Chromosomal rearrangements of the human KMT2A/MLLgene are associated with de novoas well as therapy-induced infant, pediatric, and adult acute leukemias. Here, we present the data obtained from 3401 acute leukemia patients that have been analyzed between 2003 and 2022. Genomic breakpoints within the KMT2Agene and the involved translocation partner genes (TPGs) and KMT2A-partial tandem duplications (PTDs) were determined. Including the published data from the literature, a total of 107 in-frame KMT2Agene fusions have been identified so far. Further 16 rearrangements were out-of-frame fusions, 18 patients had no partner gene fused to 5’-KMT2A, two patients had a 5’-KMT2Adeletion, and one ETV6::RUNX1patient had an KMT2Ainsertion at the breakpoint. The seven most frequent TPGs and PTDs account for more than 90% of all recombinations of the KMT2A, 37 occur recurrently and 63 were identified so far only once. This study provides a comprehensive analysis of the KMT2Arecombinome in acute leukemia patients. Besides the scientific gain of information, genomic breakpoint sequences of these patients were used to monitor minimal residual disease (MRD). Thus, this work may be directly translated from the bench to the bedside of patients and meet the clinical needs to improve patient survival.

Minimal residual disease (MRD) is a powerful prognostic factor in acute lymphoblastic leukemia (ALL) and is used for patient stratification and treatment decisions, but its precise role in Philadelphia chromosome positive ALL is less clear. This uncertainty results largely from methodological differences relating to the use of real-time quantitative PCR (qRT-PCR) to measure BCR-ABL1transcript levels for MRD analysis. We here describe the first results by the EURO-MRD consortium on standardization of qRT-PCR for the e1a2 BCR-ABL1transcript in Ph + ALL, designed to overcome the lack of standardisation of laboratory procedures and data interpretation. Standardised use of EAC primer/probe sets and of centrally prepared plasmid standards had the greatest impact on reducing interlaboratory variability. In QC1 the proportion of analyses with BCR-ABL1/ABL1ratios within half a log difference were 40/67 (60%) and 52/67 (78%) at 10−3and 36/67 (53%) and 53/67 (79%) at 10−4BCR-ABL1/ABL1. Standardized RNA extraction, cDNA synthesis and cycler platforms did not improve results further, whereas stringent application of technical criteria for assay quality and uniform criteria for data interpretation and reporting were essential. We provide detailed laboratory recommendations for the standardized MRD analysis in routine diagnostic settings and in multicenter clinical trials for Ph + ALL.

Chromosomal rearrangements of the human MLL/KMT2A gene are associated with infant, pediatric, adult and therapy-induced acute leukemias. Here we present the data obtained from 2345 acute leukemia patients. Genomic breakpoints within the MLL gene and the involved translocation partner genes (TPGs) were determined and 11 novel TPGs were identified. Thus, a total of 135 different MLL rearrangements have been identified so far, of which 94 TPGs are now characterized at the molecular level. In all, 35 out of these 94 TPGs occur recurrently, but only 9 specific gene fusions account for more than 90% of all illegitimate recombinations of the MLL gene. We observed an age-dependent breakpoint shift with breakpoints localizing within MLL intron 11 associated with acute lymphoblastic leukemia and younger patients, while breakpoints in MLL intron 9 predominate in AML or older patients. The molecular characterization of MLL breakpoints suggests different etiologies in the different age groups and allows the correlation of functional domains of the MLL gene with clinical outcome. This study provides a comprehensive analysis of the MLL recombinome in acute leukemia and demonstrates that the establishment of patient-specific chromosomal fusion sites allows the design of specific PCR primers for minimal residual disease analyses for all patients.

Children with P2RY8-CRLF2-positive acute lymphoblastic leukemia have an increased relapse risk. Their mutational and transcriptional landscape, as well as the respective patterns at relapse remain largely elusive. We, therefore, performed an integrated analysis of whole-exome and RNA sequencing in 41 major clone fusion-positive cases including 19 matched diagnosis/relapse pairs. We detected a variety of frequently subclonal and highly instable JAK/STAT but also RTK/Ras pathway-activating mutations in 76% of cases at diagnosis and virtually all relapses. Unlike P2RY8-CRLF2 that was lost in 32% of relapses, all other genomic alterations affecting lymphoid development (58%) and cell cycle (39%) remained stable. Only IKZF1 alterations predominated in relapsing cases (P=0.001) and increased from initially 36 to 58% in matched cases. IKZF1’s critical role is further corroborated by its specific transcriptional signature comprising stem cell features with signs of impaired lymphoid differentiation, enhanced focal adhesion, activated hypoxia pathway, deregulated cell cycle and increased drug resistance. Our findings support the notion that P2RY8-CRLF2 is dispensable for relapse development and instead highlight the prominent rank of IKZF1 for relapse development by mediating self-renewal and homing to the bone marrow niche. Consequently, reverting aberrant IKAROS signaling or its disparate programs emerges as an attractive potential treatment option in these leukemias.

La voie de signalisation cellulaire des RAS/MAP Kinases (RAS/MAPK) joue un rôle majeur sur le devenir des cellules en participant à la régulation de la prolifération, de la différentiation et de la survie cellulaire. Les mutations des gènes RASet des gènes de leurs effecteurs et régulateurs participent à l’oncogenèse de nombreux cancers sporadiques. Par ailleurs, au cours de ces 20 dernières années, des mutations constitutionnelles de ces mêmes gènes ont été identifiées dans plusieurs syndromes cliniques associant, de façon variable, une cardiopathie ou une cardiomyopathie, une dysmorphie, une déficience intellectuelle, des anomalies cutanées et un retard de croissance. Ces différents syndromes (syndrome de Noonan, de Costello, syndrome cardio-facio-cutané…), liés à une dérégulation de la voie RAS/MAPK, sont aujourd’hui regroupés sous le terme RASopathie. En plus des anomalies du développement qui les caractérisent, les RASopathies engendrent un risque accru de tumeurs malignes dans l’enfance. Ces patients doivent donc bénéficier d’une surveillance et d’une prise en charge adaptées à ce risque.

We have developed a strategy based on polymerase chain reaction (PCR) for detecting all possible γT-cell receptor (γTCR) rearrangements and the most common δTCR rearrangements found in B-lineage and T-acute lymphoblastic leukemia (T-ALL). The segments amplified from blasts are then directly sequenced to derive clonospecific probes. From a series of 45 patients aged 1 to 15 years (42 B-lineage ALL, 3 T-ALL), 35 (83%) could be followed for minimal residual disease with at least one clonospecific probe. Detection of clonal markers using clonospecific probes routinely allowed the detection of 1 to 10 blasts out of 106cells as determined by serial dilutions of the initial samples. Residual disease was quantitated by a competitive PCR assay based on the coamplification of an internal standard. Twenty children were prospectively followed for periods varying from 7 to 30 months. In most children, a progressive decrease of the tumor load was observed, and blasts became undetectable within 6 months after the initiation of treatment. A slower kinetics of decrease in tumor cells was found in three children. These three patients relapsed with blasts that continued to display the initial clonospecific markers. Three other children had a central nervous system relapse despite the absence of detectable medullary residual disease. The use of both Sand γTCR genes as clonal markers, as well as simplification in the methods to detect and quantify residual blasts reported here, will allow the study of the large number of patients required to determine the role of the detection of minimal residual disease by PCR in the follow-up of childhood ALL.

Le syndrome de Costello (SC) est une RASopathie liée à la mutation du gène HRAS. Elle est caractérisée par une dysmorphie faciale, un retard psychomoteur, des anomalies cardiaques et musculo-squelettiques et un risque tumoral augmenté. Les manifestations dermatologiques du SC sont constantes et caractéristiques mais restent peu connues. Les objectifs de l’étude étaient :

Le syndrome de Noonan (SN) est une RASopathie liée à la mutation des gènes PTPN11, SOS1, RIT1, KRAS, NRAS, RAF1, SHOC2, SOS2, LZTR1 et CBL. Elle se caractérise par une dysmorphie faciale, une petite taille, des anomalies cardiaques et neurologiques. Deux phénotypes de SN sont identifiés : le SN avec lentigines multiples (SNLM) et le SN avec cheveux anagènes caducs (SNCAC). Les objectifs de l’étude étaient (i) de décrire les manifestations dermatologiques des patients atteints de SN, SN-LM et SN-CAC ; (ii) de les comparer aux données exhaustives de la littérature internationale et ; (iii) d’établir une éventuelle corrélation phénotype-génotype en fonction de la présence ou non de la mutation de PTPN11, principal gène incriminé au cours du SN ou SNLM.

Les nævi pigmentaires multiples congénitaux (NPMC) résultent de mutations activatrices post-zygotiques dans le gène NRAS. Des anomalies extra-dermatologiques, souvent neurologiques, mélanocytaires ou non, sont parfois observées, suggérant un cadre nosologique plus large : syndrome du nævus pigmentaire congénital. Exceptionnellement, un rachitisme hypophosphatémique est rapporté dans ce syndrome.

Les rasopathies désignent des affections génétiques liées à des anomalies de la voie RAS-MAPKinase et comportant des caractères phénotypiques communs : dysmorphie faciale, anomalies squelettiques, neurologiques, cardiaques et prédisposition inconstante aux tumeurs. Les manifestations dermatologiques restent mal définies en raison du caractère rétrospectif des études, rarement conduites par des équipes dermatologiques et souvent antérieures à l’identification des gènes en cause.

Le diabète « néonatal » est une maladie génétique rare (1 naissance vivante sur 90 000 à 260 000). Il se définit par l’existence d’hyperglycémies liées à l’insuffisance ou à l’absence d’insuline circulante et survenant avant l’âge de un an. Ces hyperglycémies nécessitent un traitement soit de façon transitoire dans environ la moitié des cas, soit de façon permanente. Deux grands groupes de mécanisme expliquent la maladie : anomalie de développement du pancréas ou anomalie de fonction de la cellule β pancréatique. Les causes génétiques les plus fréquentes de diabète néonatal avec anomalie de fonction de la cellule β sont les anomalies du locus 6q24 et les mutations des gènes ABCC8, KCNJ11, codant pour les sous-unités du canal potassique de la cellule bêta impliqués dans le lien entre métabolisme de celle-ci et sécrétion d’insuline. D’autres gènes sont associés à une malformation du pancréas ou à une destruction des cellules β. Comparés aux patients avec une mutation dans ABCC8ou KCNJ11, les patients avec anomalie de 6q24 ont un poids et une taille de naissance plus faibles, un âge au diagnostic et de rémission plus précoces et une fréquence plus importante de malformations. Les patients avec une mutation dans ABCC8ou KCNJ11ont des troubles neurologiques et neuropsychologiques chez tous ceux testés finement. Jusqu’à 86 % des patients ayant une rémission récidivent leur diabète à l’âge pubertaire, sans qu’il existe une différence selon la cause génétique. Ces résultats plaident pour l’importance d’un suivi au long cours par une équipe multidisciplinaire pédiatrique puis de médecins d’adultes.
