Background: WHO has recognised the need to ensure that guideline processes are transparent and evidence based, and that the resulting recommendations are relevant and applicable. Along with decision-making criteria that require findings from effectiveness reviews, WHO is increasingly using evidence derived from qualitative evidence syntheses (QES) to inform the values, acceptability, equity and feasibility implications of its recommendations. This is the first in a series of three papers examining the use of QES in developing clinical and health systems guidelines. Methods: WHO convened a group of methodologists involved in developing recent (2010–2018) guidelines that were informed by QES. Using a pragmatic and iterative approach that included feedback from WHO staff and other stakeholders, the group reflected on, discussed and identified key methods and research implications from designing QES and using the resulting findings in guideline development. Our aim in this paper is to (1) describe and discuss how the findings of QES can inform the scope of a guideline and (2) develop findings for key guideline decision-making criteria. Results: QES resulted in the addition of new outcomes that are directly relevant to service users, a stronger evidence base for decisions about how much effective interventions and related outcomes are valued by stakeholders in a range of contexts, and a more complete database of summary evidence for guideline panels to consider, linked to decisions about values, acceptability, feasibility and equity. Conclusions: Rigorously conducted QES can be a powerful means of improving the relevance of guidelines, and of ensuring that the concerns of stakeholders, at all levels of the healthcare system and from a wide range of settings, are taken into account at all stages of the process.

BACKGROUND: After its outbreak in China, the novel COronaVIrus Disease 19 is spreading across the globe. It is an emergency the world has never seen before. MAIN TEXT: The attention of health systems is mainly focused on COronaVIrus Disease 19 patients and on the risk that intensive care units might be overwhelmed by the serious pulmonary complications. Different countries are also attempting to establish infection prevention and control strategies which proved effective in China where the outbreak was initially reported. We reflect on important lessons to be learnt from different countries. The effects that infection prevention and control strategies, such as social distancing or isolation, can have on the care of millions of patients with non-communicable diseases, who may be indirectly affected, have not been taken into consideration so much. CONCLUSIONS: When dealing with COronaVIrus Disease 19, policy makers and healthcare personnel should consider the indirect effects on the treatment of non-communicable diseases.

This study investigated the use of bark and combined paper sludge to develop a suitable plant growth substrate and to establish a vegetation cover on bark-covered land. Field experiments were established to determine the best plant growth medium, plant mixtures, and fertilization program. The best soil cover consisted of 2.5 cm of combined paper sludge on top of 2.5 cm of black bark. On this soil cover, the Savoureux mixture in combination with birdsfoot trefoil (Lotus corniculatus L.), Lab02 in mixture with MR77, and birdsfoot trefoil in mixture with bromegrass (Bromus inermis L.) gave good vegetation growth and yields. For birdsfoot trefoil over all soil covers, the phosphorus and potassium fertilizations were required at 140 kg P2O5/ha and 160 kg K2O/ha, respectively. These results suggest that combined paper sludge and black bark can be used as topsoil to favor plant establishment on bark-covered land.

By the end of the summer, the best soil cover consisted of 2.5 cm of combined paper sludge on top of 2.5 cm of black bark, which gave good plant appearance, cover, and yield. On this soil cover, the Savoureux mixture (53% perennial grasses and 37% perennial legumes) in combination with birdsfoot trefoil (Lotus corniculatus L.), Lab02 (65% perennial grasses and 35% perennial legumes) in mixture with MR77 (100% perennial grasses), and birdsfoot trefoil in mixture with bromegrass (Bromus inermis L.) gave good vegetation growth and yields. For the birdsfoot trefoil fertilization experiments, and over all soil covers, nitrogen fertilization was not required, but phosphorus and potassium fertilizations should be, respectively, 140 kg P2O5/ha and 160 kg K2O/ha to obtain good plant growth and yield. These first results suggested that combined paper sludge and black bark can be used as topsoil to favor plant establishment on bark-covered land.

Ethanol production from paper sludge (PS) by simultaneous saccharification and fermentation (SSF) is considered to be the most appropriate way to process PS, as it contains negligible lignin. In this study, SSF was conducted using a cellulase produced from PS by the hyper cellulase producer, Acremonium cellulolyticus C-1 for PS saccharification, and a thermotolerant ethanol producer Saccharomyces cerevisiae TJ14 for ethanol production. Background: Recruiting to primary care studies is complex. With the current drive to increase numbers of patients involved in primary care studies, we need to know more about successful recruitment approaches. There is limited evidence on recruitment to focus group studies, particularly when no natural grouping exists and where participants do not regularly meet. The aim of this paper is to reflect on recruitment to a focus group study comparing the methods used with existing evidence using a resource for research recruitment, PROSPeR (Planning Recruitment Options: Strategies for Primary Care). Methods: The focus group formed part of modelling a complex intervention in primary care in the Resources for Effective Sleep Treatment (REST) study. Despite a considered approach at the design stage, there were a number of difficulties with recruitment. The recruitment strategy and subsequent revisions are detailed. Results: The researchers' modifications to recruitment, justifications and evidence from the literature in support of them are presented. Contrary evidence is used to analyse why some aspects were unsuccessful and evidence is used to suggest improvements. Recruitment to focus group studies should be considered in two distinct phases; getting potential participants to contact the researcher, and converting those contacts into attendance. The difficulty of recruitment in primary care is underemphasised in the literature especially where people do not regularly come together, typified by this case study of patients with sleep problems. Conclusion: We recommend training GPs and nurses to recruit patients during consultations. Multiple recruitment methods should be employed from the outset and the need to build topic related non-financial incentives into the group meeting should be considered. Recruitment should be monitored regularly with barriers addressed iteratively as a study progresses.

Background: Colorectal cancer (CRC) is the third most common cancer type worldwide. Colorectal cancer treatment costs vary between countries as it depends on policy factors such as treatment algorithms, availability of treatments and whether the treatment is government-funded. Hence, the objective of this systematic review is to determine the prevalence and measurements of financial toxicity (FT), including the cost of treatment, among colorectal cancer patients. Methods: Medline via PubMed platform, Science Direct, Scopus, and CINAHL databases were searched to find studies that examined CRC FT. There was no limit on the design or setting of the study. Results: Out of 819 papers identified through an online search, only 15 papers were included in this review. The majority (n = 12, 80%) were from high-income countries, and none from low-income countries. Few studies (n = 2) reported objective FT denoted by the prevalence of catastrophic health expenditure (CHE), 60% (9 out of 15) reported prevalence of subjective FT, which ranges from 7 to 80%, 40% (6 out of 15) included studies reported cost of CRC management– annual direct medical cost ranges from USD 2045 to 10,772 and indirect medical cost ranges from USD 551 to 795. Background: Colorectal cancer (CRC) is the third most common cancer type worldwide. Colorectal cancer treatment costs vary between countries as it depends on policy factors such as treatment algorithms, availability of treatments and whether the treatment is government-funded. Hence, the objective of this systematic review is to determine the prevalence and measurements of financial toxicity (FT), including the cost of treatment, among colorectal cancer patients. Methods: Medline via PubMed platform, Science Direct, Scopus, and CINAHL databases were searched to find studies that examined CRC FT. There was no limit on the design or setting of the study. Results: Out of 819 papers identified through an online search, only 15 papers were included in this review. The majority (n = 12, 80%) were from high-income countries, and none from low-income countries. Few studies (n = 2) reported objective FT denoted by the prevalence of catastrophic health expenditure (CHE), 60% (9 out of 15) reported prevalence of subjective FT, which ranges from 7 to 80%, 40% (6 out of 15) included studies reported cost of CRC management– annual direct medical cost ranges from USD 2045 to 10,772 and indirect medical cost ranges from USD 551 to 795. Background: Palliative care is an integral part of health care, which in term has become increasingly technologized in recent decades. Lately, innovative smart sensors combined with artificial intelligence promise better diagnosis and treatment. But to date, it is unclear: how are palliative care concepts and their underlying assumptions about humans challenged by smart sensor technologies (SST) and how can care benefit from SST? Aims: The paper aims to identify changes and challenges in palliative care due to the use of SST. In addition, normative guiding criteria for the use of SST are developed. Methods: The principle of Total Care used by the European Association for Palliative Care (EAPC) forms the basis for the ethical analysis. Drawing on this, its underlying conceptions of the human and its socio-ethical aspects are examined with a phenomenological focus. In the second step, the advantages, limitations, and socio-ethical challenges of using SST with respect to the Total Care principle are explored. Finally, ethical-normative requirements for the application of SST are derived. Results and Conclusion: First, SST are limited in their measurement capabilities. Second, SST have an impact on human agency and autonomy. This concerns both the patient and the caregiver. Third, some aspects of the Total Care principle are likely to be marginalized due to the use of SST. The paper formulates normative requirements for using SST to serve human flourishing. Background: Electromyography (EMG) is a classical technique used to record electrical activity associated with muscle contraction and is widely applied in Biomechanics, Biomedical Engineering, Neuroscience and Rehabilitation Robotics. Determining muscle activation onset timing, which can be used to infer movement intention and trigger prostheses and robotic exoskeletons, is still a big challenge. The main goal of this paper was to perform a review of the state-of-the-art of EMG onset detection methods. Moreover, we compared the performance of the most commonly used methods on experimental EMG data. Methods: A total of 156 papers published until March 2022 were included in the review. The papers were analyzed in terms of application domain, pre-processing method and EMG onset detection method. The three most commonly used methods [Single (ST), Double (DT) and Adaptive Threshold (AT)] were applied offline on experimental intramuscular and surface EMG signals obtained during contractions of ankle and knee joint muscles. Results: Threshold-based methods are still the most commonly used to detect EMG onset. Compared to ST and AT, DT required more processing time and, therefore, increased onset timing detection, when applied on experimental data. The accuracy of these three methods was high (maximum error detection rate of 7.3%), demonstrating their ability to automatically detect the onset of muscle activity. Recently, other studies have tested different methods (especially Machine Learning based) to determine muscle activation onset offline, reporting promising results. Conclusions: This study organized and classified the existing EMG onset detection methods to create consensus towards a possible standardized method for EMG onset detection, which would also allow more reproducibility across studies. Hemodynamic assessment is an integral part of the diagnosis and management of cardiovascular disease. Four-dimensional cardiovascular magnetic resonance flow imaging (4D Flow CMR) allows comprehensive and accurate assessment of flow in a single acquisition. This consensus paper is an update from the 2015 '4D Flow CMR Consensus Statement'. We elaborate on 4D Flow CMR sequence options and imaging considerations. The document aims to assist centers starting out with 4D Flow CMR of the heart and great vessels with advice on acquisition parameters, post-processing workflows and integration into clinical practice. Furthermore, we define minimum quality assurance and validation standards for clinical centers. We also address the challenges faced in quality assurance and validation in the research setting. We also include a checklist for recommended publication standards, specifically for 4D Flow CMR. Finally, we discuss the current limitations and the future of 4D Flow CMR. This updated consensus paper will further facilitate widespread adoption of 4D Flow CMR in the clinical workflow across the globe and aid consistently high-quality publication standards.

BACKGROUND Spinal manipulative therapy (SMT) is offered by many health professions, most often by chiropractors. While SMT can be effective for some musculoskeletal disorders, there is no evidence that SMT improves human immunity in a clinically meaningful way. Despite this, we showed previously that Twitter misinformation about chiropractic/SMT improving immunity increased sharply at the start of the COVID-19 pandemic. Here, we perform a two-year follow-up. METHODS We previously employed specialized software (i.e. Talkwalker) to search the entirety of Twitter activity in the months before and after the COVID-19 pandemic was declared (March 11, 2020). In this paper, we conducted follow-up searches over two successive 12 month periods using terms related to SMT, immunity and chiropractic. The resulting tweets were then coded into those promoting/refuting a relation between SMT and immunity (tone) and messaging about chiropractic/interventions (content). Further analyses were performed to subcategorize tweet content, tally likes, retweets and followers, and evaluate refuting tweets and the country of origin. Finally, we created a chronology of Twitter activity superimposed with dates of promoting or refuting activities undertaken by chiropractic organizations. RESULTS Over the 27 month study period, Twitter activity peaked on March 31, 2020 then declined continuously. As in our first paper, our follow-up data showed that (1) the ratio of refuting/promoting tweets remained constant and (2) tweets that refuted a relationship between SMT and immunity were substantially more liked, retweeted and followed than those promoting. We also observed that promoting tweets suggesting that SMT improves immunity decreased more rapidly. Overwhelmingly, promoting tweets originated in the USA while refuting tweets originated in Canada, Europe and Australia. In order to exploit this knowledge by automated systems, there is a growing interest in developing text mining methodologies to extract, structure, and analyze in the shortest time possible the knowledge encoded in the large volume of medical literature. In this paper, we use the Latent Dirichlet Allocation approach to analyze the correlation between funding efforts and actually published research results in order to provide the policy makers with a systematic and rigorous tool to assess the efficiency of funding programs in the medical area. Results We have tested our methodology in the Revista Medica de Chile, years 2012-2015. 50 relevant semantic topics were identified within 643 medical scientific research papers. Relationships between the identified semantic topics were uncovered using visualization methods. We have also been able to analyze the funding patterns of scientific research underlying these publications. We found that only 29% of the publications declare funding sources, and we identified five topic clusters that concentrate 86% of the declared funds. Conclusions Our methodology allows analyzing and interpreting the current state of medical research at a national level. The funding source analysis may be useful at the policy making level in order to assess the impact of actual funding policies, and to design new policies.

Background: CrossFit® combines different types of activities (weightlifting, gymnastics, and cardiovascular training) that challenge aerobic and anaerobic pathways. Over the last few years, the scientific interest in CrossFit® has increased considerably. However, there have been no published reviews characterizing the physical demands and physiological responses to CrossFit®. The present study synthesizes current evidence on the physical demands and physiological responses to CrossFit®. Methods: The search was performed in three electronic databases (PubMed, Scopus, and Web of Science). Manuscripts related to the physical and physiological performance of adult CrossFit® participants written in English, Portuguese, and Spanish were retrieved for the analysis. Results: In addition, a meta-correlation was conducted to examine the predictors of CrossFit® performance. A total of 68 papers were included in the review. Physical and physiological markers differed between the different workouts analyzed. In addition, 48 to 72 hours are needed to recover from a CrossFit® challenge. Specific tests that involve CrossFit® movements were more related to CrossFit® performance than non-specific. Conclusion: Although the characterization of CrossFit® is dependent on the workout examined, the benefits of muscle hypertrophy are aligned with the recent findings of concurrent training. The characterization of CrossFit® entire sessions and appropriate recovery strategies should be considered in future studies to help coaches manipulate and adjust the training load.

Background: In 2015, the Ministry of Health in Mali included the treatment of severe acute malnutrition (SAM) into the package of activities of the integrated Community Case Management (iCCM). This paper aims to analyze the impact of including community health workers (CHWs) as treatment providers outside the Health Facilities (HFs) on the coverage of SAM treatment when scaling up the intervention in the three largest districts of the Kayes Region in Mali. Methods: A baseline coverage assessment was conducted in August 2017 in the three districts before the CHWs started treating SAM. The end-line assessment was conducted one year later, in August 2018. Coverage was assessed by the standardized methodology called Semi-Quantitative Evaluation of Access and Coverage (SQUEAC). The primary outcome was treatment coverage and other variables evaluated were the geographical distribution of the HFs, CHW’s sites and overlapping between both health providers, the estimation of children with geographical access to health care and the estimation of children screened for acute malnutrition in their communities. Results: Treatment coverage increased in Kayes (28.7–57.1%) and Bafoulabé (20.4–61.1%) but did not in Kita (28.4–28.5%). The decentralization of treatment has not had the same impact on coverage in all districts, with significant differences. The geospatial analyses showed that Kita had a high proportion of overlap between HFs and/or CHWs 48.7% (39.2–58.2), a high proportion of children without geographical access to health care 70.4% (70.1–70.6), and a high proportion of children not screened for SAM in their communities 52.2% (51.9–52.5). Conclusions: Working with CHWs in SAM increases treatment coverage, but other critical aspects need to be considered by policymakers if this intervention model is intended to be scaled up at the country level. Methods: We performed our search based on the conclusions of a previous systematic review published in 2014. Inclusion criteria were articles published in the indexed databases Pubmed, Scopus, Sciencedirect, IEEE and Cochrane published since January 1, 2014. Exclusion criteria were papers with a target group other than PD patients exclusively, or contributions not based on exergames. Sixty-four publications out of 525 matches were selected. Results: The analysis of the 64 selected publications confirmed the putative improvement in motor skills suggested by the results of the previous review. The reliability and safety of both Microsoft Kinect and Wii Balance Board in the proposed scenarios was further confirmed by several recent studies. Clinical trials present better (n = 5) or similar (n = 3) results than control groups (traditional rehabilitation or regular exercise) in motor (TUG, BBS) and cognitive (attention, alertness, working memory, executive function), thus emphasizing the potential of exergames in PD. Pilot studies (n = 11) stated the safety and feasibility of both Microsoft Kinect and Wii Balance Board, potentially in home scenarios as well. Technical papers (n = 30) stated the reliability of balance and gait data captured by both devices. A debate about emerging models of urban living labs (ULLs) 2.0 is taking place in the literature, highlighting the importance of social aspects. The aim of this paper is to examine feminist ULLs as a potential source of social sustainability. This is conducted through a case study of a feminist ULL in Sweden, where girls and young women, together with multiple professional stakeholders, used Minecraft to redesign a public square in their neighbourhood - a marginalised suburban area of Stockholm. Data was collected through participant observations of six experimental labs during 1 year, surveys of the ULL participants, and an analysis of the fnal Minecraft designs. The empirical fndings reveal a process of mutual learning between girls and professional stakeholders, where the girls were viewed as place experts. The fndings also reveal that the girls’ visions include a socially sustainable transition of the urban square: from patriarchal to inclusive, from grey to green, and from a transit area to a meeting-place. There were some diferences between girls and professional stakeholders. Whereas the professionals tended to emphasise security in their visions, the girls emphasised liveability. Although more research is needed, this paper concludes that feminist ULLs can be a possible source of social sustainability. However, the ULL process must both be inclusive, and at the same time unlock long-term transformation in the physical environment.
