The aim of this study was to analyse the effects of auditory stress on peritoneal and alveolar macrophages in C57 BL/6J mice of advanced age, and to compare the results to those obtained in old mice submitted to a sham stress, and to those observed in young mice submitted to the same auditory stress. We used a chemiluminescence assay to measure the production of free oxygen radicals (FOR) by macrophages. Eight 22 month-old mice were exposed to a sound stress of 110 dB for three consecutive nights; nine were submitted to a sham stress. The results were compared to those obtained in young (8 week-old) mice, 21 submitted to noise stress, and 17 controls. The corticosterone level was not increased after stress in any group. FOR production in old mice was significantly higher than that in young mice. Stress did not induce significant changes in FOR production by alveolar cells in young mice; however, the FOR production by alveolar cells was significantly higher in the stressed group than in the control group of old mice. These results show that noise stress is associated with modifications of macrophage functions that are influenced by cell localization, the behaviour of alveolar and peritoneal macrophages of old mice being clearly different in our experimental model. Abstract: Rheumatoid nodulosis (RN) is a rare condition associating rheumatoid nodules, episodes of arthritis, cystic bone lesions and, generally, positive rheumatoid factors (RF). It is considered a benign variant of rheumatoid arthritis (RA). In this study, we determined the HLA DRB1* alleles of our RN patients and compared the distribution of these alleles to those of 74 healthy controls and 104 RA patients with and without nodules. Four RN patients were observed. All had subcutaneous nodules and RF were negative in three patients. Of the 104 RA patients, 18 had nodules (nodRA). Systemic manifestation (including vasculitis, peripheral neuropathy or lung involvement) were found in seven of these nodRA cases (33.8%) and most had positive RF and erosive changes on X-rays. Only one RN patient had a RA-associated allele (DRB1*0101). The frequencies of the HLA DRB1* alleles encompassing the “rheumatoid” shared epitope were similar to those of other RA series: *0101, 34.6% (P=0.03 compared with controls); *0401, 26.9% (P<0.0001); *0404, 12.5% (P=0.04); *0405, 4.8% (P=0.8); *1001, 8.6% (P=0.5). Of the nodRA and seronegative RA patients, 77.7% and 53.3%, respectively, presented the shared epitope. Thus, there was a tendency to decreased expression of the RA-associated alleles in RN (25%) compared with nodRA and seronegative RA patients. This study is restricted by the small number of tested RN patients, but the results suggest that the RA-associated alleles are poorly expressed in RN.

Summary We describe the beneficial effect of recombinant alpha 2b interferon in a woman with an idiopathic and refractory IgM-mediated autoimmune hemolytic anemia due to low-titer, high-thermal-amplitude cold agglutinins. The response was partial but sustained, 2 months after interruption of interferon, with hemoglobin concentrations ranging between 8.5 and 10 g/dl. In the literature, such rarely encountered hemolyzing warm-reacting agglutinins are described as causing severe hemolysis with a frequently fatal clinical outcome.

To the Editor.—We read with interest the letter by Balcells-Gorina et al (1981;246:1659) about a case of periarteritis nodosa manifested as polyserositis. In this letter, we attempt to demonstrate that serositis can be observed in other forms of inflammatory angiitis, such as temporal arteritis.Report of a Case.—A 66-year-old woman was admitted to the hospital after a three-week illness, which consisted of a temperature reaching 39 °C, weight loss, and thoracic pain. A pericardial rub was noted. There was no history of viral or bacterial infection in the upper respiratory tract. The patient complained of neither arthralgia nor diffuse muscular stiffness of the shoulders and pelvic girdle. The skin was normal. No sign of Raynaud's phenomenon was noted. The liver and the spleen were of normal size. There was no pain in the temporal arteries, and palpation was normal. The chest x-ray film revealed a mild cardiomegaly. The
