Mining industry activities are fraught with inherent exposure to occupational health and safety hazards, often with fatal outcomes, injuries and occupational diseases. This occurs amidst the introduction of contemporary mining methods and targeted health and safety regulation. This historical study evaluated company annual reports through document analysis, on a textual level, to evaluate the extent of OHS performance reporting, a measure of the attainment status of the zero-harm aspirational goal. The reporting of OHS performance indicators in annual reports by South African mining companies is widespread, though variable in format, content and context. There were variances in the reporting of OHS indicators by the case mining companies which obfuscate attempts for intra- and inter-company comparisons of OHS performance. The case companies included have not attained the stated goal of zero harm in view of the historically reported and protracted incidences of fatalities, injuries and occupational diseases, a direct threat to the decent work principle. The status quo challenges all affected stakeholders, including regulators, employers, employees and unions alike, to continuously investigate measures for arresting the situation.

Introduction Cancer incidence and mortality are increasing worldwide, and pain is the most common symptom patients experience. Despite developments in cancer pain management and guidelines, the pain often remains undertreated. Effective pain management ultimately involves overcoming several complex institutional, patient, and healthcare provider-related barriers. Objective The objective of the study was to explore and describe healthcare providers’ perceptions of the cancer pain management barriers at a hospital in Zambia. Method A descriptive qualitative study was conducted. Data were collected from 10 purposively sampled medical doctors and registered nurses using prepiloted semistructured individual interviews. Interviews were audio recorded and transcribed verbatim. Thematic analysis was done, and trustworthiness was enhanced based on the criteria described by Lincoln and Guba. Ethical principles, as outlined in the Declaration of Helsinki, were adhered to. Results Three themes emerged, namely patient-related barriers (biographic characteristics, patient knowledge and perceptions), healthcare provider-related barriers (knowledge and perceptions and language barriers), and institution-related barriers (resource limitations and lack of standards and guidelines). Healthcare providers’ views that cultural beliefs about cancer being caused by witchcraft and the use of traditional medicine and services are barriers to cancer pain management were unique to this study. Conclusion Cancer pain management requires a total pain management approach that addresses the barriers to pain management strategies from a patient, healthcare provider, and institutional perspective. The knowledge deficit among patients and healthcare providers is a barrier to cancer pain management and one of the most common challenges reported in the literature. This study further points toward a need to develop culturally competent healthcare providers.

Background: The South African government has developed many policies for the prevention and control of non-communicable diseases. However, non-communicable diseases remain among the major causes of morbidity and mortality in South Africa. Although these diseases are linked to interaction of multiple risk factors, many of which are modifiable, they continue to cause much suffering particularly among the marginalised and people from the lower socio-economic status. Objectives: The objective of this research was to explore and present the inclusion of social determinants of health in the policies meant for the prevention and control of non-communicable diseases in South Africa. Method: The qualitative document analysis approach was used to conduct policy analysis of purposefully selected policies for prevention and control of cancers, obesity and mental and behavioural disorders in South Africa. Results: The analysis revealed that policies for prevention and control of cancers, obesity and mental and behavioural disorders included policy intervention activities that focused on five social determinants of health: (1) governance, (2) social policies, (3) public policies, (4) material circumstances and (5) health system. Conclusion: Excluding most of the important social determinants of health in the policies for prevention and control of non-communicable diseases means that these policies would continue to fail in preventing these diseases from the root causes. Contribution: This article points out weaknesses in the policies meant for prevention and control of obesity, cancers and mental and behavioural disorders. This article further suggests policy improvement strategies that may be considered to effectively address these diseases.

Background: Hospitalisation is a stressful event for the admitted child and the family. The unfamiliar and stressful hospital environment could increase children’s anxiety and pain experiences. Family-centred care has the potential to promote families’ holistic health, but its implementation is limited. Aim: To describe the barriers to family-centred care at a specific hospital in Gauteng. Setting: The study was contextual and was conducted at a specific hospital situated in Gauteng. Methods: A descriptive qualitative research design was used to collect data from 11 nurses and 14 primary caregivers of hospitalised children. Purposive sampling was used. Data were collected using semi-structured interviews and analysed using qualitative content analysis. Rigour through measures to enhance trustworthiness was ensured and ethical principles related to research with human participants were adhered to. Results: Three themes indicating the barriers to family-centred care emerged from the data, namely nurse-primary caregiver relationship, primary caregiver involvement, and ward structure and policy. Conclusion: Barriers to family-centred care involved interpersonal, environmental, and managerial dimensions of the hospital environment where children received care and treatment. A need to enhance family-centred care was therefore identified in order to address relational dimensions of the nurse-primary caregiver relationship, ward structure, and revision of current policies. Contribution: The article highlights barriers to family-centred care to enable action to be taken in the clinical environment to enhance a family-centred approach and improve the hospitalisation experience for children and caregivers.

Background: The articulation of the scope of practice in nursing is important to provide boundaries for registered nurses in which to practice. Registered nurses in Botswana have frequently experienced challenges and raised concerns with their scope of practice. Research related to registered nurses’ knowledge, attitudes and practice regarding their scope of practice appears to be limited in the African context, particularly in Botswana. Aim: The aim of this study was to develop guidelines for professional nurses to explore and describe registered nurses’ knowledge, attitude and practice regarding their scope of practice in Botswana. Methods: A convergent parallel mixed-methods design was employed using a three-tier sampling approach to ensure a representative sample of various settings, health facilities and nurses. For the purpose of this article, the data from the qualitative component are reported. Thirty registered nurses, working in the public health sector in Botswana, participated in semi-structured interviews. Data were analysed using thematic content analysis. Findings: Data analysis revealed that registered nurses’ scope of their knowledge was lacking. Registered nurses’ attitudes were reflected in the adaptation process to expanded practice, as demonstrated through emotive aspects, adjustments to practice beyond scope and the learning of new skills considered beyond scope. Participants reported implementing many skills deemed beyond their scope, whilst their motive to do so included their experience of a lack of control over practice, lack of resources or they were doing so out of consideration for the patient. Guidance in terms of their scope was found to be inadequate. Conclusion: As in other resource-limited countries in Africa, registered nurses in Botswana experience challenges with their scope of practice. Inadequate boundaries may result in compromised nursing care and may have detrimental consequences for both the patient and the nurse.

Background: Child-centred research requires researchers to develop research designs that will enable children to express their views in an ethical, appropriate and child-friendly manner and generate rigorous findings. These requirements challenge researchers to develop new methods to generate data with children, and the ‘younger the child, the more intense the challenge’. Aim: The aim of this article was to describe and reflect on the use of therapeutic play as a data generation method by nurse researchers with young children in a child-centred qualitative, explorative, descriptive and contextual study using a multiple case study strategy. Setting: The study was conducted with young children attending an outpatient department at a public hospital in Gauteng. Methods: Four young children, purposively sampled, participated in six sessions, each based on therapeutic play grounded in the work of Oaklander’s Gestalt play therapeutic approach, facilitated by a nurse researcher. Critical reflection was made about the play therapeutic approach as a data generation method, based on its ability to generate useful data, its implications for data analysis and its ability to be child-enabling and child-centred. Results: The play therapeutic approach, as a data generation method, is capable of generating useful data and amplifying children’s voices in the process. Conclusions: The nurse researcher needs to be highly skilled in this approach as it requires the ability to implement the specific play therapeutic approach in a safe and skilful manner.

Background: The increase in nurses enrolling in postgraduate programmes as well as the need to improve their completion requires academics to establish environments conducive for postgraduate studies. The challenges experienced during postgraduate studies have to be identified to establish conducive environments. Objective: The objective of this study was to explore and describe the challenges experienced by postgraduate nursing students enrolled in postgraduate coursework and research programmes at a South African university. Methods: An exploratory, descriptive and qualitative design was used. The study was contextual in nature. Purposive sampling was used. Fifteen honours, master’s and doctoral students participated in the study. Data were analysed through qualitative content analysis and measures to ensure trustworthiness, and ethical implementation of the study were implemented. Results: Three themes with categories were identified, namely personal challenges (i.e. finances, employment, family and accommodation), academic and institutional challenges (i.e. workload and time constraints, contact sessions, subject information and assessment) and research-related challenges (i.e. information literacy, supervisory relationship and supervisory structure and process). Conclusion: Institutional support addressing personal, academic and research-related challenges should be provided to enhance student experiences and completion.

Background: Intrapersonal, interpersonal, social and economic factors associated with chronic mental illness influence mothers' parenting abilities which in turn influence childhood mental health. Aim: The aim of the study was to explore and describe the parenting experiences of mothers with a chronic mental illness. Methods: An explorative, descriptive and qualitative research design was used. Ten participants were purposively sampled until saturation occurred. Data was collected using in-depth individual interviews and a content analysis was carried out. Results: Mothers with a chronic mental illness experienced various challenges and expressed their family support needs. Conclusion: A family-centred approach to mental healthcare, treatment and rehabilitation is proposed.

Current recommendations endorse the involvement of people living with HIV in the development of programmes for people living with the virus. The purpose of the study was to explore and describe the health literacy needs of women living with HIV. The research design was qualitative, explorative, descriptive and contextual. After women living with HIV/AIDS were sampled purposively, semi-structured interviews were conducted with eight women and qualitative content analysis done. The findings revealed that the women expressed a need to increase their knowledge about HIV/AIDS. The knowledge they needed ranged from basic pathophysiology about HIV/AIDS, to the impact of HIV/AIDS on their health, to an awareness of the modes of HIV transmission and methods of protecting others from being infected. Other important health literacy needs related to self-care and correct antiretroviral use. A need for psychosocial skills was also identified in order for women to build and maintain their relationships. Recommendations were made for nursing practice, education and further research, based on these findings.
