Cervical cancer is considered to be rare among nuns. The British Department of Health currently does not consider screening for cervical cancer necessary for nuns, virgins, or spinsters. However, various studies that have examined the incidence of cervical cancer among nuns do not confirm that this malignancy is particularly rare among nuns. Most studies have reported a large number of tumors of unspecified sites among nuns, and some of these may be cases of cervical cancer. Hence, the incidence of cervical cancer among nuns may actually be underreported. Cervical cancer is more common among women who have been married than among women who have never been married, and is most frequent among women aged 55 years and older. However, cancer of the uterus is more common among women with no previous pregnancy, such as nuns. Cervical cancer causes 74 deaths per million women annually, and ranks seventh in causes of death due to malignancy among women. The frequency of cervical cancer is similar to that of cancer of the bladder, esophagus, and uterus, and leukemias. Review of the literature indicates that the death rate due to cervical cancer among nuns is similar to that among other single women. The false belief that cervical cancer is rare among nuns could result in misdiagnosis and failure to provide appropriate treatment; reluctance to record the correct diagnosis due to disbelief or desire to protect the nun's reputation; and absence of routine screening for cervical cancer among nuns. The development of symptoms of cervical cancer, including irregular vaginal bleeding, bleeding between menstrual periods, and recurrent discharge from the vagina in a nun should suggest a need for a thorough pelvic examination. Placing quantum materials into optical cavities provides a unique platform for controlling quantum cooperative properties of matter, by both weak and strong light–matter coupling1,2. Here we report experimental evidence of reversible cavity control of a metal-to-insulator phase transition in a correlated solid-state material. We embed the charge density wave material 1T-TaS2into cryogenic tunable terahertz cavities3and show that a switch between conductive and insulating behaviours, associated with a large change in the sample temperature, is obtained by mechanically tuning the distance between the cavity mirrors and their alignment. The large thermal modification observed is indicative of a Purcell-like scenario in which the spectral profile of the cavity modifies the energy exchange between the material and the external electromagnetic field. Our findings provide opportunities for controlling the thermodynamics and macroscopic transport properties of quantum materials by engineering their electromagnetic environment.

Piano-stool iron complexes with N-heterocyclic carbene (NHC) ligands have shown high potential in the activation and conversion of C═O bonds in aldehydes, ketones, and esters. Here we demonstrate the expansion of this activity to the functionalization of CO2with amines to form N-formamide products in the presence of a silane (PhSiH3). This synthetically valuable utilization of CO2as C1-synthon is catalyzed by iron complexes containing a variety of different NHC ligands. Structural variations of the NHC ligands did not reveal any correlation with catalytic activity, with imidazole and triazole-derived NHCs reaching similar conversions. However, the use of BF4–as counterion in cationic complexes, and also incorporating a Cp* instead of a Cp spectator ligand led to a marked decrease in catalytic activity. While NHC iron complexes reach mediocre activity (about 50% conversion at 5% catalyst loading in 24 h), the corresponding imidazolium and triazolium carboxylates as precursors of free carbenes display excellent catalytic activity with essentially full conversion after 1 h at 1 mol % loading. Experiments at variable iron complex loading as well as carbene trapping experiments with S8together with spectroscopic investigations demonstrated a high stability of the Fe–CNHCbond during catalysis and lend strong support to a NHC iron unit as catalytically active species rather than free NHC resulting from Fe–NHC dissociation, even though free NHCs are excellent catalysts for this reaction. Optimized of reaction conditions, in particular the addition of another equivalent of silane allowed to reach high conversions of piperidine and CO2to N-formylpiperidine using a triazolylidene iron complex as catalyst precursor.

Electrochemical hydrogen storage in porous activated carbons is a rapidly advancing technology, yet the composition and role of oxygen-containing surface functionalities in hydrogen storage remain underexplored. This study provides a detailed investigation of the surface and bulk properties of porous activated carbon derived from phenolic resin (aC PR) using a comprehensive multi-technique approach, including scanning electron microscopy (SEM), transmission electron microscopy (TEM), Brunauer-Emmett-Teller (BET), X-ray photoelectron spectroscopy (XPS), temperature programmed desorption (TPD), Fourier transform infrared spectroscopy (FTIR), and Raman spectroscopy. The aC PR exhibits an exceptional BET surface area of approximately 4400 m2/g, with a well-balanced distribution of mesopores, micropores, and ultra-micropores. Quantitative analyses reveal that aC PR is composed of 95.45 % carbon and 4.55 % oxygen, with oxygen functionalities distributed as carboxylic acid (~8 %), anhydride (~28 %), phenol (~17 %), carbonyl and quinones (~21 %), and lactones (~23 %). Post-TPD treatment, the oxygen content reduces to 2.25 %, with minimal impact on the material's hydrogen storage capacity, which remains at ~0.60 ± 0.05 wt% H. H-storage was measured using Proton Battery. In the proton battery, protons are generated by water splitting towards the oxygen side and are stored towards the C side in the negatively charged ac PR electrode. Ab initiomolecular dynamics simulations demonstrate that both acidic and basic oxygen-containing groups have similar proton affinities, suggesting that the type of oxygen functional group plays a minimal role in hydrogen storage capacity. This work underscores the critical role of oxygen functionalities in hydrogen storage and offers new insights into the design and optimization of next-generation carbon materials for scalable hydrogen storage technologies.

The vertebrate body displays a segmental organization that is most conspicuous in the periodic organization of the vertebral column and peripheral nerves. This metameric organization is first implemented when somites, which contain the precursors of skeletal muscles and vertebrae, are rhythmically generated from the presomitic mesoderm. Somites then become subdivided into anterior and posterior compartments that are essential for vertebral formation and segmental patterning of the peripheral nervous system1–4. How this key somitic subdivision is established remains poorly understood. Here we introduce three-dimensional culture systems of human pluripotent stem cells called somitoids and segmentoids, which recapitulate the formation of somite-like structures with anteroposterior identity. We identify a key function of the segmentation clock in converting temporal rhythmicity into the spatial regularity of anterior and posterior somitic compartments. We show that an initial ‘salt and pepper’ expression of the segmentation gene MESP2in the newly formed segment is transformed into compartments of anterior and posterior identity through an active cell-sorting mechanism. Our research demonstrates that the major patterning modules that are involved in somitogenesis, including the clock and wavefront, anteroposterior polarity patterning and somite epithelialization, can be dissociated and operate independently in our in vitro systems. Together, we define a framework for the symmetry-breaking process that initiates somite polarity patterning. Our work provides a platform for decoding general principles of somitogenesis and advancing knowledge of human development.

In this work, we investigate the use of drones in a delivery scenario formed by two contiguous areas. In one area the drones can freely fly on straight lines between any two locations (Euclidean metric), while in the other one the drones must follow the open space above the roads (Manhattan metric). We model this delivery scenario as a Euclidean-Manhattan-Grid (EM-grid). Given a set of customers to be served in an EM-grid, the objective is to find the distribution point (DP) for the drone that minimizes the overall traveled distance, considering that the drone has to do multiple round trips to/from the DP. In our view, the DP is optimized with respect to the set of customers and its computation must be light because it needs to be recomputed every time the set of customers varies. Accordingly, we define the Single Distribution Point Problem (SDPP) and devise sub-optimal time-efficient algorithms for solving it. We numerically compare the cost of our sub-optimal solutions with that of an optimal solution computed with a brute-force approach. Finally, using the BlueSky open air simulator, we compare the cost of our best solution with the cost of a solution that serves the costumers from a fixed DP, like the location of a delivery company’s depot. The fixed DP can perform very poorly for some customer instances, while our solution is highly adaptive and reduces the time and the distance covered by the drone.

MITICA is one of the two ongoing experiments at the ITER neutral beam test facility (NBTF) located in Padua (Italy). MITICA aims to develop the full-size neutral beam injector of ITER and, as such, its Control and Data Acquisition System will adhere to ITER CODAC directives. In particular, its timing system will be based on the IEEE1588 PTPv2 protocol and it will use the ITER time communication network (TCN) on a dedicated Ethernet local-area network. Following the ITER device catalog, the National Instruments PXI-6683H precision time protocol (PTP) timing modules will be used to generate triggers and clocks synchronized with a PTP grandmaster clock (GMC). The “Lazy Trigger” technique, then, will be adopted to implement event-driven data acquisition without the need for any hardware link in addition to the Ethernet connections used to transfer data and timing synchronization. To evaluate the accuracy over time that can be achieved with different network topologies and configurations, a test system has been set up consisting of a GMC, two PXI-6683H devices, and two PTP-aware network switches. In addition, a detailed simulation of the network and the involved devices has been performed using the OMNET++ discrete event simulator. The simulation parameters include not only the network and switches configuration, but also the PID parameters used in the clock servo controllers. A comparison between simulated and measured statistics is reported, together with a discussion of the possible optimal configuration strategies.

At the ITER Neutral Beam Test Facility (NBTF), the source for the production of ions of deuterium extracted from radio frequency plasma (SPIDER) has been in operation since 2018 aiming at prototyping the heating and diagnostic neutral beam appliances in view of the ITER demanding requirements for plasma burning conditions and instabilities control. For the sake of safety, machine protection, and efficiency, it is necessary to follow an accurate planning strategy and approval action of the experiment parameter settings. Although the initial tools available for the SPIDER integrated commissioning and early campaigns offered the basic functionality to perform the necessary tasks, there were a set of relevant issues that were identified as needing improvements for an efficient and safe configuration environment. Namely, the fact that there were no tools indicating the parameters change since the previous pulse, or the lack of a comparison tool between a new set of parameters and a previous setup, demanded a tedious and error prone verification of all parameters in the sequence. Moreover, several verifications should be automated according to the machine safety limits, increasing the safety check by human and automated machine (algorithmic) validation. This contribution depicts: 1) the approval sequence designed for SPIDER safe operation; 2) the configuration environment requirements according to the SPIDER pulse preparation procedure; 3) the decision of the development tools used for implementation and design; and 4) the implementation details and preliminary tests of the global environment.

BackgroundIgG antibodies against myelin oligodendrocyte glycoprotein (MOG-IgG) define a subset of associated disorders (myelin oligodendrocyte glycoprotein associated disorders (MOGAD)) that can have a relapsing course. However, information on relapse predictors is scarce. The utility of retesting MOG-IgG over time and measuring their titres is uncertain. We aimed to evaluate the clinical relevance of longitudinal MOG-IgG titre measurement to predict relapses in patients with MOGAD.MethodsIn this retrospective multicentre Italian cohort study, we recruited patients with MOGAD and available longitudinal samples (at least one >3 months after disease onset) and tested them with a live cell-based assay with endpoint titration (1:160 cut-off). Samples were classified as ‘attack’ (within 30 days since a disease attack (n=59, 17%)) and ‘remission’ (≥31 days after attack (n=295, 83%)).ResultsWe included 102 patients with MOGAD (57% adult and 43% paediatric) with a total of 354 samples (83% from remission and 17% from attack). Median titres were higher during attacks (1:1280 vs 1:640, p=0.001). Median onset titres did not correlate with attack-related disability, age or relapses. Remission titres were higher in relapsing patients (p=0.02). When considering the first remission sample available for each patient, titres >1:2560 were predictors of relapsing course in survival (log rank, p<0.001) and multivariate analysis (p<0.001, HR: 10.9, 95% CI 3.4 to 35.2). MOG-IgG seroconversion to negative was associated with a 95% relapse incidence rate reduction (incidence rate ratio: 0.05, p<0.001).ConclusionsPersistent MOG-IgG positivity and high remission titres are associated with an increased relapse risk. Longitudinal MOG-IgG titres could be useful to stratify patients to be treated with long term immunosuppression.

In this work, we investigate the impact of the wind in a drone-based delivery system. For the first time, to the best of our knowledge, we adapt the trajectory of the drone to the wind. We consider a truck-drone tandem delivery system. The drone actively reacts to the wind adopting the “most tailwind” trajectory available between the truck’s path and the delivery. The truck moves on a predefined route and carries the drone close to the delivery point. We propose the Minimum-energy Drone-trajectory Problem (MDP) which aims, when the wind affects the delivery area, at planning minimum-energy trajectories for the drone to serve the customers starting from and returning to the truck. We then propose two algorithms that optimally solve MDP under two different routes of the truck. We also analytically study the feasibility of sending drones with limited battery to deliver packages. Finally, we first numerically compare our algorithms on randomly generated synthetic and real data, and then we evaluate our model simulating the drone’s flight in the BlueSky simulator.

An overview of the recent operations and the main results of cesium injection in the Source for the Production of Ions of Deuterium Extracted from Rf plasma (SPIDER) negative ion source are described in this contribution. In experiments without cesium injection, all SPIDER plants were tested to verify the basic expectations on the operational parameters (e.g., electron cooling effectiveness of magnetic filter field) and to determine its operational region. For beam properties, it was shown that the current density varies across the beam in the vertical direction. In preliminary cesium experiments, the expected increase of negative ion current and simultaneous decrease of co-extracted electrons were found, along with the influence of the control parameters (polarization of the plasma electrodes, magnetic filter field) on the SPIDER beam uniformity in the horizontal and vertical directions. It was shown that non-Gaussian tails can be identified in the angular distribution on the plane perpendicular to the beam propagation direction. Stray particles, nonhomogeneous beam and large divergence might result in unexpected heat and particle loads over ITER neutral beam injector (NBI) accelerator grids; it is the goal of SPIDER to assess and possibly to identify suitable methods for controlling these beam features. A major shutdown, planned for late 2021, to solve the issues identified during the operation and to carry out scheduled modifications, is outlined. Such improvements are expected to allow SPIDER to pursue the ITER requirements in terms of negative ion current, electron-to-ion ratio, and beam duration.

An indispensable step towards coordinating the actions of distribution and transmission system operators (dsos-tso) is to estimate the range of flexibility that can be offered to the tso by dsos. Within this context, a data-driven probabilistic approach is proposed to evaluate the capability of a distribution network in providing active and reactive power support in real-time. To this end, in an offline phase, a linear discriminant analysis model, together with a piecewise linear model of the distribution network are trained to delineate the boundary of a region representing the adherence to distribution network operational constraints. In the implementation phase, this region comprises the feasible set of a series of optimization problems, formulated to determine the support provision capability. These optimization problems are of iterative linear programming type, which allows for real-time applicability. The evaluated support capability can be deemed as the available reserve in real-time transmission operation, which enables providing a coordinated response towards unexpected events, and facilitates the participation of distributed resources in the balancing market by granting an up-to-date estimation of available supports. This approach is tested on the IEEE 123-node system and verified through comparison with an AC optimal power flow technique.

Background: Covid-19 pandemic has boosted telemedicine in medical clinical practice. Experience in the management of chronic neurological disorders is limited as well as patient opinion. During Covid-19 pandemic, we evaluated patients’ satisfaction and opinion about televisits in a large group of patients with chronic neurological disorders. Methods: All patients with chronic neurological disorders who had a virtual visit during the first phase of pandemic were invited to fill an online anonymous questionnaire about their global satisfaction and satisfaction regarding continuity of care, possibility to stay at home, doctor–patient relationship, the future of teleconsultation after pandemic and the possibility of understanding medical information and instructions. Results: We received 123 questionnaires among 232 e-mail (response rate 53%). Almost all (120 out of 121 patients, 99%) were satisfied with the overall experience with video-consultation. Comprehension of medical information was the same for 113 out of 122 patients (93%) and also the doctor–patient relationship was the same for 107 out of 122 respondents (88%) or better for 10 (8%). Ninety-three percent of patients (112 out of 120) were keen to integrate televisits with the traditional modality and only 11 out of 121 patients (9%) judged televisits as an option to discard. As a whole, 114 out of 122 respondents (93%) would suggest this modality to other patients. Conclusions: Our large cohort of patients with chronic neurologic disorders rated experience with televisits satisfactory. Comprehension of medical information and doctor–patient interaction was considered good. Eventually, patients are keen to integrate this modality with traditional follow-up visits.

Source for the production of ions of deuterium extracted from a radio frequency plasma (SPIDER) is the full-scale prototype for the ITER heating neutral beam (HNB) source. Beam operation in SPIDER, with the eventual aim of achieving 100-keV, 46-A hydrogen negative ion beam for 1 h, has mainly been performed in volume operation without cesium. Recently, with the introduction of cesium, SPIDER has moved to the surface operational regime, with a noted increase in beam current. Electrical and calorimetric measurements of the beam current have previously shown discrepancies between the two, possibly due to background gas interactions in the accelerator and upstream of the STRIKE calorimeter. Taking advantage of the reduced number of open apertures due to the plasma grid (PG) mask, a series of fluxgate current transducers have been installed in SPIDER to measure the current of individual beamlets. These beamlet current monitors (BCMs), positioned downstream of the grounded grid (GG) around individual grid apertures, have provided direct measurement of the current of up to five individual beamlets during the recent SPIDER campaigns with and without cesium. This contribution describes the installation of the BCM dc sensors, analysis of the dc signal, and initial characterization of the beamlet direct current component in volume and surface operational regimes.

The electrical insulation of the Megavolt ITER Injector and Concept Advancement (MITICA) beam source (BS) at 1 MV in vacuum is a challenging issue, which could not be fully addressed so far on the basis of experimental results and of theoretical models available in literature. A specific high-voltage (HV) test campaign is being prepared to validate and optimize the voltage holding capability of the BS insulation under realistic conditions, using full-size mockup electrodes reproducing in detail the geometry of the BS and accelerator. The proposed test strategy will address both the single-gap and the multistage insulation, so as to obtain a verification of voltage holding at 1 MV before the installation of the real components. This approach is intended to reduce the risk related to the HV insulation at 1 MV and, if necessary, to allow the development of effective corrections. In this article, the test motivations and requirements are defined, and the electrode implementation and diagnostic setup are described. Finally, the test configurations and the experimental procedure are discussed.
