Treatment-seeking patients with fibromyalgia have significantly more psychiatric co-morbidity compared with fibromyalgia non-patients or healthy controls. Fibromyalgia patients should be routinely screened for psychological distress. Disruptive mood disorders will need to be directly treated in addition to other fibromyalgia treatments. The diagnosis and treatment of 271 transgendered patients is described. Characteristics of the transgendered patients seen by the author between 1979 and 2001 reveal four distinct groups not specified in the current Diagnostic and Statistical Manual (DSM-IV) description. These characteristics are important because they determine the internal and external difficulties the patients present to the clinician.
