Previous studies have indicated that age is a risk factor for severe falciparum malaria in nonimmune patients. The objectives of this study were to reevaluate previous findings with a larger sample and to find out how strongly clinical outcomes for elderly patients differ from those for younger patients. Results of adjusted analyses indicated that the risks of death due to falciparum malaria, of experiencing cerebral or severe disease in general, and of hospitalization increased significantly with each decade of life. The case-fatality rate was almost 6 times greater among elderly patients than among younger patients, and cerebral complications occurred 3 times more often among elderly patients. Antimalarial chemoprophylaxis was significantly associated with a lower case-fatality rate and a lower frequency of cerebral complications. Women were more susceptible to cerebral complications than were men. Our study provides evidence that falciparum malaria is more serious in older patients and demonstrates that clinical surveillance networks are capable of providing quality data for investigation of rare events or diseases

Malaria continues to have a high morbidity rate associated among European travelers. Thorough recording of epidemiological and clinical aspects of imported malaria has been helpful in the detection of new outbreaks and areas of developing drug resistance. Sentinel surveillance of data collected prospectively since 1999 has begun within TropNetEurop, a European network focusing on imported infectious diseases. TropNetEurop appears to cover ∼10% of all patients with malaria seen in Europe. Reports of 1659 immigrants and European patients with Plasmodium falciparum malaria were analyzed for epidemiological information and data on clinical features. Regional data were quite diverse, reflecting local patterns of immigration and international travel. By far, the most infections were imported from West Africa. Europeans had more clinical complications; consequently, all deaths occurred in this group. Compared with European standards, the mortality rate was low (0.6% in Europeans). Data from TropNetEurop member sites can contribute to our understanding of the epidemiological and clinical findings regarding imported falciparum malaria

Within the last years, however, the number of pertussis cases has increased steadily, not only in children but also in adolescents and adults, indicating both insufficient herd immunity and vaccine coverage. Waning immunity in the host and/or adaptation of the bacterium to the immunised hosts could contribute to the observed re-emergence of pertussis. In this study we therefore addressed the genetic variability in B. pertussis strains from several Austrian cities. Between the years 2002 and 2008, 110 samples were collected from Vienna (n = 32), Linz (n = 63) and Graz (n = 15) by nasopharyngeal swabs. DNA was extracted from the swabs, and bacterial sequence polymorphisms were examined by MLVA (multiple-locus variable number of tandem repeat analysis) (n = 77), by PCR amplification and conventional Sanger sequencing of the polymorphic regions of the prn (pertactin) gene (n = 110), and by amplification refractory mutation system quantitative PCR (ARMS-qPCR) (n = 110) to directly address polymorphisms in the genes encoding two pertussis toxin subunits (ptxA and ptxB), a fimbrial adhesin (fimD), tracheal colonisation factor (tcfA), and the virulence sensor protein (bvgS). Finally, the ptxP promoter region was screened by ARMS-qPCR for the presence of the ptxP3 allele, which has been associated with elevated production of pertussis toxin. The MLVA analysis revealed the highest level of polymorphisms with an absence of MLVA Type 29, which is found outside Austria. Only Prn subtypes Prn1/7, Prn2 and Prn3 were found with a predominance of the non-vaccine type Prn2. The analysis of the ptxA, ptxB, fimD, tcfA and bvgS polymorphisms showed a genotype mixed between the vaccine strain Tohama I and a clinical isolate from 2006 (L517). The major part of the samples (93%) displayed the ptxP3 allele. The consequences for the vaccination strategy are discussed.

Die japanische Enzephalitis (JE) ist eine in Teilen Asiens vorkommende virale Infektionskrankheit, die ursprünglich in Japan als japanische B-Enzephalitis beschrieben wurde. Sie ist jedoch wesentlich weiter verbreitet, vor allem in ländlichen Gebieten Asiens. Die Infektion wird durch Stechmücken der Gattung Culex übertragen und kann neben den Einheimischen auch Reisende betreffen. Überall dort, wo Hausschweine gehalten werden, die neben Reihern und anderen Wasservögeln das Reservoir für das Virus bilden, besteht eine Gefahr der Virusübertragung durch diese Mücken. Jährlich werden in Asien zwischen 30 000 und 50 000 klinisch manifeste Fälle beobachtet. Die Dunkelziffer könnte aber aufgrund der fehlenden Melderegister höher sein. Darüber hinaus reduzieren landesweite Impfkampagnen in vielen Ländern Asiens (z. B. in Japan, Indien, Thailand, China) diese Fallzahlen. Auch in Japan kommt das Virus noch vor, obwohl die Infektionen dort bei der einheimischen Bevölkerung durch Impfkampagnen sehr stark zurückgegangen sind. Die Mehrzahl der Infektionen verläuft asymptomatisch. Von 250 Fällen erscheint nur einer als klinisch manifeste JE-Infektion – typischerweise als Enzephalitis. Aufgrund fehlender Immunität sind Kinder häufiger betroffen als Erwachsene, aber die Krankheit scheint mit zunehmendem Alter schwerer zu verlaufen. Etwa 30% der Erkrankten überleben die Infektion nicht. Über ein Drittel leidet an den neurologischen Folgesymptomen der Erkrankung. Auch Reisende können betroffen sein. Zwischen 1978 und 2006 wurden mindestens 34 Fälle bei Reisenden und Auswanderern gemeldet. Einige Touristen waren sogar an beliebten Zielen wie Bali betroffen. Auch außerhalb der klassischen Regenzeiten wurden Fälle beschrieben.
