19. Prevention of Type 1 Diabetes Mellitus.

Abstract

The ultimate goal of understanding the epidemiology of any disease is to use that information in the development of programs aimed to prevent or eradicate the disease in question. The pathogenesis of Type 1 diabetes involves genetic predisposition to the disease, environmental factors and activation of immune mechanisms targeted against pancreatic islet beta cells. Two major genetic susceptibility loci, IDDM1 and IDDM2, account for 60% of Type 1 diabetes. Environmental factors include viral infection, e.g. with Coxsackie B, chemical toxins such as nitrosourea compounds and neonatal nutrition. The disease process consists of selective destruction of the insulin-producing beta cells in the pancreas, probably by T cells. This can be divided into eight stages, outlined here. Prevention strategies invoke elimination of environmental triggers, enhancement of environmental protective factors and interruption of the immune sequence leading to beta cell destruction. These may be used after diagnosis or preclinically in at-risk individuals: options for identifying such individuals are discussed. [ABSTRACT FROM PUBLISHER]
