Background: The NeuroVizr[TM] is a new generation of Audio-Visual Entrainment (AVE) devices, which involves the use of light and sound to help relax and balance the brain, body, and nervous system. (1,4,5,7-9,14,16) After two years of consistently witnessing positive changes in users of the NeuroVizr[TM], beyond what had been seen with other AVE devices, we designed a small pilot study to: 1. Gain greater understanding of how the NeuroVizr[TM] influences users. 2. Contribute to the data showing AVE devices to be a valuable and promising tool for increased states of relaxation and well-being. 3. Explore how the NeuroVizr[TM] may be unique to other AVE devices as we investigate the concepts of engagement and bottom up (rather than top down) stimulation. Methods: A study was conducted of 12 people between the ages of 18-70 that presented to three clinic sites in Southern California with self-reported PTSD or with self-reported insomnia/sleep disturbances. Data was collected to measure markers of increased relaxation and beneficial health states after a single n-minute session on the NeuroVizr[TM]. Metrics used were changes in brain wave states on electroencephalogram (EEG), heart rate variability (HRV), skin conductance (sEMG), respiratory rate and pre- and post-intervention subjective questionnaire. The hypothesis for this pilot study was that the NeuroVizr[TM] device [tested in two settings: Sleep Angel (SA) and Alpha Primer (AP)] would have a positive effect on relaxation states. Results: The subjective data showed most all participants experienced improvements in relaxation with statistical significance-with a mean improvement of 1.6 in score. Objective measurements in EEG findings showed a statistically significant improvement in high beta brain wave states showing increased relaxation, as well as suggestive improvements in surface electromyography (sEMG). Participants also reported significant increased presence, body connection, mental focus, relaxation, and happiness, as well as decreased stress and physical pain. Additionally, seventy-three percent (4:6 in SA and 3:5 in AP) of participants fell asleep during the session. Abstract Early childhood (birth-8 years), particularly the first 3 years, is the most critical time in development because of the highly sensitive developing brain. Providing appropriate developmental care (i.e., nurturing care, as defined by the World Health Organization [WHO]) during early childhood is key to ensuring a child's holistic development. Pediatricians are expected to play a critical role in supporting early childhood development (ECD) through providing developmental services such as developmental monitoring, anticipatory guidance, screening, and referral to medical and/or community-based services when delay is identified. Pediatricians are also expected to serve as advocates within their clinics and communities for improved delivery of ECD services, such as advocating for increasing funding for ECD initiatives, increasing insurance coverage of ECD services, and working to increase other pediatricians' awareness of the principles of ECD and how to deliver developmental services. However, this does not always occur. Typically, pediatricians' training and practice emphasizes treating disease rather than enhancing ECD. Pediatricians are further hindered by a lack of uniformity across nations in guidelines for developmental monitoring and screening. In this article, we present the vision of the International Pediatric Association (IPA) of the roles that pediatricians, academic departments, medical training programs, and pediatric associations should fulfill to help support ECD, including raising ECD to higher levels of priority in routine pediatric care. First, we present the challenges that face these goals in supporting ECD. We then propose, with supportive literature, strategies and resources to overcome these challenges in collaboration with local and international stakeholders, including the IPA, the WHO, UNICEF, and the World Bank.

Abstract Background We performed a bibliometric analysis of the American Journal of Surgery (AJS) to identify, characterize and place within a historical context, its published classic cited papers (CCPs). Methods Bibliometric data from papers published in the AJS between January 1, 1945, and December 31, 2021 was extracted from the Web of Science database. Analysis was performed utilizing Bibliometrix and VOSViewer software. Results 27,070 papers were published in the AJS over the study period. There were 16 CCPs, including 5 Top CCPs, identified. Review of the Top CCPs reveals that they are based on careful clinical observations, innovation and generally build on prior published work. Top CCPs usually are specific to a particular diagnosis or a commonly performed procedure, as such papers frequently present a scoring or classification system, or important details related to new operative approaches or techniques. Conclusions Bibliometric study of the AJS has allowed for identification, characterization and appreciation of many of the key changes that have occurred in the discipline throughout the history of modern surgery. Key Words Incidental findings; best practices; quality improvement Abstract Purpose Actionable incidental findings (AIFs) are common in radiologic imaging. Imaging is commonly performed in emergency department (ED) visits, and AIFs are frequently encountered, but the ED presents unique challenges for communication and follow-up of these findings. The authors formed a multidisciplinary panel to seek consensus regarding best practices in the reporting, communication, and follow-up of AIFs on ED imaging tests. Methods A 15-member panel was formed, nominated by the ACR and American College of Emergency Physicians, to represent radiologists, emergency physicians, patients, and those involved in health care systems and quality. A modified Delphi process was used to identify areas of best practice and seek consensus. The panel identified four areas: (1) report elements and structure, (2) communication of findings with patients, (3) communication of findings with clinicians, and (4) follow-up and tracking systems. A survey was constructed to seek consensus and was anonymously administered in two rounds, with a priori agreement requiring at least 80% consensus. Discussion occurred after the first round, with readministration of questions where consensus was not initially achieved. Results Consensus was reached in the four areas identified. There was particularly strong consensus that AIFs represent a system-level issue, with need for approaches that do not depend on individual clinicians or patients to ensure communication and completion of recommended follow-up. Conclusions This multidisciplinary collaboration represents consensus results on best practices regarding the reporting and communication of AIFs in the ED setting.
