Ulcers pose significant quality-of-life and economic costs. The poor usability of existing hand-held devices for domiciliary care ulcer treatment documentation favours paper-based care plans are used instead. This introduces the risk of healing postponement due to suboptimal communication between nurses. Here we present a decision support and documentation system together with results of preliminary usability tests. The system was developed using standard methods and principles. Tests in the form of heuristic evaluation, technical user tests and clinical user test were conducted. The system design and implementation was revised and improved after the heuristic test. In the heuristic evaluation, no severe design or logical usability problems were found. In the technical user test, difficulty, logical arrangement, and appearance were rated above 8 out of 10; program navigability was rated 5 out of 10. In the clinical user test, the users could perform the task and they gave positive response on the user's experience. The hypothesis of a long-term glucose counter-regulation is of particular interest in decision support in diabetes, but no long-term studies have been performed. Without an understanding of how hypoglycemia influences the glucose metabolism, diabetes patients are at higher risk of an unstable glucose control. The aim of this study was to evaluate the phenomenon using continuous glucose monitoring data with a data mining oriented approach. In a retrospective quasi-experimental data analysis we compared continuous glucose monitoring data from people with diabetes after spontaneous hypoglycemia with matched natural hypoglycemia-free control periods. Hypoglycemic events were free of preceding and subsequent hypoglycemia. Control periods were matched regarding subject and hypoglycemia-time and free of both near-hypoglycemia, preceding near-hypoglycemia and subsequent near-hypoglycemia. Insulin intake was also compared. 10 hypoglycemic events in 10 patients with type 1 diabetes were included. No significant difference in blood glucose was seen after hypoglycemia, but insulin intake was significantly higher for hours 6-24 after hypoglycemia. The little difference in blood glucose indicates no long-term glucose counter-regulation to hypoglycemia. The hypoglycemic effect of increased insulin intake after hypoglycemia compared to in the control period together with resulting similar glucose levels could indicate hyperglycemic effects of hypoglycemia.
