The present study was designed to evaluate the antihyperlipidemic and antioxidant activities of leaves of Houttuynia cordata Thunb. The ethanolic extract was obtained by percolation method and acute oral toxicity tests were performed according to OECD (Organization for Economic Cooperation and Development) guidelines. Hyperlipidaemia was induced by feeding the rats with a high-fat diet consisting of coconut oil and vanaspati ghee, in a ratio of 2:3 v/v at a dose of 10 ml/Kg body weight. The extract was given at a dose of 500mg/kg body weight. Lipid profile, Malondialdehyde (MDA), Catalase (CAT), and Superoxide Dismutase (SOD) were measured using standard methods. The extract showed a significant decrease in total cholesterol, triglycerides, LDL, and MDA in the blood. On the other hand, HDL, CAT, and SOD increased significantly. The study demonstrates that the ethanolic extract of leaves of Houttuynia cordata Thunb., decreases blood lipid levels and lipid peroxidation. Land use is the physical form of the earth's surface that is influenced by human activities. The increase in population results in changes in land use for settlements that tend to increase and are not controlled, so it is necessary to monitor the rate of these changes so that the balance of life and the environment can be maintained and in accordance with what has been regulated by the government in the regional spatial plan (RTRW). This study aims to identify changes in settlement area from 2010-2020 in Solok City and assess the suitability of land use for settlements with residential spatial patterns (RTRW) in 2012-2031 Solok City. This study uses remote sensing data in the form of spot images. The steps taken by the image interpretation approach with manual digitization methods and arcgis applications, accuracy sampling is done by random sampling with the confusion matrix accuracy test technique carried out to answer the goal. The results of the study found that the image accuracy was 91.66%. Then, the condition of settlements in Solok City continues to increase in area every year in the 2010-2020 period with a total of 353.8 Ha, where the largest area of settlement changes is in Lubuk Sikarah District. In this study, it can be stated that the area of settlement in 2020 is 306.50 Ha which is not in accordance with the area contained in the spatial pattern (RTRW) of Solok City

To the Editor, This is the case of a 67-year-old woman admitted due to a 6-month history of exertional angina. Stress echocardiography showed severe anterior wall ischemia. The patient was referred for coronary angiography that revealed the presence of a chronic total coronary occlusion of the proximal left anterior descending coronary artery (LAD) immediately before the bifurcation with the first diagonal branch (D1) from the septal collaterals of the right coronary artery (figure 1, video 1 of the supplementary data). The patient signed the written informed consent, and a chronic total coronary occlusion recanalization was performed 6 months after the index angiography. Figure 1. Se derivó a la paciente para practicarle una coronariografía, que reveló la presencia de una oclusión coronaria total crónica en el segmento proximal de la descendente anterior (DA) inmediatamente antes de la bifurcación con la prime-ra rama diagonal (D1) perfundida por colaterales septales desde la coronaria derecha (figura 1A, vídeo 1 del material adicional). Abstract Background Guillain–Barré syndrome (GBS) is an acute inflammatory neuropathy with a heterogeneous presentation. Although some evidences support the role of autoantibodies in its pathogenesis, the target antigens remain unknown in a substantial proportion of GBS patients. The objective of this study is to screen for autoantibodies targeting peripheral nerve components in Guillain–Barré syndrome. Methods Autoantibody screening was performed in serum samples from all GBS patients included in the International GBS Outcome study by 11 different Spanish centres. The screening included testing for anti-ganglioside antibodies, anti-nodo/paranodal antibodies, immunocytochemistry on neuroblastoma-derived human motor neurons and murine dorsal root ganglia (DRG) neurons, and immunohistochemistry on monkey peripheral nerve sections. We analysed the staining patterns of patients and controls. The prognostic value of anti-ganglioside antibodies was also analysed. Results None of the GBS patients (n = 100) reacted against the nodo/paranodal proteins tested, and 61 (61%) were positive for, at least, one anti-ganglioside antibody. GBS sera reacted strongly against DRG neurons more frequently than controls both with IgG (6% vs 0%; p = 0.03) and IgM (11% vs 2.2%; p = 0.02) immunodetection. No differences were observed in the proportion of patients reacting against neuroblastoma-derived human motor neurons. Reactivity against monkey nerve tissue was frequently detected both in patients and controls, but specific patterns were only detected in GBS patients: IgG from 13 (13%) patients reacted strongly against Schwann cells. Finally, we confirmed that IgG anti-GM1 antibodies are associated with poorer outcomes independently of other known prognostic factors. Conclusion Our study confirms that (1) GBS patients display a heterogeneous repertoire of autoantibodies targeting nerve cells and structures; (2) gangliosides are the most frequent antigens in GBS patients and have a prognostic value; (3) further antigen-discovery experiments may elucidate other potential antigens in GBS.

A inclusão educacional do público alvo da Educação Especial configura-se como objeto de discussão entre professores, gestores, especialistas, pesquisadores e familiares desse público. Esse estudo objetiva tratar de algumas dessas demandas, visando diagnosticar como professores da Educação Básica, dentro de diferentes áreas do conhecimento, se posicionam frente a alguns dos desafios da inclusão educacional, relacionados a identificação do aluno com necessidades educacionais especiais; ao processo de Avaliação Psicoeducacional no Contexto Escolar e as adaptações curriculares de pequeno porte. Busca-se compreender, utilizando-se, para análise e discussão dos resultados, a Análise de Conteúdo de Bardin (2011), como a Avaliação Psicoeducacional no Contexto Escolar promove/sustenta as adaptações curriculares de pequeno porte e qual sua repercussão na prática pedagógica dos professores da Educação Básica. Os resultados encontrados remetem a práticas ainda distantes de uma efetiva inclusão educacional, onde há o distanciamento entre o Atendimento Educacional Especializado (AEE) e o trabalho do professor do ensino comum, que vê no professor especialista o responsável pela Avaliação Psicoeducacional no Contexto Escolar, não se colocando dentro do processo. Revelam, a importância da formação e capacitação do professor, voltados a inclusão educacional que exige um trabalho compartilhado, onde a finalidade deve ser a educação, em toda sua dinâmica, acreditando que a escola tem como responsabilidade, desenvolver as capacidades acadêmicas, cognitivas, afetivo-emocionais e sociais de todos os alunos com deficiência ou não.

Resumen: Introducción: Los pacientes con enfermedad renal crónica (ERC) tiene alto riesgo de morbimortalidad cardiovascular. Las alteraciones estructurales cardiacas subclínicas tienen valor pronóstico en estos pacientes. El objetivo fue estudiar la calcificación valvular, su evolución y relación con diferentes factores de riesgo. Material y métodos: Se seleccionó aleatoriamente parte de la muestra del estudio NEFRONA analizando la calcificación valvular aórtica y mitral en ecocardiogramas de la visita basal y a los 24 meses. Resultados: Se estudiaron 397 pacientes con filtrado glomerular estimado (FGE) basal de 33 ml/min/1.73 m2 con disminución significativa hasta 30.9 ml/min/1.73 m2 Se produjo aumento del área de placa carotidea y femoral, así como aumento de los pacientes con calcificación valvular a los 24 meses. Se observó asociación positiva de la calcificación mitral a 24 meses con la edad, el índice tobillo brazo (ITB) y el producto calcio fósforo (CaxP) basal, sin asociación con el FGE. La calcificación aórtica a los 24 meses presentó asociación positiva con el área total de placa carotidea, el fósforo y la edad basal, sin relación con el FGE. Conclusiones: Se objetivó en pacientes con ERC, sin enfermedad cardiovascular conocida progresión de la calcificación valvular a dos años independientemente del FGE. Presentaron mayor calcificación valvular mitral aquellos de mayor edad, mayor ITB y producto CaxP. Presentaron mayor calcificación valvular aórtica aquellos de mayor edad, mayores niveles de fósforo y mayor área total de placa carotidea. La identificación de estos pacientes con mayor riesgo, podría ayudar a evitar eventos cardiovasculares futuros intensificando el seguimiento. Abstract: Introduction: Patients with chronic kidney disease (CKD) are at high risk of cardiovascular morbidity and mortality. Subclinical cardiac structural alterations have prognostic value in these patients. The aim was to analyse the prevalence of valvular calcification, the evolution and the relationship with different risk factors. Material and methods: Part of the sample of the NEFRONA study was randomly selected. Aortic and mitral valve calcification were analysed in echocardiograms performed at the baseline visit and at 24 months. Results: We included 397 patients, the estimated basal glomerular filtrate (eGFR) was 33 ml/min with significant decrease to 30.9 ml/min. There was an increase in the area of carotid and femoral plaque, as well as an increase in patients with aortic and mitral calcification at 24 months. A positive association of mitral calcification at 24 months with age, ankle-brachial index (ABI) and calcium-phosphorus product (CaxP) at baseline visit was observed, without association with eGFR. Aortic calcification at 24 months was positively associated with age, phosphorous and total carotid plaque area at baseline, with no relationship to eGFR. Conclusions: A significant prevalence of valvular calcification was observed in patients with CKD without known cardiovascular disease.Two-year progression was observed independently of the eGFR. Patients with higher risk of mitral valve calcification were those with older age, higher ABI and CaxP product. Patients with a higher risk of aortic calcification were those with older age, higher phosphorous levels and larger area of carotid plaque. Identifying these higher risk patients would help to avoid future cardiovascular events intensifying follow-ups.

Aortic and mitral valve calcification were analysed in echocardiograms performed at the baseline visit and at 24 months. Results: We included 397 patients, the estimated basal glomerular filtrate (eGFR) was 33 ml/min with significant decrease to 30.9 ml/min. There was an increase in the area of carotid and femoral plaque, as well as an increase in patients with aortic and mitral calcification at 24 months. A positive association of mitral calcification at 24 months with age, ankle-brachial index (ABI) and calcium-phosphorus product (CaxP) at baseline visit was observed, without association with eGFR. Aortic calcification at 24 months was positively associated with age, phosphorous and total carotid plaque area at baseline, with no relationship to eGFR. Conclusions: A significant prevalence of valvular calcification was observed in patients with CKD without known cardiovascular disease.Two-year progression was observed independently of the eGFR. Patients with higher risk of mitral valve calcification were those with older age, higher ABI and CaxP product. Patients with a higher risk of aortic calcification were those with older age, higher phosphorous levels and larger area of carotid plaque. Identifying these higher risk patients would help to avoid future cardiovascular events intensifying follow-ups. Resumen: Introducción: Los pacientes con enfermedad renal crónica (ERC) tiene alto riesgo de morbimortalidad cardiovascular. Las alteraciones estructurales cardiacas subclínicas tienen valor pronóstico en estos pacientes. El objetivo fue estudiar la calcificación valvular, su evolución y relación con diferentes factores de riesgo. Material y métodos: Se seleccionó aleatoriamente parte de la muestra del estudio NEFRONA analizando la calcificación valvular aórtica y mitral en ecocardiogramas de la visita basal y a los 24 meses. Resultados: Se estudiaron 397 pacientes con filtrado glomerular estimado (FGE) basal de 33 ml/min/1.73 m2 con disminución significativa hasta 30.9 ml/min/1.73 m2 Se produjo aumento del área de placa carotidea y femoral, así como aumento de los pacientes con calcificación valvular a los 24 meses. Se observó asociación positiva de la calcificación mitral a 24 meses con la edad, el índice tobillo brazo (ITB) y el producto calcio fósforo (CaxP) basal, sin asociación con el FGE. La calcificación aórtica a los 24 meses presentó asociación positiva con el área total de placa carotidea, el fósforo y la edad basal, sin relación con el FGE. Conclusiones: Se objetivó en pacientes con ERC, sin enfermedad cardiovascular conocida progresión de la calcificación valvular a dos años independientemente del FGE. Presentaron mayor calcificación valvular mitral aquellos de mayor edad, mayor ITB y producto CaxP. Presentaron mayor calcificación valvular aórtica aquellos de mayor edad, mayores niveles de fósforo y mayor área total de placa carotidea. La identificación de estos pacientes con mayor riesgo, podría ayudar a evitar eventos cardiovasculares futuros intensificando el seguimiento.
