Free microvascular muscle flaps represent well-established reconstructive options for complex soft tissue defects. However, due to their lack of cutaneous capillary beds, they are difficult to monitor postoperatively. To this end, random and axial-pattern adipocutaneous skin paddles are often included. The objective of the study was to compare the impact of random-pattern versus perforator-based adipocutaneous skin paddles on operative efficacy and muscle flap safety. Between August 2014 and July 2016, a total of 120 free muscle flaps were included in this retrospective monocentric cohort study. Based on their skin-paddle type, they were either grouped into a 'perforator-based' (group Pb) or 'random-pattern' (group Rp) cohort. The electronic medical records and operative reports of all patients were subsequently reviewed and patient, defect, and flap characteristics of both groups were compared. The effect of the competing skin paddle types on the overall operative time, incidences of flap loss or microvascular complications, and total length of hospital stay were then assessed. Group Pb comprised 72 flaps, whereas 48 flaps constituted group Rp. Patient, defect, and flap characteristics were similar between both groups. Groups Pb and Rp were comparable regarding patient age (group Pb: 61 (10–90) vs. Rp: 59 (13–81), p = 0.556), ASA (American Society of Anesthesiologists) class (group Pb: 3 (1–4) vs. Rp 3 (1–3), p = 0.977), and comorbidities, summarized by the Charlson comorbidity index (CCI; group Pb: 1 (0–4 vs. Rp: 1 (0–5), p = 0.295). Both types of monitoring skin paddles were equally reliable. There was no significant difference in the mean operation time between both groups (group Pb: 373 ± 122 min vs. Rp: 342 ± 84 min, p = 0.124). In-patient treatment after flap surgery and total length of hospital stay were significantly shorter in group Pb (group Pb: 24 ± 10 days vs. Rp: 32 ± 17 days, p = 0.002 and group Pb: 39 ± 15 vs. Rp: 48 ± 24, p = 0.022). Perforator-based skin paddles are a reliable tool for postoperative perfusion monitoring of free muscle flaps and help avoid additional surgical interventions as opposed to their random-pattern counterparts. Résumé La position de la palette cutanée sur l’aire mammaire est un élément fondamental pour les reconstructions par grand dorsal avec prothèse. Faut-il comme Millard, recréer le défaut initial et l’inclure dans la cicatrice de mastectomie ou est-il préférable de contre-inciser à distance comme le présentent les autres auteurs. Cette étude compare les résultats morphologiques à long terme de ces deux attitudes, avec ou sans contre-incision. Patients et méthodes Quatre-vingt-deux reconstructions mammaires par grand dorsal avec prothèse sont revues à distance (recul moyen : 9,5 ans) et cotées de 1 à 5 par un panel expert et non-expert. Les patientes ont noté aussi leur propre reconstruction. Différents paramètres du sein reconstruit ont été ainsi évalués. On comparait les résultats, selon le positionnement de la palette cutanée : avec contre-incision (50 cas) ; sans contre-incision (32 cas). Les caractéristiques de la cicatrice de mastectomie sur l’aire mammaire, haute ou basse, horizontale ou oblique, définissaient 6 groupes où étaient comparés les résultats des deux options chirurgicales. Résultats Seules les reconstructions avec contre-incision obtiennent des résultats significativement supérieurs à ceux sans contre-incision ( p < 0,05). Ils concernent les deux panels, pour les cicatrices de mastectomies hautes. En revanche, pour les cicatrices basses horizontales ou obliques, la contre-incision ne s’impose pas. Conclusions Si la réalisation d’une incision supplémentaire peut être perçue comme inadaptée sur une région déjà « mutilée », nous plaidons pour cette solution dans des cas sélectionnés. Ceci apporte un bénéfice objectivé et stable dans le temps, en termes de résultat global, de forme et même de rendu cicatriciel. Should, as Millard advocated, to recreate the initial defect and include it in the mastectomy scar or is it better in an additional incision as have others authors. This study compares the long-term morphological results of these two attitudes, with or without additional incision. The characteristics of the mastectomy scar on the breast area, high or low, horizontal or oblique, defined 6 groups where the results of the 2 surgical options were compared. Results Only the reconstructions with additional incision get significantly higher results than those without additional incision ( P < 0.05). This involves the two panels, in the case of high mastectomy scars. In the others cases the additional incision is not essential. Conclusions If the realization of an additional incision can be perceived like misfit on an area already “mutilated”, we plead for this solution in selected cases. Background Perforator flaps remain challenging in their design, especially as free flaps.We used a cadaveric model to help refine the design of perforator flaps by studying their vascular features. We define the angle of perfusion of a perforator as a tool to achieve safer flap designs. Methods A total of 83 flaps were designed from20 fresh cadaveric anterolateral thigh flaps. The most dominant perforator larger than 0.5 mm was used as the reference point on the midline of the flap, and the tip of the flap was set at 5 cm (n = 10), 2 cm (n = 5), or 10 cm(n = 5) from this perforator. The perforator was injected with contrast agent, and the flap was scanned with computed tomography (CT) angiography. The vascular territory of the injected perforator was drawn twice by two different investigators. Perfused volumes were then obtained through a computerized algorithm on the CT workstation. Flaps were then flushed with heparinized saline and cut at decreasing angles (120, 90, 60, and 45 degrees) and rescanned with contrast for each perfusion angle. The perfused volumes were calculated for each angle. Results Volume and percentage of perfusion were significantly decreased with decreasing angles of perfusion, regardless of perforator location (2 cm, p = 0.002; 5 cm, p = 0.02; 10 cm, p < 0.001). Conclusions Acute angles of perfusion were associated with fewer incorporated linking vessels and lower flap perfusion. This phenomenon was less apparent in centrally located perforators. The objective of this study was to classify the skin paddles based on the dominance of vascular contribution by these axial vessels through their different perforator systems. Materials and Methods: A retrospective analysis of 5-year data of 386 free fibula flaps used in oro-mandibular reconstruction was done and the skin paddle vascularity was studied. While majority of the skin paddles received their blood supply from the peroneal septocutaneous perforators, a few had their dominant supply from the soleus musculocutaneous perforators in addition to peroneal septocutaneous perforators. In few cases, the soleus musculocutaneous perforators were the sole source of blood supply to the skin paddle. The limitation in this study was the inability to augment the clinical observation with cadaveric study. Results: The skin paddle of the free fibula flap was classified into four different types (a-d) based on the dominance of vascular contribution by axial vessels of the leg. Conclusion: The skin paddle of the free fibula flap has reliable blood supply, but a thorough knowledge of the variations in vascular pattern of the skin paddle is required especially to salvage the larger paddles used in the reconstruction complex oro-mandibular defects.

Summary: Background: Ischaemic flap complications can be a problem following harvest of the latissimus dorsi (LD) musculocutaneous flap or thoracodorsal artery perforator (TAP) flap. We investigate the reliable locations of the perforators of the thoracodorsal artery. Methods: Twenty latissimus dorsi flaps harvested from cadavers were used in the study. In fifteen flaps the thoracodorsal artery was injected with coloured latex, and the locations of perforators were recorded. In five flaps perfusion of the skin paddle was evaluated using 3D CT angiography following injection of the thoracodorsal artery with a barium sulphate/gelatin mixture. Results: At least one perforator originating from the descending branch of the thoracodorsal artery was found in all specimens, whereas no perforators from the transverse branch were found in 33% of flaps. At least one perforator originating from the descending branch in all flaps was found between 9.5 and 15.4cm from the posterior axillary fold, within 4.3cm of the lateral border of the latissimus muscle. 58% of all perforators from the descending branch, and 39% of all perforators from the thoracodorsal artery were found in this region. CT scanning of the hemiback flaps demonstrated contrast in the superior two-thirds. Perfusion of the skin paddles in three locations was demonstrated- superior transverse (bra strap), vertical and lower transverse. Conclusions: Thoracodorsal artery perforators could be found within a reliable region. Positioning of the TAP flap skin paddle over this region will maximise the chances of including a perforator and may reduce the risk of ischaemia. For a musculocutaneous LD flap, the skin paddle can be in any of the 3 popular locations, i.e. upper transverse, vertical and lower transverse, as they were shown to be well vascularised. In reconstructive techniques, the use of the composite gracilis skin flap may be complicated by a total or partial necrosis of the cutaneous paddle. Our study was aimed to describe the vascular anatomy of the gracilis muscle and the related skin, in order to have a better understanding of the skin’s blood supply, and to help in determining a safer cutaneous paddle during transfer. We performed a detailed anatomical description of the gracilis vessels on 32 human cadavers’ thighs. Selective injections of methylene blue and then colored fluid latex were carried out in the principal pedicle of the gracilis, to color and to measure the perforator vessels and the associated cutaneous paddle. The contour and the anatomical situation of the paddles were digitized in order to have precise data and to try to model a standard theoretical skin paddle. The 32 dissections showed that the principal pedicle of the gracilis originated mainly from the adductor artery (26/32), it penetrated into the muscle 92.8 mm (78–114) below the pubis with a small dispersion of the values that assessed a rather reliable parameter of location. Each muscle had 1–6 perforators (at mean 3.78); 80% of these perforators had their exits from the opposite side to the principal pedicle, included within a 60 mm length of muscle. A skin paddle was quite constantly colored (31/32), located over the proximal half of the muscle with a complex design. Its mean dimensions of length and width were 110 (72–158) and 91.4 mm (65–144), respectively. The computerized surface of the paddle was at a mean of 79.5 cm2 (42.4–139.3), and it was significantly correlated to the number of the perforators from the muscle. Our quantitative values concerning the skin paddle related to the perforator arteries may be of significant interest to the surgeon who aims to harvest a composite musculocutaneous gracilis flap, in order to improve its reliability by reducing the risk of cutaneous necrosis. Abstract: Subject: Several authors used musculocutaneous free flaps of serratus anterior. However, the localization and dimensions of its skin paddle are not clearly defined in the literature. Thus, we realized an anatomical study trying to prove the reality of this skin paddle and its localisation, before considering clinical applications. Patient and methods: Twenty-six dissections on 13 fresh anatomical subjects were realized. The protocol consisted in catheterising the thoracic branch of the thoracodorsal artery to inject a mixture of iodized product and methylene blue. The results were obtained by direct measurement of the blue cutaneous area and by radiographic evaluation of the radiopaque area, compared to reproducible anatomical reference marks. Results: Twenty-five dissections were interpretable with superposable results. The skin paddle was always located compared to the 5th, 6th and 7th ribs between the anterior axillary line and the mamelonnary line. The area of the skin paddle was 11,75 cm of length (10–14), 10,75 cm of width (9–12) and 125 cm2 from surface (110–140). Then, we realized between January and December 2003 several clinical applications of this anatomical study. We report 12 cases of free flaps of serratus anterior with skin paddle for the monitoring of the flap (4 musculocutaneous, 5 osteomusculocutaneous, 3 of fascia with skin paddle). Conclusion: This study comes to the support from the anatomical observations from Salmon, the peroperational observations of several authors and our own clinical experiment. It certifies the autonomy of cutaneous vascularization by branches resulting from the surface fascia of the muscle. It specifies the localization and dimensions of the skin paddle of the serratus anterior flap in its muscular, osteomuscular or fascial form, which still increases its versatility of use. Free flap reconstruction of the head and neck is a widespread procedure. The aesthetic outcome is frequently compromised by color mismatch between the donor site skin and the complex pigmentation of the face. Various surgical procedures have been described to improve the appearance of external skin paddles. Medical tattooing is commonly used for nipple pigmentation in breast reconstruction and cosmetic procedures such as permanent makeup. This article describes the technique and its application to head and neck reconstruction. Medical tattooing can be used to improve the cosmetic appearance of head and neck free flaps. There is no donor site morbidity and subtle changes in color can be replicated. The article describes the technique of medical tattooing with the use of illustrative cases. Medical tattooing is a viable alternative for improving the appearance of cutaneous skin paddles following head and neck reconstruction with free flaps. Its advantages include no donor site morbidity, availability of an infinite range of colors, no requirement for general anesthesia, and the ability to use multiple colors in the one flap for complex pigmentation requirements. Its disadvantages include the need for specialized skills and equipment and the fading of color over time.
