El sector assegurador té la funció principal de protegir els clients dels diferents riscos a què s'exposen diàriament. En particular, són les agències asseguradores les encarregades de fer les diferents gestions sobre els seus clients. En aquest treball es porta a terme una segmentació de les agències de l'empresa Zurich en funció de la seva composició. Es plantegen tres algoritmes clustering jeràrquic: kmeans, kmedoids i kamila, destacant similituds i diferències entre ells. Es comparen i contrasten els diferents grups formats mitjançant test estadístics de Shapiro-Wilks i Kruskal-Wallis, així com les seves característiques més rellevants. I per últim, se n'extreuen diverses conclusions rellevants de cada a futurs passos amb les segmentacions obtingudes.

Objective: To investigate the relationship between ambient temperature and atmospheric pressure (AP) and the severity of heart failure (HF) decompensations.

To investigate the relationship of ambient temperature and atmospheric pressure (AP) at patient discharge after an episode of acute heart failure (AHF) with very early post-discharge adverse outcomes. We analyzed 14,656 patients discharged after an AHF episode from 26 hospitals in 16 Spanish cities. The primary outcome was the 7-day post-discharge combined adverse event (emergency department –ED- revisit or hospitalization due to AHF, or all-cause death), and secondary outcomes were these three adverse events considered individually. Associations (adjusted for patient and demographic conditions, and length of stay -LOS- during the AHF index episode) of temperature and AP with the primary and secondary outcomes were investigated. We used restricted cubic splines to model the continuous non-linear association of temperature and AP with each endpoint. Some sensitivity analyses were performed. Patients were discharged after a median LOS of 5 days (IQR = 1–10). The highest temperature at discharge ranged from − 2 to 41.6 °C, and AP was from 892 to 1037 hPa. The 7-day post-discharge combined event occurred in 1242 patients (8.4%), with percentages of 7-day ED-revisit, hospitalization and death of 7.8%, 5.1% and 0.9%, respectively. We found no association between the maximal temperature and AP on the day of discharge and the primary or secondary outcomes. Similarly, there were no significant associations when the analyses were restricted to hospitalized patients (median LOS = 7 days, IQR = 4–11) during the index event, or when lag-1, lag-2 or the mean of the 3 post-discharge days (instead of point estimation) of ambient temperature and AP were considered. Temperature and AP on the day of patient discharge are not independently associated with the risk of very early adverse events during the vulnerable post-discharge period in patients discharged after an AHF episode.

After analysing the impact of a cognitively oriented multicomponent home-based exercise training program on independence, perceived health status, and lower limb strength in a group of multimorbid and/or palliative older patients (MPO-P), we studied the associations between improvements following training and the participants and health personnel’s opinion of the program. 13 MPO-P, (80.15+4.20 years) completed four months of training progressing in autonomy (from two supervised sessions and one autonomous weekly -60min session-, to two autonomous and one supervised). This was followed by fourautonomous (detraining) weeks with a recommendation to continue the exercises. Volume and intensity were increased by introducing more global exercises, with a greater neuromuscular load and double tasks, and a shorter resting interval. No exercise was repeated during the session (EFAM-UV©). Independence (Barthel) and perceived health (physical SF-36, with no changes in the mental domain), were improved after supervised training, with a tendency to improve in strength (sitting and standing for 30-s) and perceived health (total SF-36). This effect was maintained after the autonomous detraining phase, with a very favourable final perception by both patients and health personnel, as indicated in questionnaires. The gain in independence correlated negatively with the users’perception, but this requirement ensured the improvement. There was no association between perceived health and the evaluation of the program., Tras analizar el impacto de un programa de entrenamiento domiciliario multicomponente, con orientación cognitiva, sobre independencia, percepción subjetiva de salud, y fuerza del miembro inferior en un grupo de adultos mayores crónicos multimórbidos y/o paliativos (MCM-Ps), se estudiaron las asociaciones entre las mejoras obtenidas y la valoración del programa de participantes y personal sanitario. 13 MCM-Ps (80.15+4.20 años) completaron cuatro meses de entrenamiento progresando en autonomía (de dos sesiones supervisadas y una autónoma semanalmente -60 min/sesión-, a dos autónomas y una supervisada). Le siguieron cuatro semanas autónomas (desentrenamiento) con recomendación de ejercicio. El volumen y la intensidad se aumentaron introduciendo más ejercicios integrales, con mayor carga neuromuscular y doble tarea, y menor tiempo de descanso entre ellos, sin repetirlos en la sesión (EFAM-UV©). Independencia (Barthel) y salud percibida (SF-36 físico, sin cambios en dominio mental) mejoraron tras el entrenamiento supervisado, con tendencia a la mejora de fuerza (sentarse y levantarse 30-s) y la salud percibida (SF-36total). El efecto permaneció tras la fase autónoma/desentrenamiento, con una percepción final muy favorable de pacientes y personal sanitario-determinada mediante cuestionarios-. La ganancia en independencia correlacionó negativamente con la valoración del usuario, pero esta exigencia aseguró su mejora. No hubo asociación entre salud percibida y valoración del programa.

Fundamentos: La Higiene de Manos (HM) es la medida más importante para prevenir las infecciones asociadas a la asistencia sanitaria. El objetivo de este trabajo fue conocer el impacto de la pandemia de COVID-19 en el grado de cumplimiento (GC) de la HM en un Servicio de Urgencias (SU). Métodos: Estudio transversal. Se calculó el GC de la HM en el SU desde 2005 hasta 2021. Se estudió la asociación entre el GC de la HM y distintas variables (edad, sexo, estamento, área de urgencias, momento de la OMS, etc) calculando la Odds Ratio (OR) y su intervalo de confianza al 95% (IC95%). Para estudiar el impacto de la pandemia, se comparó el GC de la HM antes y después del inicio de la pandemia de COVID-19, se calculó la OR y su IC95%. Resultados: El GC de la HM aumentó al 75,9% (IC95%: 68,6%-83,2%) en el periodo posterior al inicio de la pandemia con respecto al periodo de referencia anterior que fue del 48,9% (IC95%: 43,4%-54,5%) (p<0,001). Los factores asociados de forma significativa e independiente al GC de la HM fueron el periodo de tiempo (antes o después de la pandemia), asistir a sesiones formativas, disponer de solución alcohólica en formato bolsillo y los momentos 3,4 y 5 de la OMS. Conclusiones: El GC de la HM en el SU ha alcanzado el máximo valor en 17 años de monitorización tras la pandemia de COVID-19. Este incremento refleja un cambio muy positivo en el comportamiento del personal sanitario con respecto a la HM., Background: Hand Hygiene (HH) is the most important measure to prevent healthcare-associated infections. Factors significantly and independently associated with DC of HH were time period (before or after pandemic), attending training sessions, having alcohol solution in pocket format and WHO moments 3,4 and 5. Conclusions: DC of HH in the ED has reached the highest value in 17 years of monitoring following the COVID-19 pandemic. This increase reflects a very positive change in the behavior of healthcare personnel with respect to HH.

Exercise counteracts aging and pathology symptoms, but there is still scarce research on exercise programs for multimorbid and/or palliative old patients (MPO-Ps). In order to analyze whether the multicomponent physical–cognitive training is beneficial for this population, 17 MPO-Ps (81.59 ± 5.63 years) completed a >26 weeks home-based intervention (20–50 min/session, three sessions/week). Twenty-eight supervised and thirty-two autonomous sessions were gradually distributed along three phases: supervised training (ST), reduced supervision training (RST), and autonomous training (AT). Physical function (gait speed, hand grip and lower-limb strength, balance, and agility), mental status (MMSE), and autonomy in daily living (the Barthel Index) were assessed. Categorical analyses regarding the changes in the walking aids used in the test were added to improve the assessment of strength and agility along the intervention. Despite important study limitations, such as the small sample size and lack of a control group, and despite the MPO-Ps’ very low baseline fitness and initial exercise intolerance, they benefited from the dual-tasking approach, especially in autonomy, lower-limb strength, and balance. Agility improvements were shown only by categorical analyses. As expected, most benefits increased the supervision (ST phase). Gait speed and cognitive status maintained despite the total autonomy in training in the last phase. Further research with larger samples should confirm if multicomponent physical–cognitive exercise, individualized and tailored on daily-basis, together with technical assistance and medical supervision, benefits this MPO-Ps population, and if it can be prescribed to them with security, in spite some of them already being palliative patients.

Introduction Social vulnerability is a known determinant of health in respiratory diseases. Our aim was to identify whether there are socio-demographic factors among COVID-19 patients hospitalized in Spain and their potential impact on health outcomes during the hospitalization. Methods A multicentric retrospective case series study based on administrative databases that included all COVID-19 cases admitted in 19 Spanish hospitals from 1 March to 15 April 2020. Socio-demographic data were collected. Outcomes were critical care admission and in-hospital mortality. Results We included 10,110 COVID-19 patients admitted to 18 Spanish hospitals (median age 68 (IQR 54–80) years old; 44.5% female; 14.8% were not born in Spain). Among these, 779 (7.7%) cases were admitted to critical care units and 1678 (16.6%) patients died during the hospitalization. Age, male gender, being immigrant, and low hospital saturation were independently associated with being admitted to an intensive care unit. Age, male gender, being immigrant, percentile of average per capita income, and hospital experience were independently associated with in-hospital mortality. Conclusions Social determinants such as residence in low-income areas and being born in Latin American countries were associated with increased odds of being admitted to an intensive care unit and of in-hospital mortality. There was considerable variation in outcomes between different Spanish centers.

Emplazamiento. Intervención realizada en la Unidad de Hospitalización a Domicilio (UHD) del Hospital General de Alicante, durante 2014-2016. Participantes. 17 pacientes ACP-P, edad media 81,6 años (DE 5,6). Intervención. 3 sesiones semanales durante 26 semanas del programa de entrenamiento funcional-cognitivo EFAM-UVH©; 3 fases, reduciendo progresivamente la presencia del entrenador para fomentar la autonomía (28 sesiones supervisadas y 20 autónomas en total). Mediciones principales. Evaluación de Sobrecarga del cuidador (Zarit) y Nivel de independencia (Índice de Barthel “IB”), apoyados por indicadores fisiológicos y funcionales. Resultados. IB mejoró con el entrenamiento supervisado (p<0,05), y se mantuvo o descendió leve, aunque no significativamente, al aumentar la autonomía. Estas mejoras se acompañaron de una reducción en la sobrecarga del cuidador, con una dinámica similar, fundamentalmente en cuidadores del grupo <79 años y de los que no andan. Location. Intervention carried out in the Home Hospitalization Unit (HHU) of the General Hospital of Alicante, during 2014-2016. Participants. 17 ACP-P patients [81.6 years (SD5.6)]. Intervention. 3 weekly sessions for 26 weeks of the EFAM-UVH© multicomponent training program (functional + cognitive); 3 phases, progressively reducing the presence of the coach to promote autonomy (28 supervised and 20 autonomous sessions in total). Main measurements. Evaluation of caregiver overload (Zarit) and level of independence (Barthel Index “BI”) of the ACP-P, supported by physiological and functional indicators. Results. BI improved with supervised training (p<0.05), and remained or fell slightly, although not significantly, when increasing the autonomy. These improvements were accompanied by a reduction in the overload of the caregiver, with a similar dynamic, mainly in the caregivers of the group <79 years and those who do not walk. Multiple studies have evaluated copeptin, a surrogate for arginine vasopressin, in the diagnosis of acute myocardial infarction (AMI) with mixed results. A systematic review and collaborative meta-analysis were performed for diagnosis of AMI and assessment of prognosis in patients presenting to the emergency department with chest pain. MEDLINE/PubMed, Cochrane CENTRAL, and EMBASE were searched for studies assessing copeptin in such patients. Study investigators were contacted, and many provided previously unpublished data. Random-effects methods were used to compare the data for copeptin, troponin, and their combination. There were a total of 9,244 patients from the 14 included studies. Mean age was 62 years; 64% were men; and 18.4% were ultimately diagnosed with AMI. Patients with AMI had a higher presentation copeptin level than those without AMI (22.8 vs 8.3 pmol/L, respectively, p <0.001). Although troponin had better diagnostic accuracy than copeptin for AMI, the combination of copeptin and troponin significantly improved the sensitivity (0.905 [0.888 to 0.921] vs 0.686 [0.661 to 0.710], respectively, p <0.001) and negative predictive value (0.97 [0.964 to 0.975] vs 0.93 [0.924 to 0.936], respectively, p <0.001) compared with troponin alone. Elevation in copeptin carried a similar risk of all-cause mortality to an elevation in troponin (odds ratio 5.84 vs 6.74, respectively, p = 0.67). In conclusion, copeptin not only identifies patients at risk of all-cause mortality, but its addition to troponin improved the sensitivity and negative likelihood ratio for diagnosis of AMI compared with troponin alone. Thus, copeptin may help identify patients who may be safely discharged early from the emergency department.
