple device to support the knee. In slight weight bearing all knees but one had normal femorotibial alignment in the lateral view. During full weight bearing sagittal displacements between 2 mm and 17 mm were recorded in 14 patients. The largest displacements were obtained at different angles of inclination of the leg and flexion of the knee joint and were independent of the point of support of the knee joint. The reproducibility of this new standing technique was within 2 mm. A high correlation with previous methods of radiographic measurements of sagittal laxity was found. By the use of the normal relationship between the tibial eminence and the femoral condyles it is possible to record and measure sagittal displacements on a single lateral radiograph of the standing knee joint.

Twenty-one computed tomographic (CT) and 17 conventional sinographies were performed on 17 patients with lesions in the musculoskeletal system. The CT sinography was superior to the conventional examination in all cases, giving detailed information on the relationship between the sinuses and neighboring soft tissue and skeletal structures. CT sinography is recommended as the primary method for examination of musculoskeletal sinuses.

Fifty-two patients (52 knees) with medial gonarthrosis were subjected to high tibial osteotomy using the Tjornstrand guide instrument. The intended wedge was calculated from a whole lower limb radiograph. The correction aimed at was an overcorrection of 4° in valgus of the hip-knee-ankle angle. All but 1 case were corrected to ±3° of the intended angle. All but two cases healed within ±3° of the achieved surgical correction, i.e., a substantial improvement compared with our previous freehand technique where one fifth were outside this interval. We conclude that in knee surgery a guide is as important for osteotomies as for arthroplasties.

Earlier studies have demonstrated an adverse effect of radiographic contrast media (CM) on granulocyte phagocytosis. Most studies in the past have depended on granulocyte separative procedures that may themselves affect granulocyte functions. This study was performed to evaluate the effect of CM on phagocytosis using a flow cytometric assay allowing more physiological assay conditions.

In a 7-year-old boy with clinical symptoms of transient synovitis of the hip, an intracapsular effusion was diagnosed with computed tomography and ultrasonography. An intracapsular pressure of 50 mmHg was recorded prior to aspiration of 5.5 ml of clear fluid. Repeated ultrasonography immediately after aspiration verified a reduction in effusion, and serial examinations on the following days did not reveal any recurrence of the effusion

An 81-year-old woman had severe hip pain after a fall. Radiography was normal, but computed tomography and sonography showed a minor, displaced acetabular fracture and hemarthrosis. Aspiration of 15 ml of blood reduced the intracapsular pressure in the neutral position from 30 kPa to atmospheric pressure with relief of pain and increased joint motion. This case confirms that acetabular fracture may escape radiographic diagnosis and cause hip joint tamponade. Aspiration should be considered as a palliative and possibly therapeutic measure.

Fractures of the pubic rami are almost invariably associated with fractures of the posterior arch of the pelvic ring. Two women, aged 50 and 67 years, with septic arthritis of the symphysis pubis attended by severe low back pain, were followed with CT and MR imaging of the pelvis, as well as bone scintigraphy in one patient. In the first patient sacral fractures with severe displacement were revealed, prompting stabilizing symphysiodesis. In the second patient an undisplaced fatigue fracture was confirmed in the right half of the sacrum. In patients with pelvic laxity following arthritis of the symphysis and post-traumatic osteolysis associated with low back pain, displaced or occult fractures of the bones adjacent to the sacroiliac joints should be considered.
