There is an increasing interest in complementary and alternative veterinary medicine (CAVM). There is, however, an uncertainty of the efficacy of these methods. Therefore, the aim of this systematic literature review is to assess the evidence for clinical efficacy of 24 CAVM therapies used in cats, dogs, and horses. A bibliographic search, restricted to studies in cats, dogs, and horses, was performed on Web of Science Core Collection, CABI, and PubMed. Relevant articles were assessed for scientific quality, and information was extracted on study characteristics, species, type of treatment, indication, and treatment effects. Of 982 unique publications screened, 42 were eligible for inclusion, representing nine different CAVM therapies, which were aromatherapy, gold therapy, homeopathy, leeches (hirudotherapy), mesotherapy, mud, neural therapy, sound (music) therapy, and vibration therapy. For 15 predefined therapies, no study was identified. The risk of bias was assessed as high in 17 studies, moderate to high in 10, moderate in 10, low to moderate in four, and low in one study. In those studies where the risk of bias was low to moderate, there was considerable heterogeneity in reported treatment effects. Therefore, the scientific evidence is not strong enough to define the clinical efficacy of the 24 CAVM therapies.

Mesotherapy, or intradermal therapy, is a therapeutic approach that is gaining popularity, but there is still a significant lack of information on its mechanisms of action or the pharmacokinetics of the therapeutic regimens. This retrospective study on 220 records compared the short-term and long-term effects of mesotherapy using a mixture of drugs versus normal saline solution in the treatment of patients with chronic spinal pain (CSP). At the end of treatment, outcome measures showed a significant improvement (P<0.003) in both groups, which persisted at the follow-up assessments. At 12 weeks of follow-up, the improvement was significantly greater in patients treated with the drug cocktail than with the saline solution (P<0.05). Mesotherapy was effective in patients affected by CSP, with high patient satisfaction reported irrespective of the agent used. Considering the risks and costs of drugs, normal saline solution appears to be the best agent in cost-benefit terms for treating localized pain by mesotherapy in CSP.

Because of low injection volume, the recently marketed injectable solution of diclofenac in complex with -cyclodextrin (Akis, IBSA Farmaceutici Italia) is an ideal candidate for mesotherapy applications. In this study, we investigated the solubility of Akis, 25 and 50mg/kg, in combination with various local anaesthetics (lidocaine, mepivacaine, bupivacaine, levobupivacaine, and ropivacaine) at different concentrations in aqueous vehicles (normal saline, sterile water, or bicarbonate). Final injection mixtures were classified as limpid, turbid, or milky at visual analysis under standardized conditions.We found that (i) the use of sterile water for injections or normal saline as vehicles to dilute Akis in combination with whatever local anaesthetic normally results in milky solutions and therefore is not recommended; (ii) using bicarbonate, optimal solubility was obtained combining Akis with lidocaine, both 1 and 2%, or mepivacaine, both 1 and 2%, whereas solutions were turbid in combination with bupivacaine, levobupivacaine, or ropivacaine.Thus, we recommend that Akis is used in combination with lidocaine or mepivacaine in a bicarbonate vehicle.

The latest scientii c knowledge on the effects of electric i elds in the body tissues has deve loped a new form of therapy instrumental with electric currents as a physical vehicle for the transdermal application of topical drugs , mainly NSAIDs. The traditional technique of mesothe rapy, i rst described in 1952 , is leading to new ways of applying active substances by electric current , radiation, etc. , without allowing to in i ltrate the tissue. These techniques are reuni ted with traditional treatments sonoforesis and iontophoresis , although a recent form of elec trotherapy 1MHz pulsed currents has provided scientii c evidence of increased penetration of the active substance in body tissues when this type of current is used instead of other tech niques of traditional physical therapy. Electro poration as an alternative treatment and ionto phoresis ultrasonophoresis show, to present a theoretical model of mechanism of action im proved over both techniques herein., El reciente conocimiento cientíi co sobre los efectos de los campos eléctricos en los tejidos corporales ha desarrollado una nueva modalidad de terapia instrumental con corrientes eléctricas como vehículo físico para la aplicación transdérmica de fármacos tópicos, principalmente antiinflamatorios no esteroideos. La técnica tradicional de mesoterapia, descrita por primera vez en 1952, está dando lugar a nuevas modalidades de aplicación de sustancias activas por medio de corrientes eléctricas, radiaciones, etc, que permiten prescindir de infiltrar el tejido. Estas técnicas se reencuentran con los tratamientos tradicionales de sonoforesis e iontoforesis, aunque una reciente forma de electroterapia con corrientes pulsadas de 1MHz ha aportado evidencias científicas de una mayor penetración de la sustancia activa en los tejidos corporales cuando esta modalidad de corriente es empleada en lugar de otras técnicas de terapia física tradicionales. En el presente texto se presenta la electroporación como alternativa de tratamiento a la iontoforesis y la ultrasonoforesis, por presentar un modelo teórico de mecanismo de acción mejorado respecto a ambas técnicas. Also, the clinical picture indicated the areas where cellulite appears, the stages and the factors of development. Following are the sensory responses and treatments restore the skin aesthetic. Finally, the work indicated in mesotherapy, and the used and how applied.
