Root canal re-treatment (RCR) cases are considered some of the most challenging cases in the field of endodontics, as they are mostly associated with various iatrogenic errors such as ledge formation, incomplete biomechanical preparation, file separation, and incomplete obturation. These iatrogenic errors lead to defective niches within root canals that may act as reservoirs for various viable microorganisms. Such residual microbial niches may cause postoperative pain even after thorough debridement and reshaping the canals, ultimately leading to a poor prognosis for the tooth. Nowadays, prevention of postoperative pain in re-treatment cases and prognosis are effectively managed by photobiomodulation therapy (PBMT).

This report discusses the case of a failed root canal treatment in the right mandibular first molar, which exhibited a chronic periapical pathology in its distal and mesial roots. The tooth was managed surgically by periapical curettage and apicoectomy. This approach is indicated in teeth having periapical pathosis which does not resolve by routine endodontic treatment. The aim of periapical curettage is to make the apical periodontium and root canals of the treated tooth free of bacteria and their products. With the advancements in magnification, armamentaria, and materials, the ease of operation and success rate of apicoectomy have underwent a significant improvement. The treated case showed a complete resolution of clinical symptoms and the periapical pathology, thus substantiating the effectiveness of this treatment approach.

Conventional freehand techniques for removing fiber posts are time-consuming, sometimes result in iatrogenic errors, and heavily rely on the practitioner’s experience. The endodontic static guide can be an alternative method. While the use of an endodontic 3D-printed static guide for fiber post removal has been reported as highly successful, it can also cause complications. Skipping any critical steps during the guide construction or its clinical application can lead to errors. This case report presents the saving of a compromised tooth with a fractured fiber post and a periapical lesion around the apex through the use of an endodontic static guide for fiber post removal. This study describes possible sources of error that may happen during construction and clinical use of the guide.
