IntroductionPenetrating traumatic brain injury (TBI) is the most common cause of death in current military conflicts, and results in significant morbidity in survivors. Identifying those physiological and radiological parameters associated with worse clinical outcomes following penetrating TBI in the austere setting may assist military clinicians to provide optimal care.MethodAll emergency neurosurgical procedures performed at a Role 3 Medical Treatment Facility in Afghanistan for penetrating TBI between 01 January 2016 and 18 December 2020 were analysed. The odds of certain clinical outcomes (death and functional dependence post-discharge) occurring following surgery were matched to existing agreed preoperative variables described in current US and UK military guidelines. Additional physiological and radiological variables including those comprising the Rotterdam criteria of TBI used in civilian settings were additionally analysed to determine their potential utility in a military austere setting.Results55 casualties with penetrating TBI underwent surgery, all either by decompressive craniectomy (n=42) or craniotomy±elevation of skull fragments (n=13). The odds of dying in hospital attributable to TBI were greater with casualties with increased glucose on arrival (OR=70.014, CI=3.0399 to 1612.528, OR=70.014, p=0.008) or a mean arterial pressure <90 mm Hg (OR=4.721, CI=0.969 to 22.979, p=0.049). Preoperative hyperglycaemia was also associated with increased odds of being functionally dependent on others on discharge (OR=11.165, CI=1.905 to 65.427, p=0.007). Bihemispheric injury had greater odds of being functionally dependent on others at discharge (OR=5.275, CI=1.094 to 25.433, p=0.038).ConclusionsWe would recommend that consideration of these three additional preoperative clinical parameters (hyperglycaemia, hypotension and bihemispheric injury on CT) when managing penetrating TBI be considered in future updates of guidelines for deployed neurosurgical care.

The relationship between animal welfare at slaughter and slaughterhouse profitability is complex, with potential trade-offs between animal welfare costs and benefits. Slaughterhouses currently lack data support for decisions on investments that can improve both animal welfare and profitability. Therefore, this study mapped the economic impacts for slaughterhouse businesses of improved cattle and pig welfare at slaughter. Specific aims were to: (i) highlight the possible economic impact of animal welfare improvements, based on the scientific literature; (ii) develop an economic model demonstrating the theoretical contribution of animal welfare to slaughterhouse profitability; and (iii) validate the economic model through focus group interviews with slaughterhouse personnel in Sweden. The findings indicated that investing in animal welfare improvements could result in accumulation of an intangible asset that can be considered together with other production factors in the economic model. Model validation stressed the importance of selling by-products for the economic outcome and of smooth workflow for productivity. The study thus improves understanding of the economic impacts of animal welfare at slaughter and incentives for slaughterhouse businesses to improve animal welfare. Im Jahr 2012 hat die Landeskrebsgesellschaft Hessen in Zusammenarbeit mit der Stiftung Leben mit Krebs e. V., der Agentur SUPERSIEBEN und einer großzügigen Unterstützung der Deutschen Krebsstiftung e. V. das Projekt „du bist kostbar“ entwickelt und für Präventionszwecke und Anliegen der Krebspatientinnen und Krebspatienten und deren Familien auf den Weg gebracht. Dieses Projekt möchte den Präventionsgedanken fördern und denen die an Krebs erkrankt sind Lebensmut und Optimismus vermitteln.

A mild fractionation process to extract functional biomolecules from green microalgae was implemented. The process includes bead milling, centrifugation, and filtration with several membrane cut-offs. For each fraction, the corresponding composition was measured, and the surface activity and gelation behavior were determined. A maximum protein yield of 12% was obtained in the supernatant after bead milling and between 3.2 and 11.7% after filtration. Compared to whey protein isolate, most of the algae fractions exhibited comparable or enhanced functionality. Surface activity for air–water and oil–water interfaces and gelation activities were notably superior for the retentate fractions compared to the permeates. It is proposed that such functionality in the retentates is due to the presence of hydrophobic compounds and molecular complexes exhibiting a similar behavior as Pickering particles. We demonstrated that excellent functionality can be obtained with crude fractions, requiring minimum processing and, thus, constituting an interesting option for commercial applications.

How should the somewhat vague term of restoration success be measured? This is a critical question rooted in European law, where in fact the creation of proper replacement habitats is a prerequisite for permitting projects that trigger a loss of species or habitats. Previous studies have used indices that relied on a comparison to reference sites, for example the number of a predefined pool of target species or compositional similarity. However, since restoration sites have rarely the same biotic and abiotic conditions as reference sites, plant communities in restored sites will not perfectly match the reference sites. Furthermore, such indices fail when reference sites are lacking or degraded. Hence, there is a need for an alternative approach that evaluates the conservation value of a restored site independently from reference sites. We propose that naturalness indicator values can be an option to measure restoration success. The approach of using naturalness indicator values makes use of the fact that plants are able to indicate environmental parameters, including degradation and regeneration. We compared and measured the restoration success of three well-established methods for grassland restoration (sod transplantation, hay transfer, seeding) with three commonly used indices (diversity, number of target species, similarity to reference sites). The results verified earlier studies and showed that sod transplantation led to the highest restoration success followed by hay transfer and seeding of sitespecific seed mixtures. Further, we used those well-established indices for an evaluation of novel, naturalness-based indices (unweighted and cover-weighted mean naturalness indicator values, the sum of naturalness indicator values). While calculating the means of naturalness indicator values failed to offer conclusive information on restoration success, we could show that the sum of naturalness indicator values was highly correlated with the number of target species and compositional similarity to reference sites. Thus, our case study demonstrated that naturalness indices can be an excellent option to estimate success in grassland restoration.

Family farming is still the main source of income for many people in the tropical regions of the world. At the same time, modern society is quickly becoming more aware of the welfare of animals for human consumption. The main objective of this study was to illustrate the need to modify certain aspects of the original Welfare Quality® (WQ) protocols developed by the EU-funded WQ project, under the conditions of small community farmers in the tropics. Thirty-four dual purpose farms in the State of Chiapas, Mexico, which had their main production focus on milk but for whom beef production was also of significant value, were evaluated utilising a merged version of the WQ protocols for dairy and beef cattle. Based on their average score, the farms obtained at least an acceptable level in each indicator of welfare. However, after merging indicators from the dairy and beef cattle protocols of WQ in order to adjust it to the prevailing conditions in the tropics, a number of sections are not applicable. This is particularly true of the section related to good housing, where most of the items do not apply due to the absence of infrastructure; the farms obtained a very high score in this section but further studies to verify whether this reflects an accurate assessment of the welfare status should be carried out. In general, the approach of the WQ protocol was useful, however certain aspects are quite different from the conventional intensive farming systems predominantly used in Europe and there is a need to implement a number of modifications. The Organ Procurement and Transplantation Network (OPTN) Deceased Donor Potential Study, funded by the Health Resources and Services Administration, characterized the current pool of potential deceased donors and estimated changes through 2020. The goal was to inform policy development and suggest practice changes designed to increase the number of donors and organ transplants. Donor estimates used filtering methodologies applied to datasets from the OPTN, the National Center for Health Statistics, and the Agency for Healthcare Research and Quality and used these estimates with the number of actual donors to estimate the potential donor pool through 2020. Projected growth of the donor pool was 0.5% per year through 2020. Potential donor estimates suggested unrealized donor potential across all demographic groups, with the most significant unrealized potential (70%) in the 50–75‐year‐old age group and potential Donation after Circulatory Death (DCD) donors. Actual transplants that may be realized from potential donors in these categories are constrained by confounding medical comorbidities not identified in administrative databases and by limiting utilization practices for organs from DCDdonors. Policy, regulatory, and practice changes encouraging organ procurement and transplantation of a broader population of potential donors may be required to increase transplant numbers in the United States. Findings from the OPTN Deceased Donor Potential Study suggest possible transplant center and organ procurement organization practice changes, as well as potential transplant system regulatory changes, which may expand the donor pool.

In infants with severe left diaphragmatic hernia, fetoscopic endoluminal tracheal occlusion (FETO) has been associated with greater survival rates but also higher rates of premature birth. This study aimed to determine the effects of FETO on pregnancies affected by moderate CDH compared with expectant prenatal care.

(Engl J Med. 2021;385:107–118)Congenital diaphragmatic hernia (CDH) can lead to serious lifelong complications, but may be treated if diagnosed before birth. In fetoscopic endoluminal tracheal occlusion (FETO), a balloon is inserted into the fetal trachea in utero to stimulate fetal lung growth and is removed prior to birth. One study of 210 fetuses undergoing FETO for severe lung hypoplasia due to isolated CDH found a nearly doubled survival rate among fetuses who received FETO compared with controls (49% vs. 24%). This current study, Tracheal Occlusion to Accelerate Lung Growth (TOTAL), aimed to test whether fetuses with surgical treatment of severe left CDH had superior survival rates compared with fetuses who received expectant prenatal care.
