Amaç: Polikistik over sendromu (PKOS) üreme çağındaki kadınlarda en sık rastlanan endokrinolojik bozukluktur. Metabolik ve kardiyovasküler ek hastalıklara zemin oluşturması yanı sıra kemik metabolizmasını da etkilemektedir. Çalışmamızda PKOS olgularının kemik mineral yoğunluğunu değerlendirmek ve bunu etkileyen faktörleri belirlemeyi amaçladık. Gereç ve yöntem: Araştırmaya Ocak 2019 ve Ocak 2021 tarihleri arasında poliklinik başvurusu olan 17-44 yaş arası 109 PKOS tanısı alan kadın dahil edilmiştir. PKOS tanısı Rotterdam kriterleri baz alınarak konulmuştur. Olguların kemik mineral yoğunluğu (KMY) dual enerji X ray absorbsiyometri (DEXA) ile ölçüldü. Ayrıca katılımcıların biyokimyasal ve hormonal değerlendirmeleri de yapılarak sol femur boynu ve lumbal 2,3 ve 4. vertebra bölgelerinden alınan DEXA ölçümlerinin Z ve T skorları, katılımcılar vücut kitle indeksine (VKİ) göre gruplandırılarak karşılaştırıldı. Bulgular: Fazla kilolu olan olguların insülin (p=0,006), homeaostatic model assessment –insulin resistance (HOMA-IR) (p=0,006) ve kortizol (p=0,009) ölçüm ortalamaları normal kilolu olgulara göre istatistiksel açıdan anlamlı olarak daha yüksek iken, tiroid stimülan hormon (TSH) ölçüm ortalamaları normal kilolu olgulara göre anlamlı olarak daha düşüktü (p=0,030). Fazla kilolu olan olguların femur boynu T skoru (FBTS) (p=0.038) ve femur boynu Z skoru (p=0.022) ölçüm ortalamaları normal kilolu olgulara göre daha yüksek olarak saptandı. Olgulara ait VKİ ölçümleri ile FBTS ölçümleri arasında pozitif bir (r=0.238) korelasyon saptanmıştır (p=0.013) ve bel çevresi/boy oranı ölçümleri ile FBZS ölçümleri arasında anlamlı, düşük düzeyde (r=0.198) ilişki saptanmıştır (p=0.039). Olguların FBZS (r=-0.212, p=0.028), lumbal 2-4 vertebra T skoru (L2L4TS)(r=-0.217, p=0.024) ve lumbal 2-4 vertebra Z skoru (L2L4ZS) (r=-0.277, p=0.004) ile dehidroepiandrosteron sülfat (DHEAS) arasında anlamlı negatif bir korelasyon saptanmıştır. Sonuç: Sonuç olarak VKİ ve bel çevresi/boy oranı PKOS olgularında KMY’i olumlu yönde etkilerken, DHEAS’nin olumsuz yönde etkilediği kanaatine vardık. Ancak mekanizmayı ortaya koymak için prospektif ve moleküler çalışmalara ihtiyaç vardır.

Amaç: Coronavirus Disease 2019 (COVID-19) birçok insanı olduğu kadar gebeleri de etkileyen bir pandemidir. Çalışmamızın amacı inflamatuar belirteçlerin gebelerde COVID-19 seyrine etkisini araştırmaktır. Gereç ve yöntem: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pozitif olan 125 ve negatif olan 40 gebenin dahil edildiği bu kesitsel tipe tanımlayıcı çalışmada, COVID-19 olan hastalar oksijen satürasyonu, akciğer tutulumu bulgularına göre hafif orta ve ağır olarak gruplandırılmıştır. Fibirnojen, ferritin ve prokalsitonin değerleri karşılaştırılmıştır. INTRODUCTION: Adolescent pregnancies, which are undesirable situations, can adversely affect maternal and infant health. The aim of this study is to examine the effect of adolescent pregnancies on early neonatal outcomes. METHODS: In this study, electronic files of adolescents (14-19 years old) and 20 years old mothers (control group) who gave birth and their babies in a tertiary hospital were retrospectively analyzed. Demographic and clinical data of the mother and baby were evaluated. RESULTS: A total of 2638 patients, 1842 (69.82%) adolescents and 796 (30.17%) 20 years old mothers, were included in this study. Purpose: We aimed to investigate the relationship between first trimester vitamin D, vitamin B12 and ferritin levels with preeclampsia and neonatal outcomes. Materials and Methods: 191 patients diagnosed with preeclampsia (PE) and 99 healthy pregnant women were included in the study.. Age, gravida, parity, body mass index (BMI), mode of delivery, birth weight, APGAR score, neonatal intensive care unit (NICU) admission and delivery week were recorded. First trimester vitamin D, vitamin B12 and ferritin levels were determined in laboratory analyzes. Evaluation was made by separating three groups as without PE, with PE and with severe PE. Results: Differences were found between BMI measurements, birth weeks, birth weights and NICU acceptance values (p=0.003; 0.001; 0.001; 0.001 respectively). Babies in the PE group had lower mean birth weeks, birth weights and APGAR scores. In the PE group had a higher rate of cesarean section (p=0.001). The vitamin D level of the PE group was lower than the group without PE. It was found that the vitamin D level decreased in proportion to the severity of PE. The B12 level of the non-PE group was higher than PE group (p=0.001). The ferritin level of the non-PE group was found to be lower than the PE group (p=0.001). Conclusion: Low vitamin D and B12 levels and high ferritin levels in the first trimester may be beneficial in predicting preeclampsia. We think that especially vitamin D, B12 and iron supplementation is a simple but effective approach to reduce perinatal morbidity and mortality in preeclampsia.

Objective This multi-center study aims to determine the efficiency and safety of endometrial myomectomy (EM) for the removal of uterine fibroids during cesarean section (CS). Methods Retrospective review of 360 women diagnosed for fibroids during pregnancy. They all delivered by CS between 2014 and 2019. The study groups included 118 women who only underwent EM, 120 women who only had subserosal myomectomy by traditional technique and 122 women with fibroids who decided to avoid cesarean myomectomy, as control group. They were analyzed and compared the surgical outcomes. Results The EM, subserosal myomectomy and control groups were statistically (p > 0.05) similar for to age, body mass index (BMI), gravidity, parity, gestational age at delivery, indications for CS, number of excised fibroids, size of the largest myoma. Postoperative hemoglobin values and ? (?) hemoglobin concentrations were lower in SM group (10.39gr/dl vs 9.98 gr/dl vs 10.19 – 1.44 gr/dl vs 1.90 gr/dl vs 1.35; p = 0.047, p = 0.021; respectively) Hybrid fibroids were significantly more frequent in the EM group than subserosal myomectomy and control groups (respectively, 33.1% vs 23.3% vs 27.0%, p = 0.002). Surgery time was significantly longer in the subserosal myomectomy group than EM and control groups (respectively, 46.53 min vs 37.88 min vs 33.86 min, p = 0.001). Myomectomy took significantly longer time in the subserosal myomectomy than EM group (13.75 min vs 8.17 min, p = 0.001). Conclusions Endometrial myomectomy is a feasible choice for treatment of fibroids during CS, and, basing on our results could be an alternative to traditional cesarean subserosal myomectomy.

Oocyte maturation abnormalities (OMAS) are a poorly understood area of reproductive medicine. Much remains to be understood about how OMAS occur. However, current knowledge has provided some insight into the mechanistic and genetic origins of this syndrome. In this study, current classifications of OMAS syndromes are discussed and areas of inadequacy are highlighted. We explain why empty follicle syndrome, dysmorphic oocytes, some types of premature ovarian insufficiency and resistant ovary syndrome can cause OMAS. We discuss live births in different types of OMAS and when subjects can be offered treatment with autologous oocytes. As such, we present this review of the mechanism and understanding of OMAS to better lead the clinician in understanding this difficult-to-treat diagnosis.

Background: The aim of this prospective cohort study was to investigate the presence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in the cerebrospinal fluid (CSF) of pregnant women with neurological symptoms due to coronavirus disease 2019 (COVID-19). Materials and Methods: This research was carried out in the Samsun University. Department of Anesthesiology and Reanimation from December 2021 to May 2022. Pregnant women exhibiting neurological symptoms linked to COVID-19 were assigned to one of the two groups, depending on the severity of the disease–mild (Group 1) and severe (Group 2). Specimens were collected from patients’ CSF, and the presence of SARS-CoV-2 was investigated using the reverse transcription-polymerase chain reaction (RT-PCR) method. SARS-CoV-2 was also investigated using RT-PCR by collecting oropharyngeal swab specimens from infants in the first 6 h after birth. Results: One hundred fifty patients were enrolled, 75 in both groups. The most frequent neurological symptoms were dizziness in Group 1 (52%) and headache in Group 2 (32%). No significant differences were determined in neutrophils, lymphocytes, hemoglobin, leukocytes, platelets, ferritin, D-dimer, C-reactive protein, prothrombin time, aspartate aminotransferase, alanine aminotransferase, creatinine, sedimentation, or fibrinogen values. SARS-CoV-2 was identified in the CSF of only one patient, from Group 2. Infant oropharyngeal swab specimens tested negative for SARS-CoV-2 using RT-PCR. Conclusion: This study indicates that the detection of SARS-CoV-2 in CSF via RT-PCR is rare. We suggest that neurological symptoms linked to SARS-CoV-2 are not caused by direct invasion and that other etiologies represent more likely mechanisms.

Objective To investigate cyclophilin A in early second trimester of pregnancy before the onset of preeclampsia. Methods In this prospective case-control study, 51 pregnant women whose serum were collected and stored and who developed preeclampsia in later follow-up and 41 pregnant women as control group were included. Results Maternal serum cyclophilin A levels in the study group who developed preeclampsia in later follow-up were significantly higher than those of normal healthy pregnant women. Conclusion Cyclophilin A may be a valuable predictor for pregnant women who subsequently develop preeclampsia in their pregnancies.

Background: The number of sperm washes to maximize outcomes for intra-uterine insemination has not been well investigated. Therefore, we undertook to compare the pregnancy and live birth rates of triple sperm washing and double sperm washing for density gradient preparation for intrauterine insemination (IUI) cycles. Methods: A retrospective cohort study including 279 couples (136 couples with triple sperm washing and 143 couples with double sperm washing) with a diagnosis of unexplained infertility and mild male subfertility who had IUI cycles between April 2015 and April 2017 were evaluated. After overnight incubation of the sperm, subjects underwent either traditional double sperm washing or Triple sperm washing which consists of use of a third gradient and spinning procedure to the conventional double gradient sperm washing in order to obtain a higher quantity of motile sperm. Results: Total sperm count after triple washing was higher than double sperm washing (98.25 ± 62.06 vs. 81.08 ± 31.57; p = 0.003). Positive β-hCG test and live birth per cycle were higher in triple sperm washing (25.8% vs. 13.3%, p = 0.009; 18.4% vs. 9.8%, p = 0.039; respectively) than in the double sperm washing group. Conclusions: The use of motile sperms obtained from triple sperm washing may increase the rates of pregnancy and live birth in IUI cycles of women with unexplained and mild male factor infertility. A prospective randomized study should be undertaken to confirm the results.
