Background:secondary prevention programmes help manage risk factors, prevent progression of disease, minimise complications and reduce readmissions to hospitals. Referrals to these programmes are suboptimal even though they improve survival and quality of life.Aim:to explore referral of individuals with a primary diagnosis of acute coronary syndrome, cerebrovascular disease or peripheral vascular disease to secondary prevention programmes.Methods:a non-experimental descriptive retrospective design was used to analyse 176 patient records from a tertiary referral hospital in Melbourne, Australia. The study population consisted of individuals over 18 years of age admitted to hospital with a primary diagnosis of acute coronary syndrome, cerebrovascular disease or peripheral vascular disease within a 12-month period in 2009.Findings:from the patient records reviewed, 34% (n=60) were referred to secondary prevention programmes compared with 66% (n=116) not referred. Individuals with acute coronary syndrome were more likely to be referred than those with peripheral vascular disease or cerebrovascular disease. Those most likely to be referred were young males, married, living in a rural area and English speaking.Discussion:the study highlights that referrals to secondary prevention programmes are suboptimal. There is a need to investigate reasons for under-referral and the best approach to manage referrals.Conclusion:referrals to secondary prevention programmes are an important area of research: these are beneficial, yet some individuals are not being referred. Nurses are well placed to promote referrals to these programmes potentially. reducing under-referral.

Contemporary demands for workplace learning and development in real time have guided one health service to create a new role, that of the clinical coach. Clinical coaches provide point of care educational interventions to achieve clinical skill and practice development for nurses and midwives and to stimulate a culture of learning and development within the organisation. Clinical coaches use coaching processes and mantras, facilitation skills, practice development principles, adult learning strategies, supported practice and clinical assessment tools to achieve these goals within a person-centred philosophy. Specific point of care accountabilities of the coaches related to staff development include supporting clinical induction requirements, supporting preceptor and learner practices, supporting evidence-based clinical development, ensuring that mandatory training requirements are met, and coaching for the maintenance of safe and competent practice. The clinical coach role has evolved throughout the health service over a number of years. Organisational data reveal the acceptability of the coaching role in the organisation along with successful outcomes. Based on this case experience, it is recommended that other health services consider clinical coaching as a relevant mechanism for advancing point of care workplace integrated learning and development.

The quality of undergraduate nursing education is essential to producing graduates that are safe and effective practitioners, relevant to the broader contexts of care. National accreditation standards are focused on ensuring the quality of nursing programs in terms of public interest and safety. To ensure that nursing programs achieve the outcomes expected of them, effectively designing curricula as part of an accredited program is central to quality assurance in nursing education. The aim of the study is to explore the process of curriculum design in the context of national accreditation processes. A qualitative grounded theory methodology was adopted with interviews and document analysis undertaken. Findings revealed significant issues with the approaches used to inform curriculum design, resourcing, and staff capacity in the context of undergraduate nursing education. Strong whole-of-course curriculum design processes form the foundation of a quality system of undergraduate nursing education. The deficiencies in current practice have significant implications for the future of the nursing profession. While no clear and immediate solution is evident, identifying the nature of such limitations and implementing systematic, evidence-informed approaches to the process is crucial to improving the quality of undergraduate nursing education.

The introduction of mixed gender wards that have permeated Australian hospitals in recent years may have begun as an ill-conceived experiment, but their continued existence despite expressed patients’ wishes is an affront to modern health service provision. While the UK has witnessed an uproar resulting in a ban on mixed gender wards, Australian services have been slow to react to this trend. We examine the literature documenting the introduction of mixed gender wards, focusing especially on their evaluation by staff and patients. There is little if any evidence showing any benefits of mixed gender wards and research suggests that they are antithetical to basic human rights and person-centred care. It appears that the barrier to their eradication is little more than a short-term focus on financial ‘feasibility’.

Urinary incontinence is a common and distressing condition. Using focus groups, we explored the views of ten ethnic language groups in Melbourne about knowledge and awareness of incontinence. The 218 participants (with or without incontinence) spoke with trained interpreters. Twenty focus group discussions of single and mixed sex groups were audio-recorded and transcribed into English. Narratives were analyzed using thematic analysis with open coding and also incorporated themes from literature. Participants’ knowledge of incontinence was low and incontinence was thought to be an inevitable consequence of ageing. There was little understanding of treatments or assistance available under government-funded programmes. No group was aware of the national continence programme or phone helpline. Sensitivities of the topic plus language barriers in immigrant culturally and linguistically diverse communities may impose barriers to accessing help. Several groups thought they would cope with incontinence by themselves, while all groups suggested they would be able to discuss the condition with a doctor. Various preferences voiced about social limitations and permissible communications with others are described. Nurses should be aware of the needs and communication preferences of ethnic language groups regarding continence information and continence service delivery.

Many health services with Intensive Care Units have developed ward liaison programs, managed by Outreach Nurses, to facilitate the transition for patients between the intensive care and general wards. This paper reports a case study of clinical supervision for two Outreach Nurses as they adapted to their new, largely autonomous role in an Australian tertiary hospital.

Practice development (PD) and knowledge translation (KT) have emerged recently as methodologies which assist advancement in gathering and using evidence in practice. For nursing to benefit from these methodologies there is a need to advance the dialogue between academia and the service sector concerning the use and further development of these methodologies as well as how we create the most effective partnerships between academia and practice. To advance this dialogue and to gain insights into the similarities and differences between KT and PD and between the academic and the service sectors, four conversations from different leaders in these sectors have been gathered and are presented here.

While the treatment of schizophrenia and other psychotic disorders has advanced over the past decades, the management of women with this serious mental illness, who become pregnant, and their babies remains a concern. The use of psychotropic medications is necessary in a large number of women of childbearing potential who have a serious mental illness however the use of psychotropics, particularly antipsychotics, may interfere with the reproductive process. The literature regarding the effect of psychotropic medications on the reproductive ability of women is presented in this article. The authors have found varied reports regarding fertility rates and outcomes for this group of women including a lower rate of fertility, fewer children, despite being less likely to use contraceptives. That atypical antipsychotic medications effect fertility is outlined. Implications for the women and clinicians are discussed. Coinciding with the developments in medication management have been many changes in the delivery of mental health care and social changes which have led to women developing and maintaining relationships and wanting to have children. Many factors may influence the fertility of women. Women generally prefer not to take medications if they are wishing or trying to become pregnant. Adherence to medication treatment may have implications in relation to this. There is little consistency in the literature around this, and little is really known about the fertility rates of women with serious mental illness.

The first perfluorovinyl alkyl-containing phosphines of the type PR2CFCF2 R  Et, iPr, Cy are reported. The reactivity of these air- and moisture-stable materials has been explored, both at the phosphorus centre and at the fluorovinyl moiety. When PR2CXCF2 X  F, Cl is reacted with LiAlH4a mixture of PR2CXCFH isomers and other defluorinated materials are produced, but the reaction with LiAlHOBut3affords the single products Z-PR2CFCFH or E-PR2CClCFH, respectively, in high yields. Reaction of the fluorovinyl alkyl phosphines with hydrogen peroxide, elemental sulfur or selenium yields fluorovinyl-containing phosphine oxides, sulfides and selenides, respectively. The phosphine sulfide SPPri2CFCF2 is the first perfluorovinyl phosphorusv compound to be characterised crystallographically and it exhibits an unusually short [1.93589 ] PS bond. Reaction of fluorovinyl phosphines with XeF2results in compounds of the type F2PR2CFCF2, identified on the basis of multinuclear NMR studies. These compounds decompose in the presence of moisture to yield the respective phosphine oxides. Reaction of OPPh2CFCF2 with Br2results in bromine addition across the double bond to give OPPh2CFBrCF2Br.

Crystal structure prediction was used to explore possible modes of assembly of 2-amino-4-nitrophenol (ANP) molecules in the solid state. The results from this simulation were employed for the rational selection of solvents for the isolation of solid phases by solution crystallisation. Implementation of this crystal engineering based approach is seen as a first step in utilising predicted structural data and in this example resulted in the isolation of three crystal forms, one non-solvated structure and two solvated forms. Though too complex to be predicted, each contained elements of the packing modes identified from the crystal modelling simulations. This work demonstrates the potential application and limitations of available crystal structure modelling techniques as part of a strategy for the discovery of new crystal forms of molecular materials.

Complexes of diphenyl-2-styrylphosphine (sp) with gold, first reported in 1971, have been reinvestigated. [AuX(sp)] can be oxidised with X₂ for X = Cl or Br but not for X = I. The initial product, [AuX₃(sp)], undergoes a reversible rearrangement in which an Au–X bond adds across the styryl double bond to give [AuX₂(sp-X)] (sp-X = 2-CH(CH₂X)C₆H₄PPh₂). The carbon-bound halide can be replaced by methoxy by reaction with methanol. [AuX₂(sp-Y)] (X = Cl or Br; Y = Cl, Br or OMe) react with iodide to give eventual reduction to [AuI(sp)], confirming that non-chelated [AuX₂(Y)(sp)] is unstable when X = I. Reduction reactions of this type complicate attempts to study halide-substitution reactions of [AuX₂(sp-Y)], but a variety of substituted products have been identified (e.g. those in which X = OAc CN, SCN, ½SCH₂CH₂NH₂ or ½>S₂CNEt₂). Monosubstitution has been achieved with PPh₂R (R = Me or Ph). The crystal structure of [Au(SCH₂CH₂NH₂)(sp-OMe)]PF₆ has been determined; in it and all other substituted products the softer, incoming ligand is trans to phosphorus.

Reaction of perfluorovinyllithium, derived from CF3CH2F, with chloro-substituted phosphines generated perfluorovinylphosphines of the type PPhm(CF&z.dbd6;CF2)n and P(CF&z.dbd6;CF2)nClm (n 4;+ 4;m 4;= 4;3) in high yields. A low-temperature crystal structure determination of the air- and moisture-stable compound PPh(CF=CF2)2 provided the first reported structural data for any perfluorovinyl-containing material. There is considerable variation in the C–F bond distances [1.310(4), 1.321(4), 1.353(3) 4;Å] within the perfluorovinyl group in the phosphine. The co-ordination chemistry of these ligands has been investigated via the synthesis of examples of late transition-metal complexes. The results of single crystal structural determinations of cis-[PtCl2{PPh2(CF&z.dbd6;CF2)}2] and [{AuCl[PPh2(CF&z.dbd6;CF2)]}2]·0.5CH2Cl2 are reported. In both of these molecules short metal–phosphorus distances [d(Pt–P)av 4;= 4;2.231(3) and d(Au–P)av 4;= 2.217(2) 4;Å] are observed compared with typical distances for similar phosphine complexes. A comparison of the electronic properties of these ligands with those of other phosphines, halogenophosphines and phosphites was made on the basis of a spectroscopic investigation of the carbonyl stretching frequencies of the complexes [Mo(CO)5{PPhm(CF&z.dbd6;CF2)n}] (n 4;+ 4;m 4;= 4;3).

A variety of R3PI4 compounds have been synthesized and characterised by elemental analysis, NMR and visible spectroscopy. Apart from Ph3PI4, all are reported for the first time. X-Ray crystallographic studies on two of these materials, Pri3PI4 and (Prn2N)3PI4, revealed different solid-state structures which are, in turn, different to both the two polymorphs of Ph3PI4 and Fc3PI4 (Fc 4;= 4;ferrocenyl) the crystal structures of which have been previously reported. The compound Pri3PI4 exists as [(IPPri3)2I3]I3, which contains two [IPPri3]+ cations independently weakly linked to one of the two terminal iodine atoms of the same triiodide anion. The structure also contains a discrete triiodide anion. The compound (Prn2N)3PI4 exhibits very little anion–cation interaction. Of the two molecules present in the asymmetric unit, one exhibits no anion–cation interaction, while the other shows only a very weak contact [d(I–I)] (cation–anion) 4;= 4;3.944(2) Å.

The reactions of the selenoamides N,N-dimethyl-2-selenourea, dmsu, selenourea, su and N-methylbenzothiazole-2-selone, mbts, with dibromine has been studied. In all reactions the bulk product formed is the 1∶1 T-shaped addition compound, selenoamide·Br2. The crystal structure of dmsu·Br2 1 has been determined and compared to its solvated analogue, dmsu·Br2·CH2Cl2. Despite the different crystal packing in these two adducts, an asymmetry in d(Se–Br) is exhibited by both, ruling out the possibility that differences in Se–Br bond lengths are attributed solely to crystal packing forces. Both structures are essentially zwitterionic, a negative charge resides on the SeBr2 moiety and the positive charge is supported by the two nitrogen atoms. The recently reported mbts·Br2, previously described as a carbene interacting with SeBr2, was reinvestigated. Considering findings from a low temperature X-ray study and the 13C NMR spectrum, this compound is in fact also best described as zwitterionic, analogous to 1 and previously described structures. An interesting minor product from the reaction of two equivalents of dibromine with dmsu has also been characterised crystallographically. This complicated ionic structure of formula 5[(H2N)(Me2N)CBr]+[SeBr6]2–[Se2Br9]–2[Br3]– illustrates the susceptibility of certain selenoamides to carbon–selenium bond cleavage when a dihalogen that is more strongly oxidising than I2 or IBr is treated with them in a stoichiometric ratio greater than 1∶1. This curious (and disordered) structure represents the first report of the anion [Se2Br9]–.

6-Bromo-2-methoxynaphthalene 1 was converted into 6-diphenylphosphanyl-2-methoxynaphthalene 2a by preparing its Grignard and quenching with PPh2Cl. Compound 2a was demethylated on refluxing in HBr yielding 6-(diphenylphosphanyl)naphthalen-2-ol 2b in good yield. Oxidation of 2b with either H2O2 or S8 afforded the corresponding phosphine oxide 3a or sulfide 3b respectively. Treatment of 2b with a stoichiometric amount of NaH and quenching of the anion with [4-R-C6H4N2][BF4] (R 4;= 4;H, Me, Et, iPr, tBu, NO2 or NMe2) yielded the C–N coupled azo-containing phosphines in good yield; similar coupling reactions of 3a, 3b afforded analogous compounds, again in good yield. Evidence is presented that shows the coupling reaction does not proceed through a P–N coupled intermediate, which would subsequently need to rearrange to the observed C–N coupled products. The latter all exist as tautomeric mixtures of the azo and hydrazone forms. The tautomerisation in some cases was suppressed on conversion into their acetic acid esters by reaction with NaH followed by acetyl chloride. All of the new compounds have been characterised by elemental analysis (C, H, N), FAB mass spectrometry, 1H, 13C-{1H}, 31P-{1H} NMR and in selected cases by Uv-visible spectroscopy. The position of the C(2) resonance in the 13C-{1H} NMR spectra has been used to calculate the position of the azo/hydrazone equilibrium and hence the mole fraction of each tautomer present in solution. These data were used to interpret the Uv-visible data. In addition, three compounds have been further characterised by single crystal X-ray diffraction studies.

The reactions of the tertiary phosphine sulfides R3PS (R 4;= 4;Ph, Me2N or C6H11) with X2 (X 4;= 4;I or Br) and IX (X 4;= 4;Br or Cl) have been studied. Reaction of R3PS with I2 or IX results in quantitative isolation of the CT complexes R3PSIX (X 4;= 4;I, Br or Cl), except for Ph3PS with I2 which produces the unusual compound (Ph3PSI2)2I2, which has been crystallographically characterised by earlier workers. The crystal structure of (Me2N)3PSI2 has been determined and compared to Ph3PSI2, previously described. The greater d(I–I) for (Me2N)3PSI2, 2.856(1) Å, compared to that of 2.823(1) Å for Ph3PSI2 clearly illustrates that d(I–I) in R3PSI2 compounds is sensitive to R, although this effect is less pronounced when compared to that of analogous R3PSeI2 compounds. The crystal structure of the product from the reaction of Ph3PS with IBr has also been determined and represents the first example of a tertiary phosphine sulfide interhalogen CT complex. It has the formula Ph3PS(I0.89Br0.11)Br and is isomorphous with Ph3PSI2. The reaction of R3PS with Br2 is complex. In the case of Ph3PS, phosphorus–sulfur bond cleavage occurs quantitatively to produce Ph3PBr2 and elemental sulfur. Reaction of (Me2N)3PS with Br2 gives, as one product, [{(Me2N)3PS}2S][Br3]2 in moderate (ca. 30%) yield. This unusual dication is compared to the previously reported [(But3PTe)2Te][SbF6]2. Tricyclohexylphosphine sulfide reacts with Br2 in solvents of low relative permittivity (Et2O) to produce the 1∶1 addition complex (C6H11)3PSBr2; however, dissolution of this material in solvents of higher relative permittivity results in phosphorus–sulfur bond cleavage to produce (C6H11)3PBr2 and elemental sulfur.

Both exhibit the charge transfer ‘spoke’ structure consisting of a linear Se–I–X arrangement (X 4;= 4;Br or Cl) where d(C–Se) increases from that seen for mbts by 0.058(5) and 0.062(8) Å (1 and 2 respectively); d(C–N) decreases by 0.019(4) and 0.019(9) Å. This indicates electron density moves towards the Se–I–X moiety upon co-ordination, causing a partial positive charge to reside on the nitrogen atom. The I–X bond lengthens with respect to the unco-ordinated species for both molecules. The addition of two equivalents of ICl to mbts gave a dark purple material of stoichiometry C8H7Cl2.9I1.1NSSe 3; the fractional stoichiometry for iodine and chlorine may be indicative of ‘halogen scrambling’. An additional product from this reaction was examined crystallographically and found to consist of the ion pair [C8H7NSCl]+[ICl2]–4, i.e. the C–Se double bond has been completely cleaved and replaced by a C–Cl single bond. The positive charge is again supported mostly by the nitrogen atom; d(C–N) decreases to 1.294(10) Å indicating a double bond.

Regular readers of the Australian Journal of Physiotherapywill have noticed a number of cosmetic changes made to the journal since it received its last major facelift at the beginning of 1999. Most of these changes have been designed to make the journal a more attractive “read”. But the real changes go more than skin deep. The Editorial Board has been implementing structural changes to the way that the journal is produced and disseminated, with the aim of ensuring that scientifically credible and clinically important information is delivered to as wide an audience as possible.

Three new ‘Chinese lantern’ complexes [XMn(μ-dppO₂)₄MnX]2+2X− ·4MeOH·Me₂CO [X = Cl, Br, I; dppO₂ = 1,3-bis(diphenylphosphinoyl)propane], have been structurally characterised using single-crystal X-ray diffraction and shown to have increasing affinity for SO₂ across the series Cl < Br < I via thermogravimetric measurements.

The title molecule, (iodobromo)triphenylarsenic(III), [As(BrI)(C6H5)3], has a dart shape, with the phenyl rings arranged in a propeller conformation [I—As—C—C 61.3 (4), 43.8 (4) and 54.1 (4)°]. There is no indication that the halogen atoms have mixed site occupancies. Packing forces displace the I atom away from one phenyl ring [I—As—C 117.3 (2)°] towards the other two [I—As—C 109.8 (2) and 108.3 (2)°] and produce an even more pronounced leaning of the terminal bromine [As—I—Br 174.78 (2)°].

The acetic acid esters of 1-(4-methylphenylazo)naphthalen-2-ol 1 and 2-(4-methylphenylazo)-4-methylphenol 3are prepared and characterised by single crystal X-ray diffraction studies and 13C{1H}NMR spectroscopy; the position of the C(2)13C resonance for the ester is used to predict the position of resonant frequency of the equivalent carbon in the parent alcohols and hence, calculate the position of the azo-hydrazone equilibrium in these compounds.
