The institution of American policing is at an inflection point. While most Americans connect the police with safety, highly publicized police killings of unarmed people of color have brought racial disparities to the forefront of the American consciousness, and pushed many people to question the fundamental role and function of the police. The purpose of this dissertation is to analyze the institution of American policing, starting with its historical origins, to develop theory on the role and function of police in an attempt to identify strategies for safety. Combining historical analysis with qualitative data collected from police officers and community members in a mid-sized American city, this dissertation identifies new insights into how deeply rooted racial divisions are to the establishment and structure of American policing, the centrality of communication to the role and function of police and how police use communication as a strategy to create and maintain safety, perceptions of safety for community members, and community-based strategies for safety. Findings from the historical analysis indicate that police violence persists because it is operating within larger systems of structural violence exerted against racialized and marginalized groups. Additionally, findings from the qualitative research signify the importance of communication for police officers and community residents and the central connections between communication and safety for both groups. Taken together, these findings can inform policy intended to create greater safety, both for police officers and community residents.

Line intensity mapping (LIM) is an emerging technique in measuring galaxy evolution and the large-scale structure of the universe. LIM surveys measure the cumulative emission from all galaxies emitting a given line at a particular redshift, which trace the distribution of dark matter throughout the universe. In this proceeding, we provide an introduction to LIM modeling, focusing on power spectrum calculation. Beyond these calculations, we describe how these power spectra may be used to constrain properties of galaxy evolution and large-scale structure cosmology. Throughout, we use the anticipated EXCLAIM signal of ionized carbon ([CII]) at redshift $z=3$ as a case study. Our goal is to provide a starting point to non-experts, e.g. upper-level undergraduate and graduate students familiar with the basics of cosmology, with the tools necessary to understand the literature and generate LIM power spectrum models themselves, while also describing a wide array of literature for continued studies.

Purpose: The purpose of this paper is to investigate the overall campaign reach and impact of the ABCs of Mental Health in Denmark; a secondary objective is to investigate how mental health-promoting beliefs and actions are associated with good mental health. Design/methodology/approach: A questionnaire was administered to two representative cross-sectional samples of the Danish population (1,508 respondents in 2019; 1,507 respondents in 2021) via an online survey. The data were subsequently pooled together into one sample consisting of 3,015 respondents. In addition to questions pertaining to campaign reach and impact, the questionnaire also included a validated scale for mental well-being and questions about beliefs and actions in regard to enhancing mental health. Findings: About 7.6% had been reached by the campaign (familiar with ABC name or messages), or 11.9% when also counting familiarity with campaign slogans. Among these, respondents reported (proportions in parentheses) that the campaign had 1) made them reflect on their mental health (74.2%), talk to friends and family about mental health (35.5%), given them new knowledge about what they can do to enhance mental health (78.4%), or take action to enhance their own mental health (16.2%). An internal well-being locus of control and proactive behaviours towards enhancing mental health are shown to be associated with higher mean scores on mental well-being, lower odds of low mental well-being and higher odds of higher mental well-being. Originality/value: An internal well-being locus of control and proactive behaviours towards enhancing mental health are suggested to both prevent low levels of mental well-being and promoting high levels of mental well-being. The results indicate that the ABCs of Mental Health campaign may be implemented to promote such beliefs and actions universally throughout the population.

However, what roles are involved in intersectoral MHP and the interrelatedness of these roles have received limited attention in the existing literature. Therefore, the aim of this study is, first, to develop a practice-oriented taxonomy of roles within intersectoral MHP practices and, secondly, to investigate interactions across roles within the implementation of MHP practices. We conducted secondary analyses on data from a study (conducted between 2019 and 2021) evaluating MHP efforts within a Danish partnership. Data included 17 interviews, 10 telephone interviews, two group interviews, observational data, and documents. Analytical processes were inspired by the approach of Template Analysis and drew on results of prior evaluations of the partnership and literature on MHP. We propose a taxonomy consisting of five roles: 1) decision-maker; 2) MHP specialist; 3) MHP co-ordinator; 4) MHP practitioner; and 5) end user. Furthermore, applying these roles in a case study of the implementation of MHP practices in a Danish community setting, we elucidate and exemplify the interactions and interrelatedness across roles. The proposed taxonomy extends the current evidence-base for MHP by offering a language that can be used to clarify roles and functions within MHP practices across sectors and professions. It is, to our knowledge, the first of its kind specifically for MHP. Overall, our results add to the literature on MHP and facilitate a greater understanding of the intersectoral actions of MHP, which is relevant for strengthening practices and capacity building within MHP. Image attribution -- matching an image back to a trusted source -- is an emerging tool in the fight against online misinformation. Deep visual fingerprinting models have recently been explored for this purpose. However, they are not robust to tiny input perturbations known as adversarial examples. First we illustrate how to generate valid adversarial images that can easily cause incorrect image attribution. Then we describe an approach to prevent imperceptible adversarial attacks on deep visual fingerprinting models, via robust contrastive learning. The proposed training procedure leverages training on $\ell_\infty$-bounded adversarial examples, it is conceptually simple and incurs only a small computational overhead. The resulting models are substantially more robust, are accurate even on unperturbed images, and perform well even over a database with millions of images. In particular, we achieve 91.6% standard and 85.1% adversarial recall under $\ell_\infty$-bounded perturbations on manipulated images compared to 80.1% and 0.0% from prior work. We also show that robustness generalizes to other types of imperceptible perturbations unseen during training. Finally, we show how to train an adversarially robust image comparator model for detecting editorial changes in matched images.

The EXperiment for Cryogenic Large-Aperture Intensity Mapping (EXCLAIM) is a balloon-borne cryogenic telescope that will survey the spectrum of diffuse emission from both the Milky Way and the cosmic web to probe star formation, the interstellar medium, and galaxy evolution across cosmic time. EXCLAIM's primary extragalactic science survey maps 305 deg$^2$ along the celestial equator with an R=512 spectrometer over the frequency range \nu=420-540 GHz, targeting emission of the [CII] line over redshifts 2.5

This paper describes a cryogenic optical testbed developed to characterize u-Spec spectrometers in a dedicated dilution refrigerator (DR) system. u-Spec is a far-infrared integrated spectrometer that is an analog to a Rowland-type grating spectrometer. It employs a single-crystal silicon substrate with niobium microstrip lines and aluminum kinetic inductance detectors (KIDs). Current designs with a resolution of 512 are in fabrication for the EXCLAIM (Experiment for Cryogenic Large Aperture Intensity Mapping) balloon mission. The primary spectrometer performance and design parameters are efficiency, NEP, inter-channel isolation, spectral resolution, and frequency response for each channel. Here we present the development and design of an optical characterization facility and preliminary validation of that facility with earlier prototype R=64 devices. We have conducted and describe initial optical measurements of R = 64 devices using a swept photomixer line source. The current state of far-infrared astronomy drives the need to develop compact, sensitive spectrometers for future space and ground-based instruments. Here we present details of the $\rm \mu$-Spec spectrometers currently in development for the far-infrared balloon mission EXCLAIM. The spectrometers are designed to cover the $\rm 555 - 714\ \mu$m range with a resolution of $\rm R\ =\ \lambda / \Delta\lambda\ =\ 512$ at the $\rm 638\ \mu$m band center. The spectrometer design incorporates a Rowland grating spectrometer implemented in a parallel plate waveguide on a low-loss single-crystal Si chip, employing Nb microstrip planar transmission lines and thin-film Al kinetic inductance detectors (KIDs). The EXCLAIM $\rm \mu$-Spec design is an advancement upon a successful $\rm R = 64\ \mu$-Spec prototype, and can be considered a sub-mm superconducting photonic integrated circuit (PIC) that combines spectral dispersion and detection. The design operates in a single $M{=}2$ grating order, allowing one spectrometer to cover the full EXCLAIM band without requiring a multi-order focal plane. The EXCLAIM instrument will fly six spectrometers, which are fabricated on a single 150 mm diameter Si wafer. Fabrication involves a flip-wafer-bonding process with patterning of the superconducting layers on both sides of the Si dielectric. The spectrometers are designed to operate at 100 mK, and will include 355 Al KID detectors targeting a goal of NEP ${\sim}8\times10^{-19}$ $\rm W/\sqrt{Hz}$. Microwave Kinetic Inductance Detectors (MKIDs) are highly scalable detectors that have demonstrated nearly background-limited sensitivity in the far-infrared from high-altitude balloon-borne telescopes and space-like laboratory environments. In addition, the detectors have a rich design space with many optimizable parameters, allowing highly sensitive measurements over a wide dynamic range. For these reasons, MKIDs were chosen for the Experiment for Cryogenic Large-Aperture Intensity Mapping (EXCLAIM), a balloon-borne telescope targeting nearly background-limited performance in a high-altitude atmospheric environment from 420-540 GHz. We describe MKID optimization in the specific context of EXCLAIM and provide general results that apply to broader applications. Extending the established approach of tone frequency tracking, we show that readout power optimization enables significant, further improvement in dynamic range.

Current and future generations of intensity mapping surveys promise dramatic improvements in our understanding of galaxy evolution and large-scale structure. An intensity map provides a census of the cumulative emission from all galaxies in a given region and redshift, including faint objects that are undetectable individually. Furthermore, cross-correlations between line intensity maps and galaxy redshift surveys are sensitive to the line intensity and clustering bias without the limitation of cosmic variance. Using the Fisher information matrix, we derive simple expressions describing sensitivities to the intensity and bias obtainable for cross-correlation surveys, focusing on cosmic variance evasion. Based on these expressions, we conclude that the optimal sensitivity is obtained by matching the survey depth, defined by the ratio of the clustering power spectrum to noise in a given mode, between the two surveys. We find that mid- to far-infrared space telescopes could benefit from this technique by cross-correlating with coming galaxy redshift surveys such as those planned for the Nancy Grace Roman Space Telescope, allowing for sensitivities beyond the cosmic variance limit. Our techniques can therefore be applied to survey design and requirements development to maximize the sensitivities of future intensity mapping experiments to tracers of galaxy evolution and large-scale structure cosmology., Comment: 10 pages, 5 figures

Hydrogen intensity mapping is a new field in astronomy that promises to make three-dimensional maps of the matter distribution of the Universe using the redshifted $21\,\textrm{cm}$ line of neutral hydrogen gas (HI). Several ongoing and upcoming radio interferometers, such as Tianlai, CHIME, HERA, HIRAX, etc. are using this technique. These instruments are designed to map large swaths of the sky by drift scanning over periods of many months. One of the challenges of the observations is that the daytime data is contaminated by strong radio signals from the Sun. In the case of Tianlai, this results in almost half of the measured data being unusable. We try to address this issue by developing an algorithm for solar contamination removal (AlgoSCR) from the radio data. The algorithm is based on an eigenvalue analysis of the visibility matrix, and hence is applicable only to interferometers. We apply AlgoSCR to simulated visibilities, as well as real daytime data from the Tianlai dish array. The algorithm can remove most of the solar contamination without seriously affecting other sky signals and thus makes the data usable for certain applications.

Incarceration is a pervasive issue in the United States that is enormously costly to families, communities, and society at large. The path from prison back to prison may depend on the relationship a person has with their probation or parole officer (PPO). If the relationship lacks appropriate care and trust, violations and recidivism (return to jail or prison) may be more likely to occur. Here, we test whether an "empathic supervision" intervention with PPOs-that aims to reduce collective blame against and promote empathy for the perspectives of adults on probation or parole (APPs)-can reduce rates of violations and recidivism. The intervention highlights the unreasonable expectation that all APPs will reoffend (collective blame) and the benefits of empathy-valuing APPs' perspectives. Using both within-subject (monthly official records for 10 mo) and between-subject (treatment versus control) comparisons in a longitudinal study with PPOs in a large US city (NPPOs = 216; NAPPs =∼20,478), we find that the empathic supervision intervention reduced collective blame against APPs 10 mo postintervention and reduced between-subject violations and recidivism, a 13% reduction that would translate to less taxpayer costs if scaled. Together, these findings illustrate that very low-cost psychological interventions that target empathy in relationships can be cost effective and combat important societal outcomes in a lasting manner.

The EXperiment for Cryogenic Large-Aperture Intensity Mapping (EXCLAIM) is a cryogenic balloon-borne instrument that will map carbon monoxide and singly-ionized carbon emission lines across redshifts from 0 to 3.5, using an intensity mapping approach. EXCLAIM will broaden our understanding of these elemental and molecular gases and the role they play in star formation processes across cosmic time scales. The focal plane of EXCLAIM's cryogenic telescope features six {\mu}-Spec spectrometers. {\mu}-Spec is a compact, integrated grating-analog spectrometer, which uses meandered superconducting niobium microstrip transmission lines on a single-crystal silicon dielectric to synthesize the grating. It features superconducting aluminum microwave kinetic inductance detectors (MKIDs), also in a microstrip architecture. The spectrometers for EXCLAIM couple to the telescope optics via a hybrid planar antenna coupled to a silicon lenslet. The spectrometers operate from 420 to 540 GHz with a resolving power R={\lambda}/{\Delta}{\lambda}=512 and employ an array of 355 MKIDs on each spectrometer. The spectrometer design targets a noise equivalent power (NEP) of 2x10-18W/\sqrt{Hz} (defined at the input to the main lobe of the spectrometer lenslet beam, within a 9-degree half width), enabled by the cryogenic telescope environment, the sensitive MKID detectors, and the low dielectric loss of single-crystal silicon. The EXperiment for Cryogenic Large-Aperture Intensity Mapping (EXCLAIM) is a balloon-borne far-infrared telescope that will survey star formation history over cosmological time scales to improve our understanding of why the star formation rate declined at redshift z < 2, despite continued clustering of dark matter. Specifically,EXCLAIM will map the emission of redshifted carbon monoxide and singly-ionized carbon lines in windows over a redshift range 0 < z < 3.5, following an innovative approach known as intensity mapping. Intensity mapping measures the statistics of brightness fluctuations of cumulative line emissions instead of detecting individual galaxies, thus enabling a blind, complete census of the emitting gas. To detect this emission unambiguously, EXCLAIM will cross-correlate with a spectroscopic galaxy catalog. The EXCLAIM mission uses a cryogenic design to cool the telescope optics to approximately 1.7 K. The telescope features a 90-cm primary mirror to probe spatial scales on the sky from the linear regime up to shot noise-dominated scales. The telescope optical elements couple to six {\mu}-Spec spectrometer modules, operating over a 420-540 GHz frequency band with a spectral resolution of 512 and featuring microwave kinetic inductance detectors. A Radio Frequency System-on-Chip (RFSoC) reads out the detectors in the baseline design. The cryogenic telescope and the sensitive detectors allow EXCLAIM to reach high sensitivity in spectral windows of low emission in the upper atmosphere. Psychotic experiences (PEs) are common in the general population in preadolescence. The implications of PEs on socioeconomic outcomes, including educational attainment, are scarcely described. We aimed to estimate how preadolescent PEs were associated with later healthcare costs, school performance, and health-related quality of life (HRQoL) in adolescence. A total of 1607 preadolescents from the general population Copenhagen Child Cohort 2000 were assessed for PEs at age 11-12 years and followed up over 5 years using register-based data on mental and somatic healthcare costs, and school performance at age 16. Furthermore, HRQoL was assessed for a subsample of the children at age 16-17. We adjusted for perinatal and family sociodemographic adversities, prior parental mental illness and healthcare use, child IQ-estimate at age 11-12, and parent-rated general psychopathology of their child. PEs were associated with slightly poorer school performance. However, preadolescents with PEs more often reported HRQoL within the lowest 10th percentile (OR = 2.74 [95% CI 1.71-4.37]). Preadolescents who reported PEs had higher average total healthcare costs over the following 5 years. The costs for individuals with PEs were higher for mental healthcare services across primary to tertiary care, but not for somatic care. After adjustments, PEs remained independently associated with higher costs and poorer HRQoL, but not with poorer school performance. In conclusion, PEs are important in mental health screening of preadolescents and identify a group of young people with increased healthcare service-use throughout adolescence and who report poorer HRQoL in adolescence, over and above parent-rated general psychopathology of their child.

This work describes the optical design of the EXperiment for Cryogenic Large-Aperture Intensity Mapping (EXCLAIM). EXCLAIM is a balloon-borne telescope that will measure integrated line emission from carbon monoxide (CO) at redshifts z < 1 and ionized carbon ([CII]) at redshifts z = 2.5-3.5 to probe star formation over cosmic time in cross-correlation with galaxy redshift surveys. The EXCLAIM instrument will observe at frequencies of 420--540 GHz using six microfabricated silicon integrated spectrometers with spectral resolving power R = 512 coupled to kinetic inductance detectors (KIDs). A completely cryogenic telescope cooled to a temperature below 5 K provides low-background observations between narrow atmospheric lines in the stratosphere. Off-axis reflective optics use a $90$-cm primary mirror to provide 4.2' full-width at half-maximum (FWHM) resolution at the center of the EXCLAIM band over a field of view of 22.5'. Illumination of the 1.7 K cold stop combined with blackened baffling at multiple places in the optical system ensures low (< -40 dB) edge illumination of the primary to minimize spill onto warmer elements at the top of the dewar., Comment: 15 pages, 8 figures

The Tianlai Dish Pathfinder Array is a radio interferometer designed to test techniques for 21~cm intensity mapping in the post-reionization universe as a means for measuring large-scale cosmic structure. It performs drift scans of the sky at constant declination. We describe the design, calibration, noise level, and stability of this instrument based on the analysis of about $\sim 5 \%$ of 6,200 hours of on-sky observations through October, 2019. Beam pattern determinations using drones and the transit of bright sources are in good agreement, and compatible with electromagnetic simulations. Combining all the baselines, we make maps around bright sources and show that the array behaves as expected. A few hundred hours of observations at different declinations have been used to study the array geometry and pointing imperfections, as well as the instrument noise behaviour. We show that the system temperature is below 80~K for most feed antennas, and that noise fluctuations decrease as expected with integration time, at least up to a few hundred seconds. Analysis of long integrations, from 10 nights of observations of the North Celestial Pole, yielded visibilities with amplitudes of 20-30~mK, consistent with the expected signal from the NCP radio sky with $<10\,$mK precision for $1 ~\mathrm{MHz} \times 1~ \mathrm{min}$ binning. Hi-pass filtering the spectra to remove smooth spectrum signal yields a residual consistent with zero signal at the $0.5\,$mK level., Comment: 30 pages, 38 figures

BACKGROUND: Torture survivors typically present with varied and complex symptoms, which may challenge assessment by general practitioners (GPs). This study explored the prevalence of torture and trauma history among immigrants born in non-Western countries presenting to GPs in Denmark and the extent to which GPs ask this population about torture or trauma history. METHODS: Based on a self-reported questionnaire among non-western immigrant patients, we used bivariate analyses to determine the prevalence of torture and trauma history and the proportion of patients being asked by their GP about this. Data were analysed using multivariate logistic regression. RESULTS: From 46 GP clinics, 300 questionnaires were finalized by immigrant patients. Twenty-eight percent of the patients had a history of torture. Of these, significantly more were men (70%) than women (29%). About half of the torture survivors (55%) had been asked by their GP about torture history. The odds ratio (OR, 95% confidence interval) for being asked about torture history by the GP was 1.28 (0.46-3.53) among women compared with men. Compared with Southeast Europe, OR for being a torture survivor among male immigrants from Middle East-North African region and South and East Asia was 1.83 (0.81-4.15) and 0.25 (0.08-0.82), respectively. CONCLUSIONS: Our results suggest that torture and trauma are widespread among immigrants presenting to GPs. In our study, the GPs had managed to detect half of the torture survivors. A more systematic approach to detection in General Practice is advisable, and more knowledge on how and when to ask is needed.

This study evaluates the potential of a new screening algorithm to identify children with MHP. Methods The study population comprises 2,015 children from the Copenhagen Child Cohort 2000 whose mental health was assessed at age 11–12 years and who had no prior use of specialised mental health services. A new algorithm based on the Strengths and Difficulties Questionnaire (SDQ) is utilised to identify MHP by combining parent-reported scores of emotional and behavioural problems and functional impairments. The screening is done on historical data, implying that neither parents, teachers nor health care professionals received any feedback on the screening status. The screening status and results of an IQ-test were linked to individual-level data from national registries. These national registers include records of each child’s school performance at the end of compulsory schooling, their health care utilisation, as well as their parents’ socio-economic status and health care utilisation. Results 10% of the children screen positive for MHP. The children with MHP achieve a significantly lower Grade Point Average on their exams, independently of their IQ-score, perinatal factors and parental characteristics. On average, the children with MHP also carry higher health care costs over a five-year follow-up period. The higher health care costs are only attributed to 23% of these children, while the remaining children with MHP also show poorer school performance but receive no additional health care. Conclusions The results demonstrate that children with MHP and a poor prognosis can be identified by the use of the brief standardised questionnaire SDQ combined with a screening algorithm.

The two types of arrays were chosen to determine the advantages and disadvantages of each approach. The primary goal of the Pathfinder is to make 3D maps by surveying neutral hydrogen over large areas of the sky %$20,000 {\rm deg}^2$ in two different redshift ranges: first at $1.03 > z > 0.78$ ($700 - 800$~MHz) and later at $0.21 > z > 0.12$ ($1170 - 1270$~MHz). The most significant challenge to $21$~cm intensity-mapping is the removal of strong foreground radiation that dwarfs the cosmological signal. It requires exquisite knowledge of the instrumental response, i.e. calibration. In this paper, we provide an overview of the status of the Pathfinder and discuss the details of some of the analysis that we have carried out to measure the beam function of both arrays. We compare electromagnetic simulations of the arrays to measurements, discuss measurements of the gain and phase stability of the instrument, and provide a brief overview of the data processing pipeline., Comment: 18 pages, 10 figures

Increasing litter size in hyperprolific sows has led to the need for management systems for surplus piglets, one of which is the use of nurse sows. The aim of this study was to investigate physiological changes in salivary cortisol, heart rate and number of milk let-downs in nurse sows compared to non-nurse sows. Sows were divided into three treatments: 1) control (non-nurse) sows nursed their own piglets until weaning at 26 days of age; 2) nurse1 sows had their own piglets removed and replaced with newborn piglets (between 6−24 h old) at Day 7, these were weaned at Day 33 of the sow's lactation period and 3) nurse2 sows weaned their own piglets at Day 21 and received a litter of 7 day old piglets from a nurse1 sow. These new piglets were weaned at Day 40 of the nurse2 sow's lactation period. Saliva samples were collected for cortisol analyses and the sows were fitted with pulse belts to monitor heart rate. Cameras were placed above the pens to record milk let-downs. Overall, there was no influence of treatment on salivary cortisol, heart rate or the number of milk let-downs/h. There was an effect of time as cortisol levels fell throughout lactation (P<0.001), and heart rate increased (P<0.001). Nurse1 sows had a lower cortisol concentration on Day 31 compared to Day 24 (P<0.028). The same was found for nurse2 sows, where the salivary cortisol concentration on Day 31 and Day 38 was significantly lower than on Day 24 (P<0.001). The present study found no differences in short-term (when the sows received new piglets) or long-term (throughout the lactation period) cortisol and heart rate measurements between different treatments. In addition, the frequency of milk let-down/h was the same for nurse sows as for non-nurse sows.

Transferring artistic styles onto everyday photographs has become an extremely popular task in both academia and industry. Recently, offline training has replaced on-line iterative optimization, enabling nearly real-time stylization. When those stylization networks are applied directly to high-resolution images, however, the style of localized regions often appears less similar to the desired artistic style. This is because the transfer process fails to capture small, intricate textures and maintain correct texture scales of the artworks. Here we propose a multimodal convolutional neural network that takes into consideration faithful representations of both color and luminance channels, and performs stylization hierarchically with multiple losses of increasing scales. Compared to state-of-the-art networks, our network can also perform style transfer in nearly real-time by conducting much more sophisticated training offline. By properly handling style and texture cues at multiple scales using several modalities, we can transfer not just large-scale, obvious style cues but also subtle, exquisite ones. That is, our scheme can generate results that are visually pleasing and more similar to multiple desired artistic styles with color and texture cues at multiple scales., Comment: Accepted by CVPR 2017

BACKGROUND: In Crohn's disease patients failing infliximab therapy, interventions defined by an algorithm based on infliximab and anti-infliximab antibody measurements have proven more cost-effective than intensifying the infliximab regimen.AIM: This study investigated long-term economic outcomes at the week 20 follow-up study visit and after 1 year. Clinical outcomes were assessed at week 20.METHODS: Follow-up from a 12-week, single-blind, clinical trial where patients with infliximab treatment failure were randomized to infliximab intensification (5 mg/kg every 4 weeks) (n = 36), or algorithm-defined interventions (n = 33). Accumulated costs, expressed as mean costs per patient, were based on the Danish National Patient Registry.RESULTS: At the scheduled week 20 follow-up study visit, response and remission rates were similar in all study subpopulations between patients treated by the algorithm or by infliximab intensification. However, the sum of healthcare costs related to Crohn's disease was substantially lower (31 %) for patients randomized to algorithm-based interventions than infliximab intensification in the intention-to-treat population: $11,940 versus $17,236; p = 0.005. For per-protocol patients (n = 55), costs at the week 20 follow-up visit were even lower (49 %) in the algorithm group: $8,742 versus $17,236; p = 0.002. Figures were similar for patients having completed the 12-week trial as per protocol (50 % reduction in costs) (n = 45). Among patients continuing the allocated study intervention throughout the entire 20-week follow-up period (n = 29), costs were reduced by 60 % in algorithm-treated patients: $7,056 versus $17,776; p < 0.001. Cost-reduction percentages remained stable throughout one year.CONCLUSION: Economic benefit of algorithm-based interventions at infliximab failure is maintained throughout 1 year.

Predefined co-primary end points at week 12 were proportion of patients responding (Crohn's Disease Activity Index (CDAI) decrease ≥ 70, or ≥ 50% reduction in active fistulas) and accumulated costs related to treatment of Crohn's disease, expressed as mean cost per patient, based on the Danish National Patient Registry for all hospitalisation and outpatient costs in the Danish healthcare sector.RESULTS: Costs for intention-to-treat patients were substantially lower (34%) for those treated in accordance with the algorithm than by IFX dose intensification: € 6038 vs € 9178, p<0.001. However, disease control, as judged by response rates, was similar: 58% and 53%, respectively, p=0.81; difference 5% (-19% to 28%).
