The incidence of infections caused by the Mycobacterium avium complex (MAC) has risen significantly, posing diagnostic and therapeutic challenges. This study analyzed 134 clinical isolates of the Mycobacterium avium complex from southern Spain, performing in vitro antimicrobial susceptibility testing using a commercial microdilution technique to generate additional data, refine treatment strategies, and improve patient outcomes. Phenotypic susceptibility testing revealed clarithromycin and amikacin as the most effective antibiotics, with susceptibility rates exceeding 90%, while linezolid and moxifloxacin exhibited limited activity, with resistance rates of 49.3% and 41.8%. A comparative analysis between M. avium and M. intracellulare showed significant differences in resistance to amikacin and linezolid, with M. avium exhibiting higher resistance rates. Additionally, species-specific differences were observed in MIC distributions for ethionamide, ciprofloxacin, and streptomycin. Our data reveal regional variability in resistance patterns, particularly for moxifloxacin and linezolid, which exhibit differing resistance rates compared to studies from other regions. The significant MIC differences for several antibiotics between M. avium and M. intracellulare underscore the importance of species-level identification and the heterogeneity in resistance mechanisms within MAC. Klebsiella pneumoniae (K. pneumoniae) is an opportunistic bacterium that has drawn attention due to its resistance to carbapenem antibiotics. The treatment of patients with severe infections has been challenging. Thus, silver nanoparticles (AgNPs) have been applied for their antimicrobial effects. This work aims to analyze the synergistic effect of the carbapenem antibiotic Imipenem with AgNPs against different susceptibility clinical profiles of K. pneumoniae. The silver nanoparticles were synthesized by bottom-up methodology and capped with alpha-lipoic acid. Susceptibility tests were performed using four K. pneumoniae strains with different susceptibility profiles to Imipenem. The strains were induced to form a biofilm for 48 h. Crystal violet and Resazurin assays were performed to determine biofilm formation and minimal inhibitory concentration, respectively. The reduction in Imipenem concentration with the association of nanoparticles was found in all strains studied in planktonic form, and the synergism between silver nanoparticles and Imipenem was demonstrated through the analysis of the fractional inhibitory concentration index. The viability percentage was reduced at rates ≥80% in the biofilm analysis, characterized by the minimal biofilm inhibitory concentration. The study's proposed association resulted in inhibitory effects on different K. pneumoniae profiles, both in planktonic forms and biofilm, with peculiar behavior in the Imipenem-resistant profile. Background: It has been suggested that Mycobacterium avium, Mycobacterium intracellulare, and M. chimaera have differential drug susceptibility patterns. We prospectively analyzed and compared the drug susceptibility patterns among these species over an 8.5-year period. Methods: A microdilution method (Slomyco®) was performed for drug susceptibility testing of 402 M. avium, 273 M. intracellulare, and 139 M. chimaera clinical isolates. Results: M. avium showed significantly higher resistance to moxifloxacin, ciprofloxacin, rifampicin, ethambutol, streptomycin, linezolid, cotrimoxazole, and clarithromycin. M. avium also showed higher minimum inhibitory concentrations (MIC) than M. intracellulare and M. chimaera against all drugs except ethionamide, to which M. intracellulare and M. chimaera showed greater resistance. Conclusions: Our series demonstrated differential drug resistance patterns among the most frequent M. avium complex species. M. avium was more resistant than M. intracellulare and M. chimaera versus eight antibiotics and showed greater MIC values to most of the antibiotics studied. This study was conducted to determine the occurrence and antimicrobial susceptibility of Staphylococcus piscifermentans nasal colonization among apparently healthy ruminants and their handlers in Maiduguri, Borno State, Nigeria. A total of six hundred and five (605) samples were collected aseptically using sterile swab sticks (Oxoid, UK) using a simple random technique from the ruminants and their human handlers at the Maiduguri central abattoir and livestock market (Kasuwan shanu). One hundred and fifty-one (151) samples were collected from each species (cattle, sheep and goat), while 152 samples were collected from animal handlers. The samples were immediately subjected to standard bacteriological analysis. A total of 195 (32.2%) coagulase-negative Staphylococcus species (CoNS) were presumptively identified; out of the 195 identified CoNS, 9 (4.62%) was Staphylococcus piscifermentans nasal colonization. The results revealed that out of 9 isolates, S. piscifermentans was highly isolated 4(8.3%) from sheep, followed by cattle and human handlers 2 (3.8%) respectively, and the least was from goats 1 (2.3%). Meanwhile, all the 9 (4.62%) isolates were negative for both tube and slide coagulase test. However, they were positive for DNase, Ornithine Decarboxylase (ODC) test, betta – hemolysis and synergistic hemolysis. 4(44.4%) of the 9 isolates were positive for Staphylococcus Protein A (SPA) virulence gene, none 0(0%) for mecA virulence gene, and 5(55.6%) were positive for slime (Biofilm) production. The antibiotic susceptibility pattern of these isolates was found to be highly variable to all the used drugs. Therefore, it is concluded that Staphylococcus piscifermentans isolated from these ruminants and their handlers contain virulence genes that could be responsible for pathogenicity. There is a need for more studies using higher technologies in other parts of the country to reveal the thorough status of the disease in Nigeria.

Background: Chronic Suppurative Otitis Media (CSOM) is a major health concern in developing countries due to its association with hearing impairment, particularly among children as it may affect their communication skills. Serious complications like meningitis and brain abscess have been reported as a cause of death. Objective: The study aimed to determine the bacteriology, susceptibility and clinical presentation of chronic suppurative otitis media among ENT patients at Mulago, Uganda. Methodology: We performed a cross sectional study and enrolled 89 patients. Pus was collected from the middle ear for microbial laboratory examination. Our primary outcome was microbial isolates, sensitivity patterns and common clinical features. Results: The commonest isolates identified were Pseudomonas aeruginosa (17.32%), Klebsiella pneumoniae (17.32%), Proteus mirabilis (13.39%), Escherichia coli (9.5%) and Staphylococcus aureus (9.5%). Pseudomonas aeruginosa was found to be 64.7% sensitive to ciprofloxacin, 57.1% to chloramphenicol, and 41.2% to gentamicin. More than 60% of patients had a hearing impairment; 78% had a central perforation. Conclusion: Susceptibility patterns to antimicrobial agent greatly varied but most demonstrated sensitivity to ciprofloxacin followed by chloramphenicol and gentamicin.

Management of superficial aspergillosis is a major challenge owing to the frequent relapses and treatment failure, which may pose a potential risk, thereby gradually developing resistant species. Therefore, necessitating the development of new antifungals with higher potency should be considered as alternative strategies for efficient management of infections. We aimed to investigate the susceptibility of Aspergillus isolates toward a novel triazole, efinaconazole, in comparison with various classes of antifungal drugs. Antifungal susceptibility testing was performed according to the Clinical and Laboratory Standards Institute M38-A2 guidelines. Efinaconazole exhibited poor activity against mutant A. fumigatus strains, A. niger sensu stricto, and A. tubingensis with GM MIC values of 3.62, 1.62, and 2 μg/ml, respectively; however, surprisingly, it efficiently inhibited the growth of A. terreus sensu stricto, followed by wild-type A. fumigatus and A. flavus with GM MIC values of 0.29, 0.42, and 0.52 μg/ml, respectively. Presumably, efinaconazole is inefficient in aspergillosis treatment due to the low susceptibility of A. niger sensu stricto, A. tubingensis, and mutant A. fumigatus; however, it may be effective in treating superficial aspergillosis caused by wild-type A. fumigatus, A. terreus sensu stricto, and A. flavus. Abstract Background Pyogenic liver abscesses (PLA) remain a significant clinical problem. Unfortunately, little is known about current bacterial susceptibility profiles and the incidence of multidrug resistant organisms (MDROs) causing PLA in Western countries. Yet, this crucial information is pivotal to guide empirical antibiotic therapy. Aim of this study was to provide detailed characteristics of PLA with a special focus on underlying bacterial pathogens and their susceptibility to antibiotics. Methods A retrospective study of patients diagnosed with PLA from 2009 to 2015 in a large tertiary reference center in Germany was performed in order to characterize PLA and antimicrobial susceptibility profiles of causative bacterial species. Results Overall, 86 patients were included. The most common causes of PLA were bile duct stenosis/obstruction (31.4%) and leakage of biliary anastomosis (15.1%). Frequent predisposing diseases were malignancies (34.9%), diabetes (24.4%) and the presence of liver cirrhosis (16.3%). Of note, Enterococcus spp. were the most frequently cultured bacterial isolates (28.9%), and in 1/3 of cases vancomycin resistance was observed. In addition, a relevant frequency of gram-negative MDROs was identified. In particular, an alarming 10% and 20% of gram-negative bacteria were resistant to carbapenems and tigecycline, respectively. Of note, MDRO status did not predict ICU stay or survival in multivariate regression analysis. The mortality rate in our series was 16.3%. Conclusion Our study demonstrates an as yet underreported role of Enterococcus spp., often associated with vancomycin resistance, as well as of gram-negative MDROs causing PLA.

Summary: Cystic fibrosis (CF) is associated with increased rates of morbidity and mortality due to fungal and bacterial colonisation of the airways or respiratory infections. The prevalence of fungi in Iranian CF population has been underestimated. Therefore, the current study was conducted to define the frequency of fungi in respiratory specimens obtained from Iranian CF patients based on conventional and molecular assays. Furthermore, in vitro antifungal susceptibility testing was performed on the obtained isolates according to the guidelines from the Clinical and Laboratory Standards Institute. A cohort of 42 CF patients, including 29 males and 13 females, were categorised according to the referenced diagnostic criteria. Candida albicans (n = 24, 80%), C. dubliniensis (n = 2, 6.6%), C. parapsilosis (n = 2, 6.6%), C. tropicalis (n = 1, 3.3%), C. glabrata (n = 1, 3.3%) and Meyerozyma caribbica (n = 1, 3.3%) were isolated from 73.8% of the CF patients. Aspergillus terreus (n = 3, 42.8%) was identified as the most common Aspergillus species, followed by A. fumigatus (n = 2, 28.5%), A. oryzae (n = 1, 14.2%) and A. flavus (n = 1, 14.2%). Bacterial and fungal co‐colonisation was detected in 7 (16.6%) and 22 (52.3%) samples that were positive for Aspergillus and Candida species, respectively. However, Scedosporium species and Exophiala dermatitidis never were detected. In terms of geometric mean (GM) minimum inhibitory concentrations (MICs), posaconazole (0.018 μg/mL) and caspofungin (0.083 μg/mL) exhibited the highest antifungal activities against all Candida species. In addition, posaconazole exhibited the lowest MIC range (0.008‐0.063 μg/mL) against all Aspergillus species, followed by caspofungin (0.016‐0.125 μg/mL) and voriconazole (0.125‐0.25 μg/mL). To conclude, it is essential to adopt a consistent method for the implementation of primary diagnosis and determination of treatment regimen for the CF patients. However, further studies are still needed to better define the epidemiology of fungal organisms in CF patients from the Middle East and the clinical significance of their isolation. The frequency of Aspergillus terreus and related (cryptic) species in clinical specimens, as well as the percentage of azole-resistant strains remains to be studied.Methods: A global set (n = 498) of A. terreus and phenotypically related isolates was molecularly identified (beta-tubulin), tested for antifungal susceptibility against posaconazole, voriconazole, and itraconazole, and resistant phenotypes were correlated with point mutations in the cyp51A gene.Results: The majority of isolates was identified as A. terreus (86.8%), followed by A. citrinoterreus (8.4%), A. hortai (2.6%), A. alabamensis (1.6%), A. neoafricanus (0.2%), and A. floccosus (0.2%). One isolate failed to match a known Aspergillus sp., but was found most closely related to A. alabamensis. According to EUCAST clinical breakpoints azole resistance was detected in 5.4% of all tested isolates, 6.2% of A. terreus sensu stricto (s.s.) were posaconazole-resistant. Posaconazole resistance differed geographically and ranged from 0% in the Czech Republic, Greece, and Turkey to 13.7% in Germany. In contrast, azole resistance among cryptic species was rare 2 out of 66 isolates and was observed only in one A. citrinoterreus and one A. alabamensis isolate. The most affected amino acid position of the Cyp51A gene correlating with the posaconazole resistant phenotype was M217, which was found in the variation M217T and M217V.Conclusions:Aspergillus terreus was most prevalent, followed by A. citrinoterreus. Posaconazole was the most potent drug against A. terreus, but 5.4% of A. terreus sensu stricto showed resistance against this azole. In Austria, Germany, and the United Kingdom posaconazole-resistance in all A. terreus isolates was higher than 10%, resistance against voriconazole was rare and absent for itraconazole.

Abstract Candidiasis is a major challenge among renal transplant recipients (RTRs) worldwide and is associated with high morbidity and mortality rates. Fluconazole is the most commonly used agent for Candida infections. However, frequent relapse and treatment failure are still reported among patients affected with this infection. In the present study, Candida species obtained from RTRs were characterized based on conventional and molecular assays. Furthermore, the antifungal susceptibility profiles of these species were determined. This study was conducted on a total of 126 RTRs within 2012–2016. The patients were categorized according to the referenced diagnostic criteria. The identification of Candida species was accomplished based on conventional examination, assimilation profile test, and polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method. The minimum inhibitory concentrations (MICs) of amphotericin B, fluconazole, itraconazole, voriconazole, posaconazole, and caspofungin were determined based on the guidelines of Clinical and Laboratory Standards Institute. The patients with Candida infection were diagnosed with urinary tract candidiasis (n = 17), peritonitis (n = 8), intra-abdominal candidiasis (n = 6), candidemia (n = 4), hepatosplenic candidiasis (n = 3), and Candida pneumonia (n = 3). A total of 41 Candida isolates, including C. albicans (n = 18), C. famata (n = 8), C. kefyr (n = 4), C. tropicalis (n = 4), C. parapsilosis (n = 3), C. glabrata (n = 2), and C. lusitaniae (n = 2), were isolated from 32.5% (41/126) renal transplant recipients. Fluconazole-resistance was observed in seven isolates, entailing C. albicans (n = 6) and C. tropicalis (n = 1). Fluconazole MIC for C. lusitaniae isolates was above the epidemiologic cut-off value (4–16 μg/ml). Furthermore, MIC range values of fluconazole against C. famata and C. kefyr were obtained as 4–32 μg/ml and 4–8 μg/ml, respectively. Posaconazole exhibited potent activity against Candida isolates, followed by caspofungin. The identification of Candida species, together with susceptibility testing, provides important data about the geographic trends of the fluconazole-resistance profiles of Candida species. INTRODUÇÃO: Nos últimos anos, o oportunismo das candidíases vem se tornando cada vez mais recorrente e de difícil tratamento, principalmente em virtude da emergência de novas espécies, bem como devido à diminuição à suscetibilidade aos antifúngicos. O sucesso do tratamento das infecções causadas por Candida depende cada vez mais do conhecimento da espécie e do seu perfil de sensibilidade. OBJETIVOS: Esse estudo teve por objetivo realizar uma investigação do perfil de suscetibilidade de amostras de candidíase oral e vulvovaginal, de pacientes residentes no oeste de Santa Catarina, aos antifúngicos anfotericina B, fluconazol e miconazol. MATERIAL E MÉTODOS: A suscetibilidade de 60 cepas de Candida spp. foi avaliada frente aos antifúngicos. As determinações das concentrações inibitórias mínimas e concentrações fungicidas mínimas foram realizadas com base no documento M27-A2 do Clinical and Laboratory Standards Institute (CLSI). RESULTADOS: No presente estudo foi observado alto índice de resistência para o fluconazol e para a anfotericina B. O miconazol foi o antifúngico que demonstrou melhor eficácia sobre as cepas testadas. DISCUSSÃO: As amostras dessa região do estado de Santa Catarina demonstram baixa suscetibilidade aos antifúngicos, o que explica a alta taxa de recidivas nas infecções, mas alerta para a emergência de cepas com resistência adquirida. CONCLUSÃO: A prévia avaliação por testes de suscetibilidade in vitro deve nortear a conduta de um tratamento antifúngico efetivo para os casos de candidíase de repetição, principalmente em locais de alta prevalência desta infecção oportunista, como no oeste de Santa Catarina.INTRODUCTION: In recent years, the opportunism of yeast infections is becoming more recurrent and difficult to treat, mainly due to the emergence of new species as well as due to decreased susceptibility to antifungal agents. The successful treatment of infections caused by Candida is increasingly dependent on knowledge of the causative species and its susceptibility profile to antifungals. OBJECTIVES: This study aimed to conduct an investigation of the susceptibility profile of samples of oral and vulvovaginal candidiasis in patients of western Santa Catarina to the antifungal amphotericin B, fluconazole and miconazole. MATERIAL AND METHODS: The susceptibility of 60 strains of Candida spp. was evaluated for antifungal agents. The determination of minimum inhibitory concentrations and minimal fungicidal concentrations were based on document M27-A2 of the CLSI. RESULTS: In this study, we observed a high rate of resistance to fluconazole and amphotericin B. The miconazole was the antifungal agent significantly more effective on the strains tested. DISCUSSION: The samples in this region of the state of Santa Catarina showed a low susceptibility to antifungal agents, which explains the high rate of relapses in infections, but alert to the emergence of strains with acquired resistance. CONCLUSION: The preliminary assessment by in vitro susceptibility testing should guide the conduct of effective therapy for cases of recurrent mainly in areas of high prevalence of opportunistic infection, as in western of Santa Catarina.

Sub-clinical mastitis is a main pathology of dairy husbandry because it is not clinically recognized by the owners and the veterinarians. For this reason, its economic loss is usually underestimated in milk production. This study has been undertaken in order to evaluate the epidemiologic situation of sub-clinicalmastitis in Batna and Setif governorates (East of Algeria). For this purpose, a detailed bacteriological study of all bacterial strains isolated from sub-clinical mastitis followed by a study of their antibacterial susceptibility profiles has been undertaken. 89 bacterial strains distributed as follows were studied: 27 strains of staphylococci among which 23 were coagulase-negative staphylococci (CNS) that are generally incriminated in sub-clinical mastitis. 39 strains of streptococci among which 10 were Lactococcus lactis ssp lactis strains. 23 strains of enterobacteria represented mainly by Escherichia coli (E.coli). All these bacterial strains were isolated from cow milk of 3 different farms. The antibacterial susceptibility profiles have revealed a susceptibility of the isolated strains to a large number of antibiotics mainly to the Neomycin, the Cephalexin and the Spiramycin. [Vet. Inadequate protection of water sources, and poor household hygienic and handling practices have exacerbated fecal water contamination in Kenya. This study evaluated the rate and correlates of thermotolerant coliform (TTC) household water contamination in Kericho District, Western Kenya. Culture and multiplex polymerase chain reaction (PCR) techniques were used to characterize TTCs. The disk diffusion method was used for antibiotic susceptibility profiling of pathogenic Escherichia coli. Out of the 103 households surveyed, 48 (46.6%) had TTC contaminated drinking water (TTC levels of >10 cfu/100 mL). Five of these households were contaminated with pathogenic E. coli, including 40% enteroaggregative E. coli, 40% enterotoxigenic E. coli, and 20% enteropathogenic E. coli. All these pathogenic E. coli strains were multidrug resistant to sulfamethoxazole/trimethoprim, ampicillin, tetracycline and ampicillin/sulbactam. Rural household locality, drinking water hand contact, water storage container cleaning practice, hand washing before water withdrawal, water source total coliforms <10 cfu/100 mL, temperature, and free chlorine levels were associated with TTC contamination of household drinking water. Significant proportions of household drinking water in Kericho District are contaminated with TTCs including with pathogenic multidrug-resistant E. coli. Objective: To assess colonization of Streptococcus agalactiae [group B streptococcus (GBS)], and delineate capsular serotype distribution and antibiotic susceptibility profiles among pregnant women in Trinidad and Tobago., Methods: Vaginal swabs were collected from 248 pregnant women attending antenatal clinics in northern Trinidad, and processed using standard microbiological laboratory tests to confirm GBS. Polymerase chain reaction detected atr and cps serotype genes. Antimicrobial susceptibility tests were performed using the Kirby-Bauer method, and SPSS Version 25 was used for statistical analysis. Prevalence ratio measured the risk, and P ≤0.05 was considered to indicate significance., Results: The GBS carriage rate was 29% (72/248, 95% confidence interval 23.3-34.8), and carriage was significantly associated with variables including gestational diabetes ( P =0.042), age 25-35 years ( P =0.006), multiparity ( P =0.035) and marital status ( P =0.006). The most common serotype was type II [47.2% (34/72)], and serotypes V, VI, VII and VIII were not encountered. GBS showed high resistance to amoxicillin-clavulanic acid (37.5%), erythromycin (30.6%), trimethoprim-sulphamethoxazole (58.3%) and tetracycline (97.2%)., Conclusion: GBS colonization among pregnant women and resistance to commonly used antibiotics are high in Trinidad and Tobago. Candida africana has been recovered principally as a causative agent of vulvovaginal candidiasis (VVC) from different countries, which is likely to be misidentified as the typical Candida albicans or Candida dubliniensis. The current study aimed to characterize C. albicans species complex obtained from VVC based on conventional and molecular assays. Furthermore, in vitro antifungal susceptibility testing was performed based on CLSI documents. Additionally, due to low knowledge concerning C. africana infections, we reviewed all published papers from 1991 to 2019. One hundred forty-four out of 287 patients were identified with Candida infection, among whom 151 isolates of Candida were obtained. Candida albicans 109 (72.1%), Candida glabrata 21 (13.9%), Candida krusei 8 (5.2%), Candida tropicalis 5 (3.3%), Candida africana 3 (1.9%), Candida parapsilosis 3 (1.9%) and C. dubliniensis 2 (1.3%) were isolated from patients. MIC results showed that C. africana isolates were susceptible to all tested antifungal drugs. Candida africana infections were more prevalent in Africa. One hundred fifteen (40.6%) of patients with C. africana candidiasis were from seven African countries, and Madagascar and Angola had the majority of cases. The epidemiological data, phenotypic, clinical features, ecologic similarity, and antifungal susceptibility profiles for better understanding of the pathogenic mechanisms and optimal treatment underlying non- Candida albicans Candida vulvovaginitis are highly recommended.
