Genetic heterogeneity contributes to clinical outcome and progression of most tumors, but little is known about allelic diversity for epigenetic compartments, and almost no data exist for acute myeloid leukemia (AML). We examined epigenetic heterogeneity as assessed by cytosine methylation within defined genomic loci with four CpGs (epialleles), somatic mutations, and transcriptomes of AML patient samples at serial time points. We observed that epigenetic allele burden is linked to inferior outcome and varies considerably during disease progression. Epigenetic and genetic allelic burden and patterning followed different patterns and kinetics during disease progression. We observed a subset of AMLs with high epiallele and low somatic mutation burden at diagnosis, a subset with high somatic mutation and lower epiallele burdens at diagnosis, and a subset with a mixed profile, suggesting distinct modes of tumor heterogeneity. Genes linked to promoter-associated epiallele shifts during tumor progression showed increased single-cell transcriptional variance and differential expression, suggesting functional impact on gene regulation. Thus, genetic and epigenetic heterogeneity can occur with distinct kinetics likely to affect the biological and clinical features of tumors.

Babesia gibsoni is a haemoprotozoan parasite of emerging global importance. The clinical presentation of babesial infections is diverse and the systemic inflammatory response induced by infection is considered to be a major feature of the pathophysiology of canine babesiosis. An experimental case-controlled longitudinal study was conducted to assess the clinical, haematological, cytokine and acute phase protein changes that occur during experimental B. gibsoni infection of beagle puppies. Infected dogs became transiently pyrexic and anaemic, intermittently neutropenic and transiently, but profoundly, thrombocytopenic, although this had no apparent adverse clinical effect. Experimental B. gibsoni infection also induced an acute phase response, characterised by a marked increase in the concentration of C-reactive protein, which was delayed in onset following infection but preceded the detection of peripheral parasitaemia. Experimental B. gibsoni infection was also associated with marked increases in the concentration of multiple cytokines which were also delayed in onset following infection and occurred subsequent to the detection of peripheral parasitaemia and the acute phase response. This study furthers our understanding of the immune response that occurs during babesial infections and the role that systemic inflammation plays in the pathophysiology of canine babesiosis.

To prevent new infections with human immunodeficiency virus type 1 (HIV-1) in sub-Saharan Africa, UNAIDS recommends targeting interventions to populations that are at high risk of acquiring and passing on the virus. Yet it is often unclear who and where these ‘source’ populations are. Here we demonstrate how viral deep-sequencing can be used to reconstruct HIV-1 transmission networks and to infer the direction of transmission in these networks. We are able to deep-sequence virus from a large population-based sample of infected individuals in Rakai District, Uganda, reconstruct partial transmission networks, and infer the direction of transmission within them at an estimated error rate of 16.3% [8.8–28.3%]. With this error rate, deep-sequence phylogenetics cannot be used against individuals in legal contexts, but is sufficiently low for population-level inferences into the sources of epidemic spread. The technique presents new opportunities for characterizing source populations and for targeting of HIV-1 prevention interventions in Africa.
