Abstract Purpose Dual kidney transplantation (DKT) offers a way to extend the use of kidneys from expanded criteria donors (ECDs). Here, we compared the outcomes of DKT with those of single kidney transplantation from standard criteria donors (SCDs) and ECDs. Methods In 2014, we began performing DKT using both kidneys from deceased donors greater than 70 years of age with one of two risk factors: serum creatinine (sCr) level over 3.0 mg/dl or eGFR under 30 ml/min. Many members of the Saprolegniales (Oomycete) cause mycoses and disorders of fishes, of which Achlya and Saprolegnia are most ubiquitous genera worldwide. During a survey of the diversity of freshwater oomycetes in Korea, we collected seven isolates of Achlya, for which morphological and molecular phylogenetic analyses enabled them to identify as Achlya americana and Achlya bisexualis. In Korea, only a species of Achlya, A. prolifera, has been previously found to cause seedling rot on rice (Oryza sativa), but none of the two species have been reported yet. Importantly, A. bisexualis was isolated from a live fish, namely rice fish (Oryzias sinensis), as well as freshwater, and this is the first report of Achlya-causing mycoses on freshwater fishes in Korea. The presence of A. americana and A. bisexualis on live fish in Korea should be closely monitored, as considering the well-known broad infectivity of these species it has the potential to cause an important emerging disease on aquaculture industry.

The distribution and occurrence of rare ascomycete fungi within freshwater samples in Korea was investigated. Three rare fungal strains, CNUFC-YR537-1, CNUFC-CNUP1-1, and CNUFC-NDR3-1, were isolated using serial dilution method. On the basis of their morphological characteristics and phylogenetic analysis of their internal transcribed spacer regions and 28S rDNA sequences, the three isolates were identified as Acrostalagmus luteoalbus, Bartalinia robillardoides, and Collariella carteri, respectively. To our knowledge, these are the first records of rare genera Acrostalagmus, Bartalinia, and Collariella from Korea, and the first reports of A. luteoalbus, B. robillardoides, and C. carteri from freshwater samples.

During a survey of fungal diversity in the class Sordariomycetes, 3 fungal strains, CNUFC-KMHY6-1, CNUFC-MSW24-2-11, and CNUFC-GW2S-4 were isolated from soil and freshwater samples, respectively in Korea. The strains were analyzed both morphologically and phylogenetically on the basis of internal transcribed spacer and RNA polymerase II second largest subunit gene sequences. On the basis of their morphology and phylogeny, CNUFC-KMHY6-1, CNUFC-MSW24-2-11, and CNUFC-GW2S-4 isolates were identified as Arcopilus aureus, Memnoniella echinata, and Stachybotrys sansevieriae, respectively. To the best of our knowledge, Ar. aureus and M. echinata have not been previously recorded in Korea, and this is the first report of S. sansevieriae from freshwater niche.

Percutaneous endoscopic gastrostomy (PEG) is widely used to provide nutritional support for patients with dysphagia and/or disturbed consciousness preventing oral ingestion, and PEG tube placement is a relatively safe and convenient non-surgical procedure performed under local anesthesia. However, the prevention of PEG-insertion-related complications is important. A 64-year-old man with recurrent pneumonia underwent tracheostomy and nasogastric tube placement for nutritional support and opted for PEG tube insertion for long-term nutrition. However, during the insertion procedure, needle puncture had to be attempted twice before successful PEG tube placement was achieved, and a day after the procedure his hemoglobin had fallen and he developed hypotension. Abdominal computed tomography revealed injury to a pancreatic branch of the superior mesenteric artery (SMA) associated with bleeding, hemoperitoneum, and pancreatitis. Transarterial embolization was performed using a microcatheter to treat hemorrhage from the injured branch of the SMA, and the acute pancreatitis was treated using antibiotics and supportive care. The patient was discharged after an uneventful recovery. Clinicians should be mindful of possible pancreatic injury and bleeding after PEG tube insertion. Possible complications, such as visceral injuries or bleeding, should be considered in patients requiring multiple puncture attempts during a PEG procedure.

While evaluating fungal diversity in freshwater, grasshopper feces, and soil collected at Dokdo Island in Korea, four fungal strains designated CNUFC-DDS14-1, CNUFC-GHD05-1, CNUFC-DDS47-1, and CNUFC-NDR5-2 were isolated. Based on combination studies using phylogenies and morphological characteristics, the isolates were confirmed as Ascodesmis sphaerospora, Chaetomella raphigera, Gibellulopsis nigrescens, and Myrmecridium schulzeri, respectively. This is the first records of these four species from Korea.

The order Mucorales, the largest in number of species within the Mucoromycotina, comprises typically fast-growing saprotrophic fungi. During a study of the fungal diversity of undiscovered taxa in Korea, two novel mucoralean strains, CNUFC-GWD3-9 and CNUFC-EGF1-4, were isolated from specific habitats including freshwater and fecal samples, respectively. On the basis of their morphological characteristics and sequence analyses of internal transcribed spacer and large subunit ribosomal DNA, the CNUFC-GWD3-9 and CNUFC-EGF1-4 isolates were confirmed to be Gilbertella persicaria and Pilobolus crystallinus, respectively. It is ecologically, pathologically, and mycologically significant to find such rare zygomycetous fungi in such specific habitats.

Purpose: The survival of advanced colon cancer patients has increased due to the development of surgical techniques and adjuvant chemotherapy. The administration of adjuvant chemotherapy after curative resection is generally accepted as a standard of care. The primary endpoint of chemotherapy should include not only tumor response and survival, but also impact on the quality of life (QoL). We evaluated changes in QoL during adjuvant chemotherapy in patients with colon cancer. Methods: Between October 2009 and February 2012, 56 patients with stage II and III colon cancer received the combination adjuvant chemotherapy 5-flurouracil/folinic acid with oxaliplatin (FOLFOX). Patients were asked to complete the QoL questionnaire QLQ-C30 version 3 before and after 6 cycles of adjuvant chemotherapy. Results: There was no significant difference in the QoL between the start of chemotherapy and after the completion of 6 cycles. After completion of 6 cycles, global QoL was worse in patients >70 years of age. The functional scale score was low in patients with chemotherapy schedules delayed more than 2 times due to adverse events. Patients with body weight increases greater than 5% scored lower on symptom scales. Interestingly, patients with peripheral neuropathy scored higher on symptom scales. Conclusion: QoL changes during adjuvant chemotherapy did not show significant differences. After the sixth chemotherapy, QoL was affected by age, body weight gain, delay of the scheduled chemotherapy, and peripheral neuropathy. Therefore, the proper attitude of physicians focused on reassurance and education of patients is very important during chemotherapy.

The purpose of the study was to describe and analyze the self-efficacy of South Korean adapted physical education (APE) majors in teaching persons with disabilities. The research design was a qualitative descriptive case study. The participants were five APE majors (four men and one woman, age ranged from 20 to 25) attending a university in South Korea. The data sources were interviews, program curriculum reviews, and a researcher's journal entries; and analyzed using thematic analysis. Four themes emerged, which were: (a) determinants of self-efficacy, (b) relationships with APE faculty, (c) increased awareness, and (d) I am not confident. Findings exposed APE majors as more likely to gain confidence after volunteering to work with persons with disabilities and developing relationships with them. The results showed the importance of integrating hands-on practicum teaching experiences in APE courses for the majors to develop their positive self-efficacy in teaching persons with disabilities.

Objective To investigate the effect-site concentration of remifentanil required to blunt haemodynamic responses during tracheal intubation with a single-lumen tube (SLT) or a double-lumen tube (DLT). Methods Patients scheduled for thoracic surgery requiring one-lung ventilation were randomly allocated to either the SLT or DLT group. All patients received a target-controlled infusion of propofol and a predetermined concentration of remifentanil. Haemodynamic parameters during intubation were recorded. The effect-site concentration of remifentanil was determined using a delayed up-and-down sequential allocation method. Results A total of 92 patients were enrolled in the study. The effective effect-site concentrations of remifentanil required to blunt haemodynamic responses in 50% of patients (EC 50 ) estimated by isotonic regression with bootstrapping was higher in the DLT than the SLT group (8.5 ng/ml [95% confidence interval (CI) 8.0–9.5 ng/ml] versus 6.5 ng/ml [95% CI 5.6–6.7 ng/ml], respectively). Similarly, the effective effect-site concentrations of remifentanil in 95% of patients in the DLT group was higher than the SLT group (9.9 ng/ml [95% CI 9.8–10.0 ng/ml] versus 7.0 ng/ml [95% CI 6.9–7.0 ng/ml], respectively). Conclusions This study demonstrated that a DLT requires a 30% higher EC 50 of remifentanil than does an SLT to blunt haemodynamic responses during tracheal intubation when combined with a target-controlled infusion of propofol. The purpose of the study was to analyze South Korean college students with disabilities' recollections about their experiences in segregated (self-contained) special education classes and integrated physical education classes during elementary, middle, and/or high school in South Korea. The research design was retrospective multiple-case study. The participants were five South Korean college students matriculating at a university in South Korea. The primary data sources were semistructured interviews and researcher's journal entries. The data were analyzed using thematic analysis and exposed four interrelated themes, which were (a) isolated, (b) felt not supported, (c) segregated from peers, and (d) felt relieved and safe. Overall, the findings exposed challenges the participants all had experienced in integrated physical education classes, including feeling isolated and excluded from meaningful participation in class activities. In contrast, they reflected on their experiences in the segregated (self-contained) special education classes with a sense of belonging and acceptance.

The purpose of this case study was to examine a South Korean (SK) immigrant parent's views on physical education programming in the context of special education in the U.S., using a phenomenological research design. The data collected were a series of semi-structured interviews and artifacts related to the child's education. A qualitative data analysis software was used to organize and analyze the data to look for themes to answer the research questions. The two themes that emerged from the data illustrate the parent's perspectives of the challenges in the U.S. education system and potential partnerships between immigrant parents, General Physical Education (GPE), and Adapted Physical Education (APE) teachers. The parent identified cultural and language differences when working with school personnel and the Korean church community as the primary support to overcome these barriers. The parent also shared his lack of knowledge about physical education programming as well as a lack of collaboration with the GPE and APE teachers. Therefore, it is suggested that school personnel should be sensitive to the needs of their students and parents from different cultures. The efforts of school personnel can support immigrant parents to become active participants in the education of their children with disabilities. As it relates to physical education, GPE and APE teachers need to assist immigrant parents with understanding the importance and the benefits of physical education programming. Just like non-immigrant parents in the United States, immigrant parents want quality for appropriate special educational services for their child with a disability. However, immigrant parents often have negative experiences working with school professionals who provide special education services. According to Lai and Vadeboncoeur (2013), immigrant parents believe that language and cultural differences impede the establishment of trusting relationships with school staff. However, parental involvement, including that of immigrant parents, is mandated as a part of the Individuals with Disabilities Education Improvement Act (2004), as well as essential to a child's success in school. The purpose of this paper is therefore to provide recommendations for GPE and APE teachers to establish a trusting relationship with immigrant parents of children with disabilities. Recommendations for GPE and APE teachers include: (a) finding effective communication methods, (b) working with classroom teachers and paraprofessionals, (c) providing educational resources, (d) inviting parents to GPE and APE classes, (e) educating parents about PE and their rights, and (f) understanding cultural differences. Purpose: Metformin can reduce diabetes-related complications and mortality. However, its use is limited because of potential lactic acidosis-associated adverse effects, particularly in renal impairment patients. We aimed to investigate the association of metformin use with lactic acidosis and hyperlactatemia in patients with type 2 diabetes. Materials and Methods: This was a cross-sectional study from a tertiary university-affiliated medical center. A total of 1954 type 2 diabetes patients were recruited in 2007-2011, and stratified according to the estimated glomerular filtration rate of 60 mL/ min/1.73 m2. Lactic acidosis was defined as plasma lactate levels >5 mmol/L and arterial pH <7.35. Results: Metformin was used in 61.4% of the patients with type 2 diabetes mellitus. Plasma lactate levels were not different in the patients with and without metformin use. There was no difference in prevalence of hyperlactatemia and lactic acidosis between the patients with and without metformin use (18.9% vs. 18.7%, p=0.905 for hyperlactatemia and 2.8% vs. 3.3%, p=0.544 for lactic acidosis). Similar results were observed in the patients with estimated glomerular filtration rate <60 mL/min/1.73 m2. Most patients with lactic acidosis had at least one condition related to hypoxia or poor tissue perfusion. Multiple regression analysis indicated no association between metformin use and lactic acidosis, whereas tissue hypoxia was an independent risk factor for lactic acidosis [odds ratio 4.603 (95% confidence interval, 1.327-15.965)]. Conclusion: Metformin use was not associated with hyperlactatemia or lactic acidosis in patients with type 2 diabetes. [ABSTRACT FROM AUTHOR]

During a biodiversity survey of undiscovered taxa in Korea, two zygomycetous fungal strains were isolated. The first strain, EML-FSDY6-1 was isolated from a soil sample collected at Dokdo Island in the East Sea of Korea in 2013, and the second strain, EML-DG-NH3-1 was isolated from a rat dung sample collected at Chonnam National University garden, Gwangju, Korea in 2014. Based on the morphological characteristics and phylogenetic analysis of the internal transcribed spacer, 18S and 28S rDNA, actin and translation elongation factor-1α genes. EML-FSDY6-1 and EML-DG-NH3-1 isolates were confirmed as zygomycete species, Absidia pseudocylindrospora and Absidia glauca, respectively. Neither species has previously been described in Korea. Background: Although disability is not ethnicity or race specific, it is known that cultural differences do occur in how disability is perceived (Daudji et al., 2010). Parental involvement influences how school personnel provide resources for students with a disability (An & Hodge, 2013; Barge & Loges, 2003). Aims: This study aims to describe perceptions of disability among Asian immigrant parents of children with disabilities in a western U.S. state that is considered to be both multicultural and urban, and to explore how these perceptions influence how services that may be available to their children are utilized. Methods: A descriptive mixed-method research design was employed. Semi-structured interviews and a Likertscale survey given to seventeen parents who immigrated to the USA from Asia, and whose children have disabilities and receiving direct services as part of their Individualized Education Program. Results: Themes were identified that directly relate to parents' cultural beliefs, knowledge of curriculum and parental rights. Percentages were high with regard to feelings of available and accessible services for their children with disabilities, and low percentages with regard to their comfort level of utilizing specific services which can be seen as barriers. Discussion and Conclusions: This research showed the importance of Asian immigrant parents' perceptions and how perceived barriers can actually become opportunities with the knowledge, value and belief in and of expert professionals within such a community. Combining Hofstede’s cultural dimensions, value–belief–norm theory, and social exchange theory, this study explores the impact of individualism and social cohesion on pro-environmental behavior (PEB) as well as the moderating role of social cohesion in the individualism–PEB link in the context of Korean society. Using the 2021 Korean General Social Survey and multiple linear regression analyses, we found that individualism is negatively related to PEB, whereas social cohesion is positively related to PEB. Further analysis showed that social cohesion attenuates the negative relationship between individualism and PEB. Our findings suggest that although individuals with high levels of individualism are less likely to perform PEB than those with a high level of collectivism, social cohesion is a valuable community resource that encourages them to engage in eco-friendly activities even when they seek to achieve person-oriented goals and pursue their own interests. The implications and contributions of these findings regarding environmental psychology are discussed.
