Abstract: Background: Impairment of ventilation and perfusion (V/Q) matching is a common mechanism leading to hypoxemia in patients with acute respiratory failure requiring intensive care unit (ICU) admission. While ventilation has been thoroughly investigated, little progress has been made to monitor pulmonary perfusion at the bedside and treat impaired blood distribution. The study aimed to assess real-time changes in regional pulmonary perfusion in response to a therapeutic intervention. Methods: Single-center prospective study that enrolled adult patients with ARDS caused by SARS-Cov-2 who were sedated, paralyzed, and mechanically ventilated. The distribution of pulmonary perfusion was assessed through electrical impedance tomography (EIT) after the injection of a 10-ml bolus of hypertonic saline. The therapeutic intervention consisted in the administration of inhaled nitric oxide (iNO), as rescue therapy for refractory hypoxemia. Each patient underwent two 15-min steps at 0 and 20 ppm iNO, respectively. At each step, respiratory, gas exchange, and hemodynamic parameters were recorded, and V/Q distribution was measured, with unchanged ventilatory settings. Results: Ten 65 [56–75] years old patients with moderate (40%) and severe (60%) ARDS were studied 10 [4-20] days after intubation. Gas exchange improved at 20 ppm iNO (PaO2/FiO2 from 86 ± 16 to 110 ± 30 mmHg, p = 0.001; venous admixture from 51 ± 8 to 45 ± 7%, p = 0.0045; dead space from 29 ± 8 to 25 ± 6%, p = 0.008). The respiratory system's elastic properties and ventilation distribution were unaltered by iNO. Hemodynamics did not change after gas initiation (cardiac output 7.6 ± 1.9 vs. 7.7 ± 1.9 L/min, p = 0.66). The EIT pixel perfusion maps showed a variety of patterns of changes in pulmonary blood flow, whose increase positively correlated with PaO2/FiO2 increase (R2 = 0.50, p = 0.049). Abstract: Background: External chest-wall compression (ECC) is sometimes used in ARDS patients despite lack of evidence. It is currently unknown whether this practice has any clinical benefit in patients with COVID-19 ARDS (C-ARDS) characterized by a respiratory system compliance (Crs) < 35 mL/cmH2O. Objectives: To test if an ECC with a 5 L-bag in low-compliance C-ARDS can lead to a reduction in driving pressure (DP) and improve gas exchange, and to understand the underlying mechanisms. Methods: Eleven patients with low-compliance C-ARDS were enrolled and underwent 4 steps: baseline, ECC for 60 min, ECC discontinuation and PEEP reduction. Respiratory mechanics, gas exchange, hemodynamics and electrical impedance tomography were recorded. Four pigs with acute ARDS were studied with ECC to understand the effect of ECC on pleural pressure gradient using pleural pressure transducers in both non-dependent and dependent lung regions. Results: Five minutes of ECC reduced DP from baseline 14.2 ± 1.3 to 12.3 ± 1.3 cmH2O (P < 0.001), explained by an improved lung compliance. Changes in DP by ECC were strongly correlated with changes in DP obtained with PEEP reduction (R2 = 0.82, P < 0.001). The initial benefit of ECC decreased over time (DP = 13.3 ± 1.5 cmH2O at 60 min, P = 0.03 vs. baseline). Gas exchange and hemodynamics were unaffected by ECC. In four pigs with lung injury, ECC led to a decrease in the pleural pressure gradient at end-inspiration [2.2 (1.1–3) vs. 3.0 (2.2–4.1) cmH2O, P = 0.035]. Conclusions: In C-ARDS patients with Crs < 35 mL/cmH2O, ECC acutely reduces DP. ECC does not improve oxygenation but it can be used as a simple tool to detect hyperinflation as it improves Crs and reduces Ppl gradient. ECC benefits seem to partially fade over time. ECC produces similar changes compared to PEEP reduction.

Abstract: Background: Results from retrospective studies suggest that use of neuromuscular blocking agents during general anaesthesia might be linked to postoperative pulmonary complications. We therefore aimed to assess whether the use of neuromuscular blocking agents is associated with postoperative pulmonary complications. Methods: We did a multicentre, prospective observational cohort study. Patients were recruited from 211 hospitals in 28 European countries. We included patients (aged ≥18 years) who received general anaesthesia for any in-hospital procedure except cardiac surgery. Patient characteristics, surgical and anaesthetic details, and chart review at discharge were prospectively collected over 2 weeks. Additionally, each patient underwent postoperative physical examination within 3 days of surgery to check for adverse pulmonary events. The study outcome was the incidence of postoperative pulmonary complications from the end of surgery up to postoperative day 28. Logistic regression analyses were adjusted for surgical factors and patients' preoperative physical status, providing adjusted odds ratios (ORadj) and adjusted absolute risk reduction (ARRadj). This study is registered with ClinicalTrials.gov, number NCT01865513. Findings: Between June 16, 2014, and April 29, 2015, data from 22 803 patients were collected. The use of neuromuscular blocking agents was associated with an increased incidence of postoperative pulmonary complications in patients who had undergone general anaesthesia (1658 [7·6%] of 21 694); ORadj 1·86, 95% CI 1·53–2·26; ARRadj −4·4%, 95% CI −5·5 to −3·2). Only 2·3% of high-risk surgical patients and those with adverse respiratory profiles were anaesthetised without neuromuscular blocking agents. Abstract: BACKGROUND: The SARS-CoV-2 pandemic increased the number of patients needing invasive mechanical ventilation, either through an endotracheal tube or through a tracheostomy. Tracheomalacia is a rare but potentially severe complication of mechanical ventilation, which can significantly compli-cate the weaning process. The aim of this study was to describe the strategies of airway management in mechanically ventilated patients with respiratory failure due to SARS-CoV-2, the incidence of severe tracheomalacia, and investigate the factors associated with its occurrence. METHODS: This retrospec-tive, single-center study was performed in an Italian teaching hospital. All adult subjects admitted to the ICU between February 24, 2020, and June 30, 2020, treated with invasive mechanical ventilation for respiratory failure caused by SARS-CoV-2 were included. Clinical data were collected on the day of ICU admission, whereas information regarding airway management was collected daily. RESULTS: A total of 151 subjects were included in the study. On admission, ARDS severity was mild in 21%, moderate in 62%, and severe in 17% of the cases, with an overall mortality of 40%. A tracheostomy was performed in 73 (48%), open surgical technique in 54 (74%), and percutaneous Ciaglia technique in 19 (26%). Subjects who had a tracheostomy performed had, compared to the other subjects, a longer duration of mechanical ventilation and longer ICU and hospital stay. Tracheomalacia was diagnosed in 8 (5%). The factors associated with tracheomalacia were female sex, obesity, and tracheostomy. CONCLUSIONS: In our population, approximately 50% of subjects with ARDS due to SARS-CoV-2 were tracheostomized. Tracheostomized subjects had a longer ICU and hospital stay. In our population, 5% were diagnosed with tracheomalacia. This percentage is 10 times higher than what is reported in available literature, and the underlying mechanisms are not fully understood.

Abstract: Background: Management of mechanical ventilation is a key issue in the prevention of postoperative pulmonary complications (PPCs) and the improvement of surgical outcome. This is especially true in cardiac surgery where the use of the cardiopulmonary bypass (CPB) increases the risk of lung injury. In the last years a growing number of studies have shown that protective ventilation has led to excellent results. However, the literature in this regard is lacking in cardiac surgery and there are no univocal guidelines in this sense. The aim of this survey was to investigate the actual clinical practice about ventilation techniques used in the Italian cardiac surgery centers. Methods: A questionnaire of 32-item was sent to 69 Italian cardiac surgery centers, 56 of which return a completed form (81.2%). The questionnaire was assembled by three independent researchers and the final version was e-mailed to all members of the SIAARTI (Italian society of anesthesia resuscitation and intensive care medicine) Study Group on Cardiothoracic and Vascular Anesthesia. The answers were collected using a Google Forms sheet. In case of multiple questionnaires returned from the same center (i.e., different physicians from the same center responded) the head of department was asked to give a definite answer. Furthermore, for the 17 centers who reported multiple questionnaires, no large differences were found between the responses of different doctors belonging to the same center (12.3%±4.2% of discordant answers). Results: Intraoperatively, patients were ventilated with a tidal volume (TV) of 6−8 mL/kg (91.1% of centers), a positive end-expiration pressure of 3−5 cmH 2 O (76.8% of centers) and a fraction of inspired oxygen (FiO 2 ) of 50−80% (60.7% of centers). Abstract: All-Vanadium Redox Flow Batteries (VRFBs) are emerging as a novel technology for stationary energy storage. Numerical models are useful for exploring the potential performance of such devices, optimizing the structure and operating condition of cell stacks, and studying its interfacing to the electrical grid. A one-dimensional steady-state multiphysics model of a single VRFB, including mass, charge and momentum transport and conservation, and coupled to a kinetic model for electrochemical reactions, is first presented. This model is then extended, including reservoir equations, in order to simulate the VRFB charge and discharge dynamics. These multiphysics models are discretized by the finite element method in a commercial software package (COMSOL). Numerical results of both static and dynamic 1D models are compared to those from 2D models, with the same parameters, showing good agreement. This motivates the use of reduced models for a more efficient system simulation.

Abstract: The first results obtained in the RFX reversed field pinch experiment after the 1995 machine modifications are reported. The confinement, for fully stationary discharges at 0.6 MA, has now reached the expected values, even in the presence of MHD wall locked modes. Studies on locked mode effects have evidenced currents flowing from the plasma into the vessel in the region of locking. Measurements on plasma rotation and radial electric field have shown a perpendicular velocity shear at the edge similar to what is found in tokamaks and stellarators. New measurements on edge superthermal electrons and some evidence of their correlation with plasma core characteristics are included.

Abstract: The results of the first year of operation of the experiment RFX are reported. Profiles of electron density, electron and ion temperature and impurity emission have been measured at plasma current I<0.7 MA. The energy confinement parameters at different density are reported, the best values ( tau E approximately 1ms, beta theta approximately 8%) being obtained operating at higher density. The role of the impurity content in determining the present performance of the experiment is discussed.

Abstract: This paper presents the I-band data obtained by the ESO Imaging Survey (EIS) over two patches of the sky, 6 square degrees each, centered at $\alpha \sim 5^h40^m$, $\delta \sim -24^\circ50^m$, and $\alpha \sim 9^h50^m$, $\delta \sim -21^\circ 00^m$. The data are being made public in the form of object catalogs and, photometrically and astrometrically calibrated pixel maps. These products together with other useful information can be found at "http://www.eso.org/eis". The overall quality of the data in the two fields is significantly better than the other two patches released earlier and cover a much larger contiguous area. The total number of objects in the catalogs extracted from these frames is over 700,000 down to $I\sim23$, where the galaxy catalogs are 80% complete. The star counts are consistent with model predictions computed at the position of the patches considered. The galaxy counts and the angular two-point correlation functions are also consistent with those of the other patches showing that the EIS data set is homogeneous and that the galaxy catalogs are uniform., Comment: 9 pages, 11 figures, submitted to Astronomy and Astrophysics

Abstract: This paper presents ground-based data obtained from deep optical and infrared observations of the HST Hubble Deep Field South (HDF-S) field carried out at the ESO 3.5 New Technology Telescope (NTT). These data were taken as part of the ESO Imaging Survey (EIS) program, a public survey coordinated by ESO and member states, in preparation for the first year of operation of the VLT. Deep CCD images are available for five optical passbands, reaching 2 sigma limiting magnitudes of U_AB~27.0, B_AB~26.5, V_AB~26, R_AB~26, I_AB~25, covering a region of ~25 square arcmin, which includes the HST WPFC2 field. The infrared observations cover a total area of ~42 square arcmin and include both the HST WFPC2 and STIS fields. The observations of the WFPC2 region were conducted in JHKs passbands, reaching J_AB~25, and H_AB and K_AB~24.0. Due to time constraints, the adjacent field, covering the STIS field, has been observed only in R, I and JHKs, while no observations were conducted covering the NIC3 field. This paper describes the observations and data reduction. It also presents images of the surveyed region and lists the optical and infrared photometric parameters of the objects detected on the co-added images of each passband, as well as multicolor optical and infrared catalogs. Abstract: This paper presents multicolor data obtained for a 1.7 square degree region near the South Galactic Pole (patch B) as part of the ESO Imaging Survey (EIS). So far the observations have been conducted in B,V and I but are expected to be complemented by observations in the U band later in 1998. Object catalogs extracted from single exposure images are 80% complete down to B~24, V~23.5 and I ~22.5 and once coadded should reach about 0.5 mag deeper. The data are being made public in the form of catalogs, pixel maps, target lists and image "postage stamps" which can be retrieved from the Web. Counts of stars and galaxies and the angular two-point correlation function of galaxies are computed and compared to other available data to evaluate the depth and uniformity of the extrtacted object catalogs. In addition, color distributions of stellar objects are presented and compared to model predictions to examine the reliability of the colors. Abstract: Preliminary results of a search for distant clusters of galaxies using the recently released I-band data obtained by the ESO Imaging Survey are presented. In this first installment of the survey, data covering about 3 square degrees in I-band are being used. The matched filter algorithm is applied to two sets of frames that cover the whole patch contiguously and these independent realizations are used to assess the performance of the algorithm and to establish, from the data itself, a robust detection threshold. Abstract: This paper presents 19 additional candidate clusters detected using the galaxy catalog extracted from the I-band images taken for the ESO Imaging Survey (EIS). The candidates are found over a region of 1.1 square degrees, located near the South Galactic Pole. Combined with the sample reported earlier, the number of candidates in the Southern Galactic Cap is now 54 over a total area of 3.6 square degrees. Images in V band are also available over 2.7 square degrees, and galaxy catalogs extracted from them over a uniform area of 2 square degrees have been used to further explore the reality of the I cluster candidates. Nearly all the I candidates with estimated redshifts <~ 0.5 are also identified in V. Abstract: The ESO Imaging Survey (EIS) presented in early issues of the Messenger, and with up-to-date information on the ongoing observations available on the Web (http://www.eso.org/eis), is a concerted effort by ESO and the Member State community to provide targets for the first year of operation of the VLT. It consists of two parts: a relatively wide-angle survey (EIS-WIDE) to cover four pre-selected patches of sky, 6 square degrees each, spread in right ascension to search for distant clusters and quasars and a deep, multicolor survey of four optical (SUSI-2) and two infrared (SOFI) covering the HST/Hubble Deep Field South (HDFS) and its flanking fields (EIS-DEEP). From the start, the main challenge has been to carry out a public survey in a limited amount of time requiring observations, software development and data reduction with the goal of distributing the survey data products before the call for proposal for the VLT. To cope with this one-year timetable, a novel type of collaboration between ESO and the community has been established which has allowed EIS to combine the scientific and technical expertise fo the community with in-house know-how and infrastructure. Abstract: This paper presents the preliminary results of the ESO Imaging Survey (EIS), a public survey being carried out by ESO and member states, in preparation for the VLT first-light. The survey goals, organization, strategy and observations are discussed and an overview is given of the survey pipeline developed to handle EIS data and produce object catalogs. A report is presented on moderately deep I-band observations obtained in the first of four patches surveyed, covering a region of 3.2 square degrees centered at R.A. 22h40m and Dec. -40d. The products available to the community, including pixel maps (with astrometric and photometric calibrations) and the corresponding object catalogs, are also described. In order to evaluate the quality of the data, preliminary estimates are presented for the star and galaxy number counts, and for the angular two-point correlation function obtained from the available data. Abstract: This paper presents preliminary lists of potentially interesting point-like sources extracted from multicolor data obtained for a 1.7 square degree region near the South Galactic Pole. The region has been covered by the ESO Imaging Survey (EIS) in B, V,I, and offers a unique combination of area and depth.These lists, containing a total of 330 objects nearly all brighter than I~21.5, over 1.27 square degrees (after removing some bad regions), are by-products of the process of verification and quality control of the object catalogs being produced. Among the color selected targets are candidate very low mass stars/brown dwarfs (54), white-dwarfs (32), and quasars (244). In addition, a probable fast moving asteroid was identified. Abstract: This paper presents ground-based data obtained from deep optical and infrared observations of the HST Hubble Deep Field South (HDF-S) field carried out at the ESO 3.5 New Technology Telescope (NTT). These data were taken as part of the ESO Imaging Survey (EIS) program, a public survey coordinated by ESO and member states, in preparation for the first year of operation of the VLT. Deep CCD images are available for five optical passbands, reaching 2 sigma limiting magnitudes of U_AB~27.0, B_AB~26.5, V_AB~26, R_AB~26, I_AB~25, covering a region of ~25 square arcmin, which includes the HST WPFC2 field. The infrared observations cover a total area of ~42 square arcmin and include both the HST WFPC2 and STIS fields. The observations of the WFPC2 region were conducted in JHKs passbands, reaching J_AB~25, and H_AB and K_AB~24.0. Due to time constraints, the adjacent field, covering the STIS field, has been observed only in R, I and JHKs, while no observations were conducted covering the NIC3 field. This paper describes the observations and data reduction. It also presents images of the surveyed region and lists the optical and infrared photometric parameters of the objects detected on the co-added images of each passband, as well as multicolor optical and infrared catalogs. Abstract: This paper presents the I-band data obtained by the ESO Imaging Survey (EIS) over two patches of the sky, 6 square degrees each, centered at $\alpha \sim 5^h40^m$, $\delta \sim -24^\circ50^m$, and $\alpha \sim 9^h50^m$, $\delta \sim -21^\circ 00^m$. The data are being made public in the form of object catalogs and, photometrically and astrometrically calibrated pixel maps. These products together with other useful information can be found at "http://www.eso.org/eis". The overall quality of the data in the two fields is significantly better than the other two patches released earlier and cover a much larger contiguous area. The total number of objects in the catalogs extracted from these frames is over 700,000 down to $I\sim23$, where the galaxy catalogs are 80% complete. The star counts are consistent with model predictions computed at the position of the patches considered. The galaxy counts and the angular two-point correlation functions are also consistent with those of the other patches showing that the EIS data set is homogeneous and that the galaxy catalogs are uniform., Comment: 9 pages, 11 figures, submitted to Astronomy and Astrophysics

Abstract: This paper presents multicolor data obtained for a 1.7 square degree region near the South Galactic Pole (patch B) as part of the ESO Imaging Survey (EIS). So far the observations have been conducted in B,V and I but are expected to be complemented by observations in the U band later in 1998. Object catalogs extracted from single exposure images are 80% complete down to B~24, V~23.5 and I ~22.5 and once coadded should reach about 0.5 mag deeper. The data are being made public in the form of catalogs, pixel maps, target lists and image "postage stamps" which can be retrieved from the Web. Counts of stars and galaxies and the angular two-point correlation function of galaxies are computed and compared to other available data to evaluate the depth and uniformity of the extrtacted object catalogs. In addition, color distributions of stellar objects are presented and compared to model predictions to examine the reliability of the colors. The results suggest that the overall quality of the catalogs extracted from the images is good and suitable for the science goals of the survey., Comment: 12 pages, 17 figures, submitted to Astronomy and Astrophysics

Abstract: Preliminary results of a search for distant clusters of galaxies using the recently released I-band data obtained by the ESO Imaging Survey are presented. In this first installment of the survey, data covering about 3 square degrees in I-band are being used. The matched filter algorithm is applied to two sets of frames that cover the whole patch contiguously and these independent realizations are used to assess the performance of the algorithm and to establish, from the data itself, a robust detection threshold. A preliminary catalog of distant clusters is presented, containing 39 cluster candidates with estimated redshifts 0.3 < z < 1.3 over an area of 2.5 square degrees., Comment: 13 pages, 10 figures, submitted to Astronomy and Astrophysics
