La ricerca si propone di indagare la possibile funzione educativa dei materiali audiovisivi d’animazione e, al contempo, visto il loro ruolo attivo nel produrre significati e percezioni ambientali, incentivarne l’uso come strumento educativo-didattico. I presupposti teorici da cui si muove la ricerca sono quelli dell’ecocriticism (Glotfelty, 1996), linea di pensiero che dimostra come i prodotti culturali di massa (Garrard, 2010; dell’Agnese, 2011) hanno un ruolo attivo nella produzione di idee sull’ambiente e conseguentemente nel condizionare i comportamenti umani nei confronti dello stesso. La crisi ambientale attuale impone un nuovo atteggiamento nei confronti dell’ambiente che ci circonda, richiede dunque un’azione di educazione ambientale e di promozione della consapevolezza di come le idee sull’ambiente derivino anche dai prodotti culturali di massa. La ricerca è stata costruita nell’ambito del PRIN Greening the Visual: an Environmental Atlas of Italian Landscape (2017-0422), che si propone di indagare il modo in cui le rappresentazioni visuali influenzano le idee ambientali della comunità andando a costruire un atlante visuale on-line dei paesaggi italiani. Il progetto Prin in questione ha coinvolto tre unità di lavoro, in particolare l’unità di ricerca dell’Università degli Studi di Milano-Bicocca, a cui afferisce il suddetto progetto di dottorato, si è dedicata all’analisi delle rappresentazioni relative al paesaggio costiero e marino. I materiali d’animazione sui quali si è lavorato hanno dunque riguardato lo spazio pelagico, attualmente al centro dell’attenzione mediatica e scientifica grazie alla proclamazione da parte delle Nazioni Unite della Decade of Ocean Science for Sustainable Development (2021-2030). Attenzione derivata dal riconoscimento, da parte della comunità scientifica internazionale, del grande ruolo rivestito dallo spazio marino nel mantenimento dell’equilibrio planetario così come lo conosciamo oggi. The theoretical assumptions from which the research moves are those of ecocriticism (Glotfelty, 1996), a line of thought that demonstrates how mass cultural products (Garrard, 2010; dell'Agnese, 2011) play an active role in the production of ideas about the environment and consequently in conditioning human behaviour towards it. The current environmental crisis imposes a new attitude towards the environment that surrounds us, and therefore requires environmental education and the promotion of awareness of how ideas about the environment also derive from mass cultural products. The research was carried out as part of the project Greening the Visual: an Environmental Atlas of Italian Landscapes (PRIN, 2017-0422), which aims to investigate the way in which visual representations influence the community's environmental ideas by constructing an online visual atlas of Italian landscapes that constitutes a repertoire of the ways in which Italian landscapes have been represented and thus communicated. The Prin project in question involved three work units. In particular, the research unit of the University of Milan-Bicocca, to which the PhD project in question relates, dedicated itself to the analysis of representations of the coastal and marine landscape. The animation materials on which the project worked therefore concerned the pelagic space, currently at the centre of media and scientific attention thanks to the proclamation by the United Nations of the Decade of Ocean Science for Sustainable Development (2021-2030). Un ruolo importante nella costruzione dell’immagine ambientale italiana lo svolgono i film d’animazione. Il presente contributo analizza le idee ambientali che emergono da alcuni cortometraggi ambientati in località marine e costiere così da identificarne temi, artifici narrativi e meccanismi discorsivi. Il loro riconoscimento è stato alla base del metodo con cui si sono scelti dei corti d’animazione utilizzati per svolgere attività educative e formative rivolte ad alunni e docenti della scuola primaria. La particolare efficacia del linguaggio dei cartoons ha stimolato il coinvolgimento emotivo degli alunni, capace di promuovere l’appropriazione affettiva degli spazi marini e di portare a una maggiore consapevolezza del ruolo che ciascuno può avere nella salvaguardia dell’ambiente marino. I risultati delle attività di ricerca-azione hanno confermato che l’utilizzo degli strumenti visuali è efficace per esplorare il paesaggio marino e la nostra relazione con esso., Animated films have an important role in the construction of the Italian environmental image. This paper analyses the environmental ideas emerging from some short films, set in marine and coastal sites, in order to identify their themes, narrative artifices and discursive mechanisms. Their recognition formed the basis of the method used to select short animated films used to carry out educational and training activities aimed at primary school students and teachers. The particular power of cartoons’ language stimulated emotional engagement in the young students, promoting affective appropriation of marine spaces and increasing in them the consciousness of their role in preserving the marine environment. The results of the action research activities confirmed that the use of visual tools is efficacious in terms of exploring the seascape and our relationship with it.

Geography and “the visual” are an inseparable duo: a powerful relationship that elicits research and papers in both geographic research and academic geography. Starting from the commentary on Giorda’s book, this short essay proposes a general reflection on the function that visual teaching aids (maps, images) have in producing and reproducing geographical stereotypes and imagery. The commentary specifically focuses on the value that work such as Giorda’s has for those involved in research in the field of geography in education today.

NAVIGATOR is an Italian regional project boosting precision medicine in oncology with the aim of making it more predictive, preventive, and personalised by advancing translational research based on quantitative imaging and integrative omics analyses. The project’s goal is to develop an open imaging biobank for the collection and preservation of a large amount of standardised imaging multimodal datasets, including computed tomography, magnetic resonance imaging, and positron emission tomography data, together with the corresponding patient-related and omics-related relevant information extracted from regional healthcare services using an adapted privacy-preserving model. The project is based on an open-source imaging biobank and an open-science oriented virtual research environment (VRE). Available integrative omics and multi-imaging data of three use cases (prostate cancer, rectal cancer, and gastric cancer) will be collected. All data confined in NAVIGATOR (i.e., standard and novel imaging biomarkers, non-imaging data, health agency data) will be used to create a digital patient model, to support the reliable prediction of the disease phenotype and risk stratification. The VRE that relies on a well-established infrastructure, called D4Science.org, will further provide a multiset infrastructure for processing the integrative omics data, extracting specific radiomic signatures, and for identification and testing of novel imaging biomarkers through big data analytics and artificial intelligence.

A settecento anni dalla morte di Dante, si è sviluppata un'ampia riflessione sul personaggio e sulla sua opera, in particolare sul valore che il testo conserva nell'attualità. Il presente contributo si è concentrato sul potenziale didattico del testo e delle illustrazioni dei luoghi della Divina Commedia per avvicinare i bambini alla comprensione del valore simbolico e paradigmatico del paesaggio., The Geography of Dante explained to children. – During the 7th century since Dante’s death, a broad reflection on the character and his work has developed, particularly on the value the text retains in the present day. This contribution has focused on the didactic potential that the text and the illustrations of the places of the Divine Comedy can have in order to introduce children to an understanding of the symbolic and paradigmatic value of the landscape.

Il seguente contributo vuole, in linea con il programma del «Decennio delle scienze del mare per lo sviluppo sostenibile» (2021-2030) e più in generale con l’Ocean Literacy e quindi con lo sforzo conoscitivo e divulgativo riferito all’ambiente marino, proporre l’uso di materiali cinematografici per promuovere l’educazione ambientale marina. Intende altresì riflettere sul ruolo che il visuale, in particolare il cinema d’animazione, può giocare nella didattica della geografia suggerendo alcuni esempi su come utilizzare il potere descrittivo ed evocativo dei film d’animazione in attività di discussione e riflessione con studenti e insegnanti di scuola primaria.

Il seguente contributo descrive una ricerca di dottorato, costruita all’interno del Prin Greening the Visual: an Environmental Atlas of Italian Landscapes2, che intende indagare il ruolo del cinema d’animazione, in particolare dei corti d’animazione, nel plasmare le idee ambientali e di come esse possono essere utilizzate per promuovere un’educazione ambientale marina e l’adozione di un atteggiamento di cittadinanza oceanica all’interno del contesto di scuola primaria. Il progetto di ricerca è volto contemporaneamente a verificare da un lato, il grado di consapevolezza degli insegnanti del ruolo attivo dei materiali d’animazione nel costruire l’idea di natura e del rapporto tra quest’ultima e gli esseri umani, dall’altro, a costruire un’attività di ricerca-azione in grado di rafforzare la consapevolezza dell’importanza degli ambienti naturali e della necessità di prendersene cura utilizzando come strumento educativo le fonti visive: nel contempo, rendere coscienti alunni e insegnanti che le fonti visive hanno un ruolo attivo nella costruzione della nostra idea di natura e di come sollecitino, plasmino e riorientino la percezione di essa. L’attività didattica e di ricerca si concentra in particolare sull’ambiente marino.

I libri di testo hanno un ruolo fondamentale nella trasmissione dei saperi a scuola. In occasione del decennio ONU dedicato alle scienze del mare si è voluto analizzare come i libri di testo per la scuola primaria affrontino questo spazio. La visione del mare che è veicolata da questi strumenti è fortemente terracentrica, il mare è prevalentemente descritto come il limite della terra ferma. Dal punto di vista iconografico si riscontrano delle contraddizioni tra testo e immagini, inoltre il mare è prevalentemente rappresentato sulla falsa riga della sua immagine turistica.

Nel presente contributo si utilizzano le pagine letterarie di Bernard Moitessier per analizzare la sua esperienza dell’ambiente, in particolare di quello marino e, più in dettaglio, con riferimento alla sua componente sonora. Si è voluto verificare se fosse possibile mettere in relazione la metodologia d’analisi testuale proposta da Maria de Fanis (2001) con le categorie di analisi del paesaggio sonoro proposte da Murray Schafer (1985) non solo per descrivere l’ambiente, ma, soprattutto, per far emergere come il paesaggio sonoro possa diventare la metafora delle complesse relazioni tra gli esseri umani e l’ambiente (Lando, 1993, p. 10). Il testo prescelto è La lunga rotta (1972) di Bernard Moitessier (1925-1994), un navigatore dalle notevoli capacità di scrittura, particolarmente attento alla componente sensoriale della sua esperienza del mare. L’autore sottopone gli spazi marini ad uno sguardo (e all’ascolto) peculiare che trasforma l’ambiente che lo circonda nel simbolo dei propri sentimenti, del senso della vita, sua e dell’intero genere umano e del rapporto tra quest’ultimo e il pianeta in cui vive., In this contribution we use the literary pages to analyse the experience of the environment, in particular the marine environment and, more in detail, with reference to its sound component. We wanted to see if it was possible to relate the textual analysis methodology proposed by Maria de Fanis (2001) with the categories of soundscape analysis proposed by Murray Schafer (1985) not only to describe the environment, but above all, to show how the soundscape can become a metaphor for the complex relationships between human beings and the environment (Lando, 1993, p. 10). The chosen text is La lunga rotta (1972) by Bernard Moitessier (1925-1994), a navigator with remarkable writing skills, particularly attentive to the sensory component of his experience of the sea. The author subjects the marine spaces to a peculiar look (and sound) that transforms the environment that surrounds him into a symbol of his own feelings, of the meaning of life, his and the whole human race and the relationship between the latter and the planet in which he lives., Dans cette contribution, les pages littéraires sont utilisées pour analyser l’expérience de l’environnement, en particulier l’environnement marin et, plus en détail, en référence à sa composante sonore. Nous avons voulu voir s’il était possible de relier la méthodologie d’analyse textuelle proposée par Maria de Fanis (2001) aux catégories d’analyse du paysage sonore proposées par Murray Schafer (1985) non seulement pour décrire l’environnement, mais surtout pour montrer comment le paysage sonore peut devenir une métaphore des relations complexes entre les êtres humains et l’environnement (Lando, 1993, p. 10). Children embrace the sea through Miyazaki’s narration. The contribution below aims to reflect on the role that cinematographic materials can play in the educational context and their validity in promoting knowledge and critical reflection on marine spaces in students, encouraging the adoption of an attitude of Ocean Citizenship. In particular, animated films were considered, a cultural product, popular and appreciated by young students, capable of appealing to the affective-emotional channel, using a language that is easy to understand, which encourages identification with the protagonists, allowing students to relate to the spaces represented through cognitive and affective intelligence. The example on which we will focus will be the animated feature film Ponyo on the Cliff by director Hayao Miyazaki.

PRIMAGE is a European Commission-financed project dealing with medical imaging and artificial intelligence aiming to create an imaging biobank in oncology. The project includes a task dedicated to the interoperability between imaging and standard biobanks. We aim at linking Digital imaging and Communications in Medicine (DICOM) metadata to the Minimum Information About BIobank data Sharing (MIABIS) standard of biobanking. A very first integration model based on the fusion of the two existing standards, MIABIS and DICOM, has been developed. The fundamental method was that of expanding the MIABIS core to the imaging field, adding DICOM metadata derived from CT scans of 18 paediatric patients with neuroblastoma. The model was developed with the relational database management system Structured Query Language. The integration data model has been built as an Entity Relationship Diagram, commonly used to organise data within databases. Five additional entities have been linked to the “Image Collection” subcategory in order to include the imaging metadata more specific to the particular type of data: Body Part Examined, Modality Information, Dataset Type, Image Analysis, and Registration Parameters. The model is a starting point for the expansion of MIABIS with further DICOM metadata, enabling the inclusion of imaging data in biorepositories.

Background & Aims: The ANSWER study reported that long-term albumin administration in patients with cirrhosis and uncomplicated ascites improves survival. During treatment, serum albumin increased within a month and remained stable thereafter. In this post hoc analysis, we aimed to determine whether on-treatment serum albumin levels could guide therapy. Methods: Logistic regression was used to assess the association between baseline serum albumin and mortality, as well as to determine on-treatment factors associated with mortality and to predict the achievement of a given on-treatment serum albumin level. Survival was assessed by Kaplan-Meier estimates and second-order polynomial regression. Patients whose on-treatment serum albumin remained below normal were compared with a subset of patients from the control arm matched by principal score. Results: Baseline serum albumin was closely associated with 18-month mortality in untreated patients; albumin treatment almost effaced this relationship. On-treatment serum albumin and MELD-Na at month 1 were the sole independent variables associated with mortality. Second-order polynomial regression revealed that survival improved in parallel with increased 1-month on-treatment serum albumin. Kaplan-Meier estimations showed that any value of 1-month on-treatment serum albumin (0.1 g/dl intervals) in the range 2.5–4.5 g/dl discriminated patient survival. In the normal range of serum albumin, the best discriminant value was 4.0 g/dl. Compared to untreated patients, survival even improved in patients whose on-treatment serum albumin remained below normal. Conclusion: Baseline serum albumin per se should not guide the decision to start albumin therapy. Conversely, 1-month on-treatment serum albumin levels are strongly associated with outcomes and could guide the use of albumin – 4.0 g/dl being the target threshold. However, even patients whose serum albumin remains below normal benefit from long-term albumin administration. Lay

This article's main contributions are twofold: 1) to demonstrate how to apply the general European Union's High-Level Expert Group's (EU HLEG) guidelines for trustworthy AI in practice for the domain of healthcare and 2) to investigate the research question of what does "trustworthy AI" mean at the time of the COVID-19 pandemic. To this end, we present the results of a post-hoc self-assessment to evaluate the trustworthiness of an AI system for predicting a multiregional score conveying the degree of lung compromise in COVID-19 patients, developed and verified by an interdisciplinary team with members from academia, public hospitals, and industry in time of pandemic. The AI system aims to help radiologists to estimate and communicate the severity of damage in a patient's lung from Chest X-rays. It has been experimentally deployed in the radiology department of the ASST Spedali Civili clinic in Brescia, Italy, since December 2020 during pandemic time. The methodology we have applied for our post-hoc assessment, called Z-Inspection®, uses sociotechnical scenarios to identify ethical, technical, and domain-specific issues in the use of the AI system in the context of the pandemic.

Il contributo analizza alcuni frammenti di capitelli romani reimpiegati nel complesso paleocristiano di San Lorenzo a Milano alcuni dei quali con tracce di policromia antica

Radiomics is a process of extraction and analysis of quantitative features from diagnostic images. Liquid biopsy is a test done on a sample of blood to look for cancer cells or for pieces of tumourigenic DNA circulating in the blood. Radiomics and liquid biopsy have great potential in oncology, since both are minimally invasive, easy to perform, and can be repeated in patient follow-up visits, enabling the extraction of valuable information regarding tumour type, aggressiveness, progression, and response to treatment. Both methods are in their infancy, with major evidence of application in lung and gastrointestinal cancer, while still undergoing evaluation in other cancer types. In this paper, the main oncologic applications of radiomics and liquid biopsy are reviewed, and a synergistic approach incorporating both tests for cancer diagnosis and follow-up is discussed within the context of systems medicine. Teaching Points: • Radiomics is a process of extraction and analysis of quantitative features from diagnostic images. • Most clinical applications of radiomics are in the field of oncologic imaging. • Radiomics applies to all imaging modalities. • A cluster of radiomic features is a “radiomic signature”. • Machine learning may improve the efficacy of radiomics analysis.

Background: Evidence is scarce on the efficacy of long-term human albumin (HA) administration in patients with decompensated cirrhosis. The human Albumin for the treatmeNt of aScites in patients With hEpatic ciRrhosis (ANSWER) study was designed to clarify this issue. Methods: We did an investigator-initiated multicentre randomised, parallel, open-label, pragmatic trial in 33 academic and non-academic Italian hospitals. We randomly assigned patients with cirrhosis and uncomplicated ascites who were treated with anti-aldosteronic drugs (≥200 mg/day) and furosemide (≥25 mg/day) to receive either standard medical treatment (SMT) or SMT plus HA (40 g twice weekly for 2 weeks, and then 40 g weekly) for up to 18 months. The primary endpoint was 18-month mortality, evaluated as difference of events and analysis of survival time in patients included in the modified intention-to-treat and per-protocol populations. This study is registered with EudraCT, number 2008–000625–19, and ClinicalTrials.gov, number NCT01288794. Findings: From April 2, 2011, to May 27, 2015, 440 patients were randomly assigned and 431 were included in the modified intention-to-treat analysis. 38 of 218 patients died in the SMT plus HA group and 46 of 213 in the SMT group. Overall 18-month survival was significantly higher in the SMT plus HA than in the SMT group (Kaplan-Meier estimates 77% vs 66%; p=0·028), resulting in a 38% reduction in the mortality hazard ratio (0·62 [95% CI 0·40–0·95]). 46 (22%) patients in the SMT group and 49 (22%) in the SMT plus HA group had grade 3–4 non-liver related adverse events. Interpretation: In this trial, long-term HA administration prolongs overall survival and might act as a disease modifying treatment in patients with decompensated cirrhosis. Despite this, the quantitative potential of imaging remains under-exploited because of variability of the measurement, lack of harmonised systems for data acquisition and analysis, and crucially, a paucity of evidence on how such quantitation potentially affects clinical decision-making and patient outcome. This article reviews the current evidence for the use of semi-quantitative and quantitative biomarkers in clinical settings at various stages of the disease pathway including diagnosis, staging and prognosis, as well as predicting and detecting treatment response. Lo studio e la documentazione degli affreschi esistenti sono iniziati nell’anno 2015 ed hanno visto un’intensa attività di ricerca da parte delle studiose, confluita nel testo dal titolo Le pitture della galleria superiore della cappella di S. Aquilino Milano: i motivi e le tecniche nel loro spazio architettonico, di cui si attende la pubblicazione in tempi prossimi su una rivista di archeologia specificatamente dedicata. Le pitture tardoantiche del sacello non sono mai state fatte oggetto di uno studio monografico; l’obiettivo è stato dunque quello di documentarle e descriverle al fine di studiarne, per la prima volta, i motivi ed il loro significato oltre che l’esecuzione dal punto di vista tecnico. Le pareti e le volte dipinte si inserivano in un articolato sistema decorativo con evidenti rimandi cristologici comprendente anche mosaici, sono perciò state studiate anche in relazione ad esso, per indagarne infine la loro funzionalità all’interno del contesto architettonico di appartenenza. Method: We studied 268 parents (134 couples) of healthy babies. At 12 months post-partum, both parents filled out, in a counterbalanced order, the Parenting Stress Index-Short Form, the Edinburgh Post-natal Depression Scale, the State-Trait Anxiety Inventory, and the Dyadic Adjustment Scale. Structural equation modeling was used to analyze the potential mediating effects of mental health on the relationship between parenting stress and dyadic adjustment. Results: Results showed the full mediation effect of mental health between parenting stress and dyadic adjustment. A multi-group analysis further found that the paths did not differ across mothers and fathers. Discussion: The results suggest that mental health is an important dimension that mediates the relationship between parenting stress and dyadic adjustment in the transition to parenthood.

Experienced readers interpreted the CT colonography images unassisted and then reviewed all colorectal lesion-like structures pinpointed by the CAD algorithm. Segmental unblinding of CT colonoscopy findings at colonoscopy was utilized. The sensitivity and specificity of unassisted and CAD-assisted reading in identifying individuals with 6-9-mm lesions were calculated and compared by means of pairwise analysis. RESULTS: A total of 618 participants (mean age, 57.9 years; 54.5% male) were included in the final analysis. Of these participants, 464 (75.1%) had no lesions 6 mm or larger, and 52 (8.4%) had 6-9-mm lesions. The sensitivity of CT colonography with unassisted reading and that with CAD-assisted reading in identifying individuals with 6-9-mm lesions was 65.4% (95% confidence interval [CI]: 50.9%, 78.0%) and 76.9% (95% CI: 63.2%, 87.5%; P = .016), respectively. No significant change in specificity was observed: The specificity of CT colonography with unassisted and that with CAD-assisted reading was 91.8% (95% CI: 88.9%, 94.1%) and 90.9% (95% CI: 88.0%, 93.4%; P = .063), respectively. Objectives: To develop a consensus and provide updated recommendations on liver MR imaging and the clinical use of liver-specific contrast agents. Methods: The European Society of Gastrointestinal and Abdominal Radiology (ESGAR) formed a multinational European panel of experts, selected on the basis of a literature review and their leadership in the field of liver MR imaging. A modified Delphi process was adopted to draft a list of statements. Descriptive and Cronbach’s statistics were used to rate levels of agreement and internal reliability of the consensus. Results: Three Delphi rounds were conducted and 76 statements composed on MR technique (n = 17), clinical application of liver-specific contrast agents in benign, focal liver lesions (n = 7), malignant liver lesions in non-cirrhotic (n = 9) and in cirrhotic patients (n = 18), diffuse and vascular liver diseases (n = 12), and bile ducts (n = 13). The overall mean score of agreement was 4.84 (SD ±0.17). Full consensus was reached in 22 % of all statements in all working groups, with no full consensus reached on diffuse and vascular diseases. Conclusions: The consensus provided updated recommendations on the methodology, and clinical indications, of MRI with liver specific contrast agents in the study of liver diseases. Key points: • Liver-specific contrast agents are recommended in MRI of the liver. • The hepatobiliary phase improves the detection and characterization of hepatocellular lesions. • Liver-specific contrast agents can improve the detection of HCC.

This is an official guideline of the European Society of Gastrointestinal Endoscopy (ESGE) and the European Society of Gastrointestinal and Abdominal Radiology (ESGAR). It addresses the clinical indications for the use of computed tomographic colonography (CTC). A targeted literature search was performed to evaluate the evidence supporting the use of CTC. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) system was adopted to define the strength of recommendations and the quality of evidence.

In the past, rudimentary devices were used to record surgical operations. Currently, the introduction of technologic advances such as high-definition television and the miniaturization of high-resolution digital video cameras provides an opportunity for making significantly enhanced surgical records. These enhancements, coupled with the recent advances in telemedicine and surgical simulation, will improve cardiac surgery training and skill acquisition, decrease operative times and costs, minimize morbidity, and improve overall patient care. The present paper provides a discussion of the media technology offered to surgeons for recording a surgical procedure on video. Hardware technology, including different types of cameras and analogical or digital post processing methods, are reviewed with a surgical ''eye''. This ''how to'' paper provides practical suggestions to surgeons in order to enhance surgical video recording.

BACKGROUND: Recent studies have sparked a renewed interest in the use of autogenous radial arteries in coronary operations. Some concerns have been found about sequelae of conventional harvesting. A less invasive technique for radial artery harvesting has been proposed by others using endoscopic devices. This technique is time consuming, needs expensive instrumentation and an important learning curve. METHODS: A new less invasive approach for radial harvesting has been developed with a light assisted retractor under direct vision. A small skin incision, median in the forearm, is followed by dissection of the proper plane of the artery. A subcutaneous tunnel is created around the vessel and all the branches are ligated or clipped. The dissection of the pedicle under the skin is completed with the aid of a modified light assisted retractor, originally designed for the saphenous vein harvesting. The incision is closed after heparin reversal with a small redon as drainage. RESULTS: A preliminary serie of 15 patients have been operated with this technique. In all patients the radial artery was patent and functional at the postoperative angiography. Morbidity included only a light hematoma at the beginning of our experience. CONCLUSION: This less invasive technique for the radial artery harvesting appears to be an excellent surgical compromise between the open technique and the endoscopic procedure; it is easy to perform, the learning curve is acceptable and it offers an excellent aesthetic result.

BACKGROUND: The exact incidence of associated aortic valve incompetence (AVI) and abdominal aortic aneurysm (AAA) in the general population is not known. In recent years, we have observed this association with increasing frequency. This observation is probably due to the extensive preoperative screening of the cardiac and vascular status of patients who are candidates for surgical procedures. The choice of the optimal surgical strategy is needed to achieve low operative morbidity and mortality. The present study reviews our experience with a subset of patients suffering the association of AVI and large AAA. Surgical strategy, clinical management and outcome are presented. METHODS: Between January 1982 and May 2000, 76 patients with the association of AAA and AVI have been evaluated in our institution. Forty-four patients have been treated for both AAA and aortic valve (AV) regurgitation. These patients have been divided into three groups on the basis of the surgical strategy adopted. Group 1: combined procedure (16 patients); group 2: AAA repair prior to AV surgery (nine patients); group 3: AV surgery prior to aneurysm repair (19 patients). RESULTS: Hospital mortality was 4.5% (two patients); overall mortality was 6.8% (three patients). CONCLUSIONS: In patients with AAA and AVI, an accurate and complete preoperative evaluation is essential. Surgical strategy should be individualized on the basis of the cardiac preoperative status.

Aortic root and sinotubular junction dilatation and aneurysm of ascending aorta are considered relative contra-indications to implantation of a stentless valve prosthesis, because the modified aortic geometry leads to aortic incompetence and early failure of the prosthesis. Aortic root reconstruction can be performed according to various techniques. We present a surgical technique in which a tubular graft, replacing an ascending aortic aneurysm, allows sinotubular remodeling and satisfactory implantation of a stentless prosthesis. The native aorta is inserted into the vascular prosthesis at the level of the sinotubular junction which is wrapped in order to prevent commissure spreading. Sizing of the vascular and valve prosthesis is made according to annular diameter. Since October 1999, 6 patients have been operated using this technique with good results. We report the case of a 35-year-old man who presented at our institution with intramural aortic hematoma shortly after inhaling cocaine and smoking crack cocaine. To our knowledge, such a case has not previously been reported in the English medical literature. Problems of diagnosis and the mechanisms of intramural hematoma and aortic dissection are discussed.
