Introduction: Healthcare-associated infections (HAIs) remain a common challenge in healthcare delivery, with a significant burden in low- and middle-income countries. Preventing HAIs has gained enormous attention from policy makers and healthcare managers and providers, especially in resource-limited settings. Despite policies to enforce infection prevention and control (IPC) measures to prevent HAIs, IPC compliance remains a challenge in hospital settings. In this study, we explore the experiences of healthcare providers and women in the post-natal phase and investigate factors influencing IPC practices in two hospitals in Ghana. Methods: The study used a qualitative approach involving semi-structured interviews, focus group discussions, and observations among healthcare providers and women in the postnatal phase in two maternity units from January 2019 to June 2019. Interviews were recorded and transcribed verbatim for thematic analysis. The data sets were uploaded into the qualitative software NVivo 12 to facilitate coding and analysis. Findings: Healthcare providers were driven by the responsibility to provide medical care for their patients and at the same time, protect themselves from infections. IPC facilitators include leadership commitment and support, IPC training and education. Women were informed about IPC in educational talks during antenatal care visits, and their practices were also shaped by their background and their communities. IPC barriers include the poor documentation or ‘invisibility’ of HAIs, low prioritization of IPC tasks, lack of clear IPC goals and resources, discretionary use of guidelines, and communication-related challenges. The findings demonstrate the need for relevant power holders to position themselves as key drivers of IPC and develop clear goals for IPC. BackgroundThere is growing concern about the sustainability and long-term impact of short-term medical missions (STMMs)-an increasingly common form of foreign medical aid-given that brief engagements do little to address the underlying poverty and fragmented healthcare system that plagues many low- and middle-income countries (LMICs). In the absence of formal evaluations, unintended but serious consequences for patients and local communities may arise, including a lack of continuity of patient care, poor alignment with community needs, and cultural and language barriers.ObjectiveWe conducted semi-structured interviews with Honduran healthcare providers (n = 88) in 2015 to explore local providers' perceptions of the impact and sustainability of foreign medical aid on patient needs, community health, and the country's healthcare system.MethodsRespondents represented a random sample of Honduran healthcare providers (physicians, dentists, nurses) who worked for either a government-run rural clinic or non-governmental organization (NGO) in Honduras.FindingsHonduran healthcare providers largely framed foreign medical teams as being assets that help to advance community health through the provision of medical personnel and supplies. Nonetheless, most respondents identified strategies to improve implementation of STMMs and reduce negative impacts. Many respondents emphasized a need for culturally- and linguistically-tailored medical care and health education interventions. Participants also recommended strengthening local partnerships to mitigate the risk of dependence, including on-going training and support of community health workers to promote sustainable change.ConclusionsGuidelines informed by local Honduran expertise are needed to increase accountability for more robust training of foreign physicians in the provision of context-appropriate care. Home blood pressure management, including self-monitoring and medication self-titration, is an efficient and cost-effective tool. Although its use is increasing globally, little is known about the feasibility of such interventions in low and middle-income countries. Further, the perspectives and experiences of healthcare providers who play a big role in ensuring the success of home blood pressure management interventions have not been documented. This qualitative study was conducted with a total of 35 healthcare providers (60% female, mean [SD] age = 37.3 [6.9 years] years), through 4 in-depth interviews from Peru, and 8 in-depth interviews and 4 focus groups from Cameroon. Study participants (healthcare providers) include physicians (primary care physicians), specialists (cardiologists and geriatricians), and nurses that were purposively recruited from two hospitals in two of the largest cities in both countries. Results were thematically analyzed by two researchers. Themes derived were related to feasibility and acceptability, and largely reflected providers in both countries endorsing home blood pressure management. Providers' concerns were in three main areas; 1) safety of patients when they self-titrate medications, 2) resources such as healthcare financing, local hospital policies that support communications with patients for home blood pressure management, and 3) sustainability through patient adherence, incorporating home blood pressure management within clinical guidelines and hospital policies, and complementing with continued health education and lifestyle modifications. According to providers, home blood pressure management may be feasible and acceptable if tailored multi-faceted protocols were introduced bearing in mind local contexts.

Currently, vaccination rates are suboptimal in the national Swedish school-based vaccination program. School nurses play a key role in all aspects of the vaccination process. Therefore, this study aims to explore school nurses' perceived HPV vaccination challenges. Methods: Seven focus group interviews were conducted with school nurses (n = 35) working in nine socio-demographically diverse municipalities in mid-Sweden. Data were analyzed using qualitative content analysis. Results: Participants described difficulties in encountering and handling the diversity of reasons for vaccine hesitancy. Parents known to be skeptical of vaccines in general were seen as most difficult to reach. Uncertainty was expressed concerning the extent of professional responsibility for vaccine promotion. The informants expressed a lack of guidelines for vaccine promotion and described challenges in supporting the child's own wishes. Creating a safe space for the individual child was seen as crucial. Other problems described were the challenges of overcoming children's fear of needles, supporting unvaccinated children, and being confronted with the remaining gender inequities of the pan-gender vaccination program. Conclusions: Our results suggest that school nurses, especially those new to their profession, may benefit from training and guidance22 material on how to address vaccine hesitancy.

Uncertainty exists ubiquitously within provider-patient interactions. Healthcare providers (HCPs) often face uncertainty during patient-provider interactions, for reasons including inconclusive test results, ambiguous communication, and lacking the resources to make diagnoses. When healthcare providers experience uncertainty, their behavior and communication can be negatively impacted. For example, prior research suggests when HCPs experience uncertainty, they may engage in authoritative, prejudiced and assumption-ridden behavior towards patients (Dietta & Rand, 2007; Drewniak et al., 2017: Portnoy et al., 2013; Poteat et al., 2013). To date, research on HCP uncertainty has been limited to specific health conditions and contexts such as cancer, vascular anomalies, and asthma patients (Dietta & Rand, 2007; Kerr & Sisk, 2021). Additionally, studies that have explored uncertainty mainly focused on medical uncertainty, leaving relational uncertainty largely undefined and vaguely understood. This paper defines both medical and relational uncertainty and answers a call for the exploration of how healthcare providers manage uncertainty in broad contexts (Kerr et al., 2013). Specifically, this study explored the types of patient-provider interactions that cause uncertainty and how providers communicatively and behaviorally manage these types of uncertainty. Results from semi-structure interviews (N=16) with Montana-based healthcare providers suggest that providers can experience medical uncertainty due to ambiguous medical test results and medical examinations, a lack of patient health information, whereas relational uncertainty often arises due to assumptions providers make based on patient demographics. Relational and medical uncertainty are managed differently. Heart failure contributes to the high morbidity and mortality rates due to chronic diseases in the world. This condition causes one of which is due to low self-care management. A decrease in the functioning of the patient's body is the impact of the condition. Health workers are essential in assisting patients' self-care while in the hospital and preparing for home care. This study aims to find out the description of the implementation of treatments focused on heart failure patients from the patient's perspective descriptive quantitative research method of the survey selected in this study. A total of 294 heart failure patients who met the established criteria were involved in the study. The results showed that the treatment focused on heart failure patients was assessed from the patient's point of view to have been well implemented by health workers. The analysis obtained an average value of 78.24., Gagal jantung menjadi penyumbang tingginya angka morbiditas dan mortalitas akibat penyakit kronis di dunia. Kondisi ini diakibatkan salah satunya karena manajemen perawatan diri yang rendah sebab penurunan fungsional tubuh pasien. Tenaga kesehatan memiliki peran penting dalam membantu perawatan diri pasien selama di rumah sakit dan mempersiapkan perawatan di rumah. Tujuan dari penelitian ini untuk mengetahui gambaran implementasi perawatan berfokus pada pasien gagal jantung dari perspektif pasien. Metode penelitian kuantitatif deskriptif survei dipilih dalam penelitian ini. Sejumlah 294 pasien gagal jantung yang memenuhi kriteria yang telah ditetapkan dilibatkan dalam penelitian ini. Hasil didapatkan bahwa perawatan berfokus pada pasien gagal jantung dinilai dari sudut pandang pasien telah diterapkan dengan baik oleh tenaga kesehatan, dengan hasil analisis diperoleh nilai rata-rata sebesar 78.24.

Contrasting COVID-19 is widely debated but often treated from a clinical or, at most, managerial perspective. Less attention is instead assigned to a narrative view of the problem. However, the stories of those who combat this pathogen teach us the need to use storytelling strategies to counteract the effects of the disease on patients and its consequences on healthcare providers. This contribution aims to address the COVID-19 issue from the perspective of narrative medicine, first by defining it and then by reporting some healthcare professionals' experiences. It intends to highlight the importance that medicine has not only on the patient but also on healthcare professionals. It offers a brief review of some of their narratives, focusing on experiences with different tones and struggles and emphasizing the importance of narrative in one's work in supporting patients. In addition, this contribution also aims to reason that not everything can be solved with words, as they help but do not solve everyday problems., La lucha contra el COVID-19 es ampliamente debatida, pero a menudo se trata desde una perspectiva clínica o, como mucho, gerencial. En cambio, se asigna menos atención a una visión narrativa del problema. Sin embargo, las historias de quienes combaten este patógeno nos enseñan la necesidad de utilizar estrategias de narración para contrarrestar los efectos de la enfermedad en los pacientes y sus consecuencias en los proveedores de atención médica. Background: Childhood obesity is a complex disease resulting from the interaction of multiple factors. The effective management of childhood obesity requires assessing the psychosocial and lifestyle factors that may play a role in the development and maintenance of obesity. This study centers on available scientific literature on psychosocial and lifestyle assessments for childhood obesity, and experiences and views of healthcare professionals with regard to assessing psychosocial and lifestyle factors within Dutch integrated care. Methods: Two methods were used. First, a scoping review (in PubMed, Embase, PsycInfo, IBSS, Scopus and Web of Science) was performed by systematically searching for scientific literature on psychosocial and lifestyle assessments for childhood obesity. Data were analysed by extracting data in Microsoft Excel. Second, focus group discussions were held with healthcare professionals from a variety of disciplines and domains to explore their experiences and views about assessing psychosocial and lifestyle factors within Dutch integrated care. Data were analysed using template analysis, complemented with open coding in MAXQDA. Results: The results provide an overview of relevant psychosocial and lifestyle factors that should be assessed and were classified as child, family, parental and lifestyle (e.g. nutrition, physical activity and sleep factors) and structured into psychological and social aspects. Insights into how to assess psychosocial and lifestyle factors were identified as well, including talking about psychosocial factors, lifestyle and weight; the professional-patient relationship; and attitudes of healthcare professionals. Conclusions: This study provides an overview of psychosocial and lifestyle factors that should be identified within the context of childhood obesity care, as they may contribute to the development and maintenance of obesity. PurposeThe purpose of this quality improvement initiative was to provide hospice and palliative care education to healthcare providers to increase their willingness and intent to refer patients to them. Background Hospice and palliative care have been used for many years to improve patient quality of life by decreasing pain, anxiety, and overall symptom burden for patients suffering from terminal illnesses (American Cancer Society [ACS], 2019; Center to Advance Palliative Care, 2022). While these services have well established benefits for patients and their families, they are often not utilized until very late into a patient’s terminal diagnosis (Devi, 2011). There are many potential reasons for a lack of or late referrals to hospice and palliative care, but a healthcare provider knowledge deficit may play a major role. Methods An online educational PowerPoint session (OEPS) on hospice and palliative care was administered to the healthcare providers at the Bear River Valley Hospital, along with a follow up post-then-pretest survey. The healthcare providers were asked to answer questions about their knowledge of, attitude towards and perception of intent to refer to, and experiences with hospice and palliative care both before and after viewing the OEPS. The healthcare providers were asked to provide two Likert scale scores of 1 to 5 for each question, one being associated with before the education and one being associated with after. Results Eleven healthcare providers participated in the OEPS and follow up survey. There was an overall reported increase in Likert scale score for each question following administration of the OEPS. The greatest increase in score was noted for the questions regarding knowledge of and intent to refer patients to hospice and palliative care. Conclusions Hospice and palliative care education for healthcare providers has the potential to increase their intent to refer patients to these important services. This may translate to an increase

This tradition of decision making at a group level was perceived as time-consuming and delayed HCPs from obtaining informed consent for EmOC. Low socioeconomic status and poor knowledge about maternal healthcare among users affected care seeking among women. Suboptimal EmOC at the hospital was reported to be due to miscommunication, inadequate interprofessional collaboration and lack of infrastructure. Conclusions: HCPs encountered difficulties with the provision of EmOC. A broad array of strategies targeting the community and healthcare system is needed, including training of HCPs on intracultural communication competence, interprofessional collaboration and use of alternative birth methods other than CS. Antenatal care can be used to prepare families for potential obstetric emergencies and as an opportunity to obtain written informed consent.

Objectives To explore the tactics adopted by healthcare providers in the humanitarian aid sector to meet the sexual and reproductive healthcare needs of undocumented immigrant women whose entitlements in the Danish national health services are restricted to emergency care only. Methods We conducted six semi-structured interviews between January 2020 and April 2020 with healthcare providers working across four health centers providing sexual and reproductive healthcare to undocumented migrants in Copenhagen. The interviews were digitally recorded, transcribed and subjected to a thematic network analysis. Results Healthcare providers adopted a number of tactics tomeet the SRH needs of undocumented women. They used their position as volunteers to gain women’s trust, drew on personal networks in the public healthcare sector, accompanied women to public hospitals, worked with professionals from the migrant communities, and promoted secondary prevention using their cultural knowledge to respond to the SRH needs of undocumented women. Results highlight that while “informal” settings enable these flexible tactics, fully meeting the SRH needs of undocumented migrants is negatively affected by their limited entitlements to public healthcare. Conclusion These tactics demonstrated the efforts of healthcare providers to use the NGO space to try to achieve SRH of undocumented women in a restrictive public healthcare sector. Nonetheless, these tactics, particularly the use of personal networks, did not always result in undocumented migrants having their SRH needs met, particularly in cases of serious illness, demonstrating a concerning gap in care.

A broad array of strategies targeting the community and healthcare system is needed, including training of HCPs on intracultural communication competence, interprofessional collaboration and use of alternative birth methods other than CS. Background The non-curative setting makes communication and shared decision-making in palliative care extremely demanding. This is even more so for patients with limited health literacy. So far, research in palliative care focusing on shared decision-making with patients with limited health literacy is lacking. Recent research from our team indicates that the assessment of these patients’ understanding of their situation and the implementation of shared decision-making in palliative care, needs improvement. Methods To improve communication and decision-making, especially with patients with limited health literacy, we developed and evaluated a blended training programme for healthcare providers. The training programme comprised of an e-learning and a team training. The evaluation was performed by 1. conducting interviews (n = 15) focused on evaluating the whole programme and, 2. coding video-recorded outpatient consultations on the extent to which providers involved patients in decision-making before (n = 19) and after (n = 20) the intervention, using the 5-item OPTION coding instrument. Results The interviews showed that healthcare providers valued the skills they had learned during the e-learning and team training. Providers specifically valued the teach-back technique, learned to use simpler wording and felt better able to recognize patients with limited health literacy. Many providers reported a change in communication behaviour as a consequence of the training programme. Suggestions for improvement for both e-learning and training were, amongst others, a follow-up team training course and a new scenarios for the e-learning about discussing palliative care. For both the pre- and the post-measurement, involving patients in decision-making lies between a minimal and a moderate effort; differences were not significant. Young adult healthcare consumers were considered to be highly influenced by external resources, often greatly concerned with small/vague symptoms they had difficulty explaining and unable to wait with. The healthcare provider's role was much perceived as being part of a healthcare structure-a large organisation with multiple units-and having to meet different priorities while also considering ethical dilemmas, though feeling supported by experience. CONCLUSION: Healthcare personnel view young adults as transferring too much of the responsibility of staying healthy to the healthcare system. The results of this study show that the discussion of young adults unnecessarily seeking health care includes an underlying discussion of scarcity of resources. Targeting informal healthcare providers (IHCPs) is important to achieve universal health coverage and effective antibiotic stewardship in resource-constrained settings. We, therefore, aimed to analyse the internal and external drivers that influence IHCPs' prescribing behaviour for common illnesses in children under five, with an emphasis on antibiotic use in rural areas of India. A total of 48 IHCPs participated in focus group discussions. Thematic framework analysis with an inductive approach was used, and findings were collated in the theoretical framework based on knowledge, attitude, and practice model which depicted that the decisions made by IHCPs while prescribing antibiotics are complex and influenced by a variety of external and internal drivers. IHCPs' internal drivers included the misconception that it is impossible to treat a patient without antibiotics and that antibiotics increase the effectiveness of other drugs and cure patients faster in order to retain them. Formal healthcare providers were the IHCPs' sources of information, which influences their antibiotic prescribing. We found when it comes to seeking healthcare in rural areas, the factors that influence their choice include 'rapid cure', 'cost of treatment', 'distance' and '24 h availability', instead of qualification, which may create pressure for IHCPs to provide a quick fix. Targeted and coordinated efforts at all levels will be needed to change the antibiotic prescribing practices of IHCPs with a focus on behaviour change and to help resolve misconceptions about antibiotics.

In order to provide quality care for patients with a migration background, it is important to understand which barriers healthcare professionals experience that prevent them from providing care and which factors can facilitate this. Design and methods: Qualitative research following the COREQ criteria. A total of six focus groups (n = 37) and 12 individual interviews were conducted with a multidisciplinary sample: doctors, nurses, social workers, and occupational therapists. Nursing and medical students were also included. Thematic content analysis was used. Results: Key findings suggest that the main barrier is that healthcare professionals regard people with a migration background as “the other”. Healthcare professionals do not feel secure or competent to provide care for these “others.” According to the healthcare professionals, the hospital structures—and, particularly, the managerial instances—appear to be only slightly supportive. Structural barriers at the level of the healthcare system, such as limited implementation of care coordination and austerity measures (time pressure or economic restrictions), were also perceived as barriers. Facilitators can be the healthcare professionals' attitude or the flexibility of the management. Conclusions: Healthcare professionals experience barriers in caring for people with a migration background. Othering plays a key role in building or maintaining several barriers. A multilevel approach is necessary to tackle these barriers and enable facilitators. Objectives To explore the tactics adopted by healthcare providers in the humanitarian aid sector to meet the sexual and reproductive healthcare needs of undocumented immigrant women whose entitlements in the Danish national health services are restricted to emergency care only. Methods We conducted six semi-structured interviews between January 2020 and April 2020 with healthcare providers working across four health centers providing sexual and reproductive healthcare to undocumented migrants in Copenhagen. The interviews were digitally recorded, transcribed and subjected to a thematic network analysis. Results Healthcare providers adopted a number of tactics tomeet the SRH needs of undocumented women. They used their position as volunteers to gain women’s trust, drew on personal networks in the public healthcare sector, accompanied women to public hospitals, worked with professionals from the migrant communities, and promoted secondary prevention using their cultural knowledge to respond to the SRH needs of undocumented women. Results highlight that while “informal” settings enable these flexible tactics, fully meeting the SRH needs of undocumented migrants is negatively affected by their limited entitlements to public healthcare. Conclusion These tactics demonstrated the efforts of healthcare providers to use the NGO space to try to achieve SRH of undocumented women in a restrictive public healthcare sector. Nonetheless, these tactics, particularly the use of personal networks, did not always result in undocumented migrants having their SRH needs met, particularly in cases of serious illness, demonstrating a concerning gap in care.

Objective: Both patients in the palliative phase of their disease and patients with limited health literacy (LHL) have an increased risk of being influenced by healthcare providers (HCPs) when making decisions. This study aims to explore to what extent persuasive communication occurs during shared decision-making (SDM) by (1) providing an overview of persuasive communication behaviours relevant for medical decision-making and (2) exemplifying these using real-life outpatient consultations. Methods: An exploratory qualitative design was applied: (1) brief literature review; (2) analysis of verbatim extracts from outpatient consultations and stimulated recall sessions with HCPs; and (3) stakeholder meetings. Results: 24 different persuasive communication behaviours were identified, which can be divided in seven categories: biased presentation of information, authoritative framing, probability framing, illusion of decisional control, normative framing, making assumptions and using emotions or feelings. Conclusions: Persuasive communication is multi-faceted in outpatient consultations. Although undesirable, it may prove useful in specific situations making it necessary to study the phenomenon more in depth and deepen our understanding of its mechanisms and impact. Practice implications: Awareness among HCPs about the use of persuasive communication needs to be created through training and education. Also, HCPs need help in providing balanced information.

BackgroundCOVID-19 rapidly accelerated the implementation of telemedicine in U.S. Department of Veterans Affairs (VA) specialty care clinics. This mixed-methods study was conducted at a VA medical center to understand the use of telemedicine, and the barriers and facilitators to its implementation, in cardiology outpatient clinics.MethodsQuantitative analyses modeled monthly trends of telemedicine use over 24-months (March 2019-March 2021) with segmented logistic regression and adjusted for socio-demographic predictors of patient-level telemedicine use. Qualitative interviews were conducted (July-October 2020) with eight cardiology clinicians.ResultsAt the onset of COVID-19, likelihood of telemedicine use was ∼12 times higher than it was pre-COVID-19 (p < 0.001). White (OR = 1.38, 95% CI:1.23-1.54), married (OR = 1.25, 95% CI:1.11-1.40), Veterans with other health insurance (OR = 1.19, 95% CI:1.06-1.35), were more likely to use telemedicine. Veterans with higher health risk factors were less likely (OR = 0.95, 95% CI:0.93-0.97). Facilitators to rapid expansion of telemedicine included prior telemedicine experience; provider trainings; and staff champions. In contrast, lack of technical support and scheduling grids for virtual visits and patient ability/preference served as barriers.ConclusionsFindings suggest that once mutable barriers were addressed, the medical center was able to expand its telemedicine efforts during COVID-19. Beyond the pandemic, a hybrid of virtual and face-to-face care might be feasible and likely beneficial for healthcare providers and patients in specialty care.ImplicationsThe ability to rapidly transition from in-person to virtual visits can potentially assist with the continuity of care and management of chronic disease during infectious outbreaks and other major disasters that obstruct traditional care models.

BackgroundSymptom management self-efficacy is a prerequisite for individuals to fully manage their symptoms. The literature reports associations between engagement with healthcare providers (HCPs), internalized stigma, and types of self-efficacy other than symptom management. However, the factors of symptom management self-efficacy are not well understood. This study aimed to investigate the relationship among engagement with HCPs, internalized stigma, and HIV symptom management self-efficacy in Chinese women living with HIV (WLWH).MethodsThis current analysis was part of the original randomized control trial, we used data collected from 41 women living with HIV (WLWH) assigned to an intervention arm or a control arm from Shanghai and Beijing, China, at baseline, Week 4 and Week 12. The CONSORT checklist was used. The study was registered in the Clinical Trial Registry (#NCT03049332) on 10/02/2017.ResultsThe results demonstrate that HCPs should increase engagement with WLWH when providing care, thereby improving their symptom management self-efficacy. The results suggested that participants' engagement with HCPs was significantly positively correlated with their HIV symptom management self-efficacy in the latter two time points. Internalized stigma was significantly negatively correlated with HIV symptom management self-efficacy only at the 4-week follow-up.ConclusionsThis study demonstrated the positive effect of engagement with HCPs on WLWHs' symptom management self-efficacy as well as the negative effect of internalized stigma on symptom management self-efficacy. Future research can further test the relationship between the three key concepts, as well as explore interventions to decrease internalized stigma.

Evangelical Christians are among the most hesitant to get the COVID-19 vaccine. This study examined the extent to which COVID-19 vaccination uptake among Evangelicals is explained by demographic characteristics, Health Belief Model constructs, and faith-based support factors. Survey research firm Qualtrics recruited 531 U.S. adults and conducted a survey to explore predictors of COVID-19 vaccine uptake among people who self-identified as Evangelicals in September 2021. A logistic regression showed that those reporting high perceived benefits of the COVID-19 vaccine were more likely to be vaccinated, while those reporting high perceived barriers were less likely to be vaccinated. Those whose healthcare provider asked them about the vaccine were more likely to be vaccinated than those whose healthcare provider did not ask. Finally, while those who reported information seeking from religious leaders were less likely to be vaccinated, those who reported more faith-based support for vaccination were more likely to be vaccinated. In addition to beliefs about benefits and barriers to vaccination, the role of healthcare providers and clergy were important factors influencing vaccination status. Intervention efforts that capitalize on partnerships between health providers and clergy in supportive congregations may be able to reach undecided Evangelicals.

Background/purposeDuring pandemics like SARS-CoV-2, healthcare providers' well-being and morale are in particular at stake. Burnout may substantially hinder the well-being and morale of healthcare providers, challenging our efforts at disease containment. This study investigated the relationship between perceived COVID-19 stigma and burnout symptoms among physicians and nurses. We further aimed to identify potential factors that may moderate this relationship, including profession, clinical contact with COVID-19 patients, and prior experience with 2003 SARS-CoV-1.MethodsWe used a web-based, structured survey from March 12th to 29th, 2020 to collect cross-sectional, self-reported data. Participants were provided with a link to the survey which took them on average 5-8 minutes. Survey consisted of demographic characteristics, clinical experiences, perceived COVID-19 related stigma, and burnout symptoms. Linear regression with bootstrapping techniques was adopted to test the relations between stigma and burnout, as well as other potential moderators, while adjusting for demographic and clinical factors.ResultsOf the 1421 consented respondents, 357 identified as physicians while 1064 identified as nurses. Participants reported some levels of stigma, and noticeable burnout symptoms. Burnout symptoms were positively correlated with COVID-19 stigma, profession, and currently care for confirmed/suspected COVID-19 patients. The interaction between stigma and profession (Stigma × Nurses) but no other interaction terms reached the significance level, suggesting that the slope for nurses was flatter than the slope for physicians.ConclusionThe study results suggest that COVID-19 stigma may contribute to burnout among physicians and nurses, and this relation may not vary across clinical roles and experiences but profession.

OBJECTIVE: This study aimed to explore the experiences of healthcare providers (HCPs) regarding the provision of emergency obstetric care (EmOC) with a focus on cesarean deliveries in a referral hospital and maternal and child health centers in Somaliland. METHODS: An exploratory qualitative approach using focus group discussions was employed at the main referral and teaching hospital and four maternal and child health centers in Hargeisa, Somaliland. Twenty-eight HCPs were divided into groups of 6-8 for discussions lasting 1 to 2 h. All HCPs included in the study had experiences with the provision of EmOC. Data were analyzed using thematic analysis. RESULTS: Collective family decision making was identified by HCPs as a barrier to the provision of EmOC. This tradition of decision making at a group level was perceived as time-consuming and delayed HCPs from obtaining informed consent for EmOC. Low socioeconomic status and poor knowledge about maternal healthcare among users affected care seeking among women. Suboptimal EmOC at the hospital was reported to be due to miscommunication, inadequate interprofessional collaboration and lack of infrastructure. CONCLUSIONS: HCPs encountered difficulties with the provision of EmOC. A broad array of strategies targeting the community and healthcare system is needed, including training of HCPs on intracultural communication competence, interprofessional collaboration and use of alternative birth methods other than CS. Antenatal care can be used to prepare families for potential obstetric emergencies and as an opportunity to obtain written informed consent.

Introduktion: Kvinnor utsatta för våld i nära relationer [VNR]söker ofta sjukvård, detta ger vårdpersonal en unik möjlighet och ett stort ansvar att fånga upp och hjälpa dem. Därför är det väsentligt att beskriva kvinnors upplevelser av vården och identifiera önskat bemötande. Syftet med studien var att beskriva hur kvinnor utsatta för våld i nära relationer upplever bemötandet av vårdpersonal, samt hur de önskar att bli bemötta angående sin situation. Metod: Detta är en deskriptiv litteraturstudie som granskat kvalitativa originalartiklar. Efter kvalitetsgranskning valdes tio studier som svarade mot studiens syfte. Efter noggrann genomläsning och diskussion utformades ett resultat med två teman och flera kategorier. Resultat: Kvinnorna beskrev främst barriärer de upplevt i mötet med vårdpersonal. Dessa barriärer började redan innan mötet med rädsla för vad som kunde hända om de sökte hjälp. Stress, tidsbrist och bristande kontinuitet i mötet med vården förhindrade uppbyggnad av den förtroendeingivande relation som beskrevs som essentiell för att kvinnan skulle känna trygghet. Kvinnorna upplevde även olika faser i hur redo de var att berätta och hantera våldsutsattheten. Skam, skuld och informationsbrist var andra barriärer. Rätt information vid rätt tidpunkt begränsade informationsbristen. Planscher och broschyrer på toaletter, i väntrum och i mottagningsrum upplevdes väldigt uppskattat. Direkta frågor och en motivering till varför frågan ställdes uppskattades av majoriteten av de studerade kvinnorna. Slutsats: I bemötandet av vården upplevde kvinnorna en mängd barriärer. Dessa kan överkommas med rätt information, en trygg miljö och en icke dömande och kontinuerlig personal-patientrelation, där kvinnans autonomi respekteras., Introduction: Women exposed to Intimate Partner Violence [IPV] often seek medical care, which gives healthcare providers [HCP] a unique opportunity and a great responsibility to help. The aim of this study was to describe how women exposed to IPV experience encounters with HCP and how they want these encounters to be. Method: This is a descriptive study that examines qualitative original articles. After quality reviewing, ten articles that met the study criteria were chosen. The articles were independently read and then discussed to form a result, consisting of two themes and several categories. Result: The women primarily described experiences of different obstacles they faced in the encounter with HCP. These obstacles started with fear for what could happen if they disclosed the violence. Shame, guilt and lack of information were other barriers. Stress, lack of continuity and of time hindered the essential patient-provider relationship from growing. The women also experienced different stages of readiness in dealing with the IPV. Adequate information at the right time and in the right way could reduce the lack of information. Posters and brochures in waitingrooms, bathrooms and meeting rooms were appreciated. A majority of the studied women appreciated direct questions as well as a motivation why they were asked. Conclusion: The women experienced obstacles when meeting HCP. These obstacles could be overcome through correct information, a safe environment, and a safe, non-judgemental patient-provider relationship, where the women’s autonomy is respected.
