Objective To determine the volume changes in gray and white matter during a long-term follow-up in patients suffering from pantothenate kinase-associated neurodegeneration (PKAN). Methods Magnetic resonance imaging was repeated in 13 patients and 14 age-matched controls after a mean interval of more than 7 years. T1-weighted sequences were evaluated by fully automated atlas-based volumetry, compared between groups and correlated with disease progression. Results The patients did not show generalized cerebral atrophy but did show a significantly faster volume reduction in the globus pallidus during follow-up (between -0.96% and -1.02% per year, p < 0.05 adjusted for false discovery rate) than controls, which was significantly related to the progression in their dystonia scores (p = 0.032). Conclusion The volume loss in the globus pallidus over time—together with the accumulation of iron known as the “tiger’s eye”—supports the pathophysiologic concept of this nucleus as a center of inhibition and its severe malfunction in PKAN.

Purpose: To demonstrate deviations of functional connectivity within the motor system in dystonic patients suffering from Pantothenate Kinase Associated Neurodegeneration, a genetic and metabolic disease, which is characterized by a primary lesion in the globus pallidus. Material and methods: Functional Magnetic Resonance Imaging data were measured during resting state in 12 patients suffering from a confirmed mutation of the PANK2 gene. Pantothenate Kinase-Associated Neurodegeneration (PKAN) is a form of Neurodegeneration with Brain Iron Accumulation (NBIA) associated with mutations in the pantothenate kinase 2 gene (PANK2). Pantothenate kinases catalyze the rate-limiting step of coenzyme A synthesis and Pank2 is the only pantothenate kinase isoform in humans that is localized to mitochondria. Acanthocytosis, the occurrence of spiculated erythrocytes, is observed in about 10% of the PKAN patients. Therefore PKAN is also classified together with other rare neurodegenerative diseases like Chorea Acanthocytosis (ChAc) and McLeod syndrome (MLS) into the Neuroacanthocytosis (NA) syndromes. It has not been investigated yet whether acanthocytosis in PKAN is associated with a specific subset of Pank2 mutations. In this study, we analyzed acanthocytosis of a cohort of 25 PKAN patients from the Dominican Republic that are homozygous for the c.680 A>G mutation in the PANK2 gene as compared to control donors that are heterozygous or wild-type with respect to this mutation. 3D modeling of this mutation indicated that the replacement of a tyrosine by a cysteine at position 227 in Pank2 disrupts a polar interaction within the A domain of the enzyme. Abstract Background and Importance Transtegmental brain herniation into the petrous bone is a rare cause of rhinoliquorrhea. Our case presents a combination of several typical clinical and imaging findings illustrating the ongoing etiologic discussion of such cerebrospinal fluid (CSF) fistulas. Clinical Presentation A 53-year-old man presented with nasal discharge after a strong effort to suppress coughing. Imaging revealed a transtegmental herniation of parts of the inferior temporal gyrus into the petrous bone and in addition a combination of signs of chronically increased intracranial pressure and a hyperpneumatization of the petrous bone. The fistula was closed by a middle cranial fossa approach. Conclusion The case illustrates the two main predisposing factors for development of petrous bone CSF fistulas: increased intracranial pressure and thinning of the tegmental roof due to extensive development of air cells. Because the CSF leakage repair does not change the underlying cause, patients have to be informed about the possibility of developing increased intracranial pressure and recurrences of brain herniations at other sites.

Zusammenfassung Ziel: Durch Integration des Fiber Tracking in die intraoperative Neuronavigation sollten die Lage der kortikospinalen Bahnen bei der Operation von Hirntumoren dargestellt und gleichzeitig die Lokalisation der Trajektorien mit einer unabhängigen Methode auf Ortsgenauigkeit überprüft werden. Patienten und Methodik: Dazu wurden bei 21 Patienten MR-Untersuchungen mit Diffusion Tensor Imaging (DTI) durchgeführt, die Tensorenfelder berechnet und die Bahnenverläufe mittels eines modifizierten „Line-Propagation“-Algorithmus bestimmt. Der Algorithmus kann die Richtung der Haupteigenvektoren auch über Faserkreuzungen hinweg verfolgen. Das „Tracking“ im peritumoralen Ödem ist durch die dort herabgesetzte fraktionale Anisotropie (FA) erschwert, so dass in diesen Fällen eine Modifizierung der Stopp-Kriterien des Algorithmus erforderlich war. Nach Koregistrierung der berechneten Trajektorien mit hochaufgelösten dreidimensionalen (3-D) Datensätzen und Umwandlung in das DICOM-Format wurden die Daten in ein neurochirurgisches Navigationssystem eingespeist. Ergebnisse: In allen 21 Fällen konnten die kortikospinalen Bahnen dargestellt werden. Dies wurde von den Neurochirurgen als sehr hilfreich empfunden, um eine Schädigung der Bahnen zu vermeiden. Während der Tumorentfernung wurde die Position der Bahnen bei 13 Patienten mittels elektrophysiologischer kortikaler und in 10 Fällen mit subkortikaler Stimulation überprüft und mit der Position der Trajektorien im Navigationssystem verglichen. In allen 13 Fällen war die kortikale Stimulation erfolgreich und bestätigte die Position des Gyrus praecentralis, aus dem die Trajektorien entsprangen. Eine positive Antwort auf die subkortikale Stimulation in der Wand der Tumorhöhle wurde dagegen nur in fünf von zehn Fällen erreicht, bei denen der Abstand zwischen Stimulationsort und Position der virtuellen Bahnen = 1 cm war. In allen Fällen mit negativem Ergebnis war der Abstand größer. Schlussfolgerung: Wenn sich die Zuverlässigkeit des beschriebenen Fiber Tracking bei weiteren Untersuchungen bestätigt, sollte die Methode als wichtige Komponente in die Neuronavigationssysteme aufgenommen werden. Zusammenfassung Hintergrund und Ziel: Seit langer Zeit sind typische Hirnstammsyndrome bekannt. Eines der häufigsten ist das Wallenberg- Syndrom bei dorsolateralem Medulla-oblongata-Infarkt. Ursache ist hier meist ein Verschluss der ipsilateralen PICA. Die Vielzahl der Syndrome auf der einen Seite sowie die Klassifikation der Hirnstamminfarkte in Gruppen andererseits können als Hinweis darauf angesehen werden, dass die Hirnstamminfarkte regelmäßig bestimmte Territorien und Gebiete bevorzugen. Inwieweit diese Hypothese zutrifft, sollte an einem Patientenklientel mit akuten Hirnstamminfarkten untersucht werden. Ziel unserer Arbeit war die Darstellung typischer Läsionsverteilungen bei Patienten mit akutem Hirnstamminfarkt. Patienten und Methoden: Bei 117 Patienten mit akutem, klinisch und im MRT nachgewiesenem Hirnstamminfarkt wurden die MRT-Läsionen (T2-w) normalisiert und schichtweise übereinander projiziert. Zur besseren Vergleichbarkeit wurden alle Läsionen auf dieselbe Seite projiziert. Die Läsionen wurden sowohl anatomisch mit Hilfe anatomischer Atlanten korreliert als auch zu bekannten Darstellungen der Hirnstammdurchblutung in Verbindung gesetzt. Ergebnisse: Die 117 Patienten (36 Frauen, 81 Männer) waren im Mittel 63,5 Jahre alt (Median 64,5; SD 12,7 Jahre). 85 Patienten (73%) hatten solitäre frische Hirnstamminfarkte, 19 Patienten (16%) wiesen zusätzlich frische Kleinhirninfarkte auf, neun Patienten (8%) zusätzlich frische supratentorielle Infarkte. Vier Patienten (3,4%) hatten multiple frische Infarkte in mindestens drei Gefäßterritorien. 65 Patienten (56%) wiesen im MRT keine Hinweise auf vorangegangene Ischämien auf. Die Hirnstamminfarkte zeigten typische Verteilungsmuster in unterschiedlichen Höhen in Bezug auf Ausdehnung, Schwerpunkt und im Verhältnis zur Mittellinie, den äußeren Begrenzungen des Hirnstammes und den bekannten Gefäßterritorien. In unserer Studie lag das Maximum nach Überlagerung aller Infarkte in Höhe der Medulla oblongata im lateralen Gefäßterritorium, welches von den Vertebralarterien über die PICA versorgt wird. In Übereinstimmung mit der klinischen Erfahrung, dass paramediane Ponsinfarkte die ?typischen? Ponsinfarkte darstellen, fanden wir das Maximum aller pontinen Infarkte weiter ventral in den anteromedialen und anterolateralen Gefäßterritorien, hervorgerufen durch Gefäßverschlüsse pontiner Ãste aus der Arteria basilaris. Mesenzephal schließlich lag das Maximum aller Infarkte erneut im anteromedialen und anterolateralen Gefäßterritorium. Schlussfolgerung: Diese Untersuchung kann als Basis dienen, über die Analyse der Läsionsverteilung ?typisch? lokalisierte Hirnstamminfarkte von atypischen Läsionen zu differenzieren. Zusammenfassung Ziel: Im Rahmen dieser Studie sollte untersucht werden, ob sich eine diffusionsgewichtete Sequenz eignet, unter Ausnutzung der anisotropen Diffusionsverhältnisse entlang der Markfasern die Zentralregion (ZR) bei Probanden und Patienten verlässlich darzustellen. Material und Methoden: Fünf Patienten mit Hirntumoren und 20 gesunde Probanden wurden an einem klinischen 1,5-TMR-Tomographen untersucht. Bei den Probanden dienten die ?klassischen? anatomischen Zeichen zur Lokalisation der Zentralregion an einem 3-D-T1-Datensatz als Goldstandard zur Lokalisation, bei den Patienten zusätzlich noch ein funktionelles Magnet-resonanztomogramm (fMRT) mit einem Finger-Tapping-Paradigma im Blockdesign und der intraoperative Befund. Ergebnisse: Bei den Probanden konnte die Zentralregion in allen Fällen durch ihre Faserverlaufsrichtung und dadurch selektiv in anterior-posteriorer Richtung behinderte Diffusion lokalisiert werden. Hierzu waren keine weiteren Nachverarbeitungsprozesse notwendig. Bei den Patienten bestand eine gute Korrelation zwischen Diffusionswichtung-(DWI-)Befund, den fMRT-Ergebnissen und der intraoperativen Lokalisation. Bilateral vestibular failure (BVF) is a rare disorder of the labyrinth or the eighth cranial nerve which has various aetiologies. BVF patients suffer from unsteadiness of gait combined with blurred vision due to oscillopsia. Functional MRI (fMRI) in healthy subjects has shown that stimulation of the visual system induces an activation of the visual cortex and ocular motor areas bilaterally as well as simultaneous deactivations of multisensory vestibular cortex areas. Our question was whether the chronic absence of bilateral vestibular input (BVF) causes a plastic cortical reorganization of the above-described visual–vestibular interaction. We used fMRI to measure the differential effects of horizontal visual optokinetic stimulation (OKN) on activations and deactivations in 10 patients with BVF and compared their data directly to those of pairwise age- and sex-matched controls. We found that bilateral activation of the primary visual cortex (inferior and middle occipital gyri, Brodmann area BA 17, 18, 19), the motion-sensitive areas V5 in the middle and inferior temporal gyri (BA 37), and the frontal eye field (BA 8), the right paracentral and superior parietal lobule and the right fusiform and parahippocampal gyri was significantly stronger and the activation clusters were larger than that of the age-matched healthy controls. Small areas of BOLD signal decreases (deactivations), located primarily in the right posterior insula containing the parieto-insular vestibular cortex, were similar to those in the healthy controls. No other sensory brain areas showed unexpected activations or deactivations, e.g. the somatosensory or auditory cortex areas. Our finding of enhanced activations within the visual and ocular motor systems of BVF patients suggests that they might be correlated with an upregulation of visual sensitivity during tracking of visual motion patterns. Functionally, these enhanced activations are independent of optokinetic performance, since the mean slow-phase velocity of OKN in the BVF patients did not differ from that in normals. Although psychophysical and neurophysiological tests have provided various examples of how sensory loss in one modality leads to a substitutional increase of functional sensitivity in other modalities, this study presents the first evidence of visual substitution for vestibular loss by functional imaging.
