Abstract

Summary: Medical centers are widely recognized as vital components of the healthcare system. However, academic medical centers are differentiated from their community counterparts by their mission, which typically focuses on clinical care, education, and research. Nonetheless, community clinics/hospitals fill a critical need and play a complementary role serving as the primary sites for health care in most communities. Furthermore, it is now increasingly recognized that in addition to physicians, physician-scientists, and other healthcare-related professionals, basic research scientists also contribute significantly to the emerging inter- and cross-disciplinary, team-oriented culture of translational science. Therefore, approaches that combine the knowledge, skills, experience, expertise, and visions of clinicians in academic medical centers and their affiliated community centers and hospitals, together with basic research scientists, are critical in shaping the emerging culture of translational research so that patients from the urban as well as suburban settings can avail the benefits of the latest developments in science and medicine. 'Integrating Clinical and Translational Research Networks-Building Team Medicine' is an embodiment of this ethos at the City of Hope National Medical Center in Duarte, California. It includes a series of papers authored by teams of leading clinicians, basic research scientists, and translational researchers. The authors discuss how engaging and collaborating with community-based practices, where the majority of older patients with cancer receive their care, can ensure that these patients receive the highest-quality, evidence-based care. Based on our collective experience at City of Hope, we would like to stress that the success of academic-community collaborative programs not only depends on the goodwill and vision of the participants but also on the medical administration, academic leadership, and policymakers who define the principles and rules by which cooperation within the health care industry occurs. We trust that our experience embodied in this singular compendium will serve as a 'Rosetta Stone' for other institutions and practitioners.

26. Cell Motility, Adhesion, Homing, and Migration Assays in the Studies of Tyrosine Kinases.

Abstract

The malignant potential of many cancers is characterized by the rate of metastsis. Metastasis is a multistep process that in general requires detachment from the original tissue, migration through the vascular endothelium, circulation in the blood, attachment to the inner blood vessel wall and penetration into surrounding tissue (1). For example, lung cancer cells have a very high rate of metastasis with cells invading virtually every organ in the body with a predilection to brain, bone, bone marrow, liver and adrenal glands. There are intrinsic abnormalities of cell motility of these lung cancer cells with increased propensity to invade and metastasize to the other organs (2). Most of the cell motility processes are tightly regulated by tyrosine kinases (3) and protein tyrosine phosphatases (4). In malignant cells, these crucial pathways can be brought out of balance through constitutive activation of growth factor receptors and their associated kinases or through formation of tyrosine kinase oncogenes. Overexpression and activation of receptor tyrosine kinases (RTKs) has been found in non small cell lung cancer (NSCLC) cells, including epidermal growth factor receptor (EGFR), HER2/neu, c-Met, and in small cell lung cancer (SCLC) cells, including c-Kit and c-Met receptors (5). The downstream targets of these kinases can involve cytoskeletal proteins which are important in cell motility, cell migration and invasion. It is also known that the tyrosine kinase oncogene BCR/ABL, the transforming protein in chronic myeloid leukemia (CML), is a potent kinase for inducing increased cell motility (membrane ruffling, filipodia formation, lamellopodia formation, and formation of uropod) and migration over extracellular matrix components such as fibronectin (6). 27. Role of Receptor Tyrosine Kinases in Lung Cancer.

Abstract

Lung cancer cells express receptor tyrosine kinases (RTKs) that may be important targets for therapies. RTKs are proto-oncogenes, which are key regulators for cell growth, differentiation, survival, or motility. More than 50 RTKs, in both lung and other tissues, have been identified (1). The receptors contain an N-terminal extracellular ligand-binding domain, a single transmembrane α helix, and a cytosolic C-terminal domain with tyrosine kinase activity. The binding of growth factors to the RTKs extracellular domain activates the cytosolic kinase domain. Growth factor activation of the RTK results in dimerization of the receptor, with phosphorylation of the receptor and downstream targets. Upon activation of RTKs, various biological functions are altered in lung cancer cells, including cell growth, migration/motility, alterations of reactive oxygen species (ROS), and activation of downstream signal-transduction events. 28. Detection and Analysis of Lung Cancer Cells from Body Fluids Using a Rare Event Imaging System.

Abstract

Present diagnostic techniques do not allow the detection of early metastatic spread of tumor cells, although this spread largely determines the clinical course of patients with small primary cancers, especially lung cancer. To improve the diagnosis of this occult stage of early metastasis, sensitive techniques have been developed that facilitate the detection of isolated lung carcinoma cells in hematopoietic tissues (peripheral blood or bone marrow), or lymph nodes (1-7). Abstract

Lung cancer is a devastating illness, and very few of the advances in chemotherapeutics have enhanced survival over the past decade. In order to make an impact on this disease, we must understand the molecular abnormalities to target better therapeutics. In this chapter, we will describe the better understanding at the molecular level that has been gained for lung cancer and its prognosis. Over the last decade, advances in molecular biology have provided important information about potentially significant determinants of prognosis in lung cancer (1,2). These molecular biomarkers are typically expressed in neoplastic lung tissue. Molecular abnormalities include chromosomal aberrations, telomerase expression, expression of oncogenes, and loss of tumor suppressor genes.
