Background: Lack of access to reliable transportation is a barrier to utilizing healthcare and other resources related to type 2 diabetes mellitus (T2DM). Little research has evaluated race/ethnicity-based differences in access to reliable transportation among persons with T2DM. Purpose: To examine whether access to reliable transportation for persons with T2DM differed by race/ethnicity. Methods: Analysis was conducted among persons with T2DM using 2022 Behavioral Risk Factor Surveillance System data. The outcome was access to reliable transportation (yes/no) for medical appointments, meetings, work, or getting things needed for daily living in the last year. The independent variable was race/ethnicity (White, Black/African American, Asian, American Indian or Alaskan Native (AI/AN), Hispanic, Native Hawaiian or Other Pacific Islander, or multi-racial). Weighted logistic regression models examined the association between access to reliable transportation and race/ethnicity controlling for covariates. Results: Persons with T2DM (n= 24,964) who identified as Black (adjusted odds ratio (AOR) = 1.42 (95% confidence interval (CI) = 1.10, 1.84), AI/AN (AOR = 2.03 (95% CI = 1.22, 3.36), or multi-racial (AOR = 1.99 (95% CI = 1.18, 3.5) were significantly more likely to indicate they did not have reliable access to transportation in the past year compared to whites. Persons who were females, older, married, had higher income, employed, no mobility issues, no depression, and rated their health status fair to very good were significantly less likely to report transportation issues. Conclusions: Access to reliable transportation may differ by race/ethnicity among persons with T2DM. Initiatives are needed to improve racially/ethnically equitable access to transportation for people with T2DM.

The abnormal assembly of tau protein in neurons is a pathological hallmark of multiple neurodegenerative diseases, including Alzheimer’s disease (AD). Assembled tau associates with extracellular vesicles (EVs) in the central nervous system of individuals with AD, which is linked to its clearance and prion-like propagation. However, the identities of the assembled tau species and EVs, as well as how they associate, are not known. Here, we combined quantitative mass spectrometry, cryo-electron tomography and single-particle cryo-electron microscopy to study brain EVs from individuals with AD. We found tau filaments composed mainly of truncated tau that were enclosed within EVs enriched in endo-lysosomal proteins. We observed multiple filament interactions, including with molecules that tethered filaments to the EV limiting membrane, suggesting selective packaging. Our findings will guide studies into the molecular mechanisms of EV-mediated secretion of assembled tau and inform the targeting of EV-associated tau as potential therapeutic and biomarker strategies for AD.

Approximately 1.4 vascular surgeons/100,000 persons are estimated to fulfill current patient needs in the United States (US), but an ongoing shortage exists. The aims of this study are to provide an updated nationwide state-by-state workforce analysis and compare the distribution of practicing vascular surgeons and training opportunities.

Nanoscale detection and control of the magnetic order underpins a spectrum of condensed-matter research and device functionalities involving magnetism. The key principle involved is the breaking of time-reversal symmetry, which in ferromagnets is generated by an internal magnetization. However, the presence of a net magnetization limits device scalability and compatibility with phases, such as superconductors and topological insulators. Recently, altermagnetism has been proposed as a solution to these restrictions, as it shares the enabling time-reversal-symmetry-breaking characteristic of ferromagnetism, combined with the antiferromagnetic-like vanishing net magnetization1, 2, 3–4. So far, altermagnetic ordering has been inferred from spatially averaged probes4, 5, 6, 7, 8, 9, 10, 11, 12, 13, 14, 15, 16, 17, 18–19. Here we demonstrate nanoscale imaging of altermagnetic states from 100-nanometre-scale vortices and domain walls to 10-micrometre-scale single-domain states in manganese telluride (MnTe)2,7,9,14, 15–16,18,20,21. We combine the time-reversal-symmetry-breaking sensitivity of X-ray magnetic circular dichroism12with magnetic linear dichroism and photoemission electron microscopy to achieve maps of the local altermagnetic ordering vector. A variety of spin configurations are imposed using microstructure patterning and thermal cycling in magnetic fields. The demonstrated detection and controlled formation of altermagnetic spin configurations paves the way for future experimental studies across the theoretically predicted research landscape of altermagnetism, including unconventional spin-polarization phenomena, the interplay of altermagnetism with superconducting and topological phases, and highly scalable digital and neuromorphic spintronic devices3,14,22, 23–24.

Introduction: Eosinophilic granulomatosis with polyangiitis (EGPA) is an eosinophil-associated disease (EAD) characterized by inflammation in small- to medium-sized blood vessels. In the REal-world inVestigation of Eosinophilic-Associated disease overLap (REVEAL) study, overlap among 11 EADs was assessed. In the present sub-study, we evaluated EGPA overlap with other EADs, all-cause EAD- and EGPA-related healthcare resource utilization (HCRU) and costs, and their relationship with blood eosinophil count and treatments received. Methods: REVEAL, a retrospective study, used Optum’s de-identified Clinformatics®Data Mart Database. In this sub-study, eligibility criteria included an age of ≥ 12 years, ≥ 1 EAD, continuous health-plan eligibility, and compliance with the EGPA/GPA case definition per International Classification of Diseases Ninth/Tenth Revision diagnostic codes between 1 January 2015 and 30 June 2018. Patients were grouped based on whether they had received immunomodulators/cyclophosphamide/mepolizumab (ICM) or not (non-ICM). Results: Of 701 patients with EGPA, 29.5% were in the ICM group. Overall, 72.2% had ≥ 1 overlapping EAD. The number of overlaps was similar for the ICM and non-ICM groups. In patients with blood eosinophil counts ≥ 300 cells/µL, 22.8% had ≥ 1 overlapping EAD. The mean annual all-cause cost was $98,644, 54.1% of which was from outpatients and 33.6% from inpatients. The mean annual EAD- and EGPA-related costs were $23,820 and $9,306, respectively. Patients in the non-ICM group versus the ICM group had higher all-cause ($101,560 vs $91,684) but lower EAD-related ($22,733 vs $26,412) and EGPA-related ($6,171 vs $16,786) costs. All-cause HCRU and costs increased with increasing overlap. Conclusions: EGPA was associated with substantial HCRU and costs, driven by outpatient and inpatient settings. More overlapping EADs were associated with higher HCRU and costs, highlighting the need for treatment to reduce healthcare expenditure in these patients. Infographic available for this article.

Background: Black patients with peripartum cardiomyopathy (PPCM) have disproportionately worse outcomes than White patients, possibly related to variable involvement of cardiovascular specialists in their clinical care. We sought to determine whether race was associated with cardiology involvement in clinical care during inpatient admission and whether cardiology involvement in care was associated with higher claims of guideline-directed medical therapy (GDMT) a week after hospital discharge. Methods: Using Optum’s de-identified Clinformatics® Data Mart (CDM), we included Black and White patients’ first hospital admission for PPCM from 2008 to 2021. Cardiology involvement in clinical care was defined as the receipt of attending care from a cardiovascular specialist during admission. GDMT included beta-blockers (BB) for all patients and triple therapy (BB, angiotensin-responsive medications, and mineralocorticoid receptor antagonists) for non-pregnant patients. Logistic regression was used to determine the associations between cardiology involvement in clinical care during admission and (1) patient race and (2) GDMT prescription, adjusting for age and comorbidities. Results: Among 668 patients (32.6% Black, 67.4% White, 93.3% commercially insured), there was no significant difference in the odds of cardiology involvement in clinical care by race (aOR: 1.41; 95%CI: 0.87–2.33, P=0.17). Inpatient cardiology care was associated with 2.75 times increased odds of having a prescription claim for GDMT (BB) for White patients (aOR: 2.75; 95%CI 1.50–5.06, P=0.001), and the estimated effect size was similar but not statistically significant for Black patients (aOR: 2.20, 95% CI, 0.84–5.71, P=0.11). The interaction between race and cardiology involvement in clinical care was not statistically significant for the receipt of BB prescription. Among 274 non-pregnant patients with PPCM (37.2% Black, 62.8% White), 5.8% received triple GDMT. Of these, none of the Black patients lacking cardiology care had triple GDMT. However, cardiology involvement in care was not significantly associated with triple GDMT for either race. Conclusions: Among a commercially insured population within PPCM, race was not associated with cardiology involvement in clinical care during hospitalization. However, cardiology involvement in care was associated with significantly higher odds of prescription claims for BB for only White patients. Additional strategies are needed to support equitable GDMT prescription.

As molecular imaging use expands for patients with breast cancer, it is important for breast radiologists to have a basic understanding of molecular imaging, including PET. Although breast radiologists may not directly interpret such studies, basic knowledge of molecular imaging will enable the radiologist to better direct diagnostic workup of patients as well as discuss diagnostic imaging with the patient and other treating physicians. Several new tracers are now available to complement imaging glucose metabolism with FDG. Because it provides a noninvasive assessment of disease status across the whole body, PET offers specific advantages over tissue-based assays. Paired with targeted therapy, molecular imaging has the potential to guide personalized treatment of breast cancer, including guiding dosing during drug trials as well as predicting and assessing clinical response. This review discusses the current established applications of FDG, which remains the most widely used PET radiotracer for malignancy, including breast cancer, and highlights potential areas for expanded use based on recent research. It also summarizes research to date on the U.S. Food and Drug Administration (FDA)–approved PET tracer 16α-18F-fluoro-17β-estradiol (FES), which targets ER, including the current guidelines from the Society of Nuclear Medicine and Molecular Imaging on the appropriate use of FES-PET/CT for breast cancer as well as areas of active investigation for other potential applications. Finally, the review highlights several of the most promising novel PET tracers that are poised for clinical translation in the near future.

Heavy alcohol use, cumulative marijuana use, and cumulative tobacco use were associated with a higher frequency of vasomotor symptoms. Heavy alcohol use and opioid use were associated with mood symptoms of menopause and opioid use was associated with musculoskeletal symptoms of menopause.

This study aims to explore the impacts of COVID-19 on birthing for wāhine (women) and whānau (immediate and extended family) in Aotearoa (New Zealand) during the lockdowns of 2020 and 2021. This rangahau (research) provided whānau with an opportunity to share their whakawhānau (birthing process) experiences via interviews. The data were analysed through interpretative phenomenological analysis. Themes identified in the results included the importance of whānau tautoko (family support), the experiences of isolation and trauma, and the effect of birthing under COVID-19 restrictions. Indigeneity was a core identity that was crucial to maintain, during these sacred times. Whakawhānau is a pivotal transition with the potential to engage with medical services. The findings of this study contribute to the knowledge of whānau Māori (Māori [Indigenous peoples of New Zealand] families) experiences to enhance service provision.

This digest reviews what instructional designers do, describes knowledge management, and indicates how knowledge management is influencing instructional design. The first section defines instructional design (ID) and briefly describes the ID process. The second section covers knowledge management (KM), including definitions of KM and systems, characteristics of KM systems (KMSs), specific functions provided by KMSs, uses of KM technology within business and industry, and key enabling technologies. The third section discusses KM in ID, including projects at the European Commission and Syracuse University that use a KMS in ID, the influence of KM technology on school-based learning and collaboration among instructional designers, and issues being addressed by ID researchers. It is concluded that KM tools and systems are beginning to be used for the design and development of instructional systems and learning environments, and ID practice is changing as a consequence. Individuals with minor ischaemic stroke and intracranial occlusion are at increased risk of poor outcomes. Intravenous thrombolysis with tenecteplase might improve outcomes in this population. We aimed to test the superiority of intravenous tenecteplase over non-thrombolytic standard of care in patients with minor ischaemic stroke and intracranial occlusion or focal perfusion abnormality.

Sexual revictimisation has devastating consequences for victim/survivors, yet there is limited research exploring women's experience of revictimisation in regional/rural areas. Using a community‐based participatory research (CBPR) approach, this paper reports on a qualitative study that employed a material feminist lens and Nixon's theory of “slow violence” to explore women's lived experiences of sexual revictimisation. In‐depth interviews were conducted with victim/survivors (N= 11) living in regional and rural areas of Australia. Findings show that the failure of family, community and services to respond appropriately to participant's disclosures of violence and abuse was deeply entangled with rural infrastructure, isolated landscapes, fear of social isolation, victim‐blaming discourses, idealisation of men in the community and limited relationship and sexuality education (RSE) in schools. These components collectively formed a manifestation of “slow violence,” which accumulated over the participants' life spans and gradually normalised relational violence. This paper draws attention to a complex interplay of cultural, material and interpersonal elements, including the culture and spaces of rural/regional communities, that establish conditions enhancing the likelihood of women experiencing sexual revictimisation.

Two-dimensional (2D) magnetic materials offer a promising platform for nanoscale spintronics and for exploration of novel physical phenomena. Here, we predict a diverse range of magnetic orders in cobalt-based 2D single septuple layers CoX2Y4, namely, CoBi2Te4, CoBi2Se2Te2, CoBi2Se4, and CoSb2Te4. Notably, CoBi2Te4presents intrinsic non-collinear antiferromagnetism (AFM), while the others display collinear AFM. The emergence of AFM in all CoX2Y4materials is attributed to the antiferromagnetic 90° Co–Te(Se)–Co superexchange coupling. The origin of non-collinear/collinear orders lies in competing Heisenberg exchange interactions within the Co triangular lattice. A pivotal factor governing the non-collinear order of CoBi2Te4is the vanishingly small ratio of exchange coupling between next-nearest neighbour Co and the nearest neighbour Co (J2/J1∼ 0.01). Furthermore, our investigation into strain effects on CoX2Y4lattices demonstrates the tunability of the magnetic state of CoSb2Te4from collinear to non-collinear. Our prediction of the unique non-collinear AFM in 2D suggests the potential for observing extraordinary magnetic phenomena in 2D, including non-collinear scattering and magnetic domain walls.

Avulsing rivers create new pathways on the floodplain and the associated flooding can profoundly affect society1–4. River avulsions are thought to occur when the water column becomes perched above the floodplain5or when the slope down the flanks of the channel provides a steeper descent than the existing river channel6,7. We test these classical ideas by quantifying the topography around avulsing rivers and show that these mechanisms, historically invoked separately, work together. Near coasts, rivers avulse when the slope away from the channel is steeper, not because they are perched. The opposite is true near mountain fronts; on fans, the alternative paths are similarly steep to the downstream path, so rivers avulse when they are perched above the surrounding landscape. We reconcile these findings and present a new theoretical framework that identifies which rivers are vulnerable to avulsion and predicts the path of an avulsing river. These first-order rules of avulsion suggest that avulsion risks are underestimated in many coastal environments8and that probabilistic predictions of avulsion pathfinding can efficiently map hazards with minimal information. Applying these principles for risk assessment could particularly benefit the Global South, which is disproportionately affected by avulsions.

Purpose:There is a need for stimuli that can be used in the assessment of apraxia of speech (AOS) characteristics. AOS often presents with aphasia, and the Western Aphasia Battery–Revised (WAB-R) is administered extensively in clinical practice for the assessment of aphasia. This preliminary study investigated error production of differentially diagnostic characteristics of AOS (phonetic and prosodic errors) in response to existing spoken language subtest stimuli of the WAB-R in individuals with concomitant AOS and aphasia and aphasia only.Method:Ten participants presenting with aphasia and potential motor speech impairment were included. Presence of AOS was determined for five individuals. Speech production errors were analyzed using narrow transcription methods in response to the Naming, Repetition, and Spontaneous Speech subtests of the WAB-R for both AOS and aphasia and aphasia-only groups.Results:Differences in total error production for both phonetic and prosodic errors were reported between the AOS and aphasia and aphasia-only groups. In addition, the phonetic complexity of the stimuli correlated to the phonetic and prosodic error production in both groups. An effect of WAB-R subtest was not reported for either group.Conclusion:The diversity of elicited spoken language production across multiple contexts makes the use of the spoken language subtest stimuli a feasible component of a comprehensive AOS assessment, since AOS is often concomitant with aphasia.

Family building is a human right. The high cost and lack of insurance coverage associated with fertility treatments in the United States have made treatment inaccessible for many patients. The universal uptake of “add-on” services has further contributed to high out-of-pocket costs. Expansion in access to infertility care has occurred in several states through implementation of insurance mandates, and more employers are offering fertility benefits to attract and retain employees. An understanding of the economic issues shaping fertility should inform future policies aimed at promoting evidence-based practices and improving access to care in the United States.

The reigns of Adad-nārārī II (911–891) and his son Tukultī-Ninurta II (890–884) are vital to understanding the rise of Neo-Assyria; yet, reconstruction of these is hampered by the scarce and fragmentary sources available. This study surveys the reigns of these two kings, and examines five fragmentary early Neo-Assyrian royal inscriptions from the city of Aššur published in 2009 by Frahm within Keilschrifttexte aus Assur literarischen Inhalts 3, providing solid ascriptions of three of these to Adad-nārārī II and two to Tukultī-Ninurta II on philological and historical grounds. These findings are then integrated into present knowledge of this period in order to present new portraits of these kings’ respective reigns. This results in a clearer historical articulation of Adad-nārārī II’s remarkably successful incumbency, particularly shedding light on his early victories. In turn, Tukultī-Ninurta II’s difficult reign spent consolidating his father’s territorial gains can also be better understood. Interestingly, various innovations can be ascribed to this latter king, not least the ‘calculated frightfulness’ for which his son would become so (in)famous within Assyriology. Finally, some repercussions of these findings for the study of 10th and 9th century royal inscriptions are explored.

As countries take stock of progress made in accomplishing their climate goals ahead of COP28 this year, it is increasingly apparent that countries must ratchet ambition in policy areas such as non-CO2gases and carbon dioxide removal, while halting deforestation to lead the globe on a path consistent with the goals of the Paris Agreement.

There is a significant shortage of vascular surgeons in the United States and projections for these practicing surgical specialists continue to worsen. Annual appraisal of our workforce recruitment and growth is imperative.

Emergent quantum phenomena in two-dimensional van der Waal (vdW) magnets are largely governed by the interplay between exchange and Coulomb interactions. The ability to precisely tune the Coulomb interaction enables the control of spin-correlated flat-band states, band gap, and unconventional magnetism in such strongly correlated materials. Here, we demonstrate a gate-tunable renormalization of spin-correlated flat-band states and bandgap in magnetic chromium tribromide (CrBr3) monolayers grown on graphene. Our gate-dependent scanning tunneling spectroscopy (STS) studies reveal that the interflat-band spacing and bandgap of CrBr3can be continuously tuned by 120 and 240 meV, respectively, viaelectrostatic injection of carriers into the hybrid CrBr3/graphene system. This can be attributed to the self-screening of CrBr3arising from the gate-induced carriers injected into CrBr3, which dominates over the weakened remote screening of the graphene substrate due to the decreased carrier density in graphene. Precise tuning of the spin-correlated flat-band states and bandgap in 2D magnets viaelectrostatic modulation of Coulomb interactions not only provides effective strategies for optimizing the spin transport channels but also may exert a crucial influence on the exchange energy and spin-wave gap, which could raise the critical temperature for magnetic order.

Topologically protected magnetic textures are promising candidates for information carriers in future memory devices, as they can be efficiently propelled at very high velocities using current-induced spin torques. These textures—nanoscale whirls in the magnetic order—include skyrmions, half-skyrmions (merons) and their antiparticles. Antiferromagnets have been shown to host versions of these textures that have high potential for terahertz dynamics, deflection-free motion and improved size scaling due to the absence of stray field. Here we show that topological spin textures, merons and antimerons, can be generated at room temperature and reversibly moved using electrical pulses in thin-film CuMnAs, a semimetallic antiferromagnet that is a testbed system for spintronic applications. The merons and antimerons are localized on 180° domain walls, and move in the direction of the current pulses. The electrical generation and manipulation of antiferromagnetic merons is a crucial step towards realizing the full potential of antiferromagnetic thin films as active components in high-density, high-speed magnetic memory devices.

AbstractThe Sustainable Development Goal’s (SDG) blueprint to global sustainability exemplifies the global governance trend towards the displacement of law by indicators. Indicators purport to produce objective measurement and comparison, a desirable trait for international public authorities that struggle to bolster the legitimacy of environmental and sustainability norms. This paper adopts a pragmatic approach to indicators by taking seriously their limitations, weaknesses, and dangers, but also their potential contributions to international sustainability objectives. We explore a reframing of the relationship between law and indicators in complementary, not adversarial, terms. Several examples of this complementarity are explored, including the potential use of the SDGs for evaluating the instrumental effectiveness of legal regimes, as well as the ways that international sustainability law supplements the SGDs by providing legal ramifications for violations of state-specific obligations. Finally, we argue that law and legal normativity make invaluable contributions to international environmental and sustainability governance, contributions that metrics and other managerial and technocratic forms of governance cannot make.

Introduction:There is an increasing need for occupational therapists with the skills to provide low vision services, however, there is a lack of standardized training of low vision occupational therapy services. The purpose of this study is to design and evaluate a course for occupational therapy students to improve their skills and knowledge about ophthalmology and low vision. Methods:A multifaceted, 15-week curriculum using didactic learning, clinical experience, and reflective writing was created to instruct second-year occupational therapy students in ophthalmology and low vision. Participants included 19 occupational therapy students from Thomas Jefferson University. Each trainee participated in the course modules and then submitted their course evaluations and reflective writings for review. Participants evaluated their satisfaction with the course using a Likert-scale and open-ended comments. Reflective writing from each participant was reviewed by occupational therapy faculty authors (AB, AL). Results:Nineteen students participated in the course, with 19 (100%) of participants reporting being “satisfied” or “highly satisfied” with the course. Analysis of the reflective writings revealed three main themes: the impact of eye-related medical conditions on daily life, insight into the provider-patient interaction and relationship, and the potential role of the occupational therapist on the vision team as an agent to maximize patient adjustment and function. Discussion:This collaborative, multimodal interprofessional educational model can assist in training and sensitizing occupational therapy students in the areas of ophthalmology and low vision. Implications for Practitioners:This report provides a model for medical educators to utilize in the training of occupational therapy students.

Overhead line design utilizing composite line post insulators requires an understanding of the insulator mechanical load carrying capabilities in multiple simultaneous directions. This paper covers the development of combined loading curves and offers guidance on the presentation of such curves so that informed decisions regarding the selection of composite line posts can be made.

Van der Waals electrode integration is a promising strategy to create nearly perfect interfaces between metals and 2D materials, with advantages such as eliminating Fermi-level pinning and reducing contact resistance. However, the lack of a simple, generalizable pick-and-place transfer technology has greatly hampered the wide use of this technique. We demonstrate the pick-and-place transfer of prefabricated electrodes from reusable polished hydrogenated diamond substrates without the use of any sacrificial layers due to the inherent low-energy and dangling-bond-free nature of the hydrogenated diamond surface. The technique enables transfer of arbitrary-metal electrodes and an electrode array, as demonstrated by successful transfer of eight different elemental metals with work functions ranging from 4.22 to 5.65 eV. We also demonstrate the electrode array transfer for large-scale device fabrication. The mechanical transfer of metal electrodes from diamond to van der Waals materials creates atomically smooth interfaces with no interstitial impurities or disorder, as observed with cross-section high-resolution transmission electron microscopy and energy-dispersive X-ray spectroscopy. As a demonstration of its device application, we use the diamond transfer technique to create metal contacts to monolayer transition metal dichalcogenide semiconductors with high-work-function Pd, low-work-function Ti, and semimetal Bi to create n- and p-type field-effect transistors with low Schottky barrier heights. We also extend this technology to air-sensitive materials (trilayer 1T’ WTe2) and other applications such as ambipolar transistors, Schottky diodes, and optoelectronics. This highly reliable and reproducible technology paves the way for new device architectures and high-performance devices.

Brain networks serving higher cognitive functions are widely distributed across frontal and posterior association zones. Two exceptions have been the parietal memory network (PMN) and salience network (SAL), which are typically restricted to posterior (e.g., posterior cingulate and lateral parietal cortex) and anterior (medial prefrontal and anterior insular cortex) areas, respectively. Using high-resolution neuroimaging, we show that individualized estimates of the PMN extend beyond the posterior set and encompass frontal and insula regions canonically ascribed to the SAL. This suggests that the SAL and PMN form a unified network: “SAL/PMN.” Task-based analyses confirm that both anterior and posterior components of the SAL/PMN show recognition-related activity. Comparison of 3T and 7T data suggests that high-resolution data more readily revealed the unified network, underscoring the importance of fine-scale distinctions for veridical representation of brain networks. Importantly, the unified network better matches the expected parallel distributed network organization that is characteristic of the association cortex.
