1. Implementation of the hyaluronic acid fat graft myringoplasty technique, pitfalls and lessons learned

Abstract

Background: Myringoplasty is a common procedure in otorhinolaryngology. Many techniques with different complications and outcomes have been described, one of which is hyaluronic acid fat graft myringoplasty (HAFGM). This technique, as proposed by Saliba, uses fat tissue and hyaluronic acid discs. The technique is relatively fast with a high success rate and low complications. However, what outcomes can be expected when performed by other surgeons? In this paper, we report on the technique’s success in our own hands. Materials and methods: Based on Saliba’s protocol, we performed 86 HAFGMs by a transcanal approach between 2012 and August 2014. However, our 70% success rate was significantly different from Saliba’s 92% (p value 2.8e − 05). We visited Saliba’s clinic in order to identify critical differences between our approaches. We adapted the differences we found in our protocol and analysed another 50 HAFGMs performed afterwards, between October 2014 and December 2015. Results: The success rate increased to 86–89%, this percentage is not different compared to Saliba’s results (p value .25 and .54). Conclusion: HAFGM is a reproducible technique in the hands of other surgeons, but critical following of the surgical protocol is important.

2. Non-allergic non-infectious perennial rhinitis. Pathogenesis and treatment

Abstract

Rhinitis is a very common disorder. Most people suffer from an infectious rhinitis at least once a year. The symptoms usually disappear within a week. The patients with chronic rhinitis pose a much greater problem. At least 10 % of the general population are affected by a chronic allergic or non-allergic non-infectious rhiilltis (l). The impact of the nasal complaints such as in rhinitis is often underestimated. Bousquet and Juniper demonstrated that the impact of the disease on the quality of life is greater in rhinitis than in asthma patients (2-4). There is no generally accepted system for the definition, classification and terminology of rhinitis (5). A distinction can be made between rhinitis of known and unknown etiology. Known causes for rhinitis can be subdivided in mechanical Jactors (e.g. septal deviation, foreign body,), injections (viral, bacterial, fungal), miscellaneous causes (e.g. rhinitis medicarnentosa, pregnancy, cystic fibrosis) and allergy. Syndromes of unknown etiology include non-allergic non-infectious perennial rhinitis (NANIPER), nasal polyposis and nonallergic rhinitis with eosinophilia (NARES). The subject of this thesis is the pathogenesis and treatment of NANIPER. As this teml suggests the disorder is diagnosed through the exclusion of the known causes for rhinitis. Available studies are often difficult to compare. Different authors use different methods to exclude "the known causes". The patients are sometimes presented in a study as NANIPER patients without further specification. The way in which an allergic pathogenesis is excluded varies from skin prick tests, senllu testing for specific IgE, total IgE, nasal provocation tests or a combination of these methods. Abstract

Vasomotor rhinitis (VMR) is a disorder of unknown pathogenesis. Forty patients with VMR were carefully selected on the basis of inclusion and exclusion criteria proposed by Mygind and Weeke. Nasal biopsy specimens were taken in the patient group as well as in a group of ten controls. Brush cytology was also taken in the VMR group. Inflammatory cells were identified and counted in the nasal mucosa, with the use of immunohistochemical techniques and a panel of monoclonal antibodies. Eosinophils were studied with the use of BMK13, EG2, and Giemsa. Mast cells were studied with anti-chymase (B7), anti-tryptase (G3) and toluidine blue. Sections were stained with IgE as well. There was no significant difference in the number of eosinophils, mast cells and IgE-positive cells between the two groups. Additionally, in contrast with other reports, in sections that were double-stained with anti-chymase and anti-tryptase, single chymase-positive cells were found.

Abstract

Muon pair production in the process $e^+e^-\to e^+e^-\mu^+\mu^-$ is studied using the data taken at LEP1 ($\sqrt{s}\simeq m_Z$) with the DELPHI detector during the years 1992-1995. The corresponding integrated luminosity is 138.5~pb$^{-1}$. The QED predictions have been tested over the whole $Q^2$ range accessible at LEP1 (from several GeV$^2/c^4$ to several hundred GeV$^2/c^4$) by comparing experimental distributions with distributions resulting from Monte Carlo simulations using various generators. Selected events are used to extract the leptonic photon structure function $F_2^\gamma$. Azimuthal correlations are used to obtain information on additional structure functions, $F_A^\gamma$ and $F_B^\gamma$, which originate from interference terms of the scattering amplitudes. The measured ratios $F_A^\gamma/F_2^\gamma$ and $F_B^\gamma/F_2^\gamma$ are significantly different from zero and consistent with QED predictions.
