Preclinical models recently unveiled the vulnerability of LKB1/KRAScomutated NSCLC to metabolic stress-based treatments. Because miR-17 is a potential epigenetic regulator of LKB1, we hypothesized that wild-type LKB1 (LKB1WT)NSCLC with high miR-17 expression may be sensitive to an energetic stress condition, and eligible for metabolic frailties-based therapeutic intervention.

La loi Claeys-Léonetti modifie de manière importante les dispositions concernant la fin de vie. Elles ont pour objectif de mettre en avant les décisions et les choix des patients en rendant opposables leurs directives anticipées et en leur permettant d’opter pour une sédation profonde et continue maintenue jusqu’à la fin de la vie. Ces modifications mettent en jeu les pratiques des soignants, qu’ils soient médecins ou personnels soignants. Elles ont une répercussion particulière dans les Ehpad, où sont accueillis une grande proportion de sujets en perte d’autonomie et souvent dans l’incapacité d’énoncer leurs volontés. À partir d’un cas clinique, nous mènerons une réflexion sur les enjeux et les difficultés rencontrées afin de traiter cette situation où sont impliqués de nombreux intervenants.

À la suite de la réflexion de professionnels travaillant en établissement d’hébergement pour personnes âgées dépendantes (Ehpad) sur la liberté d’aller et venir en institution, une enquête a été menée auprès de résidents afin de recueillir leur sentiment sur leur propre exercice de la liberté en établissement. Dans la majeure partie des cas, la plus grande entrave à leur liberté est causée par leurs difficultés liées à leur avancée en âge. Cependant la majorité conserve le souhait de rester en contact avec l’extérieur de l’établissement, alors même que les plus fragiles ne sont pas toujours capables de l’exprimer clairement.

Rad53 protein, the yeast orthologue of the human checkpoint kinase Chk2, presents two highly conserved phosphorylatable threonine residues (T354 and T358) in the activation domain, whose phosphorylation is critical to allow the activation of the kinase. In this study we found that Rad53 protein variants in which alanine and/or aspartate replace the threonine residues 354 and/or 358 do not retain kinase activity and do not undergo auto-phosphorylation, leading to defect in the checkpoint response and iper-sensitivity to DNA damage and DNA replication stress agents. Interestingly, we found that the rad53-T358D mutation severely affects the kinase activity and causes accumulation of the S129-phosphorylated isoform of histone H2A, even during an unperturbed cell cycle, thus indicating the accumulation of spontaneous DNA breaks. We further found that the protein level of Sml1, which is the physiological inhibitor of ribonucleotide reductase, remains high during DNA replication in rad53-T358D cells, suggesting that an inadequate pool of dNTPs in checkpoint defective cells causes the accumulation of spontaneous DNA breaks. In conclusion, our results indicate that phosphorylation of both T354 and T358 residues strongly influences the catalytic activity of Rad53 also in unperturbed cell cycles, and support the notion that Rad53 is essential to preserve genome integrity, by controlling the level of Sml1 and the functionality of ribonucleotide reductase.

Objective.Preventing hospital falls and injuries requires knowledge of fall and injury circumstances. Our objectives were to determine whether reported fall circumstances differ among hospitals and to identify predictors of fall-related injury.Design.Retrospective cohort study. Adverse event data on falls were compared according to hospital characteristics. Logistic regression was used to determine adjusted odds ratios (aORs) with 95% confidence intervals (CIs) for risk factors for fall-related injury.Setting.Nine hospitals in a Midwestern healthcare system.Patients.Inpatients who fell during 2001-2003.Results.The 9 hospitals reported 8,974 falls that occurred in patient care areas, involving 7,082 patients; 7,082 falls were included in our analysis. Assisted falls (which accounted for 13.3% of falls in the academic hospital and 9.8% of falls in the nonacademic hospitals; P<.001) and serious fall-related injuries (which accounted for 3.7% of fall-related injuries in the academic hospital and 2.2% of fall-related injuries in the nonacademic hospitals; P<.001) differed by hospital type. In multivariate analysis for the academic hospital, increased age (aOR, 1.006 [95% CI, 1.000-1.012]), falls in locations other than patient rooms (aOR, 1.53 [95% CI, 1.03-2.27]), and unassisted falls (aOR, 1.70 [95% CI, 1.23-2.36]) were associated with increased injury risk. Altered mental status was associated with a decreased injury risk (aOR, 0.72 [95% CI, 0.58-0.89]). In multivariate analysis for the nonacademic hospitals, increased age (aOR, 1.007 [95% CI, 1.002-1.013]), falls in the bathroom (aOR, 1.46 [95% CI, 1.06-2.01]), and unassisted falls (aOR, 1.83 [95% CI, 1.37-2.43]) were associated with injury. Female sex (aOR, 0.83 [95% CI, 0.71-0.97]) was associated with a decreased risk of injury.Conclusion.Some fall characteristics differed by hospital type. Further research is necessary to determine whether differences reflect true differences or merely differences in reporting practices. Fall prevention programs should target falls involving older patients, unassisted falls, and falls that occur in the patient's bathroom and in patient care areas outside of the patient's room to reduce injuries.

ABSTRACTMethods that used specimens from three genital sites (penile brushing [PB], urethral brushing [UB], and the retrieval of semen [SE]) from 50 men were examined for human papillomavirus (HPV) DNA detection. The rates of detection by PB, UB, SE, PB and UB, and PB and SE were 88.9%, 50.0%, 33.3%, 100%, and 97.2%, respectively. The use of PB and UB appears to be the most accurate method; as an alternative to UB, the use of SE with PB could be used to improve the rate of HPV DNA detection in men.

ABSTRACTA retrospective analysis by molecular-sequence-based techniques was performed to correctly identify the etiological agent of 24 Mediterranean spotted fever cases occurring in Western Sicily, Italy, from 1987 to 2001. Restriction analysis of a 632-bp PCR-amplified portion of the ompAgene allowed presumptive identification of five clinical isolates as belonging to Rickettsia conoriisubsp. israelensis, the etiological agent of Israeli spotted fever (ISF). The remaining 19 rickettsial isolates were Rickettsia conoriisubsp. conorii, the only pathogenic rickettsia of the spotted fever group reported in Italy until the present. Sequence analysis of the ompAgene confirmed the identification of all the R. conoriisubsp. israelensisisolates and demonstrated that rickettsiosis caused by R. conoriisubsp. israelensiscan be traced back to 1991 in Sicily. The recorded clinical data of the five ISF patients support the idea that these strains could correlate to more-severe forms of human disease. Three of five patients experienced severe disease, and one of them died.

A new method for rearing the citrus flower moth (Prays citriMill.) (Lepidoptera, Yponomeutidae) on lemon [Citrus limon(L.) Burm.] callus is reported. In the present research callus (an undifferentiated mass of plant cells that can be grown under sterile conditions on an artificial medium in vitro) was induced from lemon stigma and style explants cultured on Murashige and Skoog (MS) medium supplemented with 500 mg l−1malt extract, 13.3 μM 6‐benzylaminopurine, and 146 mM sucrose. Also somatic embryogenesis and plant regeneration were obtained from the cultures of styles and stigmas of lemon. Adults were obtained from larvae on infested flowers collected in the field. Different oviposition substrates were used: white oval pearls (WOP), black oval pearls (BOP), rooted shoots (RS) of lemon obtained in vitro, and artificial flowers containing lemon callus (AF). Larvae were reared on lemon callus. Adults oviposited on RS, on WOP, and on AF. BOP were rejected as oviposition substrates. The flower moth reared on callus oviposited fertile eggs. In our tests P. citricompleted three generations on callus or on callus plus shoots. In the latter case the larvae preferred callus and fed on shoots only after callus was completely eaten. The life cycle on callus at 23 ± 1 °C lasted about 21 days. There were significant differences between oviposition substrates for what concerns the number of eggs laid. It was observed that females generally preferred WOP (about 25 eggs/female) to AF (about 20 eggs/female) or RS (about 12 eggs/female) as oviposition substrate. Nevertheless the percentage of eggs that developed into adults was higher when AF sealed with stretched Parafilm were used (about 70% of eggs developed into adults). The method of rearing P. citriwith AF was labour‐saving and the feeding substrate (callus) had less tendency to become mouldy or decompose than when WOP and RS were used. Since such a diet is available for the insect all year round and callus can be produced in unlimited quantity, it could be possible to obtain a mass production of this moth.

The genus Encyrtoscelio Dodd is revised. E. apterus (Szelényi), E. mirissimus Dodd, E. turneri Waterston are redescribed, and six new species are described: E. cydni Caleca, E. japonicus Caleca, E. mediterraneus Caleca, E. miroides Caleca, E. spuratus Caleca and E. undecim Caleca. A key to separate females is presented, and is based on the number of antennomeres and claval sensillar formula, mandibular spurs and teeth, palpal formula and clypeus, and some cephalic characters. Morphological adaptations of the head and mandibles are discussed in relation to host habitat and oviposition site. Information on host associations, habitat preference, seasonal and daily occurrence of some species are also presented.

The monoterpenoid citral synergized the electroantennogram (EAG) response of male Grapholita molesta (Busck) antennae to its main pheromone compound Z8-12:OAc. The response to a 10-µg pheromone stimulus increased by 32, 45, 54, 71 and 94% with the addition of 0.1, 1, 10, 100 and 1,000 µg of citral, respectively. There was no detectable response to 0.1, 1, or 10 µg of citral; the response to 100 and 1,000 µg of citral was 31 and 79% of the response to 10 µg of Z8-12:OAc. In a flight tunnel, citral affected the mate-seeking behavior of males. There was a 66% reduction in the number of males orientating by flight to a virgin calling female when citral was emitted at 1,000 ng/min ˜1 cm downwind from a female. Pheromone and citral induced sensory adaptation in male antennae, but citral did not synergize the effect of pheromone. The exposure of antennae to 1 ng Z8-12:OAc/m3 air, 1 ng citral/m3 air, 1 ng Z8-12:OAc + 1 ng citral/m3 air, or to 1 ng Z8-12:OAc + 100 ng citral/m3 air for 15 min resulted in a similar reduction in EAG response of 47-63%. The exposure of males to these same treatments for 15 min had no effect on their ability to orientate to a virgin calling female in a flight tunnel. The potential for using citral to control G. molesta by mating disruption is discussed.
