The use of left ventricular assist devices has become common for the treatment of end-stage heart failure, both as a bridge to transplantation and as destination therapy. The nature of these devices and the comorbid conditions of the patients in whom the devices are implanted lead to high rates of device infection that are related directly to mortality.

Implant-associated anaplastic large cell lymphoma (ALCL) is the subject of much debate in the field of plastic surgery. Only a few published cases have been reported and the rarity of the disease may make proving causality exceedingly difficult. Despite this, it is of utmost importance that full attention be devoted to this subject to ensure the safety and well-being of patients. The authors report one new case of implant-associated ALCL that recently presented to their institution. Implant-associated ALCL is a poorly understood disease. It should likely be considered its own clinical entity and categorized into two subtypes: one presenting as a seroma and the other as a distinct mass or masses. When reported, only textured implants have been associated with ALCL. The United States Food and Drug Administration and American Society of Plastic Surgeons have initiated a registry and have collected critical data to gain further understanding of this disease.

The treatment of pediatric facial fractures, although similar to that of adults, requires a separate and thorough understanding of the unique developmental issues inherent to this age group. The contribution of several of these factors allows for a large portion of these injuries to be managed more conservatively; however, operative indications still exist. The optimal form of management in these situations must balance the risks of impacting dentition or future skeletal growth versus obtaining acceptable stability and reduction for healing. Although these principles have remained largely unchanged over the years, a more current discussion on the state of resorbable and titanium fixation is offered to highlight evolving management considerations. Although uniquely challenging, the proper management of pediatric facial trauma is possible if the treating physician remains aware of key anatomic, epidemiological, evaluation, and management issues.

Of all the methods for repair of the unilateral cleft lip, none has gained as much popularity as the rotation-advancement. Millard’s original principle of 50 years ago continues to guide surgeons in closure of the cleft lip. Unlike earlier procedures, the brilliance of the rotation-advancement is that it permits individual manipulation and modifications while maintaining Millard’s original surgical and anatomical goals. Millard and many other surgeons have made modifications to adjust the procedure to each specific patient, to address some of its faults, and to gain new advantages. In this article, the authors review the techniques of Drs. Ralph Millard, Steve Byrd, Court Cutting, John Mulliken, and Samuel Stal. The variations from Millard’s original technique are highlighted, including a discussion of the benefits of each modification.

The North Dakota Thunderstorm Project was conducted in the Bismarck, North Dakota, area from 12 June through 22 July 1989. The project deployed Doppler radars, cloud physics aircraft, and supporting instrumentation to study a variety of aspects of convective clouds. These included transport and dispersion; entrainment; cloud-ice initiation and evolution; storm structure, dynamics, and kinematics; atmospheric chemistry; and electrification.Of primary interest were tracer experiments that identified and tracked specific regions within evolving clouds as a means of investigating the transport, dispersion, and activation of ice-nucleating agents as well as studying basic transport and entrainment processes. Tracers included sulfur hexafluoride (SF6), carbon monoxide, ozone, radar chaff, and silver iodide.Doppler radars were used to perform studies of all scales of convection, from first-echo cases to a mesoscale convective system. An especially interesting dual-Doppler study of two splitting thunderstorms has resulted.The objectives of the various project experiments and the specific facilities employed are described. Project highlights and some preliminary results are also presented.

Simulated WSR-88D (Weather Surveillance Radar-1988 Doppler) radar data were used to investigate the effects of discrete azimuthal sampling on Doppler velocity signatures of modeled mesocyclones and tornadoes at various ranges from the radar and for various random positions of the radar beam with respect to the vortices. Results show that the random position of the beam can change the magnitudes and locations of peak Doppler velocity values. The important implication presented in this study is that short-term variations in tornado and far-range mesocyclone intensity observed by a WSR-88D radar may be due to evolution or due to the chance positions of the radar beam relative to the vortex’s maximum rotational velocities or due to some combination of both.

Significant advances in single-Doppler radar application to severe storm study and identification have been made since 1965. Mesocyclones have been detected by Doppler radar and found to precede severe weather occurrence by several tens of minutes. A typical mesocyclone evolution leading to tornado development has also been documented. The tornado vortex itself has a revealing signature in Doppler radar data, the tornadic vortex signature (TVS). Statistics of both mesocyclone and TVS association with confirmed severe weather are presented in this paper. Doppler radar provides the potential for improving severe thunderstorm warnings. Experiments are underway to test the operational use of this new tool in storm warning and flight advisory services.

Single Doppler radar measurements were made in a squall line that formed in southern Kansas during the afternoon of 2 June 1971 and moved south-southeastward through central Oklahoma. During the period of data collection, a pronounced hook echo, having at least one funnel cloud associated with it, developed. Preliminary analyses of these first Doppler velocity measurements within a radar hook echo in the tornado belt are presented.

Some results of the first 10 cm dual-Doppler radar measurements in a tornadic storm are presented. A mesoscale cyclonic circulation confirms proposed single Doppler vortex signature and indicates that the curved reflectivity hook echo is around the periphery of the circulation. The interpolated tornado position is within the mesocyclone where high-variance Doppler velocity spectra suggest strong velocity gradients.
