Background/AimsGlobally, there are three pathways to become a midwife: midwifery post nursing, direct entry midwifery and integrated nursing and midwifery. There is limited knowledge on the effectiveness of pre-service midwifery education. The aim of this study was to describe and compare midwifery students' confidence in intrapartum skills and associated factors such as type and level of education.MethodsA multi-country cross-sectional study was conducted, where midwifery students were approached in the final months of their education programme. Data were collected using a questionnaire, based on the basic skills by the International Confederation of Midwives. Intrapartum care comprised 40 skills.ResultsIn total, 1407 midwifery students from seven sub-Saharan countries responded. The 40 skills were grouped into six domains; three related to care during the first and second stage of labour and three related to care during the third stage of labour. Sex and age were significantly associated with confidence, with female students and those 26–35 years old having higher levels of confidence. Students enrolled in a direct entry programme were more confident than other students in all three domains of care related to the first and second stage of labour.ConclusionsDirect entry was found to result in higher confidence for midwifery students than post nursing programmes or integrated programmes. Further research is needed for evaluation of competence.

Background:Maternal mortality and morbidity rates remain high in sub-Saharan Africa, yet with appropriate management, these rates could be reduced significantly. Midwifery training to improve the management of women during intrapartum period is often lacking, calling for innovative training methods in midwifery clinical care.Aims:To develop and evaluate a game, aimed at the prevention and management of the main causes of maternal morbidity and mortality.Methods:In collaboration with the Lugina Africa Midwives Research Network (LAMRN), the authors developed an educational board game and field tested the final version in Kenya, Tanzania and Malawi with midwives (n=56) and student midwives (n=32).Findings:Qualitative and quantitative evaluation demonstrated that the game was well liked for its ease of use, acceptable appearance and relevance to practice.Conclusions:The board game, named ‘Crisis’, has potential to influence practice; however, further research is required to assess its ability to improve outcomes.

Objective:In sub-Saharan Africa, midwifery input into the generation of important research questions is limited. The authors aimed to address this issue by enabling midwives to set their own research priorities with the intention of developing a research strategy to conduct studies relevant to clinical practice.Methods:A survey was carried out in six countries (Kenya, Malawi, Uganda, Zambia, Zimbabwe and Tanzania). The Delphi method was used with six panels of midwives (n=118) to attain a convergence of opinion on midwifery research priorities. Consensus-building was achieved by using a series of questionnaires delivered using multiple iterations to collect data. Data were analysed using ranked means and proportions.Findings:Consensus was reached on 11 research questions which were considered regional priorities. These covered three key areas: midwifery training, clinical care and organisational issues. Conclusions: Midwives prioritised areas of research that had direct applicability to their own practice. All questions were at the applied end of the research spectrum. Priorities tended to cover broad areas within reproductive health, as opposed to being disease-focused. This is likely to reflect the desire to provide quality care that embraces the social aspects of health. Some of these research priorities are being taken forward by midwives in the group.

Male involvement in maternal and child health has been promoted since the 1994 International Conference on Population and Development in Cairo. Literature from high-resource settings demonstrates the benefits of involving males in maternal and child health including improvement in maternal mental health, increased assistance from partners and reduced stay in hospital; however, little is known about the situation in low-income countries. This paper aims to provide a descriptive review of qualitative studies on experiences of men who supported their partners during labour and birth in low-resource settings. Themes identified from the literature include: unpreparedness, pleasant but frightening experience, loss of interest in wife and lack of privacy. Further good quality studies should be performed in addition to the need for adequate preparation of men for involvement during labour and birth.

This study aimed to determine the factors that influence the use of prevention of mother to child transmission of HIV (PMTCT) services among young HIV-positive adults in the Balaka district of Malawi. A questionnaire was given to 184 HIV-positive young mothers aged 15–24 years who met the inclusion criteria and who consented to the study. The participants were selected from six health centres in the Balaka district during their visits to the under 5 clinic, HIV and AIDS support groups and HIV follow up clinic. Results showed that the participants’ knowledge of mandatory HIV testing, stigma and discrimination, and poverty influenced their use of PMTCT services. There is a need to mobilise the communities to promote PMTCT awareness and reduce stigma and discrimination towards HIV-positive mothers. Monitoring and evaluation activities and in-service education for health workers is also recommended to promote utilisation of PMTCT services in the district.

PURPOSE:To determine knowledge and practices of young women on utilization of prevention of mother-to-child transmission (PMTCT) of HIVservices.DESIGN:A cross-section descriptive study that used quantitative and qualitative data analysis methodologies.FINDINGS:Knowledge of PMTCT of HIV among respondents was universal. All respondents (100%, N= 184) stated that utilization of PMTCT services reduces transmission of HIV from mother to child. Respondents knew that HIV transmission can be reduced with exclusive breastfeeding (93%, n= 172), abrupt weaning at 6 months (44%, n= 81), taking single-dose nevirapine (SD-NVP; 43%, n= 79), and giving NVP to the baby (65%, n= 120). Very few respondents (4%, n= 7) stated that avoiding pregnancy is one way of preventing HIV transmission and that a mother who is HIV positive who has received SD-NVP or antiretroviral (ARV) therapy can still deliver a baby who is HIV positive. Actual practice was very low; only 14% breastfed exclusively and only 3% weaned their babies abruptly. Although all the 184 mothers were given NVP to take at onset of labor, very few respondents (22%) took NVP as recommended. Although it was recommended that all babies take NVP at birth and within 72 hr of birth, only 58% of the babies received NVP as recommended and only 3% of the women avoided pregnancy.CONCLUSION:There was a big discrepancy between knowledge and practice of PMTCT services. Culture was the major barrier because traditionally, babies are expected to be breastfed and supplements are fed to babies, too. Most mothers did not adhere to the taking of NVP at onset of labor. Therefore, there is a need to mobilize communities on PMTCT of HIV. The HIV education programmes should emphasize behavior-change interventions and should focus on both men and women, their partners, and significant others. There is also need to intensify monitoring and evaluation of health workers’ activities to ensure that knowledge is put into practice.

In sub-Saharan Africa midwifery educators are in short supply and opportunities to pursue advanced education are severely restricted. Postgraduate programmes that encourage critical thinking and strategic planning in midwifery education, practice, management and research are required to empower midwives to lead their profession.A unique user-led distance learning programme was developed by adopting a participatory approach to developing a curriculum for a Masters in Midwifery and Women's Health. Midwives from the East Central and South Africa (ECSA) region and the UK participated along with a representative from the Commonwealth Secretariat and International Confederation of Midwives (ICM). The curriculum was based on shared goals but adaptable to cultural and local context. Brainstorming sessions, informal workshops and formal consensus methods were used to reach decisions regarding modules and subsequent content.This article describes the process of developing a shared curriculum, the challenges faced in working across and within regions, and participants’ views of contributing to the end product.

ABSTRACTBackground: Evidence-based antenatal care is one cornerstone in Safe Motherhood and educated and confident midwives remain to be optimal caregivers in Africa. Confidence in antenatal midwifery skills is important and could differ depending on the provision of education among the training institutions across Africa.Objective: The aim of the study was to describe and compare midwifery students’ confidence in basic antenatal skills, in relation to age, sex, program type and level of program.Methods: A survey in seven sub-Saharan African countries was conducted. Enrolled midwifery students from selected midwifery institutions in each country presented selfreported data on confidence to provide antenatal care. Data were collected using a selfadministered questionnaire. The questionnaire consisted of 22 antenatal skills based on the competency framework from the International Confederation of Midwives. The skills were grouped into three domains; Identify fetal and maternal risk factors and educate parents; Manage and document emergent complications and Physical assessment and nutrition.Results: In total, 1407 midwifery students from seven Sub-Saharan countries responded. Almost one third (25-32%) of the students reported high levels of confidence in all three domains. Direct entry programs were associated with higher levels of confidence in all three domains, compared to post-nursing and double degree programs. Students enrolled at education with diploma level presented with high levels of confidence in two out of three domains.Conclusions: A significant proportion of student midwives rated themselves low on confidence to provide ANC. Midwifery students enrolled in direct entry programs reported higher levels of confidence in all domains. It is important that local governments develop education standards, based on recommendations from the International Confederation of midwives. Further research is needed for the evaluation of actual competence.

BackgroundDespite the documented benefits of prevention of mother-to-child transmission (PMTCT) of human immunodeficiency virus (HIV) services, the uptake remains low in sub-Saharan Africa. The lack of male involvement (MI) may be one of the reasons for this. However, there are limited data on strategies for MI in PMTCT.ObjectiveThe objective of this study was to identify strategies that may promote MI in PMTCT services in antenatal care (ANC) services in Blantyre, Malawi.Study designAn exploratory qualitative study was conducted from December 2012 to January 2013 at South Lunzu Health Centre (SLHC) in Blantyre, Malawi. It consisted of six face-to-face key informant interviews (KIIs) with healthcare workers and four focus group discussions (FGDs) with 18 men and 17 pregnant women attending ANC at SLHC. The FGDs were divided according to sex and age. All FGDs and KIIs were digitally recorded and simultaneously transcribed and translated verbatim into English. Data were analyzed using thematic content analysis.ResultsThree major themes with several subcategories emerged. Theme 1 was a gatekeeping strategy with two subcategories: (1) healthcare workers refusing service provision to women accessing antenatal clinic without their partners and (2) women refusing ANC attention in the absence of a partner. Theme 2 comprised extending invitations and had six subcategories: (1) word of mouth, (2) card invites, (3) woman's health passport book invites, (4) telephonic invites, (5) use of influential people, and (6) home visits. Theme 3 was information education and communication, such as health education forums and advertisements. Of all the strategies, an invitation card addressed to the male partner was most preferred by study participants.ConclusionsThere are several strategies by which men may be involved in PMTCT. Healthcare workers should offer a pregnant woman all strategies available for MI for her to select the appropriate one. Further research and consultations with men should continue to achieve higher levels of MI.
