22. The effect of the acute phase of infection on absorption of and exposure to orally administered antibiotics in non-critically ill, hospitalized patients.

Abstract

Objectives: During the acute phase of infection, IV antibiotics are preferred to ensure adequate systemic exposure. To assess whether adequate exposure may also be achieved with oral antibiotics, we investigated exposure to oral antibiotics and PTA during the acute phase of infection and after defervescence.Methods: We enrolled hospitalized, non-critically ill febrile patients treated with IV antibiotics other than amoxicillin or ciprofloxacin. The study consisted of two visits: when patients had received <24 h IV treatment; and when patients had become afebrile. On both visits, patients received one additional dose of 750 mg amoxicillin, or 500 mg ciprofloxacin, depending on the presumed infection, after which serial blood samples were obtained. The primary endpoint was the ratio of the AUC during the febrile and the afebrile phase. The AUCs were considered to be equivalent when the ratio of the mean AUCs and its 90% CI was contained within the acceptance interval of 80%-125%. The secondary endpoint was PTA.Results: Forty-four patients (15 amoxicillin, 29 ciprofloxacin) completed both study visits. The median time between the two study visits was 65.8 h (range 33.8-427.4). The ratio of the mean AUCs (study visit 1/study visit 2) was 97% (90% CI of 80%-117%) for amoxicillin and 112% (90% CI of 108%-116%) for ciprofloxacin. The PTA for amoxicillin and ciprofloxacin did not differ between the two phases and was adequate to treat common pathogens.Conclusions: The acute phase of infection in non-critically ill febrile patients does not influence the exposure to, or PTA of, orally administered amoxicillin and ciprofloxacin. Abstract

Background: Although potentially preventable, cervical cancer is the fourth most common cancer among women globally and a leading cause of cancer-related deaths. Women living in resource-limited countries are especially at risk due to poor access to cervical cancer screening and treatment. Alternative cervical cancer screening methodologies have been investigated where cytology-based screening is not feasible. This study aimed to assess the test performance of naked eye visual inspection analysis, in addition to the comparative performance of physician/colposcopist clinical impression to Reid's colposcopic index (RCI) grading system for histopathology, in the South African setting. Methods: Women living with HIV (WLWH) and HIV-negative women aged 25 to 65 were recruited from three sites in South Africa. A cross-sectional study which assessed visual inspection with acetic acid (VIA), visual inspection with lugols iodine (VILI), colposcopic impression and RCI for the detection of histologically confirmed CIN2+ and CIN3+ was performed. Test positivity rates, sensitivity, specificity, and predictive values were calculated. Results: Three hundred and forty-four WLWH and 409 HIV-negative women, with a median age of 40 years, were included in this analysis. Histologically confirmed CIN2+ was present in 38.51% and CIN3+ in 18.99%. Overall, positive test rates for VIA were 42.76%; VILI were 45.68%, colposcopic impression were 48.26% and RCI were 46.65%. Overall sensitivities/specificities for VIA and VILI for CIN3+ were 76.92/65.25% and 75.52/61.31%, respectively. The sensitivities however increased for WLWH (VIA 82.61%; VILI 80.43%) and decreased in HIV-negative women (VIA 66.67%; VILI 66.67%). Colposcopic impression/RCI performed better in WLWH (PPV 37.96/37.74%) than in HIV-negative women (PPV 25.63/26.80%). Conclusion: The current study demonstrates that visual inspection methods perform better in WLWH than in HIV-negative women. VIA and VILI performed similarly within each sub-population, as did colposcopic impression and RCI. The use of visual inspection methods in cervical cancer screening in WLWH is warranted. Abstract

Excitons spread through diffusion and interact through exciton–exciton annihilation. Nanophotonics can counteract the resulting decrease in light emission. However, conventional enhancement treats emitters as immobile and non‐interacting. It neglects exciton redistribution between regions with different enhancements and the increase in non‐radiative decay at high exciton densities. Here, the authors went beyond the localized Purcell effect to exploit exciton dynamics and turn their typically detrimental impact into additional emission. As interacting excitons diffuse through optical hotspots, the balance of excitonic and nanophotonic properties leads to either enhanced or suppressed photoluminescence. The dominant enhancement mechanisms are identified in the limits of high and low diffusion and annihilation. Diffusion lifts the requirement of spatial overlap between excitation and emission enhancements, which are harnessed to maximize emission from highly diffusive excitons. In the presence of annihilation, improved enhancement is predicted at increasing powers in nanophotonic systems dominated by emission enhancement. The guidelines are relevant for efficient and high‐power light‐emitting diodes and lasers tailored to the rich dynamics of excitonic materials such as monolayer semiconductors, perovskites, or organic crystals.
