Abstract Background The prognosis of colorectal cancer (CRC) patients can be influenced by genetic mutations and nutritional status. The relationship between these variables is unclear. The objective of the study was to verify the variables involved in the nutritional status and genetic mutations, which correlate with survival of CRC patients. Methods Patients with surgical intervention for tumor resection were evaluated using body mass index, nutritional screening, patient self-produced global subjective assessment, phase angle, and computed tomography to calculate the areas of visceral adipose tissue (VAT) and subcutaneous adipose tissue, and muscle mass for the determination of sarcopenia. Ten gene mutations involved in CRC carcinogenesis were studied (PIK3CA, KRAS, BRAF, EGFR, NRAS, TP53, APC, PTEN, SMAD4, and FBXW7). DNA was extracted from fresh tumor or paraffin tissues. Results Of the 46 patients, 29 (64.4%) were at nutritional risk and 21 (45.7%) were moderately malnourished. However, there was a high percentage of VAT in 24 (61.5%) and sarcopenia in 19 (48.7%) patients. These variables were associated with a higher risk of mortality. ABSTRACT Background: KRAS mutations are important events in colorectal carcinogenesis, as well as negative predictors of response to EGFR inhibitors treatment. Aim: To investigate the association of clinical-pathological features with KRAS mutations in colorectal cancer patients treated. Methods: Data from 69 patients with colorectal cancer either metastatic at diagnosis or later, were retrospectively analyzed. The direct sequencing and pyrosequencing techniques were related to KRAS exon 2. The mutation diagnosis and its type were determined. Results: KRAS mutation was identified in 43.4% of patients. The most common was c.35G>T (p.G12V), c.35G>A (p.G12D) and c.38G>A (p.G13D). No correlation was found between KRAS mutation and age (p=0.646) or gender (p=0.815). However, mutated group had higher CEA levels at admission (p=0.048) and codon 13 mutation was associated with involvement of more than one metastatic site in disease progression (p=0.029). Although there was no association between primary tumor site and mutation diagnosis (p=0.568), primary colon was associated with worse overall survival (p=0.009). Conclusion: The KRAS mutation was identified in almost half of patients. Mutated KRAS group had higher levels of CEA at admission and the mutation at codon 13 was associated with involvement of more than one metastatic site in the course of the disease. Colon disease was associated with the worst overall survival.

ABSTRACT Aim: to evaluate the presence of subclinical HPV-induced anal lesions with anal cytology, High-Resolution Anoscopy (HRA) and HPV genotyping by polymerase chain reaction (PCR) in the follow-up of treated condylomata acuminata (CA). Methods: seventy-nine male patients were included. One month after anal CA eradication, the patients underwent brush samples collection for anal cytology and PCR, and HRA with biopsy of acetowhite lesions. These methods were compared within all patients and between groups, according to Human Immunodeficiency Virus (HIV) infection status: HIV-negative; HIV-positive with TCD4 count above and below 350 cells/mm3. Results: the most frequent HPV types were 6 and 16. HPV DNA was isolated in 92%. HIV infection was associated with a higher number of oncogenic HPV types (p=0.038). All patients with negative PCR had negative HRA and cytology. There were no differences in cytological, HRA or histopathological findings between groups. Conclusion: the association of the findings of cytopathology, HRA and genotyping of HPV refined the diagnosis of HPV-induced lesions. The degree of immunodeficiency was not associated with increase in remnant HPV-induced anal lesions.

Abstract Purpose: To compare the effectiveness of anal and perianal condylomata treatment using argon plasma and electrofulguration. Methods: From January 2013 to April 2014, 37 patients with anal and perianal condylomata, who had been diagnosed through proctological examination, oncotic cytology, polymerase chain reaction (PCR) and histology, underwent treatment with argon plasma and electrofulguration. The perianal and anal regions were divided into two semicircles. Each semicircle was treated using one of the methods by means of simple randomization. Therapeutic sessions were repeated until all clinical signs of infection by HPV were eliminated. The patients were evaluated according to several variables like the genotype of HPV, HIV infection, oncological potential per genotype, oncotic cytology and histology. Results: Among all the variables studied, only immunosuppression due to HIV influenced the results, specifically when the fulguration method was used. There was no significant difference in effectiveness between argon and fulguration based on lesion relapse (p > 0.05). However, among HIV-positive patients, fulguration presented worse results, with a significant difference (p = 0.01). Conclusion: Regarding treatment of anal and perianal condylomata acuminata, comparison between applying fulguration and argon demonstrated that these methods were equivalent, but use of fulguration presented more relapses among HIV-positive patients.

Colorectal cancer is the third most common cancer worldwide, accounting for more than 610,000 mortalities every year. Prognosis of patients is highly dependent on the disease stage at diagnosis. Therefore, it is crucial to investigate molecules involved in colorectal cancer tumorigenesis, with possible use as tumor markers. Heparan sulfate proteoglycans are complex molecules present in the cell membrane and extracellular matrix, which play vital roles in cell adhesion, migration, proliferation, and signaling pathways. In colorectal cancer, the cell surface proteoglycan syndecan-2 is upregulated and increases cell migration. Moreover, expression of syndecan-1 and syndecan-4, generally antitumor molecules, is reduced. Levels of glypicans and perlecan are also altered in colorectal cancer; however, their role in tumor progression is not fully understood. In addition, studies have reported increased heparan sulfate remodeling enzymes, as the endosulfatases. Therefore, heparan sulfate proteoglycans are candidate molecules to clarify colorectal cancer tumorigenesis, as well as important targets to therapy and diagnosis.

PURPOSE: The controversy regarding whether loop ileostomy or loop transverse colostomy is a better method for temporary decompression of colorectal anastomosis motivated this review. METHODS: Five randomized trials were included, with 334 patients: 168 in the loop ileostomy group and 166 in the loop transverse colostomy group. The outcomes analyzed were: 1. Mortality; 2. Wound infection; 3. Time of stoma formation; 4. Time of stoma closure; 5. Time interval between stoma formation and closure; 6. Stoma prolapse; 7. Stoma retraction; 8. Parastomal hernia; 9. RESULTS: Stoma prolapse was statistically significant (p = 0.00001), but with statistical heterogeneity; the sensitive analysis was applied, excluding the trials that included emergency surgery, and this showed: p = 0.02, with I² = 0% for the heterogeneity test. CONCLUSIONS: The outcomes reported were not statistically or clinically significant except for stoma prolapse. Better evidence for making the choice between loop ileostomy or loop colostomy requires large-scale randomized controlled trials.OBJETIVO: A controvérsia entre ileostomia em alça ou colostomia em alça como a melhor forma para a descompressão temporária da anastomose colorretal motivou a realização desta revisão. RESULTADOS: Prolapso do estoma: p = 0.00001, mas com heterogeneidade estatística; a análise de sensibilidade foi aplicada excluindo os estudos que incluíram cirurgias de emergência: p = 0.02 e teste de heterogeneidade: I²=0%. CONCLUSÕES: Os resultados encontrados não foram estatística ou clinicamente significantes, exceto prolapso do estoma. A melhor evidência para a escolha entre ileostomia em alça ou colostomia em alça necessita de maior número de ensaios clínicos.

PURPOSE: To compare the results of rubber band ligation and infrared photocoagulation for the treatment of hemorrhoidal disease through the analysis of the incidence of complications after each treatment and respective success rate. METHODS: Forty-eight patients with first, second or third degree hemorrhoidal disease were randomized to recieve treatment with either rubber band ligation (n=23) or infrared photocoagulation (n=25). Each patient was assessed at 1 week and 4 week intervals after treatment. We compared the incidence of complications and efficiency of each treatment modality and Qui-square, Fisher's Exact Test and Student's t Test were used to statistical analysis. RESULTS: Bleeding occured in eigth (34,7%) patients treated with rubber band ligation and in four (16,0%) after infrared photocoagulation (p=0,243). Thirteen (52,0%) patients felt pain during infrared photocoagulation and 9 (39,1%) after rubber band ligation (p=0,546). After rubber band ligation, 14 (60,8%) required medication for pain relief. One patient (4,0%) required medication after infrared photocoagulation (pOBJETIVO: Comparar os resultados da ligadura elástica com os da fotocoagulação com radiação infravermelha no tratamento da doença hemorroidária através da análise de suas respectivas morbidades e eficácia. MÉTODOS: Foi realizado ensaio clínico casualisado com 48 pacientes portadores de doença hemorroidária graus I, II e III. Após randomização, os pacientes foram submetidos à ligadura elástica (n=23) ou fotocoagulação com radiação infravermelha (n=25). Os pacientes compareceram para consulta ambulatorial 1 semana e 4 semanas após o procedimento. A avaliação foi feita através da análise da incidência de morbidades após cada procedimento e eficácia, com quatro semanas de acompanhamento. As análises estatísticas foram realizadas através do teste do Qui-quadrado, Teste Exato de Fisher e Teste t de Student. RESULTADOS: Dos pacientes submetidos à ligadura elástica, 34,7% (8/23) apresentou sangramento após o procedimento. Após fotocoagulação, 16,0% (4/25) tiveram sangramento (p=0,243). A incidência de pacientes com dor no momento da aplicação do procedimento foi de 52,0% (13/25) após fotocoagulação com radiação infravermelha e 39,1% (9/23) após ligadura elástica (p=0,546). No entanto, ligadura elástica foi mais dolorosa após 48 horas do procedimento (p=0,044). OBJETIVO: Avaliar a relação de duas proteínas que participam do mecanismo de adesão celular com o grau de diferenciação celular e os estadiamentos TNM (T: tumor, N: linfonodo, M: metástase) I e IV no câncer de cólon e reto. MÉTODOS: Foram estudados cem pacientes (54 homens e 46 mulheres) tratados por adenocarcinoma colorretal, estádios I (44) e IV (56). Os cortes histológicos do tecido tumoral foram examinados por técnica de imuno-histoquímica em relação à imunoexpressão das proteínas caderina-E e delect in colon cancer (DCC), sendo classificados como positivos quando se detectou a imunoexpressão dessas proteínas em 50% ou mais das células tumorais. RESULTADOS: Para o TNM, imunoexpressão da caderina-E estádio I: positiva em 72,7 % e negativa em 35,7% ; estádio IV: positiva em 64,3% e negativa em 35,7%. Proteína DCC: 43,2% positiva e 56,8% negativa no estádio I, e 50% positiva e 50% negativa no estádio IV. Em relação ao grau de diferenciação celular, imunoexpressão da caderina-E - GI: positiva em 70% e negativa em 30%; GII: positiva em 68,4% e 31,6% negativa; GIII: 63,6% positiva e 36,4% negativa. Imunoexpressão da DCC - GI: 40% positiva e 60% negativa; GII: 46,8% positiva e 53,2% negativa; GIII: 54,5% positiva e 45,5% negativa. Não houve diferença significativa entre os grupos. CONCLUSÃO: Os resultados dessa pesquisa permitem concluir que não há relação da imunoexpressão das proteínas caderina-E e DCC com o estadiamento TNM (I e IV) e o grau de diferenciação celular no carcinoma colorretal.OBJECTIVE: Evaluate the relationship of two proteins, which take part in the same mechanism of cell adhesion, with the cell differentiation degree and TNM staging I and IV in colorectal cancer. METHODS: One-hundred patients (54 men and 46 women), who have received treatment for colorectal cancer, stages I (44) and IV (56), have been studied. Histological cuts of tumor tissue were examined by the immunohistochemical technique as to the expression of E-cadherin and delect in colon cancer (DCC) proteins, being classified as positive whenever it was detected immunoexpression of such proteins in 50% or more tumor cells. RESULTS: For TNM, E-cadherin immunoexpression for stage I: positive in 72.7% and negative in 35.7%; stage IV: positive in 64.3% and negative in 35.7%. For DCC protein: 43.2% positive and 56.8% negative in stage I, and 50% positive and 50% negative in stage IV. Regarding the cell differentiation degree, the immunoexpression of E-cadherin - GI: positive in 70% and negative in 30%; GII: positive in 68.4% and negative in 31.6%; GIII: positive in 63.6% and negative in 36.4%. The immunoexpression of DCC - GI: 40% positive and 60% negative; GII: 46.8% positive and 53.2% negative; GIII: 54.5% positive and 45.5% negative. There was no significant difference among groups. CONCLUSION: The results of this research make it possible to come to the conclusion that there is no relationship between the immunoexpression of E-cadherin and DCC proteins with TNM staging (I and IV) and cell differentiation degree in colorectal cancer.

PURPOSE: To analyze the correlation between p53 and bcl-2 expression and colorectal adenocarcinoma staging and prognosis. METHODS: This was a retrospective series of 125 colorectal adenocarcinoma patients (67 women and 58 men; ages 30-87 years) who underwent surgery with curative intent. The mean follow-up was 28.5 months (range: 2-96 months). TNM staging, tumor recurrence, survival and cancer-related mortality were analyzed. Immunoreactivity was evaluated using DO7 (Dako) for p53 and K492 (Dako) for bcl-2. Tumors with accumulation of staining for cytoplasmic bcl-2 or nuclear p53 in more than 10% of cells were considered positive. INTRODUÇÃO: Insulinoma é a neoplasia endócrina mais frequente dos tumores funcionantes do pâncreas. Origina-se a partir das células beta das ilhotas de Langerhans e caracteriza-se pela produção excessiva de insulina, com consequente hipoglicemia. O tratamento de escolha é a remoção cirúrgica da neoplasia. O presente relato tem como objetivo apresentar uma complicação metabólica pouco observada. RELATO DO CASO: Homem de 41 anos de idade há dois anos com tonturas, visão turva e convulsões. Os sintomas estavam bem relacionados com períodos prolongados de jejum e melhoravam com as refeições, e durante um dos episódios foi constatada a presença de hipoglicemia, melhorando os sintomas imediatamente após administração de glicose endovenosa. A pesquisa glicêmica revelou intensa hipoglicemia. Ultrassonografia, tomografia computadorizada e ressonância magnética de abdome não revelaram nenhum tipo de alterações no pâncreas. Com a hipótese diagnóstica de hiperglicemia orgânica por provável insulinoma, o paciente foi submetido à enucleação da lesão. No 5º dia do pós-operatório surgiu fístula pancreática e acidose metabólica com resolução satisfatória. O laudo histopatológico mostrou tumor endócrino de pâncreas de 1,5 cm. CONCLUSÃO: Todo paciente submetido à ressecção de insulinoma pancreático deve realizar o pós-operatório imediato em unidades de terapia intensiva, monitorando de forma rigorosa os níveis de glicemia como prevenção de acidose metabólica.INTRODUCTION: The insulinoma is the most frequent endocrine neoplasm among the functional pancreatic tumors. It originates in the beta cells of the islets of Langerhans and is characterized by the oversecretion of insulin, leading to hypoglycemia. The treatment of choice is the surgical excision of the tumor. The aim of the present report is to describe a rarely observed metabolic complication. CASE REPORT: The case is presented of a 41-year-old man with a 2-year history of dizziness, blurred vision and seizures. The symptoms were closely related to prolonged fasting and improved with eating; hypoglycemia was found during one of the episodes. Symptoms were relieved immediately after intravenous administration of glucose. Blood glucose workup showed severe hypoglycemia. Abdominal ultrasonography, computed tomography and magnetic resonance imaging did not show any alteration in the pancreas. With the diagnostic hypothesis of organic hypoglycemia from a likely insulinoma, the patient underwent the enucleation of the lesion. On the 5th postoperative day, a pancreatic fistula appeared, as well as metabolic acidosis which resolved satisfactorily. The histopathological report showed a 1.5-cm endocrine pancreatic tumor. CONCLUSION: Every patient submitted to pancreatic insulinoma resection should stay in an intensive care unit during the immediate postoperative period and their glycemic levels must be monitored closely to prevent metabolic acidosis.

A anastomose com sutura é método consagrado, apresentando desvantagens teóricas representadas pelo trauma tecidual, edema e alteração da microcirculação. Além disso, o fio de sutura, sendo corpo estranho, determina reação inflamatória, propiciando a aderência e a proliferação de bactérias, assim como de células neoplásicas que seriam evitadas ao se usar método de anastomose sem sutura. A procura por método de anastomose em que não se utilizasse fio de sutura vem sendo realizada desde o início deste século. Com o advento das novas aquisições tecnológicas foi desenvolvido método de anastomose sem sutura através do anel biofragmentável. Esse anel sofre desintegração por hidrólise, sendo eliminado com a evacuação, não permanecendo corpo estranho na anastomose. Esse instrumento permite a realização de anastomose sem sutura, por meio da compressão das paredes intestinais justapostas. O objetivo desse estudo prospectivo e casualizado, foi o de comparar os resultados clínicos e endoscópicos de anastomoses colocólicas, eletivas e de baixo risco, realizadas com anel biofragmentável e com fio de sutura não absorvível. CASUÍSTICA E MÉTODOS: A casuística foi composta por 36 doentes, divididos em dois grupos de 18, que foram denominados Grupo I (anel biofragmentável ) e Grupo II (sutura com fio não absorvível ), constituídos de doentes comparáveis. RESULTADOS: Os parâmetros de avaliação do período intra-operatório, revelaram que a anastomose com anel biofragmentável despende menor tempo para sua realização. Não se detectou diferença estatisticamente significante entre os dois grupos com relação à incidência de complicações pós-operatórias, mortalidade e avaliação clínica ambulatorial. A incidência de deiscência anastomótica só ocorreu no Grupo I. A análise endoscópica da anastomose no período pós-operatório não demonstrou diferença quanto à perviedade, contratilidade, elasticidade e grau de epitelização. O Grupo II apresentou maior incidência de fio de sutura na anastomose em relação ao Grupo I sendo esta diferença estatística significante. CONCLUSÕES: O anel biofragmentável permite a realização de anastomose de colo mais rápida do que aquela realizada com fio de sutura. Este método de anastomose determinou, na análise global da complicações clínicas, resultados semelhantes ao uso de fio de sutura. O método de anastomose sem sutura apresentou o inconveniente de determinar maior incidência de deiscência anastomótica e como vantagem estabeleceu presença de corpo estranho na anastomose em menor porcentagem.
