1. Quantitative analysis of secondary organic aerosol using a high-resolution mass spectral database

Abstract: Organic aerosol is an important factor in the atmospheric pollution, contributing up to 90% of the total aerosol. The complex system of sources and pathways of functionalisation these compounds can undergo, lead to a large number of compounds which exist in the organic fraction. The state of this organic fraction is in flux, due to the varying contributions of sources and the oxidative aging of the air mass. A purpose-built, UPLC-MS2 method, combined with a semi-targeted automated processing software was developed to compliment large datasets required for high temporal resolution. This method was validated using previous analysis of organic aerosol from London and Birmingham. The organic fraction in Beijing, China was observed from high resolution off-line filter samples during two field campaigns during winter 2016 and summer 2017. In total 646 compounds were identified during these campaigns, of which, the nitrophenolics and organosulfates were observed to be the most important groups. Hierarchical cluster analyses were applied to these two groups to determine the most important clusters within these heteroatom groups. 43 nitrophenolics were observed in the organic aerosol and 191 organosulfates. Nitrophenolics observed by this method show very high variation in ionisation efficiency based on structural isomerism, with ionisation efficiency varying of factors up to three orders of magnitude. The most significant organosulfates are more locally sourced to the Beijing area.

Abstract: Introduction: The Covid-19 pandemic limited educational and career development opportunities for medical students, requiring innovative programs to accelerate professional identity formation and clinical skills acquisition. Methods: We developed a brief coaching intervention that took place over the advanced (sub-internship) emergency medicine rotation at our institution. We trained coaches using a newly developed workshop, who met with students for an average of 4.5 hours over 3 weeks. Impact/effectiveness: We showed that this coaching program was both feasible and impactful for faculty coaches and medical students.

4. Popular policing? Sector policing and the reinvention of police accountability

Abstract: The aim of this thesis is to explain the change in the debate about police accountability in Britain that took place in the 1980s. In seeking such an explanation in the reinvention of police accountability over this period, a four dimensional analysis of accountability is presented. This is used to examine, in turn, the history of police governance in London, the debates about police accountability that took place in the 1980s, and the implications of the growing influence of community policing that culminated in the introduction by the Metropolitan Police of a new style of ‘sector policing’. A series of questions about whether and how police accountability was reinvented in the 1980s are posed, and the implications of the reconceptualisation that took place are assessed in their historical and theoretical contexts. Use is also made of empirical data drawn from a study of the implementation of sector policing on an inner city police area in North London. It is argued that far-reaching changes took place in the conceptualisation of police accountability during the 1980s on all four of the dimensions identified, and that this reinvention of the relationship between police and people made policing in London neither more democratic nor more consensual.

Abstract: Twenty-six years ago a small committee report, building on earlier studies, expounded a compelling and poetic vision for the future of astronomy, calling for an infrared-optimized space telescope with an aperture of at least 4 m. With the support of their governments in the US, Europe, and Canada, 20,000 people realized that vision as the 6.5 m James Webb Space Telescope. A generation of astronomers will celebrate their accomplishments for the life of the mission, potentially as long as 20 yr, and beyond. This report and the scientific discoveries that follow are extended thank-you notes to the 20,000 team members. The telescope is working perfectly, with much better image quality than expected. In this and accompanying papers, we give a brief history, describe the observatory, outline its objectives and current observing program, and discuss the inventions and people who made it possible. We cite detailed reports on the design and the measured performance on orbit.

6. Feasibility and acceptability to use a smartphone-based manikin for daily longitudinal self-reporting of chronic pain

Abstract: Background: As management of chronic pain continues to be suboptimal, there is a need for tools that support frequent, longitudinal pain self-reporting to improve our understanding of pain. This study aimed to assess the feasibility and acceptability of daily pain self-reporting using a smartphone-based pain manikin. Methods: For this prospective feasibility study, we recruited adults with lived experience of painful musculoskeletal condition. They were asked to complete daily pain self-reports via an app for 30 days. We assessed feasibility by calculating pain report completion levels, and investigated differences in completion levels between subgroups. We assessed acceptability via an end-of-study questionnaire, which we analysed descriptively. Results: Of the 104 participants, the majority were female (n = 87; 84%), aged 45-64 (n = 59; 57%), and of white ethnic background (n = 89; 86%). The mean completion levels was 21 (± 7.7) pain self-reports. People who were not working (odds ratio (OR) = 1.84; 95% confidence interval (CI), 1.52-2.23) were more likely, and people living in less deprived areas (OR = 0.77; 95% CI, 0.62-0.97) and of non-white ethnicity (OR = 0.45; 95% CI, 0.36-0.57) were less likely to complete pain self-reports than their employed, more deprived and white counterparts, respectively. Of the 96 participants completing the end-of-study questionnaire, almost all participants agreed that it was easy to complete a pain drawing (n = 89; 93%). Conclusion: It is feasible and acceptable to self–report pain using a smartphone–based manikin over a month. For its wider adoption for pain self–reporting, the feasibility and acceptability should be further explored among people with diverse socio–economic and ethnic backgrounds.

Abstract: BACKGROUND: Culture and ethnicity influence how people communicate about their pain. This makes it challenging to develop pain self-report tools that are acceptable across ethnic groups.OBJECTIVE: We aimed to inform the development of cross-culturally acceptable digital pain self-report tools by better understanding the similarities and differences between ethnic groups in pain experiences and self-reporting needs.METHODS: Three web-based workshops consisting of a focus group and a user requirement exercise with people who self-identified as being of Black African (n=6), South Asian (n=10), or White British (n=7) ethnicity were conducted.RESULTS: Across ethnic groups, participants shared similar lived experiences and challenges in communicating their pain to health care professionals. However, there were differences in beliefs about the causes of pain, attitudes toward pain medication, and experiences of how stigma and gender norms influenced pain-reporting behavior. Despite these differences, they agreed on important aspects for pain self-report, but participants from non-White backgrounds had additional language requirements such as culturally appropriate pain terminologies to reduce self-reporting barriers.CONCLUSIONS: To improve the cross-cultural acceptability and equity of digital pain self-report tools, future developments should address the differences among ethnic groups on pain perceptions and beliefs, factors influencing pain reporting behavior, and language requirements.

8. Observations of the Ultraviolet-Bright Star ZNG 1 in the Globular Cluster M5 (NGC 5904)

Abstract: We have analyzed archival spectra of the hot UV-bright star ZNG 1 in the globular cluster M5 (NGC 5904) obtained with the Far Ultraviolet Spectroscopic Explorer (FUSE) and the Space Telescope Imaging Spectrograph (STIS). From these data, we derive an effective temperature $T_{\rm eff} = 43{,}000 \pm 1400$ K, a surface gravity $\log g = 4.47 \pm 0.08$, a rotational velocity $v \sin i = 157 \pm 12$ km s$^{-1}$, and a mass $M = 0.92 \pm 0.17 \, M_{\odot}$. The atmosphere is helium-rich ($Y = 0.99$) and enhanced in CNO (relative to the cluster). The spectrum exhibits wind features with a terminal velocity near 1500 km s$^{-1}$ and strong discrete absorption components (DACs). The high helium abundance, stellar mass, and rotational velocity suggest that the star is a merger remnant, and its parameters are consistent with models of a pair of merging He-core white dwarfs., Comment: 12 pages, 9 figures, to be published in the Astronomical Journal

9. Evidence of Third Dredge-Up in Post-AGB Stars in Galactic Globular Clusters

Abstract: To better understand the mixing and mass loss experienced by low-mass stars as they ascend the asymptotic giant branch (AGB), I have gathered from the literature the abundances of CNO and s-process elements in post-AGB stars in Galactic globular clusters. These species are mixed to the surface during third dredge-up (3DU) events, so their abundance should increase as the star ascends the AGB. Of the 17 stars in this sample, CNO abundances are available for 11. Of these, four are enhanced in CNO relative to the RGB stars from which they descended, which I take as evidence of 3DU on the AGB. The enhancement is mainly in the form of carbon. Of the six stars for which only heavy-element abundances are available, one shows s-process enhancements that previous authors have interpreted as evidence of 3DU. Combining these 17 stars with other recent samples reveals that most globular-cluster post-AGB stars have luminosities log (L/L_sun) ~ 3.25. They are the progeny of blue horizontal-branch (HB) stars in clusters with intermediate metallicity ([Fe/H] ~ -1.5). A second group consists of sub-luminous stars associated with high-metallicity clusters ([Fe/H] ~ -1.0) with red HBs. They may be burning helium, rather than hydrogen. A third group of hot, super-luminous stars is evolving quickly across the Hertzsprung-Russell diagram. Some of them may be merger remnants., Comment: 12 pages, 4 figures

Abstract: Coding of unstructured clinical free-text to produce interoperable structured data is essential to improve direct care, support clinical communication and to enable clinical research.However, manual clinical coding is difficult and time consuming, which motivates the development and use of natural language processing for automated coding. This work evaluates the quality and consistency of both manual and automated clinical coding of diagnoses from hospital outpatient letters. Using 100 randomly selected letters, two human clinicians performed coding of diagnosis lists to SNOMED CT. Automated coding was also performed using IMO's Concept Tagger. A gold standard was constructed by a panel of clinicians from a subset of the annotated diagnoses. This was used to evaluate the quality and consistency of both manual and automated coding via (1) a distance-based metric, treating SNOMED CT as a graph, and (2) a qualitative metric agreed upon by the panel of clinicians. Correlation between the two metrics was also evaluated. Comparing human and computer-generated codes to the gold standard, the results indicate that humans slightly out-performed automated coding, while both performed notably better when there was only a single diagnosis contained in the free-text description. Automated coding was considered acceptable by the panel of clinicians in approximately 90% of cases.

Abstract: Economics is a central element of statecraft. Ricardo argued for a natural system of political order through which the free operation of partial interests would achieve the general interest. The premise of this system was the independent labourer. This premise has been the most important for any system of economics in that it is not just the first assumption but also the result to be achieved. After Ricardo we can follow the course of political economy as it sought to secure its initial assumptions within changing social circumstances. We can see the changes which occurred in political economy in the 19th century as a response to the problems posed by the emergence of a political challenge to the market. First the classical conception of poverty was replaced by the modem conception of shortage and then the concept of unemployment and so full employment replaced the old idea of redundant persons. In this light Keynes's General Theory can be regarded as the completion of the process in that it completed a history of reform that had set out to retrieve a sustainable political order on the basis of the independent labourer. In the course of this development the foundation of the economic theory evolved from a natural basis to one in which policy played a critical role; this evolution marked the emergence and containment of the collective worker.

Abstract: We present the wide field slitless spectroscopy mode of the NIRISS instrument on the James Webb Space Telescope. This mode employs two orthogonal low-resolution (resolving power $\approx 150$) grisms in combination with a set of six blocking filters in the wavelength range 0.8 to $2.3\,\mu$m to provide a spectrum of almost every source across the field-of-view. When combined with the low background, high sensitivity and high spatial resolution afforded by the telescope, this mode will enable unprecedented studies of the structure and evolution of distant galaxies. We describe the performance of the as-built hardware relevant to this mode and expected imaging and spectroscopic sensitivity. We discuss operational and calibration procedures to obtain the highest quality data. Abstract: Introduction: Medical students lack adequate training on how to correctly don and doff personal protective equipment (PPE). Simulation-based mastery learning (SBML) is an effective technique for procedural education. The aim of this study was to determine whether SBML improves proper PPE donning and doffing by medical students.Methods: This was a prospective, pre-test/post-test study of 155 medical students on demonstration of correct PPE use before and after a SBML intervention. Subjects completed standard hospital training by viewing a US Centers for Disease Control and Prevention training video on proper PPE use prior to the intervention. They then participated in a SBML training session that included baseline testing, deliberate practice with expert feedback, and post-testing until mastery was achieved. Students were assessed using a previously developed 21-item checklist on donning and doffing PPE with a minimum passing standard (MPS) of 21/21 items. We analyzed differences between pre-test and post-test scores using paired t-tests. Students at preclinical and clinical levels of training were compared with an independent t-test. Results: Two participants (1.3%) met the MPS on pre-test. Of the remaining 153 subjects who participated in the intervention, 151 (98.7%) reached mastery. Comparison of mean scores from pre-test to final post-test significantly improved from an average raw score of 12.55/21 (standard deviation [SD] = 2.86), to 21/21(SD = 0), t(150) = 36.3, P <0.001. There was no difference between pre-test scores of pre-clinical and clinical students. Conclusion: Simulation-based mastery learning improves medical student performance in PPE donning and doffing in a simulated environment. This approach standardizes PPE training for students in advance of clinical experiences.

Abstract: Objective: Clinical trials assessing systemic sclerosis (SSc)-related digital ulcers have been hampered by a lack of reliable outcome measures of healing. Our objective was to assess the feasibility of patients collecting high-quality mobile phone images of their digital lesions as a first step in developing a smartphone-based outcome measure. Methods: Patients with SSc-related digital (finger) lesions photographed one or more lesions each day for 30 days using their smartphone and uploaded the images to a secure Dropbox folder. Image quality was assessed using six criteria: blurriness, shadow, uniformity of lighting, dot location, dot angle and central positioning of the lesion. Patients completed a feedback questionnaire. Results: Twelve patients returned 332 photographs of 18 lesions. Each patient sent a median of 29.5 photographs [interquartile range (IQR) 15-33.5], with a median of 15 photographs per lesion (IQR 6-32). Twenty-two photographs were duplicates. Of the remaining 310 images, 256 (77%) were sufficiently in focus; 268 (81%) had some shadow; lighting was even in 56 (17%); dot location was acceptable in 233 (70%); dot angle was ideal in 107 (32%); and the lesion was centred in 255 (77%). Patient feedback suggested that 6 of 10 would be willing to record images daily in future studies, and 9 of 10 at least one to three times per week. Conclusion: Taking smartphone photographs of digital lesions was feasible for most patients, with most lesions in focus and central in the image. These promising results will inform the next research phase (to develop a smartphone monitoring application incorporating photographs and symptom tracking).

Abstract: This paper characterizes the actual science performance of the James Webb Space Telescope (JWST), as determined from the six month commissioning period. We summarize the performance of the spacecraft, telescope, science instruments, and ground system, with an emphasis on differences from pre-launch expectations. Commissioning has made clear that JWST is fully capable of achieving the discoveries for which it was built. Moreover, almost across the board, the science performance of JWST is better than expected; in most cases, JWST will go deeper faster than expected. Abstract: The impact of emissions of volatile organic compounds (VOCs) to the atmosphere on the production of secondary pollutants, such as ozone and secondary organic aerosol (SOA), is mediated by the concentration of nitric oxide (NO). Polluted urban atmospheres are typically considered to be "high-NO"environments, while remote regions such as rainforests, with minimal anthropogenic influences, are considered to be "low NO". However, our observations from central Beijing show that this simplistic separation of regimes is flawed. Despite being in one of the largest megacities in the world, we observe formation of gas- and aerosol-phase oxidation products usually associated with low-NO "rainforest-like"atmospheric oxidation pathways during the afternoon, caused by extreme suppression of NO concentrations at this time. Box model calculations suggest that during the morning high-NO chemistry predominates (95 %) but in the afternoon low-NO chemistry plays a greater role (30 %). Current emissions inventories are applied in the GEOS-Chem model which shows that such models, when run at the regional scale, fail to accurately predict such an extreme diurnal cycle in the NO concentration. With increasing global emphasis on reducing air pollution, it is crucial for the modelling tools used to develop urban air quality policy to be able to accurately represent such extreme diurnal variations in NO to accurately predict the formation of pollutants such as SOA and ozone.

Abstract: The Bright Star in the globular cluster 47 Tucanae (NGC 104) is a post-AGB star of spectral type B8 III. The ultraviolet spectra of late-B stars exhibit a myriad of absorption features, many due to species unobservable from the ground. The Bright Star thus represents a unique window into the chemistry of 47 Tuc. We have analyzed observations obtained with the Far Ultraviolet Spectroscopic Explorer (FUSE), the Cosmic Origins Spectrograph (COS) aboard the Hubble Space Telescope, and the MIKE Spectrograph on the Magellan Telescope. By fitting these data with synthetic spectra, we determine various stellar parameters (T_eff = 10,850 +/- 250 K, log g = 2.20 +/- 0.13) and the photospheric abundances of 26 elements, including Ne, P, Cl, Ga, Pd, In, Sn, Hg, and Pb, which have not previously been published for this cluster. Abundances of intermediate-mass elements (Mg through Ga) generally scale with Fe, while the heaviest elements (Pd through Pb) have roughly solar abundances. Its low C/O ratio indicates that the star did not undergo third dredge-up and suggests that its heavy elements were made by a previous generation of stars. If so, this pattern should be present throughout the cluster, not just in this star. Stellar-evolution models suggest that the Bright Star is powered by a He-burning shell, having left the AGB during or immediately after a thermal pulse. Its mass (0.54 +/- 0.16 M_sun) implies that single stars in 47 Tuc lose 0.1--0.2 M_sun on the AGB, only slightly less than they lose on the RGB., Comment: 23 pages, 11 figures, accepted for publication in the Astronomical Journal

21. A nationwide survey of emergency medicine resident workflow efficiency: Are training programs teaching residents to be efficient?

Abstract: BackgroundWorkflow efficiency (WFE) is essential to the practice of emergency medicine (EM), but a standardized approach to measuring and teaching it during residency is lacking. In this study we sought to describe how EM residency programs in the United States currently measure and teach WFE and to assess the relative importance of WFE teaching to EM residency program leaders.MethodsWe conducted a cross-sectional survey of all accredited EM residency training programs in the United States in Fall 2019. We invited all allopathic EM residency programs to participate in the study by directly emailing program directors and assistant/associate program directors. We conducted the study and performed descriptive statistics using SurveyMonkey software.ResultsWe received a total of 133 responses out of 190 total programs (70%) with proportionate representation from 3- and 4-year programs and all regions of the United States. When asked to what extent teaching efficiency should be a priority compared to other educational goals, 65% of program leaders responded with "significant" or "moderate" priority. Most EM programs collect WFE data on their residents, either by tracking patients per hour (78%) or by written evaluations (59%). Common methods for providing WFE data to residents were: "individual data provided along with deidentified rank" (35%), "data provided only during private feedback meetings" (26%), and "no data or rank provided to residents" (16%). Regarding targeted WFE teaching to residents, 88% reported utilizing general on-shift teaching, 48% reported teaching WFE during formal didactics, and 45% during dedicated private feedback sessions.ConclusionThis national study of allopathic U.S. EM programs suggests that most EM program leaders do value WFE teaching. However, we found no consistent approach among programs for tracking or distributing resident WFE data, and many programs lack a formalized way to teach efficiency to their residents.

22. Implementing disruptive technological change in UK healthcare: exploring development of a smart phone app for remote patient monitoring as a boundary object using qualitative methods

Abstract: Purpose Developing technological innovations in healthcare is made complex and difficult due to effects upon the practices of professional, managerial and other stakeholders. Drawing upon the concept of boundary object, this paper explores the challenges of achieving effective collaboration in the development and use of a novel healthcare innovation in the English healthcare system. Design/methodology/approach A case study is presented of the development and implementation of a smart phone application (app) for use by rheumatoid arthritis patients. Over a two-year period (2015–2017), qualitative data from recorded clinical consultations (n = 17), semi-structured interviews (n = 63) and two focus groups (n = 13) were obtained from participants involved in the app's development and use (clinicians, patients, researchers, practitioners, IT specialists and managers). Findings The case focuses on the use of the app and its outputs as a system of inter-connected boundary objects. The analysis highlights the challenges overcome in the innovation's development and how knowledge sharing between patients and clinicians was enhanced, altering the nature of the clinical consultation. It also shows how conditions surrounding the innovation both enabled its development and inhibited its wider scale-up. Originality/value By recognizing that technological artefacts can simultaneously enable and inhibit collaboration, this paper highlights the need to overcome tensions between the transformative capability of such healthcare innovations and the inhibiting effects simultaneously created on change at a wider system level.

Abstract: Isoprene-derived secondary organic aerosol (iSOA) is a significant contributor to organic carbon (OC) in some forested regions, such as tropical rainforests and the Southeastern US. However, its contribution to organic aerosol in urban areas that have high levels of anthropogenic pollutants is poorly understood. In this study, we examined the formation of anthropogenically influenced iSOA during summer in Beijing, China. Local isoprene emissions and high levels of anthropogenic pollutants, in particular NOx and particulate SO2-4 , led to the formation of iSOA under both high- A nd low-NO oxidation conditions, with significant heterogeneous transformations of isoprene-derived oxidation products to particulate organosulfates (OSs) and nitrooxyorganosulfates (NOSs). Ultra-high-performance liquid chromatography coupled to high-resolution mass spectrometry was combined with a rapid automated data processing technique to quantify 31 proposed iSOA tracers in offline PM2.5 filter extracts. The co-elution of the inorganic ions in the extracts caused matrix effects that impacted two authentic standards differently. The average concentration of iSOA OSs and NOSs was 82.5 ngm-3, which was around 3 times higher than the observed concentrations of their oxygenated precursors (2-methyltetrols and 2-methylglyceric acid). OS formation was dependant on both photochemistry and the sulfate available for reactive uptake, as shown by a strong correlation with the product of ozone (O3) and particulate sulfate (SO2-4). A greater proportion of high-NO OS products were observed in Beijing compared with previous studies in less polluted environments. The iSOA-derived OSs and NOSs represented 0.62% of the oxidized organic aerosol measured by aerosol mass spectrometry on average, but this increased to ∼ 3% on certain days. Abstract: BACKGROUND: The idiopathic inflammatory myopathies (IIMs) are a group of rare conditions characterised by muscle inflammation (myositis). Accurate disease activity assessment is vital in both clinical and research settings, however, current available methods lack ability to quantify associated variation of physical activity, an important consequence of myositis. This study aims to review studies that have collected accelerometer-derived physical activity data in IIM populations, and to investigate if these studies identified associations between physical and myositis disease activity. METHODS: A narrative review was conducted to identify original articles that have collected accelerometer-derived physical activity data in IIM populations. The following databases were searched from February 2000 until February 2019: Medline via PubMed, Embase via OVID and Scopus. RESULTS: Of the 297 publications screened, eight studies describing accelerometer use in 181 IIM cases were identified. Seven out of the eight studies investigated juvenile dermatomyositis (JDM) populations and only one reported on an adult-onset population. Population sizes, disease duration, accelerometer devices used, body placement sites, and study duration varied between each study. Accelerometer-derived physical activity levels were reduced in IIM cohorts, compared to healthy controls, and studies reported improvement of physical activity levels following exercise programme interventions, thus demonstrating efficacy. Higher levels of accelerometer-derived physical activity measurements were associated with shorter JDM disease duration, current glucocorticoid use and lower serum creatine kinase. However, no clear association between muscle strength and accelerometer-derived physical activity measures was identified. Abstract: BACKGROUND: The Knee OsteoArthritis, Linking Activity and Pain (KOALAP) study is the first to test the feasibility of using consumer-grade cellular smartwatches for health care research. OBJECTIVE: The overall aim was to investigate the feasibility of using consumer-grade cellular smartwatches as a novel tool to capture data on pain (multiple times a day) and physical activity (continuously) in patients with knee osteoarthritis. Additionally, KOALAP aimed to investigate smartwatch sensor data quality and assess whether engagement, acceptability, and user experience are sufficient for future large-scale observational and interventional studies. METHODS: A total of 26 participants with self-diagnosed knee osteoarthritis were recruited in September 2017. All participants were aged 50 years or over and either lived in or were willing to travel to the Greater Manchester area. Participants received a smartwatch (Huawei Watch 2) with a bespoke app that collected patient-reported outcomes via questionnaires and continuous watch sensor data. All data were collected daily for 90 days. Additional data were collected through interviews (at baseline and follow-up) and baseline and end-of-study questionnaires. This study underwent full review by the University of Manchester Research Ethics Committee (#0165) and University Information Governance (#IGRR000060). For qualitative data analysis, a system-level security policy was developed in collaboration with the University Information Governance Office. Additionally, the project underwent an internal review process at Google, including separate reviews of accessibility, product engineering, privacy, security, legal, and protection regulation compliance. RESULTS: Participants were recruited in September 2017. Data collection via the watches was completed in January 2018. Collection of qualitative data through patient interviews is still ongoing. Abstract: We have analyzed FUSE, COS, GHRS, and Keck HIRES spectra of the UV-bright star Barnard 29 in M13 (NGC 6205). By comparing the photospheric abundances derived from multiple ionization states of C, N, O, Si, and S, we infer an effective temperature T_eff = 21,400 +/- 400 K. Balmer-line fits yield a surface gravity log g = 3.10 +/- 0.03. We derive photospheric abundances of He, C, N, O, Mg, Al, Si, P, S, Cl, Ar, Ti, Cr, Fe, Ni, and Ge. Barnard 29 exhibits an abundance pattern typical of the first-generation stars in M13, enhanced in oxygen and depleted in aluminum. An underabundance of C and an overabundance of N suggest that the star experienced nonconvective mixing on the RGB. We see no evidence of significant chemical evolution since the star left the RGB; in particular, it did not undergo third dredge-up. Previous workers found that the star's FUV spectra yield an iron abundance about 0.5 dex lower than its optical spectrum, but the iron abundances derived from all of our spectra are consistent with the cluster value. We attribute this difference to our use of model atmospheres without microturbulence, which is ruled out by careful fits to optical absorption features. We derive a mass M_*/M_sun = 0.45 - 0.55 and luminosity log (L_*/L_sun) = 3.26 - 3.35. Comparison with stellar-evolution models suggests that Barnard 29 evolved from a ZAHB star of mass M_*/M_sun between 0.50 and 0.55, near the boundary between the extreme and blue horizontal branches., Comment: 17 pages, 13 figures

Abstract: Adverse drug reactions (ADRs) are unwanted or harmful effects experienced after the administration of a certain drug or a combination of drugs, presenting a challenge for drug development and drug administration. In this paper, we present a set of taggers for extracting adverse drug reactions and related entities, including factors, severity, negations, drug class and animal. The systems used a mix of rule-based, machine learning (CRF) and deep learning (BLSTM with word2vec embeddings) methodologies in order to annotate the data. Abstract: Personalised medicine, new discoveries and studies on rare exposures or outcomes require large samples that are increasingly difficult for any single investigator to obtain. Collaborative work is limited by heterogeneities, both what is being collected and how it is defined. To develop a core set for data collection in rheumatoid arthritis (RA) research which (1) allows harmonisation of data collection in future observational studies, (2) acts as a common data model against which existing databases can be mapped and (3) serves as a template for standardised data collection in routine clinical practice to support generation of research-quality data. A multistep, international multistakeholder consensus process was carried out involving voting via online surveys and two face-to-face meetings. A core set of 21 items ('what to collect') and their instruments ('how to collect') was agreed: age, gender, disease duration, diagnosis of RA, body mass index, smoking, swollen/tender joints, patient/evaluator global, pain, quality of life, function, composite scores, acute phase reactants, serology, structural damage, treatment and comorbidities. The core set should facilitate collaborative research, allow for comparisons across studies and harmonise future data from clinical practice via electronic medical record systems.

39. Predicting Student Retention Using Scholarship and Grant Aid

Abstract: Since the beginning, the field of higher education has represented the possibility of a better future for many. For some, education represents a way to better oneself and to prepare for a future career. This is the one of the strongest drivers of going to college, that it will open doors to a better future. The number of students attending colleges and universities has increased over the decades but so too has the expense of going to college. The dissertation report presented here attempts to look at the retention rates of colleges and universities and how forms of financial gift aid may impact the retention rate of college students. Participants for this study included all public and private colleges and universities who award at least a bachelor’s degree and receive federal financial aid. These schools are required to submit information to the National Center for Educational Statistics and an archival database was used to pull financial aid and student enrollment information for each of these schools. Using a correlational predictive design, these data were examined to see if student retention rates can be predicted by the average amount of federal, state, and institutional grant and gift aid awarded per new incoming student and if one form of aid is more predictive than the others.

Abstract: Background Lymphomas comprise a heterogeneous group of malignant diseases with highly variable prognosis. Rheumatoid arthritis (RA) is associated with a twofold increased risk of both Hodgkin’s lymphoma (HL) and non-Hodgkin’s lymphoma (NHL). It is unknown whether treatment with biologic disease-modifying antirheumatic drugs (bDMARDs) affect the risk of specific lymphoma subtypes. Methods Patients never exposed to (bionaïve) or ever treated with bDMARDs from 12 European biologic registers were followed prospectively for the occurrence of first ever histologically confirmed lymphoma. Patients were considered exposed to a bDMARD after having received the first dose. Lymphomas were attributed to the most recently received bDMARD. Results Among 124 997 patients (mean age 59 years; 73.7% female), 533 lymphomas were reported. Of these, 9.5% were HL, 83.8% B-cell NHL and 6.8% T-cell NHL. No cases of hepatosplenic T-cell lymphoma were observed. Diffuse large B-cell lymphoma (DLBCL) was the most frequent B-cell NHL subtype (55.8% of all B-cell NHLs). The subtype distributions were similar between bionaïve patients and those treated with tumour necrosis factor inhibitors (TNFi). For other bDMARDs, the numbers of cases were too small to draw any conclusions. Patients with RA developed more DLBCLs and less chronic lymphocytic leukaemia compared with the general population. Conclusion This large collaborative analysis of European registries has successfully collated subtype information on 533 lymphomas. While the subtype distribution differs between RA and the general population, there was no evidence of any modification of the distribution of lymphoma subtypes in patients with RA treated with TNFi compared with bionaïve patients.

Abstract: Objective: The relationship between insomnia symptoms and cognitive performance is unclear, particularly at the population level. We conducted the largest examination of this association to date through analysis of the UK Biobank, a large population-based sample of adults aged 40-69 yrs. We also sought to determine associations between cognitive performance and self-reported chronotype, sleep medication use, and sleep duration. Methods: This cross-sectional, population-based study involved 477,529 participants, comprising 133,314 with frequent insomnia symptoms (age: 57.4 ± 7.7 yrs; 62.1% female) and 344,215 controls without (age: 56.1 ± 8.2 yrs; 52.0% female). Cognitive performance was assessed through a touchscreen test battery probing reasoning, basic reaction time, numeric memory, visual memory and prospective memory. Adjusted models included relevant demographic, clinical and sleep variables. Results: Frequent insomnia symptoms were associated with cognitive impairment in unadjusted models, however these effects were reversed after full adjustment, leaving those with frequent insomnia symptoms showing statistically better cognitive performance over those without. Relative to intermediate chronotype, evening chronotype was associated with superior task performance, while morning chronotype was associated with the poorest performance. Sleep medication use and both long (>9hrs) and short (<7hrs) sleep duration were associated with impaired performance. Conclusions: Our results suggest that after adjustment for potential confounding variables, frequent insomnia symptoms may be associated with a small statistical advantage, which is unlikely to be clinically meaningful, on simple neurocognitive tasks. Further work is required to examine mechanistic underpinnings of an apparent evening chronotype advantage in cognitive performance, as well as impairment associated with morning chronotype, sleep medication use, and sleep duration extremes.

Abstract: We present a spectral analysis of the UV-bright star Y453 in M4. Model fits to the star's optical spectrum yield T_eff ~ 56,000 K. Fits to the star's FUV spectrum, obtained with the Cosmic Origins Spectrograph (COS) on board the Hubble Space Telescope, reveal it to be considerably hotter, with T_eff ~ 72,000 K. We adopt T_eff = 72,000 +/- 2000 K and log g = 5.7 +/- 0.2 as our best-fit parameters. Scaling the model spectrum to match the star's optical and near-infrared magnitudes, we derive a mass M_* = 0.53 +/- 0.24 M_sun and luminosity log L/L_sun = 2.84 +/- 0.05, consistent with the values expected of an evolved star in a globular cluster. Comparing the star with post-horizontal branch evolutionary tracks, we conclude that it most likely evolved from the blue horizontal branch, departing the AGB before third dredge-up. It should thus exhibit the abundance pattern (O-poor and Na-rich) characteristic of the second-generation (SG) stars in M4. We derive the star's photospheric abundances of He, C, N, O, Si, S, Ti, Cr, Fe, and Ni. CNO abundances are roughly 0.25 dex greater than those of the cluster's SG stars, while the Si and S abundances agree match the cluster values. Abundances of the iron-peak elements (except for iron itself) are enhanced by 1 to 3 dex. Abstract: OBJECTIVES: Some studies have reported a possible association between exposure to tumour necrosis factor (TNF) inhibitors and an increased risk of melanoma. The aim of this study was to investigate the incidence of invasive cutaneous melanomas in patients with rheumatoid arthritis (RA) treated with TNF inhibitors (TNFi), other biologic disease modifying drugs and non-biologic therapy.METHODS: Eleven biologic registers from nine European countries participated in this collaborative project. According to predefined exposure definitions, cohorts of patients with RA were selected. Using the country-specific general population of each register as reference, age, sex and calendar year standardised incidence ratios (SIRs) of invasive histology-confirmed cutaneous melanoma were calculated within each register. Pooled SIR and incidence rate ratios (IRRs) comparing biologic cohorts to biologic-naïve were calculated across countries by taking the size of the register into account.RESULTS: Overall 130 315 RA patients with a mean age of 58 years contributing 579 983 person-years were available for the analysis and 287 developed a first melanoma. Pooled SIRs for biologic-naïve, TNFi and rituximab-exposed patients were 1.1 (95% CI 0.9 to 1.4), 1.2 (0.99 to 1.6) and 1.3 (0.6 to 2.6), respectively. Incidence rates in tocilizumab and abatacept-exposed patients were also not significantly increased. IRR versus biologic-naïve patients were: TNFi 1.1 (95% CI 0.8 to 1.6); rituximab 1.2 (0.5 to 2.9).CONCLUSIONS: This large European collaborative project did not confirm an overall increased risk of melanoma following exposure to TNFi.

Abstract: Background: Foot surgery is common in patients with RA but research into surgical outcomes is limited and conceptually flawed as current outcome measures lack face validity: to date no one has asked patients what is important to them. This study aimed to determine which factors are important to patients when evaluating the success of foot surgery in RA Methods: Semi structured interviews of RA patients who had undergone foot surgery were conducted and transcribed verbatim. Thematic analysis of interviews was conducted to explore issues that were important to patients. Results: 11 RA patients (9 ♂, mean age 59, dis dur = 22yrs, mean of 3 yrs post op) with mixed experiences of foot surgery were interviewed. Patients interpreted outcome in respect to a multitude of factors, frequently positive change in one aspect contrasted with negative opinions about another. Overall, four major themes emerged. Function: Functional ability & participation in valued activities were very important to patients. Walking ability was a key concern but patients interpreted levels of activity in light of other aspects of their disease, reflecting on change in functional ability more than overall level. Positive feelings of improved mobility were often moderated by negative self perception ("I mean, I still walk like a waddling duck”). Appearance: Appearance was important to almost all patients but perhaps the most complex theme of all. Physical appearance, foot shape, and footwear were closely interlinked, yet patients saw these as distinct separate concepts. Abstract: Objective. Under the auspices of the European League Against Rheumatism (EULAR), a study group of investigators representing European biologic DMARD (bDMARD) registers was convened. The purpose of this initial assessment was to collect and compare a cross section of patient characteristics and collate information on the availability of potential confounders within these registers. Methods. Baseline characteristics of patients starting their first bDMARD in an arbitrary year (2008) for the treatment of RA, including demographic and disease characteristics, bDMARD drug details and co-morbidities, were collected and compared across 14 European bDMARD registers. Results. A total of 5320 patients were included. Half the registers had restricted recruitment to certain bDMARDs during the study year. All registers's collected data on age, gender, disease duration, seropositivity for IgM-RF and 28-joint DAS (DAS28). The mean DAS28 ranged from 4.2 to 6.6 and the mean HAQ from 0.8 to 1.9. Current smoking ranged from 9% to 34%. Nine registers reported co-morbidities with varying prevalence. Conclusion. In addition to demonstrating European-wide collaboration across rheumatology bDMARD registers, this assessment identified differences in prescribing patterns, recruitment strategies and data items collected. These differences need to be considered when applying strategies for combined analysis. Abstract: Our sleep timing preference, or chronotype, is a manifestation of our internal biological clock. Variation in chronotype has been linked to sleep disorders, cognitive and physical performance, and chronic disease. Here we perform a genome-wide association study of self-reported chronotype within the UK Biobank cohort (n=100,420). We identify 12 new genetic loci that implicate known components of the circadian clock machinery and point to previously unstudied genetic variants and candidate genes that might modulate core circadian rhythms or light-sensing pathways. Pathway analyses highlight central nervous and ocular systems and fear-response-related processes. Genetic correlation analysis suggests chronotype shares underlying genetic pathways with schizophrenia, educational attainment and possibly BMI. Further, Mendelian randomization suggests that evening chronotype relates to higher educational attainment. These results not only expand our knowledge of the circadian system in humans but also expose the influence of circadian characteristics over human health and life-history variables such as educational attainment.

Abstract: In chronic inflammatory rheumatic diseases, comorbidities such as cardiovascular diseases and infections are suboptimally prevented, screened for and managed. The objective of this European League Against Rheumatism (EULAR) initiative was to propose points to consider to collect comorbidities in patients with chronic inflammatory rheumatic diseases. We also aimed to develop a pragmatic reporting form to foster the implementation of the points to consider. In accordance with the EULAR Standardised Operating Procedures, the process comprised (1) a systematic literature review of existing recommendations on reporting, screening for or preventing six selected comorbidities: ischaemic cardiovascular diseases, malignancies, infections, gastrointestinal diseases, osteoporosis and depression and (2) a consensus process involving 21 experts (ie, rheumatologists, patients, health professionals). Recommendations on how to treat the comorbidities were not included in the document as they vary across countries. The literature review retrieved 42 articles, most of which were recommendations for reporting or screening for comorbidities in the general population. The consensus process led to three overarching principles and 15 points to consider, related to the six comorbidities, with three sections: (1) reporting (ie, occurrence of the comorbidity and current treatments); (2) screening for disease (eg, mammography) or for risk factors (eg, smoking) and (3) prevention (eg, vaccination). A reporting form (93 questions) corresponding to a practical application of the points to consider was developed. Using an evidence-based approach followed by expert consensus, this EULAR initiative aims to improve the reporting and prevention of comorbidities in chronic inflammatory rheumatic diseases. Next steps include dissemination and implementation.

49. Weapons for Peace or War? The Role of Military Independence in Militarized Interstate Disputes

Abstract: The global trade in weaponry has created an environment in which states are now utilizing arms transfer agreements to bolster their own domestic defense industry aspirations. Previous research concerning arms transfers has suggested that a state may alter its behavior depending on its level of dependency on foreign sourced weapons. However, previous scholarship primarily examined the effect from importing arms and not the effect that military industry will have upon state behavior. Since the number of states possessing domestic defense industries has risen by 250% since 1950, it is paramount that we understand the effect of a domestic military industry on state behavior. To explore this problem, this dissertation utilizes militarized interstate dispute and arms procurement data. 3 primary independence variables are created, all of which measure military independence in different ways. These variables include, military industry presence, arms supplier diversification, and foreign dependence on military goods. The dissertation hypothesizes that the level of military independence will have an effect on the probability that a state will be involved, initiate as well as decrease dispute duration.

50. FUSE, STIS, and Keck spectroscopic analysis of the UV-bright star vZ 1128 in M3 (NGC 5272)

Abstract: We present a spectral analysis of the UV-bright star vZ 1128 in M3 based on observations with the Far Ultraviolet Spectroscopic Explorer (FUSE), the Space Telescope Imaging Spectrograph (STIS), and the Keck HIRES echelle spectrograph. By fitting the H I, He I, and He II lines in the Keck spectrum with non-LTE H-He models, we obtain Teff = 36,600 K, log g = 3.95, and log N(He)/N(H) = -0.84. The star's FUSE and STIS spectra show photospheric absorption from C, N, O, Al, Si, P, S, Fe, and Ni. No stellar features from elements beyond the iron peak are observed. Both components of the N V 1240 doublet exhibit P~Cygni profiles, indicating a weak stellar wind, but no other wind features are seen. The star's photospheric abundances appear to have changed little since it left the red giant branch (RGB). Its C, N, O, Al, Si, Fe, and Ni abundances are consistent with published values for the red-giant stars in M3, and the relative abundances of C, N, and O follow the trends seen on the cluster RGB. In particular, its low C abundance suggests that the star left the asymptotic giant branch before the onset of third dredge-up., Comment: 15 pages, 10 figures, to be published in MNRAS
