Abstract

UnlabelledThe serotonin-1A (5-HT1A; 5-HT is 5-hydroxytryptamine) receptor is implicated in an array of neurologic and psychiatric disorders. Current PET radioligands targeting 5-HT1A receptors have limitations hindering widespread PET studies of this receptor system. The 5-HT1A-specific antagonist radioligand N-{2-[4-(2-methoxyphenyl)piperazinyl]ethyl}-N-(2-pyridyl)-N-(trans-4-(18)F-fluoromethylcyclohexane)carboxamide ((18)F-mefway) exhibited promising in vivo properties in rhesus monkeys. The goal of this work was to examine the in vivo cerebral binding profile and metabolism of (18)F-mefway in humans.MethodsDynamic (18)F-mefway PET data were acquired for 6 healthy volunteers (4 women, 2 men; age, 22-38 y). Scans were initiated with the injection of 192-204 MBq of radiotracer, and data were acquired for 2 h. Venous blood samples were collected and assayed to examine the in vivo metabolism profile of (18)F-mefway. To examine the test-retest variability of (18)F-mefway, a second PET scan was acquired at least 2 wk later for 4 subjects. Regional binding potentials (BPNDs) were calculated with the multilinear reference tissue model, and voxelwise BPND maps were calculated with Logan graphical analysis. Regions surrounding the brain were carefully inspected for uptake of radiolabeled species in bone.Results(18)F-mefway uptake in the brain occurred quickly, with a peak standardized uptake value (SUV) of 1.7. Rapid washout in the cerebellum resulted in SUVs of 0.2 at 120 min, whereas regions with specific 5-HT1A binding exhibited retention of radioligand, yielding SUVs of 0.4-0.9 at 120 min. Rapid metabolism of (18)F-mefway was observed, with no detected (18)F-fluoride ions in plasma. BPND values of 2.4 were measured in the mesial temporal lobe, with values of 1.6 in the insular cortex and 0.7-1.0 in other cortical regions. Stable BPND estimates were obtained using 90 min of dynamic data. Average test-retest variability was 8%. No evidence of radioactivity uptake in bone was observed.Conclusion(18)F-mefway exhibits favorable in vivo properties for serotonin 5-HT1A receptor measurements in humans. The simple radiosynthesis, high specific binding profile, and absence of PET signal in bone make (18)F-mefway an attractive radiotracer for PET experiments examining the 5-HT1A receptor in neuropsychiatric disorders and drug intervention.

Abstract

Background and purposeThe goal of the Stroke Treatment Academic Industry Roundtable (STAIR) meetings is to advance the development of stroke therapies. At STAIR VIII, consensus recommendations were developed for clinical trial strategies to demonstrate the benefit of endovascular reperfusion therapies for acute ischemic stroke.Summary of reviewProspects for success with forthcoming endovascular trials are robust, because new neurothrombectomy devices have superior reperfusion efficacy compared with earlier-generation interventions. Specific recommendations are provided for trial designs in 3 populations: (1) patients undergoing intravenous fibrinolysis, (2) early patients ineligible for or having failed intravenous fibrinolysis, and (3) wake-up and other late-presenting patients. Among intravenous fibrinolysis-eligible patients, key principles are that CT or MRI confirmation of target arterial occlusions should precede randomization; endovascular intervention should be pursued with the greatest rapidity possible; and combined intravenous and neurothrombectomy therapy is more promising than neurothrombectomy alone. Among patients ineligible for or having failed intravenous fibrinolysis, scientific equipoise was affirmed and the need to randomize all eligible patients emphasized. Vessel imaging to confirm occlusion is mandatory, and infarct core and penumbral imaging is desirable in later time windows. Additional STAIR VIII recommendations include approaches to test multiple devices in a single trial, utility weighting of disability end points, and adaptive designs to delineate time and tissue injury thresholds at which benefits from intervention no longer accrue.ConclusionsEndovascular research priorities in acute ischemic stroke are to perform trials testing new, highly effective neuro thrombectomy devices rapidly deployed in patients confirmed to have target vessel occlusions.

Abstract

Introduction: Age-related changes in cerebral hemodynamics are controversial and discrepancies may be due to experimental techniques. As such, the purpose of this study was to compare cerebral hemodynamics measurements of the middle cerebral artery (MCA) between transcranial Doppler ultrasound (TCD) and four-dimensional flow MRI (4D flow MRI). Methods: Twenty young (25 ± 3 years) and 19 older (62 ± 6 years) participants underwent two randomized study visits to evaluate hemodynamics at baseline (normocapnia) and in response to stepped hypercapnia (4% CO2, and 6% CO2) using TCD and 4D flow MRI. Cerebral hemodynamic measures included MCA velocity, MCA flow, cerebral pulsatility index (PI) and cerebrovascular reactivity to hypercapnia. MCA flow was only assessed using 4D flow MRI. Results: MCA velocity between the TCD and 4D flow MRI methods was positively correlated across the normocapnia and hypercapnia conditions (r = 0.262; p = 0.004). Additionally, cerebral PI was significantly correlated between TCD and 4D flow MRI across the conditions (r = 0.236; p = 0.010). However, there was no significant association between MCA velocity using TCD and MCA flow using 4D flow MRI across the conditions (r = 0.079; p = 0.397). When age-associated differences in cerebrovascular reactivity using conductance were compared using both methodologies, cerebrovascular reactivity was greater in young adults compared to older adults when using 4D flow MRI (2.11 ± 1.68mL/min/mmHg/mmHg vs. 0.78 ± 1.68mL/min/mmHg/mmHg; p = 0.019), but notwith TCD (0.88 ± 1.01 cm/s/mmHg/mmHg vs. 0.68 ± 0.94 cm/s/mmHg/mmHg; p = 0.513). Conclusion: Our results demonstrated good agreement between the methods at measuring MCA velocity during normocapnia and in response to hypercapnia, but MCA velocity and MCA flow were not related. In addition, measurements using 4D flow MRI revealed effects of aging on cerebral hemodynamics that were not apparent using TCD. [ABSTRACT FROM AUTHOR]

30. Examining cerebrovascular burden across the cognitive continuum in older adults with and without evidence of amyloidosis.

Abstract

Background: This work characterized cerebrovascular health in human subjects spanning the cognitive spectrum with and without presence of Aβ using different neuroimaging techniques (Fig1). Methods: Subjects (n=70) were clinically characterized into four groups: cognitively unimpaired (CU), CU‐Declining (at‐risk of progressing to clinical mild MCI based on traditional neuropsychological data), clinical MCI, and clinical dementia (Fig2). MRI: Vascular imaging: Whole‐brain 4D‐Flow data were acquired to characterize cerebrovascular health. pCASL with three post labeling delays was used to provide transit time corrected CBF maps. Structural imaging: T1‐weighted and T2‐FLAIR images were also collected for quantification of intracranial, hippocampal and white matter hyperintensity volumes. Hemodynamic measurements were extracted from intracranial arteries including the petrous ICAs, BA, MCAs, and veins SSS. Quantified flow parameters included: total cerebral blood flow, trans‐capillary pulse wave delay, and flow pulsatility index (PI). Trans‐capillary pulse wave delay was defined as the time shift between arterial and venous cardiac waveforms. PET: Aβ burden was assessed using 11C‐PiB PET imaging. Amyloid positivity (A‐, A+) was established as mean cortical PiB DVR >1.16. For A+ participants, amyloid time (i.e., estimated duration A+) was estimated using the sampled iterative local approximation algorithm. Differences were assessed using ANOVA followed by post‐hoc analysis (P<0.05 significance). Results: Micro‐vascular tissue perfusion was statically higher in A+/CU when compared to other A+ groups in the GM, WM, and hippocampus (Fig3 and 4). Trans‐capillary pulse wave delay was statistically longer in A+/CU when compared to A+/CU‐Declining, A+/MCI, and A+/dementia (Fig5A). For A‐ groups trans‐capillary pulse wave delay was similar; yet, flow PI was statistically higher in A‐/CU‐Declining and A‐/MCI when compared to A‐/CU in the ICAs and MCAs (Fig5 C, D). Flow PI in A+/CU was higher than in A‐/CU. Conclusions: Decreased micro‐vascular tissue perfusion and shorter trans‐capillary pulse wave delay in A+ subjects with cognitive alterations (e.g. CU‐Declining, MCI, Dementia) compared to A+/CU indicates decreased metabolism and increased small vessel stiffness may contribute to cognitive decline and/or are induced by amyloidosis. In contrast, for A‐ subjects significant differences in arterial flow pulsatility index between A‐/CU, CU‐Declining, and MCI suggests macro‐vascular arterial stiffness contributes to cognitive decline. Abstract

The COVID‐19 pandemic highlighted the challenges delivering face‐to‐face patient care across healthcare systems. In particular the COVID‐19 pandemic challenged the imaging community to provide timely access to essential diagnostic imaging modalities while ensuring appropriate safeguards were in place for both patients and personnel. With increasing vaccine availability and greater prevalence of vaccination in communities worldwide we are finally emerging on the other side of the COVID‐19 pandemic. As we learned from our institutional and healthcare system responses to the pandemic, maintaining timely access to MR imaging is essential. Radiologists and other imaging providers partnered with their referring providers to ensure that timely access to advanced MR imaging was maintained. On behalf of the International Magnetic Resonance in Medicine (ISMRM) Safety Committee, this white paper is intended to serve as a guide for radiology departments, imaging centers, and other imaging specialists who perform MR imaging to refer to as we prepare for the next pandemic.
