Dieses zweite Positionspapier der Sektion Metabolismus und Ernährung der Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin (DIVI) gibt Empfehlungen zum laborchemischen Monitoring der Makro- und Mikronährstoffzufuhr sowie zum Einsatz der indirekten Kalorimetrie im Rahmen der medizinischen Ernährungstherapie erwachsener Intensivpatient:innen. Zusätzlich werden Empfehlungen zur krankheitsbezogenen bzw. individuellen (Spiegelbestimmung) Substitution und (Hochdosis‑)Pharmakotherapie von Vitaminen und Spurenelementen vorgenommen.

Die Erhebung des Ernährungsstatus zum Zeitpunkt der Aufnahme im Intensiv- oder Intermediate Care Bereich hat sowohl prognostische als auch therapeutische Relevanz im Hinblick auf die Planung einer individualisierten medizinischen Ernährungstherapie (engl. „medical nutrition therapy“, MNT). Diese Planung wird im Rahmen der Erstversorgung eines vital bedrohlichen Krankheitsbilds nachvollziehbar nicht priorisiert, jedoch im weiteren Verlauf häufig auch oft nicht mehr angemessen durchgeführt. Vor allem bei längerer Verweildauer besteht das Risiko einer Mangelernährung mit Aufbau eines kumulativen, prognoserelevanten Makro- und/oder Mikronährstoffdefizits. Bisher gibt es für Patient*innen auf Intensiv- und Intermediate Care Einheiten keine strukturierten Empfehlungen zur Erhebung des Ernährungsstatus. Das vorliegende Positionspapier der Sektion Metabolismus und Ernährung der Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin (DIVI) beinhaltet konsensbasierte Empfehlungen zur Erfassung und zum apparativen Monitoring des Ernährungsstatus von Patient*innen auf Intensiv- und Intermediate Care Stationen. Diese Empfehlungen ergänzen die aktuelle S2k-Leitlinie „Klinische Ernährung in der Intensivmedizin“ der Deutschen Gesellschaft für Ernährungsmedizin (DGEM) und der DIVI.

Krankheitsbedingte Mangelernährung ist in deutschen Krankenhäusern weitverbreitet. Diese wirkt sich negativ auf den Therapieerfolg und die Lebensqualität der Patienten und Patientinnen aus und hat erhöhte Kosten zur Folge. Eine individuelle, durch Fachkräfte durchgeführte leitliniengerechte Ernährungstherapie senkt nachweislich die Mortalität mangelernährter Patientinnen und Patienten und wird idealerweise von einem interdisziplinären Ernährungsteam durchgeführt. Aktuelle Daten zur ernährungstherapeutischen Versorgung in deutschen Krankenhäusern sowie zu Ernährungsteams fehlen bisher.

Critical conditions such as sepsis following infection or traumatic injury disturb the complex state of homeostasis that may lead to uncontrolled inflammation resulting in organ failure, shock and death. They are associated with endogenous mediators that control the onset of acute inflammatory response, but the central problem remains the complete resolution of inflammation. Omega-3 enriched lipid emulsions (?-3+LEs) were used in experimental studies and clinical trials to establish homeostasis, yet with little understanding about their role on the resolution of inflammation and tissue regeneration. Here, we demonstrate that ?-3 lipid emulsions (LEs) orchestrate inflammation-resolution/regeneration mechanism during sterile peritonitis and murine polymicrobial sepsis. ?-3+LEs recessed neutrophil infiltration, reduced pro-inflammatory mediators, reduced the classical monocyte and enhanced the non-classical monocytes/macrophages recruitment and finally increased the efferocytosis in sepsis. The actions of ?-3+LE were 5-lipoxygenase (5-LOX) and 12/15-lipoxygenase (12/15-LOX) dependent. ?-3+LEs shortened the resolution interval by 56%, stimulated the endogenous biosynthesis of resolution mediators lipoxin A4, protectin DX and maresin 1 and contributed to tissue regeneration. ?-3+LEs protected against hypothermia and weight loss and enhanced survival in murine polymicrobial sepsis. We highlighted a role of ?-3+LEs in regulating key mechanisms within the resolution terrain during murine sepsis. This might form the basis for a rational design of sepsis specific clinical nutrition.

Die parenterale Ernährung ist elementarer Bestandteil der Therapie kritisch kranker Patienten. Dies gilt unvermindert auch vor dem Hintergrund der aktuellen Diskussion um diese Form der künstlichen Ernährung. Erst in jüngerer Zeit haben mehrere groß angelegte prospektiv randomisierte kontrollierte Studien Fragen nach der korrekten Indikation, dem optimalen Timing und der Zusammensetzung einer parenteralen Ernährung untersucht. Trotz einer zum Teil ausgesprochen kontrovers geführten Diskussion kristallisiert sich eine Strategie heraus, die die parenterale Ernährung als besonders wichtigen, ergänzenden Baustein einer möglichst früh begonnenen enteralen Ernährung sieht.

In patients mechanically ventilated for severe respiratory failure, respiratory system mechanics are non-linear, i.e., volume-dependent. We present a new computer-based multipoint method for simultaneously determining volume-dependent dynamic compliance and resistance. Our method is based on continuously determined tracheal pressure (Ptrach). Tidal volume is subdivided into six volume slices of equal size. One compliance value (intrinsic PEEP considered) and one resistance value are determined for each volume slice by applying of the least-squares-fit (LSF) analysis based on the linear RC-model; we therefore call this the SLICE method. The method gives the course of dynamic compliance and resistance within the tidal volume. The method was evaluated using physical models of the respiratory system with linear and non-linear passive mechanical properties. The relative error of the method is smaller than ±5%. The method needs no special ventilatory pattern. Using data from 14 patients mechanically ventilated for adult respiratory distress syndrome (ARDS) we found a very good correspondence between the measured end-inspiratory airway pressure (Paw,Ie) and the end-inspiratory alveolar pressure (Palv,Ie) calculated from the dynamic compliance values determined with the SLICE method (Palv,Ie= 1.02 * Paw,Ie+ 0.097; r2= 0.977). The SLICE method allows continuous monitoring of non-linear pulmonary mechanics on a breath-by-breath basis at the bedside.

Available objective tests for fat embolism are neither sensitive nor specific. We performed frozen-section analysis of blood aspirated from a pulmonary artery catheter in a trauma victim with fat embolism syndrome and found marked levels of pulmonary microvascular blood fat. The amount of fat detected in this patient with long bone fractures was higher than that observed in a pilot study of this technique in cardiopulmonary bypass patients. This technique provided diagnostic support of our clinical findings. The procedure may prove useful in patients with an equivocal diagnosis of fat embolism syndrome or when the cause of posttraumatic acute pulmonary insufficiency is unknown.

Heparin-induced thrombocytopenia with antithrombin III deficiency can be a severe, fulminant systemic disease leading to disastrous complications and often death. An eighteen-month review of the authors' surgical population revealed 7 cases of heparin-induced thrombocytopenia. They present the diagnosis and management of these patients, as well as a literature review. On the basis of their experience, a platelet count that falls below 17% of the baseline should be the impetus to test for heparin-associated antibodies, protein C and S deficiency, and antithrombin III deficiency. Their newer recommendation, along with the use of saline flush and the start of coumadin early in the postoperative period, has prevented the complications associated with heparin-induced thrombocytopenia in their hospital for the past twelve months.
