OBJECTIVE: Right ventricle and pulmonary artery pressure have always received less attention in type 1 diabetes than left ventricle. The aim of this study is to compare the right heart performance and the estimated peak systolic pulmonary artery pressure (EPSPAP) in young type 1 diabetes patients with healthy controls. PATIENTS AND METHODS: Subjects affected by type 1 diabetes without cardiovascular and respiratory diseases (n=93) and healthy controls (n=56) were evaluated with a comprehensive transthoracic echocardiography. The pulmonary peak systolic arterial pressure was calculated with an established formula based on pulmonary artery acceleration time. RESULTS: The left ventricle's function was found to be normal in all the subjects under study. The estimated peak systolic pulmonary artery pressure was significantly higher in patients with type 1 diabetes compared to the controls (38.5 ± 8.6 vs. 35.4 ± 6.7, p = 0.019). The highest value of EPSPAP was observed in smoking female patients with type 1 diabetes. Basal and mid cavity diameter of the right ventricle were higher in patients with type 1 diabetes. Factors associated with EPSPAP were sex, body mass index, mid cavity diameter and, with an inverse correlation, HDL-cholesterol. CONCLUSIONS: The present study suggests that young, uncomplicated patients with type 1 diabetes have a higher estimated peak systolic pulmonary artery pressure. Proteomics has been used to investigate cross-talk between the intestinal microbiome and host biological processes. In this study, an in ovo technique and a proteomics approach was used to address how early bacterial colonization in the gastrointestinal tract (GIT) could modulate inflammatory and immune responses in young broilers. Embryos at 18 embryogenic days were inoculated with saline (S), 102 CFU of Citrobacter freundii (CF), Citrobacter species (C2), or lactic acid bacteria mixture (L) into the amnion. At 10 days posthatch, ileum samples from 12 birds per treatment were selected for tandem mass spectrometry analysis. Our further findings indicated that treatment-specific influences on early GIT microbiota resulted in different immune responses in mature broilers. Predicted functional analyses revealed activation of inflammation pathways in broilers treated in ovo with L and CF. Exposure to L enhanced functional annotation related to activation, trafficking of immune cells, and skeletal growth based-network, while CF inhibited biological functions associated with immune cell migration and inflammatory response. These results highlighted that proper immune function was dependent on specific GIT microbiota profiles, in which early-life exposure to L-based probiotic may have modulated the immune functions, whereas neonatal colonization of Enterobacteriaceae strains may have led to immune dysregulation associated with chronic inflammation. Abstract: The ecosystem services (ES) approach is a framework for describing the benefits of nature to human well‐being, and this has become a popular instrument for assessment and evaluation of ecosystems and their functions. Grazing lands can provide a wide array of ES that depend on their management practices and intensity. This article reviews the trends and approaches used in the analysis of some relevant ES provided by grazing systems, in line with the framework principles of the Millennium Ecosystem Assessment (MA). The scientific literature provides reports of many studies on ES in general, but the search here focused on grazing systems, which returned only sixty‐two papers. This review of published papers highlights that: (i) in some papers, the concept of ES as defined by the MA is misunderstood (e.g., lack of anthropocentric vision); (ii) 34% of the papers dealt only with one ES, which neglects the need for the multisectoral approach suggested by the MA; (iii) few papers included stakeholder involvement to improve local decision‐making processes; (iv) cultural ES have been poorly studied despite being considered the most relevant for local and general stakeholders; and (v) stakeholder awareness of well‐being as provided by ES in grazing systems can foster both agri‐environmental schemes and the willingness to pay for these services. Aim To assess the influence of health locus of control and fear of hypoglycaemia on metabolic control and treatment satisfaction in people with Type 1 diabetes mellitus on continuous subcutaneous insulin infusion. Methods People with Type 1 diabetes on continuous subcutaneous insulin infusion for at least 1 year, sub-classified as an 'acceptable glucose control' group [HbA1c ≤ 58 mmol/mol (7.5%)] and a 'suboptimum glucose control' group [HbA1c > 58 mmol/mol (7.5%)], were consecutively enrolled in a multicentre cross-sectional study. Questionnaires were administered to assess health locus of control [Multidimensional Health Locus of Control ( MHLC) scale, with internal and external subscales], fear of hypoglycaemia [Hypoglycaemia Fear Survey II ( HFS- II)] and treatment satisfaction [Diabetes Treatment Satisfaction Questionnaire ( DTSQ)]. Results We enrolled 214 participants (mean ± sd age 43.4 ± 12.1 years). The suboptimum glucose control group ( n = 127) had lower mean ± sd internal MHLC and DTSQ scores than the acceptable glucose control group (19.6 ± 5.2 vs 21.0 ± 5.0, P = 0.04 and 28.8 ± 4.8 vs 30.9 ± 4.5, P < 0.001). HFS- II scores did not differ between the two groups. Internal MHLC score was negatively associated with HbA1c ( r = −0.15, P < 0.05) and positively associated with the number of mild and severe hypoglycaemic episodes ( r = 0.16, P < 0.05 and r = 0.18, P < 0.001, respectively) and with DTSQ score ( r = 0.17, P < 0.05). HFS- II score was negatively associated with DTSQ score ( r = −0.18, P < 0.05) and positively with number of severe hypoglycaemic episodes ( r = 0.16, P < 0.5). Conclusions In adults with Type 1 diabetes receiving continuous subcutaneous insulin infusion, high internal locus represents the most important locus of control pattern for achieving good metabolic control. Background Paragangliomas are rare neoplasms that originate from the neural crest. They are malignant in approximately 10% of cases, with a 50% survival rate at 5 years from diagnosis. In most cases, manifestations of malignancy (such as metastasis) are lacking, and paragangliomas are considered benign lesions. Pancreatic paragangliomas are extremely rare, with only 31 cases described in the scientific literature to date. Case summary Here we describe a case of a 55-year-old Caucasian male patient referred to our institution in September 2013 for lumbar pain lasting five months. The ultrasound and the CT scan revealed a 2.5 cm solid nodule located in the uncinate process of the pancreas. On the basis of this evidence, the preoperative diagnosis was a pancreatic neuroendocrine tumor (NET), which was further confirmed by a subsequent In-Pentetreotide Scan examination. A pylorus-preserving duodenocephalopancreasectomy was performed. Pancreatic paraganglioma was the final pathological diagnosis. Rare localizations of paraganglioma are often discovered casually, during imaging examinations for other clinical reasons, as happened in the case of our patient. It appears evident that the preoperative diagnosis of pancreatic paragangliomas is extremely challenging. Surgery represents the cornerstone of the clinical management of these neoplasms, primarily for the need of a definitive diagnosis, which is difficult to assess preoperatively in most cases. Conclusions Our strategy is the same as that adopted for the management of pancreatic NETs; the dimensional limit for a conservative resection is 2 cm, while major resections (Whipple’s approach or distal pancreatectomy) should be employed in larger tumors, which are generally associated with a worse prognosis. In the last decade, several molecular markers have been proposed to improve the diagnosis of thyroid nodules. Among these, mutations in the telomerase reverse transcriptase (TERT) promoter have been correlated to malignant tumors, characterized by highest recurrence and decreased patients' survival. This suggests an important role of TERT mutational analysis in the clinical diagnosis and management of thyroid cancer patients. The aim of the study was to demonstrate the adequacy of core needle biopsy (CNB) for the preoperative assessment of TERT mutational status, to reach a more accurate definition of malignancy and a more appropriate surgical planning. Indeed, CNB is gaining momentum for improving diagnosis of thyroid nodules deemed inconclusive by fine needle aspirate (FNA). The study included 50 patients submitted to CNB due to inconclusive FNA report. TERT mutational status was correlated with BRAF mutation, definitive histology, and post-operative TNM staging of the neoplasia. C228T mutation of the TERT promoter was reported in 10 % of the papillary carcinomas (PTC) series. When compared with final histology, all cases harboring TERT mutation resulted as locally invasive PTCs. The prevalence of TERT mutated cases was 17.6 % among locally advanced PTCs. TERT analysis on CNB allows the assessment of the pathological population on paraffin sections before DNA isolation, minimizing the risk of false negatives due to poor sampling that affects FNA, and gathering aggregate information about morphology and TERT mutational status. Advances introduced by additive manufacturing (AM) have significantly improved the control over the microarchitecture of scaffolds for tissue engineering. This has led to the flourishing of research works addressing the optimization of AM scaffolds microarchitecture to optimally trade-off between conflicting requirements (e.g. mechanical stiffness and porosity level). A fascinating trend concerns the integration of AM with other scaffold fabrication methods ( i.e. “combined” AM), leading to hybrid architectures with complementary structural features. Although this innovative approach is still at its beginning, significant results have been achieved in terms of improved biological response to the scaffold, especially targeting the regeneration of complex tissues. The scope for using sunflower cake in ration formulation was tested in 6 horses employed for equine tourism. Three experimental diets providing the same amount of proteins and energy as the control diet were formulated using rising doses of sunflower cake (SD3: 0.3 kg; SD6: 0.6 kg and SD10: 1.0 kg). Digestibility evaluation using internal markers showed that rising sunflower cake doses reduced digestibility of nutrients. Statistical analysis using the diet effect demonstrated a significant reduction in dry matter (DM) (P=0.0006), ether extract (EE) (P=0.0001), crude fibre (CP) (P=0.0279), neutral detergent fiber (NDF) (P=0.0247) and acid detergent fiber (ADF) (P=0.0269) digestibility. Comparison of the two internal markers acid insoluble ash (AIA) and acid detergent lignin (ADL) showed better digestibility values for all measures using ADL. A composite polymeric membrane to be used as electrolyte in polymer electrolyte membrane fuel cells has been prepared and characterized. The membrane is composed of an acidic polymer, sulfonated polyetheretherketone, and of basic filler, amino-diphenylsilandiol, a functionalized organically modified silane. Attenuated total reflectance (ATR)/fourier transform infrared spectroscopy (FTIR) demonstrated the occurrence of a specific interaction of the polymer SO3H groups with the basic function of the filler. Such interaction reflected in reduced swelling, enhanced thermal stability, and good proton conductivity values at intermediate temperatures (σ ≈ 10−2 S cm−1 at 100°C). © 2011 Wiley Periodicals, Inc.
