Meralgia paresthetica is a compressive neuropathy of the lateral femoral cutaneous nerve. Surgery is the gold standard for severe cases. However, no high-quality evidence exists on which strategy is best: decompression or neurectomy. Data of a consecutive series of 52 patients treated for meralgia paresthetica over 25 years (1997–2022) were retrospectively collected from medical records and telephone interviews. In total, 27 women and 25 men were operated on; 11 patients had iatrogenic meralgia paresthetica. Decompression was performed on 47 patients, and neurectomy in 8 cases (5 primary neurectomies plus 3 failed neurolysis). Out of the patients who underwent decompression, 41 (87.2%) benefited from the treatment; 3 had pain relief, but no benefit on paresthesia; and 3 reported pain persistence. The latter required neurectomy to resolve symptoms. The eight patients who underwent neurectomy experienced symptom relief but had an obvious anesthetic area persisting over years. Complications were rare (3.8%): a groin hematoma in the post-operative course and an inguinal herniation 6 months after surgery. Surgery, be it neurolysis or neurectomy, offers excellent results with low risks. Decompression has been proven to be adequate in almost all patients, avoiding the side effects of neurectomy. Background: A 26-year old male testicular cancer survivor presented for Chiropractic care with a primary complaint of intermittent thigh pain. Intervention: The patient underwent a course of subluxation-based Chiropractic care during which he was checked and adjusted using Advanced Bio-Structural Correction Technique. Outcome: Despite a singular presenting concern of intermittent thigh pain from an four-year-old injury, he returned numerous other positive outcomes he attributed to Chiropractic. This presents numerous questions as to the body's state of adaptability even years after cancer, and the role Chiropractic may play in building up this adaptability again over time. BACKGROUND: Anterior thigh pain is frequent in rugby players, and hematomas are common. Most occur due to trauma; however, if swelling occurs with no mechanism of injury, these should always be investigated. CASE PRESENTATION: A 17-year-old male presented with a 6-month history of left anterolateral thigh pain and swelling with no mechanism of injury. Examination revealed that the area was hot to touch, no wounds were visible, and the patient reported no night pain or fever. He was urgently referred where an x-ray showed no findings, and then subsequently discharged. On recommendation from the original referring clinician, an ultrasound scan was ordered, which showed a 4.5-cm foreign body causing an infection. OUTCOME AND FOLLOW-UP: Subsequent surgery removed a twig with thorns from the area. The pain was relieved, and he returned to rugby 3 weeks later with no further complications. DISCUSSION: This case is a unique presentation of anterolateral thigh pain and highlights the importance of referral and follow-up when things do not fit a clinical picture. [ABSTRACT FROM AUTHOR]

INTRODUCTION: To assess the incidence and causes of persistent thigh pain and peri-implant fractures after union in patients of intertrochanteric fractures treated with short cephalo-medullary nails. MATERIALS AND METHODS: A retrospective observational study conducted at a Level 1 Trauma centre. A total of 122 patients of intertrochanteric fractures who were operated using short cephalo-medullary nails (170mm and 200mm lengths) between January 2018 to June 2019 were included in the study. Main outcomes measured were the incidence of thigh pain and peri-implant fractures. RESULTS: Out of the 122 patients with a mean follow-up of 14.1 month, 12 patients had persistent thigh pain. Six patients had the helical blade protruding from the lateral cortex, two of them had distal tip of nail abutting on the anterior cortex and four cases had prominent proximal segment of nail which may explain the cause of their pain. Five of these patients had a combination of these findings. Two patients had pain for which no other obvious cause was found. There were no cases of peri-implant fractures in our study. CONCLUSION: Thigh pain associated with the use of short cephalon-medullary nails is often unrelated to nail length and can be prevented by using proper surgical technique. There seems to be no association between the use of short nails and peri-implant fractures.

Total hip arthroplasty (THA) for hip disorders helps to alleviate pain and improve active daily life. When determining the effects of medical therapy and the subsequent clinical results, patient quality of life (QOL) also needs to be assessed. Recently, patient-reported outcomes (PRO) have become important as evaluation criteria. This study aimed to evaluate patient QOL and various PRO between different types of cementless stems and to clarify the relationship between PRO and stem-cortical bone contact. The study comprised 138 consecutive patients undergoing cementless THA for hip osteoarthritis. We assessed three different types of PRO: the Japanese Orthopaedic Association Hip-Disease Evaluation Questionnaire (JHEQ), patient's joint perception, and peri-hip articular joint pain (PHAP). We measured the state of contact between femur and implant by density mapping. No significant difference was noted in PRO between stem types based on the JHEQ and patient's joint perception. PHAP occurred more frequently in the patients with a taper wedge stem versus a fit-and-fill stem. In both groups, distal contact was associated with PHAP but not with JHEQ results and patient's joint perception. The difference in PHAP between the two stem types groups was significant, with postoperative PHAP being higher with the taper wedge stem because of the more distal contact between the taper wedge stem and femur cortical bone. Abstract Background The trochanteric bursa (TB) is an anatomic structure, which is dissected during posterior/lateral hip approaches in Total Hip Arthroplasty (THA). Some surgeons prefer to simply resect the bursa as they believe that it may be responsible for postoperative lateral trochanteric pain (LTP). Others advocate that this tissue acts as a buffer minimizing friction between soft tissue and bone, and therefore its repair may prevent LTP after THA. Aim The purpose of this prospective randomized controlled trial was to compare the clinical results of either resecting or repairing the TB during posterior approach THA. Methods Forty-two patients with primary hip osteoarthritis undergoing THA via a posterior hip approach were randomly assigned to two groups; Group A, or TB resection group and Group B, or TB repair group. All patients in both groups were evaluated postoperatively in terms of hip function, measured by the Harris Hip Score (HHS), at 6 weeks, 3 months, 6 months, and 12 months after surgery, as well as LTP during daily routine activities and lying on the operative side. Results Forty patients completed the study. Postoperative difference in terms of leg length and femoral offset was similar among the two groups (P = 0.467 and P = 0.39, respectively). At 6 weeks, patients in Group B had higher HHS (P = 0.052) and experienced less LTP when lying on the operative side (P = 0.046) but not during activities (P = 0.759). Thereafter, all functional parameters measured had comparable values in both groups. Subgroup analysis failed to identify any correlation between high offset stems and LTP. Conclusion TB repair in posterior approach THA improves hip functional recovery as well as patients’ ability to lie on the operative side during the early postoperative period.

Background: Femoral strut allografts are used in revision hip arthroplasty for management of bone loss associated with implant failure or periprosthetic fractures. They have also been used to treat unremitting thigh pain in well-fixed cementless femoral stems, to address the differential in structural stiffness between the stem and femoral shaft. Our study used an in vitro biomechanical model to measure the effect of placement of allografts on femoral strains, to determine their load-sharing capacity. Material and methods: Three rosette strain gauges were applied to the femoral surface of each of 6 cadaveric femurs, at the stem tip level on anterior, medial, and lateral cortices. After stem implantation, cortical strut allografts were applied to the lateral femoral shaft and secured with 4 Dall-Miles cables. A fourth gauge was placed on the midpoint of the allograft. Strains were recorded in the intact femur, then the implanted femur with and without the allograft under simulated physiologic loading in a load frame. Results: Reduction in distal femoral principal strains, between 12% and 59%, was seen in all cortices following placement of the allograft. Under axial loading, 30% of the strain in the lateral cortex was borne by the allograft. Greater reductions in strain, by as much as 59%, occurred under axial load and torque. Conclusion: The results of this biomechanical model indicate that by placement of an allograft, cortical strains can be reduced to levels approaching those in an intact femur, supporting this technique for treatment of unremitting thigh pain in well-fixed prostheses.

Objective: To evaluate the clinical utility of selective magnetic resonance neurography-(MRN)-guided anterior femoral cutaneous nerve (AFCN) blocks for diagnosing anterior thigh neuralgia. Materials and methods: Following institutional review board approval and informed consent, participants with intractable anterior thigh pain and clinically suspected AFCN neuralgia were included. AFCN blocks were performed under MRN guidance using an anterior groin approach along the medial sartorius muscle margin. Outcome variables included AFCN identification on MRN, technical success of perineural drug delivery, rate of AFCN anesthesia, complications, total procedure time, patient-reported procedural experiences, rate of positive diagnostic AFCN blocks, and positive subsequent treatment rate. Results: Eighteen MRN-guided AFCN blocks (six unilateral and six bilateral blocks) were performed in 12 participants (6 women; age, 49 (30–65) years). Successful MRN identified the AFCN, successful perineural drug delivery, and AFCN anesthesia was achieved in all thighs. No complications occurred. The total procedure time was 19 (10–28) min. Patient satisfaction and experience were high without adverse MRI effects. AFCN blocks identified the AFCN as the symptom generator in 16/18 (89%) cases, followed by 14/16 (88%) successful treatments. Conclusion: Our results suggest that selective MR neurography-guided AFCN blocks effectively diagnose anterior femoral cutaneous neuralgia and are well-tolerated. [ABSTRACT FROM AUTHOR]

Purpose: Short-stem prostheses in hip arthroplasty have emerged as an alternative to conventional stems, especially in younger patients. The purpose of this study was to compare functional and radiological results of a short metaphyseal fitting cementless stem versus a conventional stem implant, in patients younger than 60 years. Methods: All patients operated from January 2006 to April 2013 were included, obtaining a minimum follow-up of 7 years. Harris Hip Score (HHS) and SF-36 (quality of life) questionnaires were applied and the presence of "thigh pain" was specifically assessed. We also compared complication rate, revision rate and average prosthesis survival. Femoral stress shielding (Gruen scale), stem subsidence, varus-valgus tilt and implant stability (Engh scale) were also compared. Results: A total of 101 short-stem and 74 conventional arthroplasties were included, with an average follow-up of 9.82 (7–14) years. HHS functional score and SF-36 were excellent in both implants and no significant difference between them (p > 0.05) was found. However, "thigh pain" was present in 7 patients with conventional stems and none with short-stems (p < 0.001). The survival rate at 13 years was 99%, for both implants, and no significant differences were found between them (χ2(2) = 0.178; p = 0.673). Conventional stems had stress shielding at the greater trochanter in 72% of the cases and 43% at the calcar, being statistically superior (p < 0.001) to the stress shielding observed in the short stems. Conclusion: According to our results, this short-stem seems to allow preservation of bone stock, with decreased stress shielding and also a lower incidence of thigh pain compared to conventional stems. Abstract Purpose Short tapered stem placement has been extensively employed in total hip arthroplasty (THA). Suboptimal fixation tends to cause postoperative complications, such as thigh pain. However, it remains unclear whether poor seating/alignment of short tapered stems contributes to thigh pain. In this study, we retrospectively examined the factors that might be associated with thigh pain. Methods Medical records of 230 patients who had undergone THAs at our hospital were reviewed retrospectively. All patients received the same mediolateral (ML) short tapered femoral stems. The association between thigh pain and patients’ demographics, radiographic findings, or the type of fitting of the femoral stems was investigated. Results In our cohort, 68 patients (27.8%) presented with thigh pain. Among 203 type I fit patients, 62 (30.5%) developed thigh pain, while only 6 out of 43 (12.2%) type II fit patients had thigh pain, with the differences being statistically significant (x 2 = 6.706, p = 0.01). In addition, hip anteroposterior radiographs exhibited that the stem angulation (mean 2.52°), the variation in angulation (mean 1.32°), and the extent of femoral stem subsidence (mean 0.29 cm) were greater in patients with thigh pain than in their counterparts without thigh pain (all p < 0.05). Conclusion Malalignment and improper seating of short tapered stems could be at least one of the reasons for post-THA thigh pain. The distal contact between the stem tip and the medial femoral cortex might result in thigh pain. Our study suggested that distal implant contact should be avoided, and stem alignment should be meticulously performed in the placement of ML short tapered femoral stems for THA.

Background: Thigh pain following a well-fixed total hip arthroplasty (THA) remains problematic and a source of patient dissatisfaction. The purpose of this study is to evaluate if the development of distal femoral cortical hypertrophy (DFCH) is associated with postoperative thigh pain after THA. Methods: All patients who underwent an uncomplicated primary THA via a direct anterior approach with the Taperloc Microplasty (Zimmer Biomet, Warsaw, IN, USA) implant between 2011 and 2015 were mailed a pain drawing questionnaire. Radiographs were reviewed at 1 year minimum to determine cortical thickness change from immediate post-op. Thigh pain was compared to DFCH. 293 patients were included in the study. Results: Mean follow-up was 3.2 years. A total of 218 hips (74%) had cortical hypertrophy in Gruen zone 3 and 165 hips (56%) had cortical hypertrophy in Gruen zone 5. 52 hips (18%) had ⩾25% cortical hypertrophy in zone 3 and 91 hips (31%) had ⩾25% cortical hypertrophy in zone 5. A total of 44 patients (15%) reported anterior thigh pain and 43 patients (15%) reported lateral thigh pain. Development of DFCH in either Gruen zone 3 or 5 was not associated with anterior or lateral thigh pain. Stem size was positively correlated with zone 3 hypertrophy and inversely related to zone 5 hypertrophy. Thigh pain was not associated with patient age, gender, activity level or stem size. Conclusions: The development of distal femoral cortical hypertrophy after THA with a short stem implant was high, but not associated with patient-reported anterior or lateral thigh pain. Objective: To describe and illustrate the magnetic resonance imaging (MRI) anatomy of the anterior femoral cutaneous nerve (AFCN) and a new technique for cryoanalgesia of the AFCN for long-term analgesic treatment of recalcitrant AFCN-mediated neuropathic pain.Materials and Methods: Using a procedural high-resolution MRI technique, we describe the MRI anatomy of the AFCN. Three patients (mean age, 48 years; range, 41-67 years) with selective nerve block-verified recalcitrant AFCN-mediated anterior thigh pain were enrolled to undergo cryoanalgesia of the AFCN. Procedures were performed under MRI guidance using clinical wide-bore MR imaging systems and commercially available cryoablation system with MR-conditional probes. Outcome variables included technical success, clinical effectiveness including symptom relief measured on an 11-point visual analog scale, frequency of complications, and procedure time.Results: Procedural MRI allowed to successfully demonstrate the course of the AFCN, accurate cryoprobe placement, and monitoring of the ice ball, which resulted in technically successful iceball growth around the AFCN in all cases. All procedures were clinically effective, with median pain intensity decreasing from 8 (7-9) before the procedure to 1 (0-2) after the procedure. The cryoanalgesia effect persisted during a 12-month follow-up period in all three patients. No major complications occurred. The average total procedure time was 98 min (range, 85-125 min).Conclusion: We describe the MRI anatomy of the AFCN and a new technique for cryoanalgesia of the AFCN using MRI guidance, which permits identification of the AFCN, selective targeting, and iceball monitoring to achieve long-term AFCN-mediated neuropathic pain relief. Case: A 70-year-old female patient underwent total hip arthroplasty (HA) using a stem cemented line-to-line without centralizer. Postoperatively, she complained of load-dependent thigh pain. Conventional radiographs identified cortical overload because of a distal cement mantle discontinuity at the level of the stem's tip, confirmed by single-photon emission computed tomography/computed tomography scan (SPECT/CT). After cement-in-cement revision using a stem with centralizer, pain ceased rapidly. The cortical overload disappeared, as confirmed on a following SPECT/CT performed for low back pain. Conclusion: In HA, the stem's tip may cause overload on the bone's cortex if the cement mantle is incomplete. Implanting a stem with centralizer avoids or cures this. [ABSTRACT FROM AUTHOR]

Despite good revision rates, well fixed diaphyseal femoral stems are not free of complications. Persistent thigh pain as well as proximal bone resorption influence the quality of treatment negatively. One reason for thigh pain is the difference of the elasticity modulus of the implant and the cortical bone of the femur. Patients with a low cortical index, osteoporosis and a high activity level have a significantly increased risk of developing chronic thigh pain after cementless hip arthroplasty. Even modern tapered-wedge stems cause thigh pain if anatomy and implant design do not match. Bone loss in the proximal aspect of the femur due to stress shielding is well known. The design of the implant and the corresponding femoral anatomy both play key roles. Using tapered-wedge stems leads to significantly less stress shielding. However, short stems show clear advantages in both the prevalence of thigh pain and stress shielding, particularly in the region of the greater trochanter, compared to straight or tapered-wedge stems. Nevertheless, not every anatomic variance can be covered. A morphotype-specific approach should be the surgeon's primary goal. This includes consideration of all available stem designs. Purpose: The occurrence of thigh pain following cementless total hip arthroplasty (THA) may be dependent on multiple factors, which may differ with design of the prosthesis. Thus, this study aimed to investigate the frequency of thigh pain following cementless THA using a short, tapered-wedge stem to identify risk factors for thigh pain. Methods: This retrospective cohort study analysed 222 joints of patients who underwent THA with a short, tapered-wedge stem. Thigh pain was evaluated using a questionnaire during each follow-up visit, and the clinical and radiographic assessments were compared. Results: Thigh pain occurred in 37 patients (16.7%) during the follow-up period. The pain started two to 24 months after THA. Multivariate analysis demonstrated that higher University of California, Los Angeles, activity rating (odds ratio 7.2; 95% confidence interval (CI) 3.0–17.2); Dorr type C femoral bone shape (odds ratio 1.5; 95% CI 1.1–2.0); and stem tip-cortical bone contact (odds ratio 8.2; 95% CI 2.3–29.4) were significant risk factors of thigh pain following THA. Post-operatively, cortical hypertrophy at Gruen zone 4 was significantly found in patients with thigh pain (p value = 0.032). Conclusion: Risk factors of pain following THA with short, tapered-wedge stem were high activity level, Dorr type C femoral bone shape, and stem tip contacting the distal bone surface. Moreover, post-operative cortical hypertrophy at the distal stem tip significantly increased the incidence of thigh pain. Therefore, we must pay attention to the aforementioned factors to avoid post-operative thigh pain when using a short, tapered-wedge stem. [ABSTRACT FROM AUTHOR]

Background: Although reported results on short stems sound very promising, the occurrence of distal femoral cortical hypertrophy is often observed. The aim of the present study was to report 5-year survival data of a commercially available trochanter sparing short stem and investigate the clinical impact of distal femoral cortical hypertrophy on the outcome. Methods: 123 total hip arthroplasties were performed on 120 patients from April 2008 to May 2010 (mean age 62, range 29–89 years; 71 hips from male patients, 58%). Clinical and radiological data were collected preoperative, at 6 weeks, 1, 2, 3, and 5 years postoperative to assess the outcome. Radiographs taken immediately postoperative as well as 1 and 5 years postoperative were used to identify and assess cortical hypertrophy. Results: 1 stem had to be revised due to aseptic loosening, resulting in a Kaplan-Meier survival analysis with endpoint for stem revision of 99.2% (95% Confidence Interval 94.1–99.9) at 5 years. 96 radiological and 95 clinical follow-ups were analysed 5 years postoperative. 68 (71%) hips showed distal femoral cortical hypertrophy after 5 years. The average Harris Hip Score and Oxford Hip Score improved 33 (standard deviation (SD) 15.1, range 2–70), 18 (SD 12.1, range -10–43) points, respectively. Overall 16% of the patients reported thigh pain, unrelated to the presence of cortical hypertrophy. Discussion: This short stem shows an excellent 5-year survival rate and good clinical outcome despite a high incidence of cortical hypertrophy. Background: Cobalt-chromium (Co-Cr) and titanium (Ti) have been the most popular materials employed for cementless implants. The purpose of this study was to compare clinical and radiological results of a single stem design with both alloys at long-term follow-up.Methods: Two hundred consecutive uncemented stems implanted in 171 patients (100 Co-Cr and 100 Ti implants) between 1999 and 2002 were studied. Mean age of the patients was 60.9 years (range, 20-84). Clinical results were evaluated using the Harris hip score. The presence of thigh pain was also analyzed. Stem fixation was graded according to Engh criteria. Radiolucent lines, osteolysis, and stem subsidence were also analyzed.Results: At 15-year follow-up, no stems had been revised. Both groups showed similar clinical results with mean Harris hip score of 93.4 (Co-Cr) vs 93.9 (Ti). There was no difference in the rate of thigh pain (11 vs 8.3, respectively, P = .507). Radiolucent lines were more frequent in the Co-Cr group (63.6% vs 35.6%, P < .001).Conclusion: Ti stems showed better osteointegration than Co-Cr stems, with a significantly lower incidence of radiolucent lines. Introduction: Short stems have been designed with the purpose of preserving bone tissue, decreasing the incidence of thigh pain and facilitating surgical techniques. The aim of our study was to assess whether a shortened tapered conventional stem was able to reduce the incidence of thigh pain. Methods: Between March 2010 and December 2012, 200 patients were enrolled in the study. Visual analogue scale (VAS) that included mapping of the pain, Harris Hip Score (HHS), Short Form-12 (SF-12) and radiographic outcomes were evaluated prior to surgery as well as at 6, 12 and 24 months post-operatively. Results: After 6 months, 6 patients (3%) had thigh pain. After 12 months, 3 patients (1.5%) complained about thigh pain. After 2 years, 2 patients (1%) had thigh pain. There was no correlation between pain and clinical, radiological, or demographic variables. Conclusion: The shortened tapered conventional stem resulted in a lower incidence of thigh pain for up to 2-years following surgery, compared with conventional or other short stems.

The incidence of esophageal adenocarcinoma is increasing, and with this increase is an influx of unusual presentations in the literature. Skeletal muscles are generally rare as sites of metastasis. We report a case of a middle aged-man who presented with a painful swelling of the thigh which turned out to be consistent with esophageal adenocarcinoma metastasis. Few reports have preceded ours. The prognosis is poor and the therapeutic modalities of such an advanced disease are limited.

Abstract Purpose The purpose of this report is to describe the clinical course of a patient who had a primary complaint of isolated right lateral thigh pain 3 years prior that was nonresponsive to conservative measures. Hypoesthesia in the lateral side of the right forearm, deltoid and biceps, as well as triceps paresis, was also diagnosed. Results Immediately after surgery, the patient reported substantial improvement in his right thigh symptoms. The signs and symptoms associated with the right C6 and C7 radiculopathy did improve. Conclusion Although the presentation described in this case is somewhat unique, the eventual myelopathic signs and symptoms were not. It was these myelopathic signs and symptoms that led to cervical magnetic resonance imaging, the diagnosis of cervical cord compressive myelopathy, and surgical management.

Despite the theoretical advantages of uncemented short stems, postoperative thigh pain is still matter of concern and can be attributed to different causes. We report a peculiar case of a stress fracture around a short cementless stem with cervico-metaphyseal fixation in an otherwise healthy patient. We implanted a MiniHipTM stem in a 43 year-old male professional golf player for the treatment of primary osteoarthritis using a ceramic on ceramic bearing. Against medical advice, the patient started to play soccer at the 4th postoperative month and was completely asymptomatic to that extent; but at 8 months follow-up and without a history of trauma he started complaining about progressive hip pain. After ruling out infection and loosening, histological analysis from a bone biopsy confirmed the diagnosis of stress fracture. Although revision surgery was initially scheduled, pain started to decrease gradually with protected weight-bearing (crutches) and disappeared around the first postoperative year, remaining the patient asymptomatic at 2 and half years of follow-up, with radiographs depicting a healed fracture with a hypertrophic callus. We encourage surgeons to be aware of the existence of periprosthetic stress fractures as a source of thigh pain (sometimes intractable), and despite being infrequent, they should always be contemplated, providing that these cases can be managed conservatively with rest and limited weight-bearing. After this uncommon case, we suggest to align the stem in order to equally distribute loads onto the medial calcar and the lateral femoral cortical.

Objective. This article presents an evidence-based algorithm to assist primary care providers with the diagnosis and management of lateral hip and thigh pain in older adults. It is part of a series that focuses on coexisting pain patterns and contributors to chronic low back pain (CLBP) in the aging population. The objective of the series is to encourage clinicians to take a holistic approach when evaluating and treating CLBP in older adults. Methods. A content expert panel and a primary care panel collaboratively used the modified Delphi approach to iteratively develop an evidence-based diagnostic and treatment algorithm. The panelists included physiatrists, geriatricians, internists, and physical therapists who treat both civilians and Veterans, and the algorithm was developed so that all required resources are available within the Veterans Health Administration system. An illustrative patient case was chosen from one of the author's clinical practices to demonstrate the reasoning behind principles presented in the algorithm. Results. An algorithm was developed which logically outlines evidence-based diagnostic and therapeutic recommendations for lateral hip and thigh pain in older adults. A case is presented which highlights the potential complexities of identifying the true pain generator and the importance of implementing proper treatment. Conclusions. Lateral hip and thigh pain in older adults can contribute to and coexist with CLBP. Distinguishing the true cause(s) of pain from potentially a myriad of asymptomatic degenerative changes can be challenging, but a systematic approach can assist in identifying and treating some of the most common causes. Background: Although thigh pain is an annoying problem after total hip arthroplasty (THA), little information has been known about its natural course.Methods: To determine the frequency, time of onset, and duration of thigh pain after cementless THA, we evaluated 240 patients (240 hips) who underwent primary THA because of femoral head osteonecrosis with the use of a single tapered stem.Results: Twenty-seven patients (11.3%) experienced thigh pain during the follow-up of 3-11 years (mean, 84 months). The pain developed 2-78 months (median, 25 months) after THA. The 75% of pain developed within 36 months. In 25 patients, the pain disappeared postoperatively 1 month-64 months (mean, 17.4 months), and 2 patients (7.4%, 2/27) had persistent thigh pain. There were no differences in the latest Harris Hip Score between the thigh pain group and no thigh pain group.Conclusion: Our results provided basic information about the natural history of thigh pain after cementless THA with a tapered stem design. Morel-Lavallée lesion is an unusual clinical entity which was first described by the French surgeon Maurice Morel-Lavallée in 1853. These lesions result from peeling off the surfaces between the fat tissue and the muscular fascia. During this decomposition, the rupture of small vessels may cause an effusion full of necrotic blood, lymph and fat cells. In prolonged cases, these lesions may become increasingly painful, leading to be confused with other diagnosis such as sciatalgia, piriformis syndrome, trochanteric bursitis or soft tissue tumor. Abstract: Background context: Despite common use of intraoperative electrophysiologic neuromonitoring, injuries to the lumbar plexus during lateral lumbar interbody fusion (LLIF) have been reported. Emerging data suggest that recombinant human bone morphogenetic protein-2 (rhBMP-2) use during an anterior or transforaminal lumbar interbody fusion may be associated with an increased risk of neurological deficit. Clinical data on the sequelae of rhBMP-2 implantation in close proximity to the lumbosacral plexus during LLIF remains to be understood. Purpose: The purpose of this study was to compare the incidence of neurologic deficits and pain in patients undergoing LLIF with and without rhBMP-2. Study design/setting: Retrospective outcome analysis in controlled cohorts undergoing the lateral exposure technique for LLIF with and without rhBMP-2. Methods: The electronic medical records of patients undergoing LLIF with and without supplemental posterior fusion for degenerative spinal conditions were retrospectively reviewed over a 6-year period. Patients with previous lumbar spine surgery or follow-up of less than 6 months were excluded. Patients were divided into 2 groups, Group 1 (rhBMP-2 use; n=72) and Group 2 (autograft/allograft use; n=72), and were matched according to the age at the time of surgery, gender, weight, body mass index, side of approach, total number of treated spinal segments, use of supplemental posterior fusion, and length of follow-up. Results: Immediately after surgery, a sensory deficit was recorded in 33 patients in Group 1 and 35 patients in Group 2 (odds ratio [OR] 0.895; 90% confidence interval [CI] 0.516–1.550; p=.739). At last follow-up, a persistent sensory deficit was identified in 29 patients whose LLIF procedure was supplemented by rhBMP-2 and 20 patients in whom autograft/allograft was used (OR 1.754; 90% CI 0.976–3.151; p=.115). A motor deficit was recorded in 37 patients immediately after the rhBMP-2 procedure and 28 patients treated with autograft/allograft (OR 1.661; 90% CI 0.953–2.895; p=.133). A persistent motor deficit was recorded in 35 and 17 patients in Groups 1 and 2, respectively, at last follow-up (OR 3.060; 90% CI 1.681–5.571; p=.002). During the first postoperative examination, 37 patients in Group 1 and 25 patients in Group 2 complained of anterior thigh or groin pain (OR 1.987; 90% CI 1.133–3.488; p=.045). At last follow-up, there was a significantly higher number of patients in Group 1 who complained of persistent anterior thigh or groin pain than Group 2 (8 vs. 0 patients) (OR 16.470; 90% CI 1.477–183.700; p=.006). Conclusions: Our results provide evidence of an increased rate of postoperative neurologic deficit and anterior thigh/groin pain after LLIF using rhBMP-2, when compared with matched controls without rhBMP-2 exposure. Background: Total hip arthroplasty (THA) for hip disorders helps to alleviate pain and improve active daily life. When determining the effects of medical therapy and the subsequent clinical results, patient quality of life (QOL) also needs to be assessed. Recently, patient-reported outcomes (PRO) have become important as evaluation criteria. This study aimed to evaluate patient QOL and various PRO between different types of cementless stems and to clarify the relationship between PRO and stem-cortical bone contact., Methods: The study comprised 138 consecutive patients undergoing cementless THA for hip osteoarthritis. We assessed three different types of PRO: the Japanese Orthopaedic Association Hip-Disease Evaluation Questionnaire (JHEQ), patient's joint perception, and peri-hip articular joint pain (PHAP). We measured the state of contact between femur and implant by density mapping., Results: No significant difference was noted in PRO between stem types based on the JHEQ and patient's joint perception. PHAP occurred more frequently in the patients with a taper wedge stem versus a fit-and-fill stem. Study Design. Retrospective analysis of 73 standalone lateral lumbar interbody fusion (LLIF) procedures to identify any association between the amount of coronal curve correction and lumbosacral plexus injuries and/or postoperative pain. Objective. To address if there is any association between the amount of correction in both the coronal and sagittal planes and the development of postoperative neurological deficit and/or anterior thigh/groin pain. Summary of Background Data. LLIF is a powerful tool for the restoration of spinal alignment including correction of small degenerative curves of the lumbar spine and increase of lumbar lordosis. Concerns remain about its safety regarding injuries of the lumbosacral plexus, which occur with a prevalence ranging from 0.7% to 23%. Methods. The medical records and spinal radiographs of patients undergoing standalone LLIF for symptomatic degenerative scoliosis of the lumbar spine were retrospectively reviewed during a 6-year period. Results. Thirty patients (73 levels) met the inclusion criteria and were followed for a mean of 21 months (range, 9-39 mo). Average age at the time of surgery was 67 years (range, 50-78 yr). Immediately after surgery, a motor deficit was recorded in 6 patients and a sensory deficit in 17 patients. Statistical analysis did not reveal any significant association between the amount of coronal curve correction, restoration of lumbar lordosis or increase in lumbar spine height, and the development of postoperative motor or sensory deficits. Seventeen patients complained of anterior thigh/groin pain immediately postoperative. A statistically significant association was identified between postoperative anterior thigh/groin pain and the magnitude of curve correction (P = 0.005), as well as the increase in lumbar lordosis (P = 0.040). Conclusion. There is a strong association between the development of postoperative anterior thigh/groin pain and the amount of coronal curve correction, as well as the increase in lumbar lordosis. Abstract: Introduction: Thigh pain may sometimes occur following total knee arthroplasty (TKA). The existence of thigh pain can interfere with postoperative rehabilitation and result in an unpleasant postoperative period. Our aim is to identify the risk factors of post-TKA thigh pain. Although the application of a pneumatic tourniquet is common while performing a TKA, it seems to play a role in the development of post-TKA thigh pain. In addition to the use of a tourniquet, some patient factors may also be associated with the occurrence of thigh pain after TKA. Therefore, we hypothesized that some factors correlated to post-TKA thigh pain exist. Materials and methods: During 2011–2012, we conducted a prospective cohort study enrolling 347 patients (with 411 cases of primary TKA) in Taipei Veterans General Hospital. There were 283 cases of unilateral and 64 cases of bilateral knee arthroplasty, respectively. All surgeries were performed by a single surgeon. Thigh pain was measured according to the patients'' response to the “squeeze test” on the next day after surgery, performed by a single technician. Thigh pain after surgery developed in 33 out of 411 cases of primary TKA. We analyzed the correlation between thigh pain and several possible risk factors, including age, gender, body mass index (BMI), tourniquet time, blood pressure, tourniquet pressure, thigh circumference, and use of patient-controlled analgesia (PCA), using a generalized estimating equation, with SPSS software version 19.0 (SPSS Inc., Chicago, IL, USA). Results: The study consisted of 77 male (22%) and 270 female (78%) patients. The mean age of the patients was 72.2 years (range, 29–89 years). The mean tourniquet time was 35.3 minutes and mean cuff pressure was 268.8 mmHg. Higher diastolic blood pressure and higher tourniquet pressure correlated to thigh pain, which achieved statistical significance. Age, gender, BMI, PCA use, and tourniquet time were found to have no significant influences on the occurrence of thigh pain. Discussion: In this study, higher diastolic blood pressure and higher tourniquet pressure were found to have a correlation with thigh pain. Tissue ischemia and reperfusion may explain this finding. Tourniquet time seemed irrelevant, the reason for which might be attributed to the short tourniquet time in our study. In conclusion, factors that influence post-TKA thigh pain exist. Objectives: Inside-out tension-free vaginal tape obturators for the cure of female stress incontinence can cause postoperative thigh pain. The aim of the present study was to analyze and compare the mid-term outcomes of two tension-free vaginal tape obturator procedures. Methods: Patients diagnosed with urinary stress incontinence were enrolled to undergo a tension-free vaginal tape obturator sub-mid urethral tape operation either by de-Leval's method or by Flam's modification. Peri- and postoperative data were collected by uninvolved researchers and analyzed. Follow up was 36 months. Results: Overall, 161 patients were included in the study. Cure and complication rates were similar between the two treatment groups. Postoperative thigh pain was significantly more frequent and lasted longer in the de-Leval group compared with the Flam group (31.9% vs 10.0%, respectively). Urinary urgency was more frequent in the de-Leval patients (20.3% vs 2.8%). Conclusion: Our findings suggest that both tension-free vaginal tape obturator procedures are effective with few adverse effects. The Flam method provides shorter and decreased levels of postoperative thigh pain, as well as reduced long-term postoperative urinary urgency. [ABSTRACT FROM AUTHOR]

Hydroxyapatite (HA) coating has been used as a biological adjuvant for noncemented femoral stem fixation. This is a long-term concise report of a previously published prospective, randomized clinical trial, comparing plasma sprayed porous noncemented stems with and without an HA coating, with focus on wear and survivorship. Between July 1991 and October 1994, 177 patients (199 hips) underwent noncemented Ranawat-Burstein (RB) Metaphyseal-Diaphyseal fit total hip replacement in a prospective randomized clinical trial. At the 5-year follow-up, the incidence of trochanter and/or thigh pain was 12% in the HA group and 24% in the non-HA group. After the mean follow-up of 17.7 ± 0.8 years, there were 3 revisions in the HA group, 1 for fracture and 2 for poly wear. There were 4 revisions in the non-HA group, 1 for infection and 3 for poly wear. There was no revision for femoral loosening in either groups. The Kaplan–Meier survivorship for failure for mechanical reasons in the HA group and non-HA group were 97.9% and 97.1%, respectively, which was not statistically significant. The association between esophageal adenocarcinoma and distant skeletal muscle metastasis is extremely rare. Only three cases have been previously reported in the literature. All reported involvement of the gastroesophageal junction. We describe a 58 year-old Caucasian man who presented with worsening right hip pain for nine months. Computerized axial tomography (CT) scan demonstrated a 10 cm by 8 cm by 12 cm intra-pelvic mass involving the right iliacus muscle with central destruction of the right mid-ileum. CT-guided biopsy of the right hip mass demonstrated poorly differentiated carcinoma. CT scan of the chest revealed a 5 cm by 4 cm circumferential mass involving the distal esophagus. Endoscopic biopsy showed poorly differentiated adenocarcinoma. Immunohistochemical analysis of each specimen correlated for pancytokeratin. Final diagnosis was primary esophageal adenocarcinoma with distant metastasis to the right ileum and iliacus muscle. We review distinctions between esophageal adenocarcinoma and adenocarcinoma of the gastroesophageal junction. A brief discussion of diagnostic modalities and treatment options are provided. Background: Femoral strut allografts are used in revision hip arthroplasty for management of bone loss associated with implant failure or periprosthetic fractures. They have also been used to treat unremitting thigh pain in well-fixed cementless femoral stems, to address the differential in structural stiffness between the stem and femoral shaft. Our study used an in vitro biomechanical model to measure the effect of placement of allografts on femoral strains, to determine their load-sharing capacity., Material and Methods: Three rosette strain gauges were applied to the femoral surface of each of 6 cadaveric femurs, at the stem tip level on anterior, medial, and lateral cortices. After stem implantation, cortical strut allografts were applied to the lateral femoral shaft and secured with 4 Dall-Miles cables. A fourth gauge was placed on the midpoint of the allograft. Strains were recorded in the intact femur, then the implanted femur with and without the allograft under simulated physiologic loading in a load frame., Results: Reduction in distal femoral principal strains, between 12% and 59%, was seen in all cortices following placement of the allograft. Under axial loading, 30% of the strain in the lateral cortex was borne by the allograft. Greater reductions in strain, by as much as 59%, occurred under axial load and torque., Conclusion: The results of this biomechanical model indicate that by placement of an allograft, cortical strains can be reduced to levels approaching those in an intact femur, supporting this technique for treatment of unremitting thigh pain in well-fixed prostheses., (© 2022 The Authors.)

Cement extrusions on the femoral side after total hip replacement can occur in approximately 0.3% of cemented primary total hip replacements. Not recognised until a postoperative x-ray is performed, the willingness to dismiss and treat these extrusions conservatively is high.~Introduction~Background~We report on 3 patients presenting with sudden onset of thigh pain associated with an inability to weight-bear after a 2 to 15 month period of uneventful healthy recovery from cemented total hip replacement. On immediate postoperative x-rays occult cement extrusion in the posterolateral circumference of the femoral component tip were present. X-rays and CT scans showed no fracture signs. Scintigraphy revealed late increased uptake at the extrusion height. With the hypothesis of imminent femoral fatigue fracture, all patients underwent revision surgery. The defect sites were surgically exposed, thoroughly cleaned of cement, filled with iliac crest bone graft and stabilised with tension band plating.~Methods~Methods~This procedure resulted in fully recovered asymptomatic patients at 6 weeks and after a mean follow-up period of 48 months, as demonstrated by their pain level and tolerance of full weight bearing.~Results~Results~These cases lead us to adopt a low threshold for immediate revision when occult cement extrusion is recognised near the tip of a cemented stem on postoperative films, and to adopt a low threshold for surgical revision when, in the presence of cement extrusion, thigh pain is a complaint. Background: The purpose of this study was to determine whether short tapered stems reduce the rate of thigh pain through a systematic review and meta-analysis of comparative studies between short tapered stems and standard-length tapered stems., Methods: We conducted a meta-analysis of comparative studies: 1) retrospective studies and 2) randomized controlled trials (RCTs), on 2 stem designs: short tapered stem versus standard-length tapered stem. Studies were selected by means of the following criteria: 1) study design: retrospective comparative studies, prospective comparative studies, RCTs; 2) study population: patients with total hip arthroplasty or hemiarthroplasty for hip disease or hip fracture; 3) intervention: short tapered stem and standard tapered stem; and 4) outcomes; thigh pain, other clinical results., Results: Among the 250 articles that were identified at the initial search, 6 studies, 4 RCTs and 2 retrospective comparative studies, were included in this meta-analysis. In the analysis of retrospective studies, the short tapered stem reduced the risk of thigh pain compared to the standard tapered stem (risk ratio [RR] = 0.13; 95% confidence interval [CI], 0.02-0.09; Z = -2.07; P = 0.039). However, in the analysis of RCTs, the incidence of thigh pain was similar between the two stem designs (RR = 1.21; 95% CI, 0.76-1.93; Z = 0.82; P = 0.410). We present the case of a ganglion cyst with 11 x 3 x 2 cm dimensions originating from the quadriceps femoris tendon. The patient presented with palpable mass and chronic pain in the anterolateral thigh region. The pain increased with walking. Ultrasonography and computed tomography revealed a cystic, well defined lesion in the vastus lateralis muscle. The patient was operated on and the cyst excised; it was seen to be a ganglion cyst. The patient is asymptomatic 6 months after surgery. This is a very rare localization for a giant synovial cyst, and preoperative diagnosis is a challenge for the surgeon. [ABSTRACT FROM AUTHOR]

Aims Enigmatic thigh pain in uncemented femoral components of a total hip replacement can be severe and disabling. Treatment can be conservative or surgical with cortical strut graft or revision of the femoral stem. Cortical strut grafting may offer good results with reduced morbidity. The aim of this study was to report the functional and radiographic outcomes of four patients with enigmatic thigh pain treated with cortical strut allograft. Materials and Methods Between 2016 and 2018, four women underwent cortical strut allografting at two centres. All patients had an uncemented, proximally porous S-ROM femoral implant (DePuy, Warsaw, In, USA). All other causes of anterolateral thigh pain were excluded. The mean age was 36.7 years (range: 29-51 years). Patients were followed up for a minimum of 14 months (range: 14-38 months). The University of California, Los Angles (UCLA) activity score, pain scores, complications, and radiographs at six weeks, three months, six months, nine months and one year were recorded. Results Mean UCLA activity scores increased from 3.2 (range: 2-4) to 6.2 (range: 6-7) post-operatively. Radiologically, all four patients had complete osseointegration of their strut grafts. Pain scores decreased at six weeks and at six months. One deep venous thrombosis occurred. One patient experienced recurrence of anterolateral thigh pain 26 months post-strut graft, which resolved with protected weight-bearing and analgesia for three months. Conclusions In uncemented femoral prostheses, cortical strut grafting to treat enigmatic thigh pain can reduce symptoms and increase activity without the need to revise a well-fixed femoral stem. Purpose: The mechanisms underlying thigh pain in patients with well-fixed cementless femoral components after total hip arthroplasty (THA) remains unclear. We hypothesized that the thigh pain is correlated with the initial contact state of the stem and aimed to investigate the relation between thigh pain and the initial contact state., Materials and Methods: A total of 209 hips of 184 patients were analysed in this retrospective case-control study. The patients were divided into a thigh pain group ( n = 13 hips) and a control group (without thigh pain, n = 196). Post-operative stem contact images were three-dimensionally visualized by a density mapping function using computed tomography data, which quantified the stem contact area according to Gruen zones. Thigh pain was defined as anterior or anterolateral pain upon loading at 3-month post-operatively., Results: Thirteen hips (6.2%) had thigh pain; however, all the hips demonstrated stable bony ingrowth radiographically. The thigh pain group had a significantly lower contact area in zone 2 ( p = 0.014). The multivariate logistic regression analysis showed that the contact area of zone 2 was negatively correlated with thigh pain [odds ratio (OR): 0.858, p = 0.018], and the canal flare index was negatively correlated with the development of thigh pain (OR: 0.336, p = 0.026)., Conclusions: We identified an association between the initial contact state and post-operative thigh pain. Our data demonstrated that proper lateral contact prevents the occurrence of thigh pain in THA using a tapered wedge stem.

Abstract: Patients do not always have typical anginal chest pain in case of acute coronary syndromes. Many atypical chest pain forms were found associated with acute coronary syndrome, especially in the upper part of the body. Bilateral thigh pain should be kept in mind as a dissimilar projection of anginal chest pain and could be accepted as a predictor of major adverse cardiovascular event in diabetic patients with coronary artery disease. Objective The purpose of this systematic review was to determine the effectiveness of exercise for the management of soft tissue injuries of the hip, thigh, and knee. Methods We conducted a systematic review and searched MEDLINE, EMBASE, PsycINFO, the Cochrane Central Register of Controlled Trials, and CINAHL Plus with Full Text from January 1, 1990, to April 8, 2015, for randomized controlled trials (RCTs), cohort studies, and case-control studies evaluating the effect of exercise on pain intensity, self-rated recovery, functional recovery, health-related quality of life, psychological outcomes, and adverse events. Random pairs of independent reviewers screened titles and abstracts and assessed risk of bias using the Scottish Intercollegiate Guidelines Network criteria. Best evidence synthesis methodology was used. Results We screened 9494 citations. Eight RCTs were critically appraised, and 3 had low risk of bias and were included in our synthesis. One RCT found statistically significant improvements in pain and function favoring clinic-based progressive combined exercises over a “wait and see” approach for patellofemoral pain syndrome. A second RCT suggests that supervised closed kinetic chain exercises may lead to greater symptom improvement than open chain exercises for patellofemoral pain syndrome. One RCT suggests that clinic-based group exercises may be more effective than multimodal physiotherapy in male athletes with persistent groin pain. Conclusion We found limited high-quality evidence to support the use of exercise for the management of soft tissue injuries of the lower extremity. The evidence suggests that clinic-based exercise programs may benefit patients with patellofemoral pain syndrome and persistent groin pain. Further high-quality research is needed. A 9-year-old boy with thigh pain, high fever, hyperleukocytosis and positive blood cultures for Staphylococcus aureus had a left obturator internus phlegmon. Lack of evidence for hip infection or osteomyelitis in a child with thigh pain, high fever and hyperleukocytosis points to a possible infection of pelvic structures. Abscess or phlegmon of the obturator internus muscle is a very rare condition. The most frequent agent is Staphylococcus aureus. CT of the pelvis is the procedure of choice to show diffuse swelling of the muscle or collection of pus. Conservative treatment with adequate antibiotics is the treatment of choice. Aim: Thigh pain following tourniquet application is a common complaint in early post operative period following total knee arthroplasty., Method: Post operative Thigh pain was evaluated in 30 consecutive simultaneous bilateral total knee arthroplasty patients between July 2013 and January 2014. Patient thigh pain was evaluated with the VAS score. The scale was applied on first, second, third day & second and six weeks after surgery., Result: There were statistically significant difference in VAS score in non-tourniquet group on first, second, third post operative day. We did not find statistically significant difference at Second and Six weeks post operatively., Conclusion: This Randomized trial demonstrates that non-tourniquet use in TKA has less early postoperative pain and leads to better recovery.

Abstract: The association between myocardial infarction and lower limb symptoms is rare and poorly recognised. To our knowledge, we present the third reported case in the English literature of a patient presenting with isolated lower limb pain as the initial symptom of a myocardial infarction. We describe the clinical case and discuss the potential pathophysiology behind this atypical presentation. [Copyright &y& Elsevier]
