I. Introduction Gambling self-exclusion agreements are a form of precommitment whereby a person may request to be prevented from gambling for some future period or indefinitely. (1) They are used [...], Gambling self-exclusion agreements enable a person to have themselves prevented from gambling for some future period. In light of evidence of their effectiveness in helping problem gamblers manage their addiction, these agreements enjoy growing popularity. In particular, several jurisdictions now oblige gambling operators to offer self-exclusion to their clientele. If self-exclusion has a unique value that is distinct from paternalistic measures, such as forced exclusion, it is surely because it prizes the gambler's autonomy. In this article, however, I will argue that self-exclusion's theoretical basis cannot, in fact, be found in a procedural theory of autonomy that only regards agents' own values and decisions. Rather, I will contend that if agents may bind their future selves in only some ways--for example, by preventing themselves from gambling but not preventing themselves from self-excluding or selling themselves into slavery--it can only be because of a normative, substantive claim. Background: Multimorbidity is a prevalent risk factor for COVID-19-related complications and death. We sought to evaluate the association of homologous booster vaccination using BNT162b2 (Pfizer-BioNTech) or CoronaVac (Sinovac) with COVID-19-related deaths among people with multimorbidity during the initial Omicron wave of the COVID-19 pandemic. Methods: Using routine clinical records from public health care facilities in Hong Kong, we conducted a territory-wide retrospective cohort study comparing people aged 18 years or older with 2 or more chronic conditions who received a homologous booster (third) dose with those who received only 2 doses, between Nov. 11, 2021, and Mar. 31, 2022. The primary outcome was death related to COVID-19. Results: We included 120 724 BNT162b2 recipients (including 87 289 who received a booster), followed for a median of 34 (interquartile range [IQR] 20-63) days and 127 318 CoronaVac recipients (including 94 977 who received a booster), followed for a median of 38 (IQR 22-77) days. Among BNT162b2 recipients, booster-vaccinated people had fewer COVID-19-related deaths than those who received 2 doses (5 v. 34, incidence rate 1.3 v. 23.4 per million person-days, weighted incidence rate ratio [IRR] 0.05, 95% confidence interval [CI] 0.02-0.16). We observed similar results among recipients of CoronaVac booster vaccination compared with those who received only 2 doses (26 v. 88, incidence rate 5.3 v. 53.1 per million person-days, weighted IRR 0.08, 95% CI 0.05-0.12). Interpretation: Among people with multimorbidity, booster vaccination with BNT162b2 or CoronaVac was associated with reductions of more than 90% in COVID-19-related mortality rates compared with only 2 doses. Background: People of African Nova Scotian (ANS) ancestry are a culturally distinct group who experience numerous socioeconomic inequities and health disparities, secondary to structural and social determinants of health. Understanding the experiences of ANS health practitioners is important in addressing antiBlack racism in health care. We sought to critically examine the leadership experiences of ANS nurses in health care practice. Methods: We used Black feminist theory to guide this qualitative study. We conducted 1-on-1 semistructured telephone interviews with ANS nurses and analyzed interview transcripts using Critical Discourse Analysis. Results: We interviewed 18 nurses of ANS ancestry. We conceptualized study findings in 3 overarching areas: People of ANS ancestry as a distinct people, institution of care, and leadership philosophy and practice. Each area, and its corresponding themes and subthemes, illustrated an emergent understanding of factors that influence leadership among ANS nurses, such as socialization, early exposure to care and diversity in health care. Participants perceived and practised leadership in a manner that transcended formal titles or designations. Interpretation: African Nova Scotian ancestry is implicated in the perception and practice of leadership among ANS nurses, who considered leadership to be a fundamental component of nursing practice that was grounded in community-oriented care. This article examines the experiences of local union presidents with respect to gender equality. Based on a qualitative case study of members of union committees dedicated to the status of women and local union presidents working in the Quebec education sector, our research points to an unprecedented breakthrough of women serving as local presidents. Nevertheless, the existence of a 'triple burden,' the persistence of gender stereotypes and a male union culture, the lack of training and mentoring, and the reliance on solicitation are all hindering the achievement of gender equality. Our research findings also highlight a weak representation of the diversity of social identities within the union organization studied. This article examines the experiences of local union presidents with respect to gender equality. In a context where equity, diversity and inclusion in the workforce are more relevant than ever, this paper aims to contribute to reflections on the democratic deficit of female representation in union organizations. The findings are based on a qualitative case study of members of two union committees dedicated to the status of women and sixteen local union presidents working in the Quebec education sector. By gathering the views of these women and men, our research makes a case for the long-neglected place of women serving as local presidents within the union organization studied. It is the efforts made by the unions themselves that have gradually allowed women to enter the traditionally male-dominated union world. Behind this unprecedented breakthrough, however, are gaps that remain in achieving gender equality. Beyond a continuing numerical deficit of women, the difficulty of managing a 'triple burden,' the persistence of gender stereotypes, the prevalence of a male union culture, the lack of training and mentoring, and the use of solicitation are major obstacles that prevent women from accessing local union leadership positions. In addition, the representation of diverse social identities (age, family status, race, etc.) is still not well represented at the local decision-making level of the union organization studied, even among elected women. Diversity and gender equality representation remain challenges to be prioritized to ensure a more sustainable and democratic union organization. A partir d'une etude de cas qualitative menee aupres des membres de comites syndicaux de la condition feminine et de presidences syndicales locales du secteur de l'education du Quebec, notre recherche fait valoir une percee sans precedent des femmes siegeant comme presidentes locales. Neanmoins, l'existence d'une <>, la persistance de stereotypes de genre et d'une culture syndicale masculine, le manque de formation et de mentorat ainsi que le recours a la sollicitation nuisent a l'atteinte de l'egalite de genre. Nos resultats de recherche font aussi ressortir une faible representation de la diversite des identites sociales au sein de l'organisation syndicale etudiee. Cet article porte sur l'experience des presidences impliquees dans les organisations syndicales locales au regard de l'egalite de genre. Dans un contexte ou l'equite, la diversite et l'inclusion de la main-d'oeuvre sont plus que jamais d'actualite, cet ecrit contribue aux reflexions entourant le deficit democratique de la representation des femmes au sein des organisations syndicales. Les resultats reposent sur une etude de cas qualitative menee aupres des membres de deux comites syndicaux de la condition feminine et de 16 presidences syndicales locales travaillant dans le secteur de l'education du Quebec. En recueillant le point de vue de ces femmes et de ces hommes, notre recherche fait valoir une avancee quant a la place longtemps delaissee des femmes siegeant localement comme presidentes au sein de l'organisation syndicale etudiee. Ce sont les efforts deployes par les syndicats eux-memes qui ont permis peu a peu aux femmes d'acceder a l'univers syndical traditionnellement masculin. Derriere cette percee sans precedent, il en ressort neanmoins que cette avancee demeure lacunaire pour atteindre l'egalite de genre. Outre un certain deficit numerique des femmes qui resiste au temps qui passe, la difficulte d'articuler une <>, la persistance de stereotypes de genre, la pregnance d'une culture syndicale masculine, le manque de formation et de mentorat ainsi que le recours a la sollicitation sont des freins importants qui empechent les femmes d'acceder aux postes syndicaux locaux. De plus, la representation de la diversite des identites sociales (age, situation familiale, race, etc.) est encore peu presente au palier decisionnel local de l'organisation syndicale etudiee, et ce, meme parmi les femmes elues. La diversite et la representation de l'egalite de genre demeurent des defis a prioriser pour assurer une organisation syndicale perenne et democratique.
