Among asymptomatic patients with severe carotid artery stenosis but no recent stroke or transient cerebral ischaemia, either carotid artery stenting (CAS) or carotid endarterectomy (CEA) can restore patency and reduce long-term stroke risks. However, from recent national registry data, each option causes about 1% procedural risk of disabling stroke or death. Comparison of their long-term protective effects requires large-scale randomised evidence.

The optimal surgical excision margins are uncertain for patients with thick (>2 mm) localised cutaneous melanomas. In our previous report of this multicentre, randomised controlled trial, with a median follow-up of 6·7 years, we showed that a narrow excision margin (2 cm vs4 cm) did not affect melanoma-specific nor overall survival. Here, we present extended follow-up of this cohort.

25. Haemodialysis access thrombosis: Outcomes after surgical thrombectomy versus catheter-directed thrombolytic infusion

Introduction: Thrombosis is one of the most common complications of dialysis vascular access and is a significant source of morbidity and healthcare-associated costs. In this retrospective study, outcomes for surgical thrombectomy and thrombolysis after access thrombosis in patients with arteriovenous fistulas or prosthetic grafts (arteriovenous grafts) were analysed.Methods: All patients with a primary episode of dialysis access thrombosis between 2005 and 2013 were included which yielded 131 patients with 149 episodes of access thrombosis (108 arteriovenous grafts; 41 arteriovenous fistulas). In all, 18 patients had two separate accesses during the study. Patient demographics, access anatomy, surgical and radiological procedural data were recorded. Kaplan–Meier estimates and Poisson regression were used for statistical analysis of access patency.Results: In total, 107 underwent surgical thrombectomy and 42 were treated with catheter-directed thrombolytic infusion. Technical success was 60% for surgical thrombectomy and 73% for thrombolysis (p = 0.18). There were no major complications and no deaths within 30 days of the procedure. More patients had adjunctive procedures in the thrombolysis group (65/107 vs 37/42; p = 0.002). There was an increasing risk of rethrombosis or a further access-related event for both arteriovenous fistulas and arteriovenous grafts after open thrombectomy compared with catheter-directed thrombolytic infusion, and arteriovenous fistulas exhibited a lower risk than arteriovenous grafts with an average increase in risk of 23.9% (95% confidence interval: 3.1–49) between each treatment group.Conclusion: Thrombolysis for thrombosis of native and prosthetic dialysis accesses appears to yield better assisted primary patency compared to surgical thrombectomy. Our results suggest that thrombolysis may be considered the first-choice method for treating the thrombosed dialysis access.

After endovascular aortic repair (EVAR) for treatment of aortoiliac aneurysms, patients commonly develop an inflammatory reaction: Postimplantation syndrome (PIS). Clinically, it may be hard to separate PIS from an infectious complication. Procalcitonin (PCT) is a diagnostic marker for severe bacterial infections and sepsis. We hypothesize that low-PCT levels facilitate the PIS diagnosis after EVAR.

32. Mechanical and Barrier Properties of Avenin, Kafirin, and Zein Films

- Abstract
Biodegradable and renewable materials can be manufactured from prolamins, which are the major storage protein fraction of cereals. This paper investigates the material properties of oat prolamin (avenin), corn prolamin (zein), and sorghum prolamin (kafirin). Glass transition temperature, dry solid content, stress at break, strain at break, oxygen permeability, and water vapor permeability were analyzed at different plasticizer contents. Avenin was plasticized with glycerol, and kafirin and zein were plasticized with a mixture of polyethylene glycol, glycerol, and lactic acid. Avenin displayed potential, although it did not exhibit the mechanical qualities of gluten, which resembles avenin at the molecular level. Compared to kafirin and zein, avenin was more extensible at low plasticizer contents, while kafirin and especially zein were more extensible at the highest plasticizer content. Avenin was far weaker than the other two at all plasticizer contents. Kafirin and zein displayed similar barrier properties, whereas avenin was notably more permeable.

The behavior of poly-γ-methyl-
l -glutamate (pMeE) at the air−water interface has been studied with the surface film balance technique. In addition, Langmuir−Blodgett (LB) films of pMeE deposited on mica and quartz have been studied by atomic force microscopy (AFM) and circular and linear dichroism (CD and LD) spectroscopy. Depending on the spreading solvent, pMeE displays strikingly different compression isotherms. When spread from chloroform or trifluoroacetic acid (TFA) the surface pressure isotherms are consistent with that of a peptide in α-helix conformation. However, the latter solvent gives rise to isotherms with a considerably smaller apparent mean molecular area, A0 . When spread from pyridine, on the other hand, pMeE yields an isotherm that is expanded and inconsistent with the presence of a monolayer consisting entirely of α-helical peptides. Isotherms and AFM images strongly suggest that peptide aggregation and solvent retention are the main factors behind the isotherm differences. When the water-soluble spreading solvent TFA is used, pMeE forms discrete wormlike aggregates embedded in a monolayer matrix. In the pyridine case, aggregation in the spreading solvent and retention of pyridine in the film result in a rough aggregate network coexisting with discrete aggregates. No aggregation takes place when chloroform is used as spreading solvent. CD and LD spectra of the LB films reveal a pronounced lateral orientation of the α-helices in films spread from chloroform and TFA, while spectra of films spread from pyridine are consistent with unordered peptide strands in β-sheet conformation. In conclusion, the results show that if water-soluble and/or low-volatile solvents are used as spreading media, hydrophobic peptides cannot, a priori, be assumed to form proper monolayers.- Published

34. Hydrophobic Homopolymers of Native α-L-Amino Acids at the Air–Water Interface: A Study by Circular Dichroism Spectroscopy, Atomic Force Microscopy, and Surface Balance Experiments

- Abstract
Films of poly-L-leucine, poly-L-valine, and poly-L-isoleucine have been studied at the air–water interface by surface balance experiments. In addition, Langmuir–Blodgett (LB) films of these polypeptides deposited onto quartz and mica have been studied by circular dichroism (CD) spectroscopy and atomic force microscopy (AFM) to elucidate the effects of polypeptide conformation and spreading agent (chloroform and trifluoroacetic acid, TFA) on film morphology and phase behavior. Monolayers of poly-L-leucine contain α-helical polypeptide strands. When spread from chloroform, the compression isotherm displays a collapse plateau and a limiting molecular area (A0) of 19 Å2per amino acid residue. The corresponding LB films are flat and featureless. When a water-soluble solvent (TFA) is used as a spreading agent, the AFM results reveal an extensive formation of polypeptide aggregates. The aggregation is accompanied by a substantial decrease in A0but has little effect on polypeptide conformation, film compressibility, and phase behavior. According to CD spectroscopy, films of poly-L-valine and poly-L-isoleucine contain polypeptide strands in β-sheet conformation. The corresponding isotherms are steep and lack a collapse plateau. When TFA is used as a spreading agent, the limiting area decreases, but AFM data do not give direct evidence for any aggregation.

- Abstract
PURPOSE: This study was designed to evaluate the results of rectocele repair and parameters that might be useful in selecting patients for this operation. METHODS: Twentyfive patients with symptom-giving rectoceles were prospectively evaluated with a standardized questionnaire, physical examination, defecography, colon transit studies, anorectal manometry, and electrophysiology. Patients underwent posterior colporrhaphy and perineorrhaphy. They were followed postoperatively (mean, 1.0 year) with the same questionnaire, physical examination, defecography, anorectal manometry, and electrophysiology. RESULTS: Constipation had improved postoperatively in 21 of 24 constipated patients (88 percent). At postoperative follow-up 13 patients (52 percent) had no constipation symptoms, 8 (32 percent) had occasional symptoms, and 4 (16 percent) had symptoms more than once per week. Four patients with rectocele at preoperative defecography, but not at physical examination, had favorable outcomes following surgery. The majority of patients not using vaginal digitalization preoperatively had improved with respect to constipation. All patients with pathologic transit studies had various degrees of constipation postoperatively. Constipation was not improved in two of five patients with preoperative paradoxic sphincter reaction. CONCLUSIONS: Rectocele is one cause of constipation that can be treated with good results. Preoperative use of vaginal digitalization is not mandatory for a good postoperative result. Defecography is an important complement to physical examination. Patients with pathologic transit study might have a less favorable outcome of rectocele repair with respect to constipation. More studies about the significance of paradoxic sphincter reaction in these patients are indicated.

The article profiles the Piano Hill Riding Centre, a horse riding training institute in New Zealand. It is reported that, the institute is set in a 33ha of farmland near Whangarei, New Zealand, and its owner Ellen Bennett has lived in the country for 20 years and helps under-achieving school children in the country through her institute. The institute, as reported, has helped the children learn new skills, and very timid children have started to come out of their shells.

The article features the community-built footbridge located in the estuary of Whananaki in Northland, New Zealand. Whananaki is a small community that resides mostly along the northern side of the estuary. It is considered the longest pedestrian bridge in the Southern Hemisphere, measured at 395 meters in length. The Whangarei County Council decided in 1946 to construct the bridge to provide students with a safe passage to and from the primary school.
