Venous thromboembolism (VTE) is rare in patients aged <21 years. Young adults aged 18 to 21 years are frequently included in adult VTE studies, whereas pediatric VTE studies include patients aged up to either 18 or 21 years. The clinical characteristics of young adult patients with VTE have not been well defined.

4. Deterioration Models and Service Life Prediction of Vertical Assets of Urban Water Systems

Abstract

This study proposes a methodology for developing deterioration models and predicting the service lives of vertical assets of urban water systems (i.e., water storage tanks and pumping stations) using regression analysis. The main factors contributing to the deterioration of these assets are analyzed. Simple and multiple linear regression models of average and maximum deterioration are calculated for 22 water storage tanks and 17 wastewater pumping stations. Data on a set of four water storage tanks are used to validate the developed deterioration models. Service life prediction is carried out using the developed models and considering two maximum deterioration levels: the maximum recommended and admissible deterioration levels. Two water storage tanks are further studied to illustrate and discuss the effect of maintenance and rehabilitation interventions on asset service life by comparing the asset deterioration before and after the interventions. Results include simple linear regression models of average and maximum deterioration indices as a function of asset age and multiple linear regression models that incorporate other physical, operational and environmental factors. The results show that simple linear regression models of asset deterioration show a better predictive power than multiple regression models. Despite the higher data variability of multiple regression models, these models allow to include the random process of asset deterioration, through the calculation of the standard deviation. This study also shows that periodic interventions are a preferable maintenance and rehabilitation strategy over major sporadic rehabilitation interventions since it allows to maintain assets in good condition and to extend their service life almost indefinitely. Urban water assets are continuously deteriorating and more investments are necessary to maintain adequate levels of service. However, investment budgets are often limited and appropriate deterioration models and reliably predicted service lives are essential for planning and scheduling maintenance actions. This paper develops deterioration models for water storage tanks and wastewater pumping stations based on the identification and classification of anomalies through visual inspection. Additionally, service lives (i.e., the period from the installation until the asset or its components fulfill the service requirements) were obtained and compared with reference values. Finally, the effect of maintenance actions and rehabilitation interventions on the service life of vertical assets was discussed. In order to maintain a good asset condition and extend its service life quasi‐indefinitely, periodic and well‐established interventions are a preferable maintenance and rehabilitation strategy over major sporadic rehabilitation interventions. Abstract

Little is known about the prognosis of patients with massive pulmonary embolism (PE) and its risk of recurrent venous thromboembolism (VTE) compared with non‐massive PE, which may inform clinical decisions. Our aim was to compare the risk of recurrent VTE, bleeding, and mortality after massive and non‐massive PE during anticoagulation and after its discontinuation. We included all participants in the RIETE registry who suffered a symptomatic, objectively confirmed segmental or more central PE. Massive PE was defined by a systolic hypotension at clinical presentation (<90 mm Hg). We compared the risks of recurrent VTE, major bleeding, and mortality using time‐to‐event multivariable competing risk modeling. There were 3.5% of massive PE among 38 996 patients with PE. During the anticoagulation period, massive PE was associated with a greater risk of major bleeding (subhazard ratio [sHR] 1.72, 95% confidence interval [CI] 1.28–2.32), but not of recurrent VTE (sHR 1.15, 95% CI 0.75–1.74) than non‐massive PE. An increased risk of mortality was only observed in the first month after PE. After discontinuation of anticoagulation, among 11 579 patients, massive PE and non‐massive PE had similar risks of mortality, bleeding, and recurrent VTE (sHR 0.85, 95% CI 0.51–1.40), but with different case fatality of recurrent PE (11.1% versus 2.4%, P= .03) and possibly different risk of recurrent fatal PE (sHR 3.65, 95% CI 0.82–16.24). In this large prospective registry, the baseline hemodynamic status of the incident PE did not influence the risk of recurrent VTE, during and after the anticoagulation periods, but was possibly associated with recurrent PE of greater severity.

Abstract

El cáncer de mama tiene una alta tasa de incidencia y un impacto negativo en la vida de las mujeres. La práctica de actividad física (AF) ha demostrado una evidencia sólida en el control de los efectos secundarios asociados a la enfermedad y su tratamiento. Sin embargo, tener un estilo de vida activo está influenciado por inequidades sociosanitarias. El objetivo fue analizar las categorías relacionadas con los significados y experiencias percibidas con la AF en sobrevivientes de cáncer de mama (SCM). Protocolo https://osf.io/7fwbs/. Se incluyeron artículos que describieran los significados de AF en SCM publicados después de 2010. Se analizaron 14 artículos mediante una codificación línea-a-línea. The aim of the present study was to evaluate the changes caused by adjuvant‐induced arthritis in liver mitochondria and to investigate the effects of the nonsteroidal anti‐inflammatory drug nimesulide. The main alterations observed in liver mitochondria from arthritic rats were: higher rates of state IV and state III respiration with β‐hydroxybutyrate as substrate; reduced respiratory control ratio and impaired capacity for swelling dependent on β‐hydroxybutyrate oxidation. No alterations were found in the activities of NADH oxidase and ATPase. Nimesulide produced: (1) stimulation of state IV respiration; (2) decrease in the ADP/O ratio and in the respiratory control ratio; (3) stimulation of ATPase activity of intact mitochondria; (4) inhibition of swelling driven by the oxidation of β‐hydroxybutyrate; (5) induction of passive swelling due to NH3/NH4+ redistribution. The activity of NADH oxidase was insensitive to nimesulide. Mitochondria from arthritic rats showed higher sensitivity to nimesulide regarding respiratory activity. The results of this work allow us to conclude that adjuvant‐induced arthritis leads to quantitative changes in some mitochondrial functions and in the sensitivity to nimesulide. Direct evidence that nimesulide acts as an uncoupler was also presented. Since nimesulide was active in liver mitochondria at therapeutic levels, the impairment of energy metabolism could lead to disturbances in the liver responses to inflammation, a fact that should be considered in therapeutic intervention. Abstract

Abstract. Objetive and design: Glycolysis and the glucose phosphorylation capacity of livers from arthritic rats were studied because alterations in these parameters are suggested by some studies.¶ Subjects: Arthritis was induced in male albino rats (Wistar; 180-220 g).¶ Treatment: The animals were injected with 100 μl heat inactivated Mycobacterium tuberculosis suspended in mineral oil at a concentration of 0.5% (w/v). Animals showing lesions after 14 to 21 days were selected.¶ Methods: Glucose phosphorylation was measured in the high speed supernatant fraction of liver homogenates and glycolysis in the isolated perfused liver.¶ Results: The glucose concentration for half-maximal rates was reduced from 18.32 ± 5.69 in normal to 9.84 ± 3.15 mM in arthritic rats (p = 0.024). Vmax was increased from 8.77 ± 0.27 in normal to 11.49 ± 0.29 nmol min-1 mg protein-1 in arthritic rats (p = 0.001). Perfused livers from arthritic rats showed a 2.43-fold higher rate of glycolysis.¶ Conclusions: Livers from arthritic rats present a higher glucose phosphorylation capacity. Possibly this phenomenon is caused by circulating inflammatory mediators produced during adjuvant-induced arthritis.

Abstract

Venous thromboembolism (VTE) is a major cause of morbidity and mortality postoperatively. The use of pharmacologic prophylaxis is effective in reducing the incidence of VTE. However, the prophylaxis is often discontinued at hospital discharge, especially for those with benign disease. The implications of this practice are not known. We assessed the data from a large, ongoing registry regarding the time course of VTE and outcomes after noncancer surgery.

Overlap exists between the risk factors for coronary artery disease and venous thromboembolism (VTE). However, a paucity of data is available on the incidence of major acute cardiovascular events (MACE) and major adverse limb events (MALE) among patients presenting with VTE. Moreover, it is unknown whether the rate of cardiovascular outcomes differs among patients with unprovoked vs provoked VTE.

The extract was found to have significant antiulcer activity against all the models studied. Ulcers were induced in rats by means of three experimental models: acute stress, acidified‐ethanol and indomethacin. The total extract and the fractions were found to have significant antiulcer activity in the case of the acute stress and acidified‐ethanol models.
