One of the surprising early findings with JWST has been the discovery of a strong "roll-over" or a softening of the absorption edge of Ly$\alpha$ in a large number of galaxies at ($z\gtrsim 6$), in addition to systematic offsets from photometric redshift estimates and fundamental galaxy scaling relations. This has been interpreted as damped Ly$\alpha$ absorption (DLA) wings from high column densities of neutral atomic hydrogen (HI), signifying major gas accretion events in the formation of these galaxies. To explore this new phenomenon systematically, we assemble the JWST/NIRSpec PRImordial gas Mass AssembLy (PRIMAL) legacy survey of 494 galaxies at $z=5.5-13.4$. We characterize this benchmark sample in full and spectroscopically derive the galaxy redshifts, metallicities, star-formation rates, and ultraviolet slopes. We define a new diagnostic, the Ly$\alpha$ damping parameter $D_{\rm Ly\alpha}$ to measure and quantify the Ly$\alpha$ emission strength, HI fraction in the IGM, or local HI column density for each source. The JWST-PRIMAL survey is based on the spectroscopic DAWN JWST Archive (DJA-Spec). All the software, reduced spectra, and spectroscopically derived quantities and catalogs are made publicly available in dedicated repositories. The fraction of strong galaxy DLAs are found to be in the range $65-95\%$ at $z>5.5$. The fraction of strong Ly$\alpha$ emitters (LAEs) is found to increase with decreasing redshift, in qualitative agreement with previous observational results, and are predominantly associated with low-metallicity and UV faint galaxies. By contrast, strong DLAs are observed in galaxies with a variety of intrinsic physical properties. Our results indicate that strong DLAs likely reflect a particular early assembly phase of reionization-era galaxies, at which point they are largely dominated by pristine HI gas accretion. Context. How galaxies form, assemble, and cease their star formation is a central question within the modern landscape of galaxy evolution studies. These processes are indelibly imprinted on the galaxy stellar mass function (SMF), and its measurement and understanding is key to uncovering a unified theory of galaxy evolution. Aims. We present constraints on the shape and evolution of the galaxy SMF, the quiescent galaxy fraction, and the cosmic stellar mass density across 90% of the history of the Universe from z = 7.5 0.2 as a means to study the physical processes that underpin galaxy evolution. Methods. The COSMOS survey is an ideal laboratory for studying representative galaxy samples. Now equipped with deeper and more homogeneous near-infrared coverage exploited by the COSMOS2020 catalog, we leverage the large 1.27 deg2 effective area to improve sample statistics and understand spatial variations (cosmic variance) particularly for rare, massive galaxies and push to higher redshifts with greater confidence and mass completeness than previous studies. We divide the total stellar mass function into star-forming and quiescent subsamples through NUVrJ color-color selection. The measurements are then fit with single- and double-component Schechter functions to infer the intrinsic galaxy stellar mass function, the evolution of its key parameters, and the cosmic stellar mass density out to z = 7.5. Finally, we compare our measurements to predictions from state-of-the-art cosmological simulations and theoretical dark matter halo mass functions. Results. We find a smooth, monotonic evolution in the galaxy stellar mass function since z = 7.5, in general agreement with previous studies. The number density of star-forming systems have undergone remarkably consistent growth spanning four decades in stellar mass from z = 7.5 2 whereupon high-mass systems become predominantly quiescent ( downsizing ). While space-borne optical and near-infrared facilities have succeeded in delivering a precise and spatially resolved picture of our Universe, their small survey area is known to under-represent the true diversity of galaxy populations. Ground-based surveys have reached comparable depths but at lower spatial resolution, resulting in source confusion that hampers accurate photometry extractions. What once was limited to the infrared regime has now begun to challenge ground-based ultra-deep surveys, affecting detection and photometry alike. Failing to address these challenges will mean forfeiting a representative view into the distant Universe. We introduce The Farmer: an automated, reproducible profile-fitting photometry package that pairs a library of smooth parametric models from The Tractor (Lang et al. 2016) with a decision tree that determines the best-fit model in concert with neighboring sources. Photometry is measured by fitting the models on other bands leaving brightness free to vary. The resulting photometric measurements are naturally total, and no aperture corrections are required. Supporting diagnostics (e.g. $\chi^2$) enable measurement validation. As fitting models is relatively time intensive, The Farmer is built with high-performance computing routines. We benchmark The Farmer on a set of realistic COSMOS-like images and find accurate photometry, number counts, and galaxy shapes. The Farmer is already being utilized to produce catalogs for several large-area deep extragalactic surveys where it has been shown to tackle some of the most challenging optical and near-infrared data available, with the promise of extending to other ultra-deep surveys expected in the near future. Background/purpose: Matching phasic pressure tracings between a fluid-filled catheter and high-fidelity pressure wire has received limited attention, although each part contributes half of the information to clinical decisions. We aimed to study the impact of a novel and automated method for improving the phasic calibration of a fluid-filled catheter by accounting for its oscillatory behavior. Methods/materials: Retrospective analysis of drift check tracings was performed using our algorithm that corrects for mean difference (offset), temporal delays (timing), differential sensitivity of the manifold transducer and pressure wire sensor (gain), and the oscillatory behavior of the fluid-filled catheter described by its resonant frequency and damping factor (how quickly oscillations disappear after a change in pressure). Results: Among 2886 cases, correcting for oscillations showed a large improvement in 28 % and a medium improvement in 41 % (decrease in root mean square error >0.5 mmHg to <1 or 1-2 mmHg, respectively). 96 % of oscillators were underdamped with median damping factor 0.27 and frequency 10.6 Hz. Fractional flow reserve or baseline Pd/Pa demonstrated no clinically important bias when ignoring oscillations. However, uncorrected subcycle non-hyperemic pressure ratios (NHPR) displayed both bias and scatter. Conclusions: By automatically accounting for the oscillatory behavior of a fluid-filled catheter system, phasic matching against a high-fidelity pressure wire can be improved compared to standard equalization methods. The majority of tracings contain artifacts, mainly due to underdamped oscillations, and neglecting them leads to biased estimates of equalization parameters. No clinically important bias exists for whole-cycle metrics, in contrast to significant effects on subcycle NHPR.

An excess of massive systems at $z\sim2.5-5.5$ with strikingly red colors, some newly identified, increase the observed number densities to the point where the SMF cannot be reconciled with a Schechter function. Systematics including cosmic variance and/or AGN contamination are unlikely to fully explain this excess, and so we speculate that there may be contributions from dust-obscured objects similar to those found in FIR surveys. A detailed comparison of the two resulting photometric catalogs is presented. Photometric redshifts are computed for all sources in each catalog utilizing two independent photometric redshift codes. Finally, a comparison is made between the performance of the photometric methodologies and of the redshift codes to demonstrate an exceptional degree of self-consistency in the resulting photometric redshifts. The i < 21 sources have subpercent photometric redshift accuracy and even the faintest sources at 25 < i < 27 reach a precision of 5%. Finally, these results are discussed in the context of previous, current, and future surveys in the COSMOS field. Compared to COSMOS2015, it reaches the same photometric redshift precision at almost one magnitude deeper. Both photometric catalogs and their photometric redshift solutions and physical parameters will be made available through the usual astronomical archive systems (ESO Phase 3, IPAC-IRSA, and CDS).

Background: Following programmatic introduction of the 13-valent pneumococcal conjugate vaccine (PCV13), there is residual carriage and disease due to PCV13-covered serotypes. Methods. PCV13-immunized children aged 13-48 months, N = 988, were enrolled between February 2014 and August 2015 (“late PCV13”), and had nasopharyngeal pneumococcal carriage compared with 7-valent pneumococcal conjugate vaccine (PCV7) immunized children, N = 567, enrolled between November 2010 and September 2011 (“early PCV13”). Nasopharyngeal pneumococci were molecularserotyped by microarray. Invasive pneumococcal disease (IPD) cases were identified through enhanced national surveillance. Results. Compared with PCV7-immunized children, carriage among PCV13-immunized children was significantly lower for serotypes 19A (odds ratio [OR], 0.08 [95% confidence interval {CI},.02-.25]), 6C (OR, 0.11 [95% CI,.03-.32]), and 7F (8 vs 0 cases). IPD incidence in children <5 years was significantly lower for serotypes 1 (incidence rate ratio [IRR], 0.03 [95% CI, 0-.19]) and 7F (IRR, 0.13 [95% CI,.05-.36]) but not 19A (IRR, 0.6 [95% CI,.3-1.12]) or serotype 3 (IRR, 2.3 [95% CI,.86-6.15]) in the late PCV13 period than in the early PCV13 period. The most significant rises in IPD incidence were for serotypes 8, 12F, and 24F. Conclusions. PCV13 has reduced serotype 19A carriage among vaccinated children. We found no impact of PCV13 on serotype 3 carriage or disease, and emergence of non-PCV13-serotype disease.

We report on the design, deployment, and first results from a scintillation detector deployed at the Murchison Radio-astronomy Observatory (MRO). The detector is a prototype for a larger array -- the Square Kilometre Array Particle Array (SKAPA) -- planned to allow the radio-detection of cosmic rays with the Murchison Widefield Array and the low-frequency component of the Square Kilometre Array. The prototype design has been driven by stringent limits on radio emissions at the MRO, and to ensure survivability in a desert environment. Using data taken from Nov.\ 2018 to Feb.\ 2019, we characterize the detector response while accounting for the effects of temperature fluctuations, and calibrate the sensitivity of the prototype detector to through-going muons. This verifies the feasibility of cosmic ray detection at the MRO. We then estimate the required parameters of a planned array of eight such detectors to be used to trigger radio observations by the Murchison Widefield Array., Comment: 17 pages, 14 figures, 3 tables

Background: Potentially some patients have symptoms that arise from their low-gradient aortic valve stenosis (AS). Comprehensive valve physiology with dobutamine stress remains incompletely characterized in this population. Methods: A cohort of 18 subjects with low-gradient AS underwent graded dobutamine infusion with invasive assessment using 0.014” pressure wires. A subset of 4 subjects received thermodilution cardiac output assessment at each stage. Results: Peak dobutamine hemodynamics could not be predicted from clinical or baseline parameters, reflecting statistically the physiologic heterogeneity of the measured pressure loss versus flow curves. While 0 subjects had a baseline aortic/left ventricular pressure ratio during ejection <0.71, 7 of 18 subjects (39%) achieved a ratio during peak dobutamine (the so-called stress aortic valve index, SAVI) below this threshold derived from a prior study of patients undergoing routine transcatheter aortic valve implantation (TAVI). Conclusion: For low-gradient AS, the hemodynamic changes from resting to peak dobutamine conditions cannot be predicted in advance due to pressure loss versus flow curve heterogeneity. A sizable minority of low-gradient AS reaches a severity during dobutamine stress equivalent to patients undergoing TAVI for established benefit. Whether this subset receives similar clinical advantage remains an unproven but natural hypothesis raised by our study.

BACKGROUND: Physical inactivity is a key contributor to the global burden of disease and disproportionately impacts the wellbeing of people experiencing mental illness. Increases in physical activity are associated with improvements in symptoms of mental illness and reduction in cardiometabolic risk. Reliable and valid clinical tools that assess physical activity would improve evaluation of intervention studies that aim to increase physical activity and reduce sedentary behaviour in people living with mental illness. METHODS: The five-item Simple Physical Activity Questionnaire (SIMPAQ) was developed by a multidisciplinary, international working group as a clinical tool to assess physical activity and sedentary behaviour in people living with mental illness. Patients with a DSM or ICD mental illness diagnoses were recruited and completed the SIMPAQ on two occasions, one week apart. Participants wore an Actigraph accelerometer and completed brief cognitive and clinical assessments. RESULTS: Evidence of SIMPAQ validity was assessed against accelerometer-derived measures of physical activity. Data were obtained from 1010 participants. The SIMPAQ had good test-retest reliability. Correlations for moderate-vigorous physical activity was comparable to studies conducted in general population samples. Evidence of validity for the sedentary behaviour item was poor. An alternative method to calculate sedentary behaviour had stronger evidence of validity. This alternative method is recommended for use in future studies employing the SIMPAQ. CONCLUSIONS: The SIMPAQ is a brief measure of physical activity and sedentary behaviour that can be reliably and validly administered by health professionals.

Aims: Echocardiography and tomographic imaging have documented dynamic changes in aortic stenosis (AS) geometry and severity during both the cardiac cycle and stress-induced increases in cardiac output. However, corresponding pressure gradient vs. flow relationships have not been described. Methods and results: We recruited 16 routine transcatheter aortic valve implantations (TAVI's) for graded dobutamine infusions both before and after implantation; 0.014″ pressure wires in the aorta and left ventricle (LV) continuously measured the transvalvular pressure gradient (ΔP) while a pulmonary artery catheter regularly assessed cardiac output by thermodilution. Before TAVI, ΔP did not display a consistent relationship with transvalvular flow (Q). Neither linear resistor (median R2 0.16) nor quadratic orifice (median R2 < 0.01) models at rest predicted stress observations; the severely stenotic valve behaved like a combination. The unitless ratio of aortic to left ventricular pressures during systolic ejection under stress conditions correlated best with post-TAVI flow improvement. After TAVI, a highly linear relationship (median R2 0.96) indicated a valid valve resistance. Conclusion: Pressure loss vs. flow curves offer a fundamental fluid dynamic synthesis for describing aortic valve pathophysiology. Severe AS does not consistently behave like an orifice (as suggested by Gorlin) or a resistor, whereas TAVI devices behave like a pure resistor. During peak dobutamine, the ratio of aortic to left ventricular pressures during systolic ejection provides a 'fractional flow reserve' of the aortic valve that closely approximates the complex, changing fluid dynamics. Because resting assessment cannot reliably predict stress haemodynamics, 'valvular fractional flow' warrants study to explain exertional symptoms in patients with only moderate AS at rest.

Initial internet-based cognitive behavioral therapy (iCBT) programs for anxiety disorders in children and young people (CYP) have been developed and evaluated, however these have not yet been widely adopted in routine practice. The lack of guidance and formalized approaches to the development and dissemination of iCBT has arguably contributed to the difficulty in developing iCBT that is scalable and sustainable beyond academic evaluation and that can ultimately be adopted by healthcare providers. BACKGROUND: The indirect effects of pneumococcal conjugate vaccines (PCVs) are mediated through reductions in carriage of vaccine serotypes. Data on PCVs in Asia and the Pacific are scarce. Fiji introduced the ten-valent PCV (PCV10) in 2012, with a schedule consisting of three priming doses at 6, 10, and 14 weeks of age and no booster dose (3 + 0 schedule) without catch-up. We investigated the effects of PCV10 introduction using cross-sectional nasopharyngeal carriage surveys. METHODS: We did four annual carriage surveys (one pre-PCV10 and three post-PCV10) in the greater Suva area in Fiji, during 2012-15, of 5-8-week-old infants, 12-23-month-old children, 2-6-year-old children, and their caregivers (total of 8109 participants). Eligible participants were of appropriate age, had axillary temperature lower than 37°C, and had lived in the community for at least 3 consecutive months. We used purposive quota sampling to ensure a proper representation of the Fiji population. Pneumococci were detected by real-time quantitative PCR, and molecular serotyping was done with microarray. FINDINGS: 3 years after PCV10 introduction, vaccine-serotype carriage prevalence declined, with adjusted prevalences (2015 vs 2012) of 0·56 (95% CI 0·34-0·93) in 5-8-week-old infants, 0·34 (0·23-0·49) in 12-23-month-olds, 0·47 (0·34-0·66) in 2-6-year-olds, and 0·43 (0·13-1·42) in caregivers. Reductions in PCV10 serotype carriage were evident in both main ethnic groups in Fiji; however, carriage of non-PCV10 serotypes increased in Indigenous Fijian infants and children. Density of PCV10 serotypes and non-PCV10 serotypes was lower in PCV10-vaccinated children aged 12-23 months than in PCV10-unvaccinated children of the same age group (PCV10 serotypes -0·56 [95% CI -0·98 to -0·15], p=0·0077; non-PCV10 serotypes -0·29 [-0·57 to -0·02], p=0·0334). One such challenge concerns its propensity to give rise to small defamation claims between ordinary people given the often-enormous costs of litigating defamation claims before the ordinary courts. This article promotes a reform agenda directed to meeting this challenge by (1) demonstrating the need for a proportionate means for resolving small defamation claims, having regard to access to justice considerations and other wider concerns; (2) establishing reasonable grounds for seriously considering deploying the traditional small-claims-proportionate response – small claims jurisdictions – for this purpose notwithstanding contraindications including the infamous complexity of defamation law; and (3) advancing a research pathway for the proportionate treatment of small defamation claims to guide decision-making and innovation. This article also advocates for consideration of this important issue in the ‘national reform process’ launched in 2018 for Australian defamation law.

Coronary vasomotion plays an important role in the regulation of coronary perfusion at rest and during exercise. Normal coronary arteries show coronary vasodilation of the proximal (+20%) and distal (+40%) vessel segments during supine bicycle exercise. However, patients with coronary artery disease show exercise-induced vasoconstriction of the stenotic vessel segments. The exact mechanism of exercise-induced stenosis narrowing is not clear but might be related to a passive collapse of the disease-free vessel wall (Venturi mechanism), elevated plasma levels of circulating catecholamines, an insufficient production of the endothelium-derived vesorelaxing factor or increased platelet aggregation due to turbulent blood flow with release of thromboxane A2 and serotonin. Various vasoactive drugs, such as nitroglycerin and calcium antagonists, prevent exercise-induced stenosis vasoconstriction. An additive effect on coronary vasodilation of the stenotic vessel segment was observed after combination of nitroglycerin with diltiazem. Thus, exercise-induced stenosis narrowing plays an important role in the pathophysiology of myocardial ischaemia during dynamic exercise. Objective: The aim is to identify the role and scope of Accredited Exercise Physiologist (AEP) services in the mental health sector and to provide insight as to how AEPs can contribute to the multidisciplinary mental health team. Method(s): A modified Delphi approach was utilised. Thirteen AEPs with experience in mental health contributed to the iterative development of a national consensus statement. Six mental health professionals with expertise in psychiatry, mental health nursing, general practice and mental health research participated in the review process. Reviewers were provided with a template to systematically provide feedback on the language, content, structure and relevance to their professional group. Result(s): This consensus statement outlines how AEPs can contribute to the multidisciplinary mental health team, the aims and scope of AEP-led interventions in mental health services and examples of such interventions, the range of physical and mental health outcomes possible through AEP-led interventions and common referral pathways to community AEP services. Outcome(s): AEPs can play a key role in the treatment of individuals experiencing mental illness. Objectives This study classified and quantified the variation in fractional flow reserve (FFR) due to fluctuations in systemic and coronary hemodynamics during intravenous adenosine infusion. Background Although FFR has become a key invasive tool to guide treatment, questions remain regarding its repeatability and stability during intravenous adenosine infusion because of systemic effects that can alter driving pressure and heart rate. Methods We reanalyzed data from the VERIFY (VERification of Instantaneous Wave-Free Ratio and Fractional Flow Reserve for the Assessment of Coronary Artery Stenosis Severity in EverydaY Practice) study, which enrolled consecutive patients who were infused with intravenous adenosine at 140 μg/kg/min and measured FFR twice. Raw phasic pressure tracings from the aorta (Pa) and distal coronary artery (Pd) were transformed into moving averages of Pd/Pa. Visual analysis grouped Pd/Pa curves into patterns of similar response. Quantitative analysis of the Pd/Pa curves identified the "smart minimum" FFR using a novel algorithm, which was compared with human core laboratory analysis. Results A total of 190 complete pairs came from 206 patients after exclusions. Visual analysis revealed 3 Pd/Pa patterns: "classic" (sigmoid) in 57%, "humped" (sigmoid with superimposed bumps of varying height) in 39%, and "unusual" (no pattern) in 4%. The Pd/Pa pattern repeated itself in 67% of patient pairs. Despite variability of Pd/Pa during the hyperemic period, the "smart minimum" FFR demonstrated excellent repeatability (bias -0.001, SD 0.018, paired p = 0.93, r2 = 98.2%, coefficient of variation = 2.5%). Our algorithm produced FFR values not significantly different from human core laboratory analysis (paired p = 0.43 vs. VERIFY; p = 0.34 vs. RESOLVE). Molecular identification of Streptococcus pneumoniae serotype 19F is routinely performed by PCR targeting the wzy gene of the capsular biosynthetic locus. However, 19F isolates with genetic similarity to 19A have been reported in the United States and Brazil. We screened 78 pneumococcal carriage isolates and found six 19F wzy variants that originated from children in Papua New Guinea and Fiji. Isolates were characterized using multilocus sequence typing and opsonophagocytic assays. The 19F wzy variants displayed similar susceptibility to anti-19F IgG antibodies compared to standard 19F isolates. Our findings indicate that these 19F variants may be more common than previously believed.

Although neuropsychological deficits in adult depression are relatively well established, findings in children/adolesc ents have been inconsistent and thus require further investigation. The current study investigated verbal fluency (VF), cognitive speed, motor speed, and executive functions in adolescents with unipolar depression. Results indicated that adolescents with minor depression showed working memory deficits and poorer VF (letter task). Adolescents with major depression showed working memory deficits and processing speed deficits from the early stages of information processing to the later stages of motor output. Executive function deficits of set-shifting and response inhibition that are well established in adults were not found, but may reflect task differences. Thus, it appears that depression subtype or severity of symptoms may impact on neuropsychological functioning and may in part explain previous inconsistent results. © The Author 2011. Gametophytes of Jamesoniella colorata (Jungermanniaceae) and Isotachis lyallii (Isotachidaceae) produce red leaves in exposed habitats, but green leaves in shaded environments. To understand the functional significance of this colour polymorphism, the anatomy, pigment composition, optical properties, and kinetics of chlorophyll a fluorescence were compared for red and green gametophytes. Both colour morphs were structurally similar, but the red leaves held unidentified red pigment(s) firmly associated with the cell wall. Green morphs contained more chlorophylls and carotenoids, and had higher ratios of chlorophylls to carotenoids, than did the red morphs. Red leaves absorbed 10% more photosynthetically active radiation, with a maximum at 540 nm, than did the green leaves. Under high irradiance, the red leaves maintained higher apparent quantum efficiencies for photosynthesis, and had larger photochemical and non-photochemical quenching values. There is a "policy" that judicial officers should not sue for defamation, save in exceptional circumstances. But this is coming under challenge on a number of fronts. Concerns about maintaining public confidence in the judiciary if this policy is relaxed or even abandoned are now largely illusory in the face of a robust judiciary as part of a modern democracy. More worrying is the potential chilling effect on criticism of the judiciary; the real issue is whether the defamation defences are up to the task of adequately protecting this form of speech - and it appears that there may be reason for concern. These issues are considered against the background of the robust views about criticism of the judiciary expressed by Justice Sackville, Chair of the Judicial Conference of Australia and a Judge of the Federal Court of Australia, in his Lucinda Lecture and other fora in 2005. The inquiry also provides insight into other wider issues including the interrelationship between the judicial restraint policy and scandalising contempt.

However, patients with coronary artery disease show exercise-induced vasoconstriction of the stenotic vessel segments. The exact mechanism of exercise-induced stenosis narrowing is not clear but might be related to a passive collapse of the disease-free vessel wall (Venturi mechanism), elevated plasma levels of circulating catecholamines, an insufficient production of the endothelium-derived vesorelaxing factor or increased platelet aggregation due to turbulent blood flow with release of thromboxane A2 and serotonin. Various vasoactive drugs, such as nitroglycerin and calcium antagonists, prevent exercise-induced stenosis vasoconstriction. An additive effect on coronary vasodilation of the stenotic vessel segment was observed after combination of nitroglycerin with diltiazem. Thus, exercise-induced stenosis narrowing plays an important role in the pathophysiology of myocardial ischaemia during dynamic exercise.
